Despite enormous efforts to achieve the goal of eliminating mother-to-child transmission of HIV-1, it remains a major challenge for many countries in sub-Saharan Africa, particularly Mali. Our objective is to assess changes in the rate of mother-to-child transmission of HIV-1. We conducted a cross-sectional study between January 1, 2009 to December 31, 2018 (10 years) of early diagnosis activity in newborns and children born to HIV-1-positive mothers at the National Institute for Public Health (INSP). The samples came from health and referral centers in mali. All samples were received at the Laboratory of Molecular Biology at the INSP. Proviral DNA extraction was performed from a blood spot sample with a Roche DNA kit, Cobas AmpliPrep/Cobas TaqMan HIV-1 qualitative Test, V2.0 (Roche Molecular System, Inc, USA) following the company procedures. Molecular diagnosis was performed using the same kits using an algorithm of three identical PCRs. The Epi Info version 7 software was used for data analysis with a significance threshold of 5%. A total of 10,714 samples of infants and children born to HIV-positive mothers were analyzed by PCR. Ninety-six percent of mothers were on ARV prophylaxis (AZT 3TC NVP and AZT NVP) and 60% of newborns received the same ARV prophylaxis. Of these children, 956 tested positive with an overall transmission rate of 8.92%, varying between 7.27% in 2009 and 08.01% in 2018. This rate was relatively low among children receiving prophylaxis at 2.04% and remained high for children who received breastfeeding at 5.62%. However, the transmission rate remains low for those who have benefited from mixed and artificial breastfeeding at 1.58% and 1.27% respectively. A significant proportion of children remained infected by their mothers during pregnancy, childbirth or breastfeeding. This study shows the importance of early diagnosis of HIV in children using molecular technology.

The department of Tillabéri is primarily affected by climatic phenomena, impacting crop yields, growing cycles, and consequently, the economic outcomes of agricultural operations. The objective of this study is to analyze these impacts of climate disruption on the economic performance of farms. The methodology adopted for this study combined documentary research with field surveys conducted on a sample of 250 randomly selected farmers. The analytical methods used mainly consisted of linear regression, profitability calculations, and linear programming. The findings indicate that all productions across different crops have experienced a decrease over the past 30 years. For instance, the production of millet, sorghum, and cowpea, which were respectively 812 kg/ha, 260 kg/ha, and 100 kg/ha between the last 30 and 20 years, has now dropped to 412 kg/ha, 106 kg/ha, and 46 kg/ha respectively. A negative and significant effect on agricultural net margin was observed due to variables such as flooding, drought, pest invasion in rice fields, and temperature changes. Smallholder farms show a relatively low margin (46%) to cover their fixed costs, which may indicate a risk if fixed expenses are high. Furthermore, the analysis results from linear programming reveal that farmers could achieve an additional net profit per hectare of 116,861 FCFA, 217201.5 FCFA, and 291988.2 FCFA respectively for small, medium, and large producers by managing variable costs and health-related expenses for households.

Chronic Osteomyelitis in Children at Zinder National Hospital: Diagnosis and Therapeutic Aspects

Objective: Chronic osteomyelitis represents a frequent complication in young children, especially those with sickle cell disease. The aim of this study was to describe the epidemiological aspects and therapeutic implications of chronic long-bone osteomyelitis in sickle-cell subjects aged 0 to 5 years at Zinder National Hospital. Methodology: This is a descriptive study with retrospective data collection of sickle cell subjects hospitalized and treated for chronic osteomyelitis from November 2023 to October 2024 at Zinder National Hospital. Results: Of the 16 patients included, male sex predominated in 56.25% (9/16). The mean age of patients was 32.69 months, ranging from 12 to 54 months. Involvement was multifocal in 75% (12/16) and bilateral in 37.5% (6/16). Bone involvement was predominantly in the pelvic limbs (87.5%). Among the bones affected, the femur was the most frequent site of involvement. All patients presented clinical symptoms such as pain, fever and swelling. Hyperleukocytosis was present in all subjects. Abscesses were present in 93.75% of cases, and sequestration was observed in 37.5% of patients. All cases were managed medico-surgically. Complications were noted in 25% (4/16), and involved residual bone deformities. Cure was confirmed in 43.75% (7/16). Conclusion: Chronic osteomyelitis in sickle-cell patients is a serious complication of acute osteomyelitis. Early management of acute osteomyelitis helps to avoid.

