Increasing Incidence of Extended-Spectrum Beta-Lactamase-Producing Enterobacteriaceae (ESBL) and the Relation to Consumption of Broad-Spectrum Antimicrobial Agents 2003-2011 in a Large Area of Copenhagen, Denmark

Purpose: To investigate 1) the development in the incidence of ESBL-producing bacteria in hospitals and primary health care, 2) the contribution of primary health care to the incidence of ESBL-producing bacteria, and 3) the development in resistance patterns for all Escherichia coli and Klebsiella pneumoniae isolates in relation to antimicrobial consumption in hospitals and primary health care. Methods: ESBL-data were retrospectively collected from bacterial isolates from all specimens received at the Department of Clinical Microbiology from 2003 to 2011 together with the corresponding patient data. ESBL-production was detected in isolates from 1067 of 59,373 patients (1.8%) with an E. coli infection and in 263 of 8660 patients (3.0%) with a K. pneumoniae infection. Results: From 2003 to 2009, an increase in patients with an ESBL-producing isolate occurred in both hospitals and primary health care at the same time as an increased consumption of broad-spectrum antimicrobial agents was seen. Interventions to reduce prescription of cephalosporins and ciprofloxacin at the hospitals from 2010 resulted in a remarkable decrease in patients with ESBL-producing K. pneumoniae whereas a continuing increase was seen in patients with ESBL-producing E. coli both at hospitals and in primary health care. The proportion of patients with community-acquired ESBL-producing E. coli was stable with an increase of only 1.4% from 2007 to 2011. Conclusions: Reduction in prescription of broad-spectrum antimicrobial agents at the hospital level had an important impact on the incidence of ESBL-producing K. pneumoniae, but not on ESBL-producing E. coli.

Background: Abdominal aortic aneurysm (AAA) and cardiovascular disease are intimately associated, the latter representing the most common cause of death in Sweden. Cardiac complications are held responsible for the majority of perioperative morbidity and mortality in patients undergoing repair of AAA. The importance of preoperative thorough cardiac assessment is therefore obvious. The aim of this study was to evaluate the prognostic significance of preoperative echocardiographic findings for 1-year mortality after elective endovascular aneurysm repair (EVAR) of infrarenal AAA. Design: Retrospective analysis. Methods: The 505 patients were identified in a prospective database for endovascular interventions between 1998 and 2011, and data were retrieved from patient records. Preoperative echocardiography reports in 380 patients were reviewed and findings were notified according to a predefined protocol. Results: The 1-year mortality rate was 6.7%. Severe valve disease was present in 8.7% of the patients, aortic valve stenosis being the leading cause of valve pathology. Severe valve disease (OR 3.5, 95% CI [1.2 - 10.7];p = 0.025) and chronic kidney disease grade ≥ 3 (OR 7.5, 95% CI [2.1 - 26.1];p = 0.002) were the only independent risk factors for increased mortality rate at 1-year. Conclusion: Echocardiography should be a part of the preoperative workup in AAA patients. Finding of severe valve disease should be further evaluated by a cardiologist prior to EVAR.

Geostatistical Correlation of Aquifer Potentials in Abia State, South-Eastern Nigeria

In this paper, a collection of statistical correlation methods is used in the study of aquifer potentials in Abia State of south-eastern Nigeria. The Physiology, geomorphology and hydrogeology of the area are first presented. Sixty-six Vertical Electrical Sounding (VES) data sets are used to determine the aquifer. Demographic studies are then carried out in 220 communities in order to determine the relationship between population size on one hand and a unit draw-down of wells due to groundwater abstraction on the other. The relationship between geological Formation, aquifer potentials and depth of boreholes are then calculated using Pearson’s correlation matrix. Results show that the mean population of persons appears to be higher in Bende-Ameki Formation (of Eocene-Oligocene age) and the late Tetiary-Early Quaternary Coastal Plain Sands, than in the Cretaceous shale Formation of Asata Nkporo. The mean population of persons sitting on these Formations is 31,200, 18,370 and 5400 respectively. Furthermore, it is observed that a population increase of about 50 persons in a community in Abia State is accompanied by a unit volume (1 m3) draw-down of wells due to groundwater abstraction. It is therefore concluded that population size is positively correlated with groundwater abstraction, aquifer potentials and geological Formation favouring aquifer in Abia State.

＜正＞ Polymorphism of genes encoding drag metabolising enzymes is a major known genetic cause for the interindividual variability in drag toxicity and response. Based on the occurrence of mutations in these genes, gene deletions and gene duplications, the populations can be divided into poor （PM）. intermediate （IM）, efficient （EM） or ultrarapid （UM） metabolisers. A major role of this polymorphism is seen for individual susceptibility to drug toxicity. where subjects lacking a particular enzyme can achieve too high plasma levels at ordinary dosage, and with respect to non response of drag treatment where UMs are overrepresented. An increasing amount of literature indicates that the genetic variability of cytochrome P450 enzymes influences the therapeutic outcome of treatment of HIV. cancer, depression, ulcer, psychosis, cardiovascular disorders, epilepsy and pain. The number of important CYP variants identified increases and recently a common CYP2C19 gene causing increased drug metabolism was identified in our laboratory. The lecture will give a state of the art view of the field today with illustrations from the clinical perspective.

On June 282022 Sweden and Finland were invited to join *** a symbolic ceremony at the NATO Madrid Summit,all the central actors gathered in front of the cameras,as if to replicate a“family photo”in mini format:representatives from Sweden,Finland,and Turkey,as well as Secretary General *** atmosphere was mixed:tense because of the dramatic backdrop of the Summit,relief because of the speedy process that had taken the process *** was finally happening:Sweden and Finland would join the Atlantic *** how did they get there?Over a few hectic months,two countries strongly associated with neutrality had moved their policies from emphasizing continuity to altering the very foundations of their *** only NATO critics were taken by *** NATO supporters were astonished by the rapid pace and the turnaround of the Swedish Social Democratic *** relatively quick turnaround in Stockholm also meant that questions about the future and the consequences of NATO membership were postponed,not least because the Swedish election campaign put a lid on debate about strategic consequences:what journey awaits in the future and what strategic consequences does a future NATO membership have?This article discusses these questions in the belief that the temporal perspectives are connected:Sweden’s historical collaboration with NATO had significance for the country’s partner relationship,and in turn this influenced how Sweden became a member and what challenges the country faces together with its allies on a new northern flank in Europe.
