Postoperative Radiotherapy in Bladder Cancer Patients: 5-Year Institutional Experience of National Cancer Institute, Cairo University

Purpose: Adjuvant radiation therapy could reduce loco regional failure, but currently has no defined role because of previously reported morbidity. NCI-Cairo routine work is to give adjuvant PORT for locally advanced bladder carcinoma patients. The aim of this work is to re-evaluate this protocol regarding its effect on prognosis and complications. Patients and Method: A retrospective study included 208 patients with pathologically proven bladder cancer who presented to the NCI, Cairo University from 2007-2011. All of them underwent RC with bilateral PLND followed by conventional post-operative radiotherapy in 2 - 6 weeks after surgery for 5000 cGy in 25 fractions, over 5 weeks using 2D technique. Analysis of data from their files was done for the treatment results, prognostic factors and complications. Results: Three years overall survival (OS) and disease free survival (DFS) for the whole group was ~60%, and 54% respectively in favour of the female gender, non-smokers, Squamous cell carcinoma patients, low grade tumours (grade 1 and 2) negative margins, N0, pT2b and early stage group showed the best prognoses. The 3 years metastases free survival (MFS) was ~71%. Only four factors showed a significant relation with the MFS which were the grade, LN status, T-stage and group staging. The local recurrence rate (LRC) at 2 years for the whole group was ~95% and 94% at 3 years. Only surgical margin status and extent of LN dissection had a significant impact on the LRC. Conclusions: Adjuvant radiotherapy shows sustained improvement in the loco regional control, and should be recommended for patients with locally advanced disease especially those with less than 10 dissected lymph nodes and those with positive margins.

Geochemistry of lamprophyres associated with uranium mineralization, Southeastern Desert, Egypt

Two brecciated shear zones (NNW-SSE) are found crosscutting cataclastic rocks. The cataclastic rocks (3.0 km2) occupy the core of the granitic pluton and enclose a roof pendant of mafic-ultramafic rocks. The NNW-SSE-extending lamprophyre dykes vary in thickness from 0.5 m to 1 m and up to 800 m long, cutting the cataclastic rocks and are composed mainly of plagioclases, amphiboles, relics of pyroxenes and K-feldspar phenocrysts embedded in fine-grained groundmass. They are characterized as being peraluminous, calc-alkaline in composition (chemical trap) and enriched in calcite, sulfide and P2O5. The lamprophyres were affected by hydrothermal alteration (chlorite-carbonate alteration) while the cataclastic rocks were affected by diagenetic alteration (K-feldspar-albite alteration). Uranium mineralization is the product of hydrothermal events and has been investigated by X-ray diffraction (XRD) and environmental scanning electron microscopy (ESEM), involving primary uranium minerals (U3O8) and secondary uranium minerals (uranophane and beta-uranophane, kasolite, torbernite, autonite and meta-autonite) in addition to U- bearing minerals (astrocyanite, betafite and fergusonite). The presence of different mineral parageneses associated with clay minerals indicates that the lamprophyres were subjected to acidic and alkaline mineralizing solutions. Moreover, the U-Zr/U, U-Ce/U values show negative correlations, confirming U-enrichment in both cataclastic rocks and shear zones while the Th-eU/eTh, Th-Zr/Th and Th-Ce/Th values show negative correlations, indicating that the U-bearing solutions are rich in Th in the cataclastic rocks only.

Disorders of the fallopian tube play a very important role in both infertility and gynaecological oncology. Tubal factor infertility is considered among the leading causes of female factor infertility. Many tubal disorders are related to infertility including congenital anomalies, acute and chronic inflammatory diseases, endometriosis and other pathologies that result in partial or total fallopian tube obstruction. In the field of gynaecological oncology, ovarian surface epithelial tumors remain one of the most fatal malignancies in women worldwide carrying the worst prognosis among female genital malignancies. For decades, the cell of origin of epithelial tumors has remained controversial and was largely believed to be surface ovarian epithelium. Recently several studies suggested that there is a major role of the fallopian tube in the development of ovarian surface epithelial tumors, mainly high grade serous carcinoma and other tumour types. In this article we review the role of the fallopian tube in both infertility and gynaecological oncology.

