Safety and efficacy of ultrasound ciliary plasty as a primary intervention in glaucoma patients

AIM: To evaluate the safety and the efficacy of the ultrasound ciliary plasty(UCP) on the intraocular pressure(IOP) control in glaucomatous eyes without previous glaucoma ***: A retrospective study included patients with primary and secondary glaucoma who underwent UCP in Dar Al Shifa Hospital, Kuwait between January 2017 to June 2018. High-intensity focused ultrasound procedures were performed under peribulbar anesthesia using the 2^(nd) generation probe with 8 s duration of each of the 6 shots. Complete ophthalmologic examinations were scheduled pre-treatment, and at 1 d, 1 wk, 1, 3, 6 and 12 mo posttreatment. Primary outcomes were the IOP reduction and success rates at 12 mo, while the secondary outcomes were the occurrence of vision threatening complications and visual ***: The records of 62 eyes of 62 patients were analyzed with mean age of 63.8 y(67.7% males). There was statistically significant reduction in the mean IOP from 35.2±8.3 mm Hg before treatment to 20.6±8.7 mm Hg at 12^(th) month(P<0.0005) with a mean percentage IOP reduction of 42.3% with significant reduction in the mean number of antiglaucomatous drugs from 3.2±0.4 before treatment to 2.1±1.02 at 12 mo(P<0.0005). Qualified success was achieved in 77.4% of eyes at 12 mo. No major intra-or posttreatment complications were ***: Second-generation UCP prove to be effective in reducing IOP in naive glaucoma patients with lower success rates in cases of neovascular and uveitic glaucomas.

In this paper, Laplace decomposition method (LDM) and Pade approximant are employed to find approximate solutions for the Whitham-Broer-Kaup shallow water model, the coupled nonlinear reaction diffusion equations and the system of Hirota-Satsuma coupled KdV. In addition, the results obtained from Laplace decomposition method (LDM) and Pade approximant are compared with corresponding exact analytical solutions.

Management of 634 Consecutive Patients with Chronic Pilonidal Sinus: A Nine-Year Experience of a Single Institute

Objectives: Different surgical techniques, with variable morbidity and recurrence rates, have been advocated for the management of chronic pilonidal sinus (PNS). This study was conducted to report the outcome of surgical treatment of 634 cases of chronic PNS at a single institute between January 2001 and January 2010. Methods: Karydakis flap was performed in 244 patients (38.5%, Group 1). Excision and midline closure was performed in 371 patients (58.5%, Group 2), while the open method was used in 19 (3.0%). Data regarding patient and sinus characteristics, operative de-tails, postoperative course, complications and recurrence were recorded. Mean follow-up was 73.5 months. Results: 571 patients were male (90.1%) and 63 were female (9.9%). Their ages ranged between 16 - 44 years (mean 25.7 years). The mean body mass index was 31.2 (range 23.6 - 41.9), and 71.5% (453/634) were hirsute. Chronic PNS was the first presentation (primary) in 504 patients (79.5%) and recurrent in 130 (20.5%).Overall complication rate was 16.1% (102/ 634) and overall recurrence rate was 8.4% (53/634). Comparing both groups showed that they were similar regarding demographic characteristics, clinical presentation, hospital stay, healing time and time off work. Operative time was insignificantly longer with Karydakis technique (mean 43.2 versus 39.1 minutes, respectively). Complications were sig-nificantly more in patients with midline closure (21%, 78/371) as compared with Karydakis procedure (9%, 22/244) (P = 0.0001). Likewise, there was a significantly (P = 0.0001) higher rate of recurrence with midline closure (12.1%, 45/371) as opposed to Karydakis technique (2.5%, 6/244). Conclusions: 1) PNS affects mainly young male adults who are usually, obese and hirsute, 2) Karydakis technique for the management of chronic PNS, whether primary or recurrent, is a non-lengthy, efficient procedure that has less overall complications and a lower recurrence rate than conventional excision and midline clo

目的评估再生疗法（MEBT／MEBO）治疗各种急慢性开放性创面的安全性和有效性。方法将2007年1月至2012年1月期间因各种开放性创伤就诊于3家不同医院的276例患者（376处创面）随机分组，接受MEBT／MEBO治疗（1组：患者141例，创面190处）或生理盐水湿敷治疗（2组：患者135例，创面186处）。前瞻性收集人口统计学、诱发疾病、合并症和创面特征等数据；每隔2周计算和对比刨面面积和愈合指数，持续12周；记录治疗后24周的二次截肢情况。结果开放性创伤包括慢性褥疮（202处）、糖尿病足溃疡（119处）和各种其它创面（55处）。受试患者中171例（63％）为男性，105例（37％）为女性；年龄介于5～102岁，平均年龄63岁±14．5岁。两组患者在人口统计学、临床特点、生化检查和创面特征方面接近。MEBT／MEBO治疗组在治疗2周时，其愈合指数显著升高、创面面积显著缩小；在12周时，MEBT／MEBO组中66．3％（126／190）的创面实现了完全愈合（HI=1），而生理盐水湿敷组仅22．6％（42／186）的创面完全愈合（，=71．4，P=0．000）。治疗12周时，MEBT／MEBO组无l例患者的愈合指数〈0．5；而生理盐水湿敷组38．7％（72／186）的患者愈合指数〈0．5X=165，P=0．000）。两组患者均无不良反应或过敏反应发生。治疗后24周，与生理盐水湿敷组相比，MEBT／MEBO治疗组的二次截肢率明显降低（3．3％：19．0％）（X。=7．52，P=0．006）。结论MEBT／MEBO不仅操作简便、使用安全，还能显著促进开放性创面的愈合，治疗2周就可显著提高创面愈合指数，12周时几乎2／3的创面可完全愈合，24周时糖尿病足溃疡患者中发生二次截肢的患者明显减少。
