Long-term follow-up of quality of life in high-risk patients undergoing transcatheter aortic valve implantation for symptomatic aortic valve stenosis

Background Transcatheter aortic valve implantation （TAVI） has become the standard treatment for patients with severe symptomatic aortic stenosis （AS） considered at very high risk for surgical aortic valve replacement. The purpose of this sub-study was to evaluate long-term （〉 4 years） health-related quality of life （QoL） in octogenarians who underwent TAVI. Methods A single center observational registry in twenty patients who underwent frame analysis assessment ≥ 4 years after TAVI. Health-related QoL was evaluated, using the Short Form-36 （SF-36）, the EuroQoL-5D （EQ-5D） and the visual analogue score （EQ-VAS） questionnaires. Results The mean SF-36 subscale scores at follow-up were physical functioning 40.8 ± 26.3, role physical functioning 67.7 ± 34.9, vitality 54.6 ± 21.6, general health 52.1 ± 20.4, social functioning 63.8 ± 37.7, role emotional functioning 70.2 ± 36.0, mental health 73.2 ± 23.3 and bodily pain 80.9 ± 22.9. The mean EQ-VAS score 〉 4 years after TAVI was 64.7 ± 15.1. With respect to functional class, 80% of the patients were in NYHA class I/II at follow-up compared to 15% prior to TAVI. Conclusions This sub-study reports a significant improvement in functional class （NYHA） in a selected group of very elderly patients 〉 4 years after TAVI. Furthermore, all patients showed a satisfactory QoL despite their age and multiple comorbidities. In addition, our study reveals a lower QoL when compared with the general age matched Dutch population.

Analysis of mid-spatial frequency wavefront distortions from a liquid-cooled flash-lamp pumped Nd:phosphate laser amplifier

Mid-spatial frequency wavefront deformation can be deleterious for the operation of high-energy laser systems. When fluid cooled high-repetition-rate amplifiers are used, the coolant flow is likely to induce such detrimental mid-spatial frequency wavefront deformations. Here, we describe the design and performance of a 90 mm × 90 mm aperture, liquid-cooled Nd:phosphate split-slab laser amplifier pumped by flash-lamps. The performance of the system is evaluated in terms of wavefront aberration and gain at repetition rates down to 1 shot per minute. The results show that this single cooled split-slab system exhibits low wavefront distortions in the medium to large period range, compatible with a focus on target, and despite the use of liquid coolant traversed by both pump and amplified wavelengths. This makes it a potential candidate for applications in large high-energy laser facilities.

Clinical studies have shown that Aggregatibacter actinomycetemcomitans(***)is associated with aggressive periodontitis and can potentially trigger or exacerbate rheumatoid arthritis(RA).However,the mechanism is poorly ***,we show that systemic infection with *** triggers the progression of arthritis in mice anti-collagen antibody-induced arthritis(CAIA)model following IL-1βsecretion and cell infiltration in paws in a manner that is dependent on caspase-11-mediated inflammasome activation in *** administration of polymyxin B(PMB),chloroquine,and anti-CD11b antibody suppressed inflammasome activation in macrophages and arthritis in mice,suggesting that the recognition of lipopolysaccharide(LPS)in the cytosol after bacterial degradation by lysosomes and invasion via CD11b are needed to trigger arthritis following inflammasome activation in *** data reveal that the inhibition of caspase-11-mediated inflammasome activation potentiates aggravation of RA induced by infection with *** work highlights how RA can be progressed by inflammasome activation as a result of periodontitis-associated bacterial infection and discusses the mechanism of inflammasome activation in response to infection with ***.

Epidemiological and Histopathological Profile of Prostate Cancer: A Retrospective Study in the Pathology Department of the University Clinics of Kinshasa

Background: Prostate cancer, the most common male cancer, represents a real public health problem in terms of its frequency and severity in different countries around the world. It disproportionately affects people of African descent wherever they live in the world [1]. To the best of our knowledge, its extent and particularities in the African environment are not well known. Objective: To determine the epidemiological and histopathological profile of prostate cancer in the CUK anatomopathology department. Methodology: This is a retrospective study conducted at the University Clinics of Kinshasa Anapathology Department from january 1, 2015 to December 31, 2022, a period of 8 years. Word processing and tables were entered using the Hp brand computer, with Microsoft Office WORD 2016 software. Data analysis was performed with SPSS version 22.0 software. Results were presented in tables and figures. Results: Prostate was diagnosed in 132 cases, i.e. 1.58% of all CUK laboratory analyses and 8% of cancers diagnosed. The age group most affected was 66-75 years, i.e. 59% of all subjects. Adenocarcinoma was the most frequent histological type, and biopsy dominated in 111 cases (84.1%). Conclusion: Prostate cancer is a real public health problem. Worldwide, and in the Democratic Republic of Congo, it is the most frequently diagnosed cancer in men, and the leading cause of cancer-related death in men. In the DRC, because of the delay in consulting our patients and the weakness of systematic screening, patients are seen at an advanced stage of the disease. Treatment is multidisciplinary, involving surgery, radiotherapy and chemotherapy (including targeted therapies). Patient awareness and screening campaigns will help to considerably reduce the delay in diagnosis and the morbidity and mortality associated with prostate cancer.