AIM： TO investigate colonic endocrine cells in lympho- cytic colitis （LC） patients. METHODS： Fifty-seven patients with LC were in- cluded. These patients were 41 females and 16 males, with an average age of 49 years （range 19-84 years）. Twenty-seven subjects that underwent colonoscopy with biopsies were used as controls. These subjects underwent colonoscopy because of gastrointestinal bleeding or health worries, where the source of bleed- ing was identified as haemorrhoids or angiodysplasia. They were 19 females and 8 males with an average age of 49 years （range 18-67 years）. Biopsies from the right and left colon were obtained from both patients and controls during colonoscopy. Biopsies were fixed in 4% buffered paraformaldehyde, embedded in paraffin and cut into 5 μm-thick sections. The sections immunostained by the avidin-biotin-complex method for se- rotonin, peptide YY （PYY）, pancreatic polypeptide （PP） enteroglucagon and somatostatin cells. The cell densi- ties were quantified by computerised image analysis using Olympus software. RESULTS： The colon of both the patient and the control subjects were macroscopically normal. Histo- pathological examination of colon biopsies from con- trols revealed normal histology. All patients fulfilled the diagnosis criteria required for of LC： an increase in intraepithelial lymphocytes （〉 20 lymphocytes/100 epithelial cells） and surface epithelial damage with increased lamina propria plasma cells and absent or minimal crypt architectural distribution. In the colon of both patients and control subjects, serotonin-, PYY-, PP-, enteroglucagon- and somatostatin-immunoreac- tive cells were primarily located in the upper part of the crypts of Lieberk0hn. These cells were basket- or flask-shaped. There was no statistically significant dif- ference between the right and left colon in controls with regards to the densities of serotonin- and PYY- immunoreactive cells （P = 0.9 and 0.1, respectively）. Utilities around the world have been considering Demand Side Management (DSM) in their strategic planning. The costs of constructing and operating a new capacity generation unit are increasing everyday as well as Transmission and distribution and land issues for new generation plants, which force the utilities to search for another alternatives without any additional constraints on customers comfort level or quality of delivered product. De can be defined as the selection, planning, and implementation of measures intended to have an influence on the demand or customer-side of the electric meter, either caused directly or stimulated indirectly by the utility. DSM programs are peak clipping, Valley filling, Load shifting, Load building, energy conservation and flexible load shape. The main Target of this paper is to show the relation between DSM and Load Forecasting. Moreover, it highlights on the effect of applying DSM on Forecasted demands and how this affects the planning strategies for utility companies. This target will be clearly illustrated through applying the developed algorithm in this paper on an existing residential compound in Cairo-Egypt.

Chelyshkov-Tau Approach for Solving Bagley-Torvik Equation

There are few numerical techniques available to solve the Bagley-Torvik equation which occurs considerably frequently in various offshoots of applied mathematics and mechanics. In this paper, we show that Chelyshkov-tau method is a very effective tool in numerically solving this equation. To show the accuracy and the efficiency of the method, several problems are implemented and the comparisons are given with other methods existing in the recent literature. The results of numerical tests confirm that Chelyshkov-tau method is superior to other existing ones and is highly accurate.

Objective: To study the effect of adding different concentrations of soybean lecithin in Tris based extender and to compare the results of Tris soybean lecithin extender with two commercial diluents (egg yolk-based: BullXcell and plant-based: OptiXcell) for Damascus goat sperm cryopreservation. Methods: The ejaculates from 4 mature male Damascus goats were obtained by using an artificial vagina. Semen samples were pooled and diluted in Tris extender supplemented with soybean lecithin at different concentrations of 1.5%, 3.0%, 6.0% and 10.0% to get better concentration to be used for further experiments, with a final concentration of 240×106 spermatozoa/mL. Semen samples were packed in straws (0.25 mL), frozen by using an automated system and stored in liquid nitrogen at -196 ℃ for 48 h. After thawing (37 ℃/30 s), the samples were evaluated for sperm quality parameters, including sperm motility, membrane integrity and acrosome integrity. Malondialdehyde concentration was estimated as a marker for lipid peroxidation. Based on the previous investigations, only Tris extender supplemented with 3.0% soybean lecithin (based on its positive results) was used versus BullXcell and OptiXcell for sperm ultrastructure evaluation and artificial insemination by using electron microscope and artificial insemination of the synchronized does. Results: There was no significant difference between Tris-soybean lecithin at 3.0% and BullXcell/OptiXcell diluents in post-thaw sperm parameters and fertility following artificial insemination;meanwhile, the other concentrations of soybean lecithin (1.5%, 6.0% and 10%) showed lower sperm parameters following cryopreservation. Conclusions: Using of Tris-soybean lecithin based extender at the level of 3.0% can be an appropriate alternative to either BullXcell or OptiXcell for Damascus goat sperm cryopreservation.