Introduction: Open leg fracture is a diaphyseal or metaphyseal, extra-articular fracture of one or both leg bones, with communication between the fracture site and the external environment. To provide a broader context, studies conducted globally have highlighted the significance of understanding open leg fractures due to their complex management and potential complications. Previous research in Africa and Europe provides comparative data that emphasizes regional differences in fracture types, causes, and treatment outcomes. This study aims to contribute to improving the management of theses fractures in Niger. Materials and Methods: This was a prospective, descriptive study carried out in the Traumatology-Orthopedics Department of the Zinder National Hospital, Niger, from December 8, 2020 to June 8, 2022 (18 months). We included all patients over 15 years of age in whom an open leg fracture was diagnosed and managed. Inclusion criteria focused on patients aged over 15 years with confirmed diagnoses of open leg fractures. Results: Over an 18-month period, the frequency of open leg fractures was 16.28%. There was a predominance of males (85.93% or n = 116). The average age of patients was 31.79 years. Road accidents were the primary circumstance of occurrence (93.33% or n = 126). The fracture line was simple in 74 patients (54.81%) and complex in 61 cases (45.19%). Surgically, the external fixator was used in 78 cases (57.80%). Postoperative follow-up was mostly straightforward. The main complications were infection (4 cases) and delayed consolidation (4 cases). Conclusion: Open leg fractures in adults are frequent at Zinder National Hospital. Public road accidents were the main cause of occurrence. An external fixator osteosynthesis was the most commonly used surgical option. These findings have important implications for healthcare policy, particularly in improving trauma care infrastructure and road safety initiatives in Niger. Future studies should prioritize the

Climate change is becoming a major issue for agriculture and the well-being of farmers. The objective of this article is to identify and analyze the production factors that may influence the competitiveness level of agricultural operations, as well as to establish a structural and functional typology of these farms. Using Principal component analysis (PCA) combined with hierarchical ascending classification (HAC) on 250 farmers, the study was able to set farms typology. Furthermore, variance analysis and econometric models (linear et quadratic) were also used for in-depth analysis. The results show the existence of three groups of farm (GA, GB, GC): GA (19.7%), GB (65.3%), and GC (15%). Drought spells and flood are the main climatic risks affecting rain-fed farm operations. For irrigated crops such as rice, the major constraints remain bird attacks, the invasion of pests and nematodes. Climate variability significantly increases the prevalence of morbidities in the region by raising the number of inactive individuals. This significantly and differentially affects the outcomes of these assets. Health expenditures represent a significant share (GB: 12% and GC: 11%) and a non-negligible share (GA: 8.4%). However, larger participations (GC) show better economic performance due to economies of scale, but all categories would benefit from adopting appropriate strategies to reduce losses and increase their resilience.

