BACKGROUND The prevalence of chronic obstructive pulmonary disease(COPD)is increasing worldwide,and at the same time it is associated with increased mortality and reduced quality of *** to build sustainable rehabilitation approaches to COPD treatment and prevention are *** system of long-term pulmonary rehabilitation care is *** main reasons for the absence of these outpatient programs are the lack of experience,the lack of interest of insurance companies in secondary prevention programs,and the lack of healthcare facilities in large geographical *** possibility of at-home pulmonary rehabilitation models(telemonitoring and telecoaching)could solve this *** SUMMARY A 71-year-old man with severe COPD,Global Initiative for Obstructive Lung Diseases stage 3 underwent an 8-wk remotely monitored inspiratory muscle training with a device based on the test of incremental respiratory endurance ***,body plethysmography,test of incremental respiratory endurance examination,6-min walking test,body mass index,airflow obstruction,dyspnea,exercise capacity index,and subjective perception of dyspnea were performed as part of the initial and final *** patient performed training at home,and the physiotherapist monitored the patient remotely through a web application that allowed the physiotherapist to evaluate all training parameters in real-time and respond to any *** 8 wk of home training,there was a significant increase in all monitored values:maximal inspiratory pressure,a novel parameter sustained maximal inspiratory pressure,forced expiratory volume in 1 s,total lung capacity,forced vital capacity,peak expiratory flow,and inspiratory *** was also an improvement in the perception of dyspnea according to the COPD Assessment Test and a modified Medical Research Council Breathlessness Scale,an increase in exercise tolerance according to the 6-min walking test,and a decrease in the exercise

Maytenus erythroxylon Reissek(Celastraceae) ethanol extract presents antidiarrheal activity via antimotility and antisecretory mechanisms

AIM To investigate the acute toxicity,phytochemical profile,antidiarrheal activity and mechanisms of action of Maytenus erythroxylon(***)ethanol *** A castor oil-induced diarrhea model was used to evaluate antidiarrheal *** transit and gastric emptying protocols were used to evaluate a possible antimotility *** channels,nitric oxide,presynapticα2-adrenergic and tissue adrenergic receptors were investigated to uncover antimotility mechanisms of action and castor oil-induced enteropooling to elucidate antisecretory *** All tested doses of the extract(62.5,125,250 and 500 mg/kg)possessed antidiarrheal activity,with a significant decrease of the evacuation *** activity is possibly related to a reduced gastric emptying(125,250 and 500 mg/kg)and to a decreased percentage of intestinal transit for all tested *** last effect seems to be modulated by nitric oxide,KATP channels and tissue adrenergic ***,the extract also presented antisecretory effect due to a decrease of intestinal fluid *** The antidiarrheal effect of *** found in this study involves antimotility and antisecretory mechanisms that may be attributed to the chemical compounds found in this species:saponins,flavonoids,tannins,triterpenes and steroids.

Average Rainfall Estimation: Methods Performance Comparison in the Brazilian Semi-Arid

Considering the rainfall’s importance in hydrological modeling, the objective of this study was the performance comparison, in convergence terms, of techniques often used to estimate the average rainfall over an area: Thiessen Polygon (TP) Method;Reciprocal Distance Squared (RDS) Method;Kriging Method (KM) and Multiquadric Equations (ME) Method. The comparison was done indirectly, using GORE and BALANCE index to assess the convergence results from each method by increasing the rain gauges density in a region, through six scenarios. The Coremas/Mae D’água Watershed employed as study area, with an area of 8385 km2, is situated on Brazilian semi-arid. The results showed the TP, as RDS and ME techniques to be employed successfully to obtain the average rainfall over an area, highlighting the MEM. On the other hand, KM, using two variograms models, had an unstable behavior, pointing the prior study of data and variogram’s choice as a need to practical applying.