Acute Bronchiolitis about 535 Cases at Zinder National Hospital, Niger

Introduction: Lower respiratory infections in general, and acute bronchiolitis in particular, represent a major cause of morbidity in children in Niger. The aim of this study was to investigate the epidemiological, diagnostic and therapeutic features of acute bronchiolitis in infants in the Pediatrics A department of the National Hospital in Zinder. Patients and Methods: This was a descriptive, cross-sectional study with prospective data collection conducted from April 1 to May 31, 2024. All infants aged 0 to 24 months hospitalized with acute bronchiolitis in the pediatric ward A of Zinder National Hospital were included in the study. Epidemiological, diagnostic and therapeutic aspects were the characteristics studied. Data analysis was performed using Epi info 7 software. Results: During the study period, acute bronchiolitis accounted for 51% of hospitalizations, and infants aged 3 - 11 months were the most affected (52.90%). The sex ratio was 1.6. The average age of patients was 10.40 ± 6.91 months, with extremes of 1 and 24 months. 98.50% of patients came from urban areas, and the socioeconomic level of families was average in 47.49% of cases. Mixed breastfeeding was practiced in 59.63% of cases. Average hospital admission time was 4.22 days. The main clinical signs were cough (43.18%), rhinorrhea (70.85%), ronchi (35.33%), crepitating rales (26.54%), sibilant rales (24.11%), signs of struggle (14.02%) and polypnea (70.85%). Paraclinical findings included chest distension, hyperleukocytosis and anemia, with 47.48%, 55.14% and 78.31% respectively. Therapeutically, nebulization was used in 93.08% of cases, respiratory physiotherapy (41.34%), corticosteroid therapy (34.08%), antibiotic therapy (21.50%), oxygen therapy (20.11%) and antipyretics (4.68%) were prescribed. Progression was favorable in 93.08% of patients, with a case-fatality rate of 1.12%. Conclusion: With its high frequency, acute bronchiolitis is a public health problem. Introduction: The objective of this work was to study maternal deaths noted on arrival in the Gynecology and Obstetrics Department at Fousseyni Daou Hospital in Kayes over a period of 10 years. Materials and Methods: This was a cross-sectional, descriptive study with data collection over a period of 10 years;The data collection was retrospective over nine years from January 1, 2013 to December 31, 2021 and prospective over one year from January 1, 2022 to December 31, 2022. This study focused on all patients whose death was noted on arrival during pregnancy, labor or in the postpartum period in the Gynecology-Obstetrics Department of Fousseyni Daou Hospital. Confidentiality and anonymity were respected. The processing and analysis of statistical data were carried out using SPSS 20.0 software. Results: During the study period, we recorded 93 cases of death noted on arrival out of a total of 606 maternal deaths, i.e., a frequency of 15.34%. The average age was 27 years with the extremes of 20 years and 34 years. They came mainly from rural areas at 74%, were married at 82%, uneducated at 51.6%, housewives at 87.1%. The profession of the spouses is worker at 37.6%. In our sample, evacuated patients were the most represented with 75.3%. Postpartum hemorrhage was the most frequent reason for admission with 22.6%. The deceased patients had no medical history at 86%. In our series, 59.5% of the deceased patients had not had antenatal consultations (CPN). Patients who died on arrival and who had given birth at home were the most represented with 54.8%. Deaths from immediate postpartum hemorrhage complicated by shock were the most frequent with 25.8% followed by severe anemia 8.6%. Deaths were mainly due to direct obstetric causes at 76.3%. In these deaths observed on arrival, the 2nd delay was identified at 48.4%. Conclusion: Maternal deaths observed on arrival remain frequent in the Kayes region. Nutritional and microbiological evaluations of chocolate-coated Chinese chestnut (Castanea mollissima) fruit for commercial use

In recent years, China has become an increasingly important and the largest chestnut producer in the world. This study aimed to evaluate the nutritional value and microbiological quality of the roasted freeze-dried Chinese chestnut (Castanea mollissima) (RFDC) coated with dark chocolate (DCC) and milk chocolate (MCC) for industrial use and commercial consumption. Chocolate coating significantly improved the nutritional value of chestnut. RFDC had high levels of starch (66.23%) and fibers (3.85%) while DCC and MCC contained significantly high amounts of sucrose, protein, fat and minerals. Furthermore, the protein content doubled in MCC rather than in DCC. This could be attributed to the different formulations in the two products. Milk powder and whey protein constituted the source of protein in MCC while cocoa powder added to MCC formulation constituted an additional source of minerals. The amino acid profile showed differences in amino acid composition related to the sample’s protein content, indicating their good nutritional quality. The moisture contents in all RFDC, DCC and MCC were suitable for industrial processing. These results provide information about the additional nutrients of chocolate-coated chestnut and confirm that the product is an interesting nutritional food. The combination of freeze-drying and chocolate-coating generally results in greater reductions on microbiological loads, extending shelf life of harvested chestnut for commercial application. This is an alternative strategy to add value to chestnut, minimizing the significant losses in harvested fruits and providing a wider range of choices of new products to the consumer disposal.