Floristic Diversity, Stand Structure and Plant Life Traits in the Forest-Savanna Mosaic at Ndjole (Centre Cameroon)

With the aim of assessing floristic diversity, stand structure and vegetation ecology, the present work was carried out in the savanna mosaic forest of the locality of Ndjole (Centre Cameroon). A total of 30 floristic plots of 40 m × 40 m (1600 m2) were delineated in the different ecosystems (forest, forest-savanna contact zone and the savanna). Within each plot, all individuals with a diameter at breast height ≥ 10 cm at 1.30 m were taken into account. ANOVA was used to compare the means of the diversity indices of the different ecosystems, and the DUNCAN test at 5% significance level (XLSTAT 2016) to separate these means. A total of 85 species belonging to 67 genera and 33 families were recorded. That is, 68 species in semi-caducifolia forest, 45 in forest-savanna contact zone and 18 in savanna. Urticaceae, Combretaceae, Apocynaceae, Myristicaceae and Euphorbiaceae are the most diverse families in the forest. On the other hand, in contact zones, the Euphorbiaceae, Fabaceae, Annonaceae and Moraceae families are the most diversified, while in the savannas, the Combretaceae, Fabaceae and Annonaceae families are the most diversified. Shannon’s diversity index values (1.7 to 2.37 bits) reflect low specific diversity;Piélou’s equitability (0.86 to 0.9) shows good distribution of individuals within species. The diametric structure of woody species shows an inverted “j” shape in all plant communities. This indicates a high regeneration potential characterized by a greater number of small trees than large trees. Mesophanerophytes and megaphanerophytes are dominant in forests and forest-savanna contact zones, and microphanerophytes in the savannas. Endozoochory is the main mode of diaspore dissemination. As regards the distribution of phytogeographical types, it shows the overall dominance of Guineo-Congolese species in all plant communities. 近年来,大震频发,地震发生机制和地震断裂作用的研究已成为当今社会的重大课题,而断裂带的变形机制,尤其是大型地震断裂带的变形机制是认识断裂活动性和地震发生机制的关键。本文通过介绍青藏高原东缘龙门山断裂带和鲜水河断裂带的最新研究成果,探讨青藏高原大型地震断裂带的变形机制,主要认识如下:(1)汶川地震使不同性质断层同时破裂,并在地壳浅部(～732.6m深度)富流体断层泥中发生了熔融作用,颠覆了地震的传统认识,深化了对浅部断层力学性质的认识。(2)龙门山断裂带映秀—北川断裂带在晚三叠世曾经发生Mw7.4～7.9级的逆冲-左行走滑大地震,断裂岩的高磁化率各向异性度值指示了大地震活动。(3)汶川—茂县断裂带在新生代时期存在三期不同构造变形,青藏高原东缘不存在下地壳流模式。(4)龙门山断裂带汶川—茂县断裂带曾经发生了摩擦热温度>500℃的大地震活动,孕震环境为还原性的含有硫化物的低温热液流体环境。(5)强震频发的鲜水河断裂带是藏东南物质外迁的主要边界,具有长期蠕滑变形行为,流体作用较强,流体的注入明显提升断层核部强矿物含量,促进了蠕滑断层的局部变强。上述认识丰富和完善了断裂作用理论,提高了对青藏高原大型地震断裂带变形机制的认识,为断裂带活动性、地震发生机制、地震危险性评估和防震减灾提供了科学依据。

Concomitant pancreatic adenocarcinoma in a patient with branch-duct intraductal papillary mucinous neoplasm

Branch duct intraductal papillary mucinous neoplasms(BD-IPMN) are pre-malignant pancreatic cystic lesions which carry a small risk of malignant transformation within the cyst. Guidelines exist with respect to surveillance of the cysts using computed tomography, magnetic resonance imaging, and/or endoscopic ultrasound(EUS). There are reports that patients with IPMNs are at increased risk of developing pancreatic adenocarcinoma, which arises in an area separate to the IPMNs. We present two cases of pancreatic adenocarcinoma arising within the parenchyma, distinct from the IPMN-associated cyst, identified with EUS. This case report highlights that patients with BD-IPMN are at increased risk for pancreatic adenocarcinoma separate from the cyst and also the importance for endosonographers to carefully survey the rest of the pancreatic parenchyma separate from the cyst in order to identify small pancreatic adenocarcinomas.

Cambrian-early Ordovician volcanism across the South Armorican and Occitan domains of the Variscan Belt in France:Continental break-up and rifting of the northern Gondwana margin

The Cambrian-lower Ordovician volcanic units of the South Armorican and Occitan domains are ana- lysed in a tectonostratigraphic survey of the French Variscan Belt. The South Armorican lavas consist of continental tholeiites in middle Camhrian-Furongian sequences related to continental break-up. A significant volcanic activity occurred in the Tremadocian, dominated by crustal melted rhyolitic lavas and initial rifting tholeiites. The Occitan lavas are distributed into five volcanic phases：（I） basal Cambrian rhyolites, （2） upper lower Cambrian Mg-rich tholeiites close to N-MORBs but crustal contaminated, （3） upper lower-middle Cambrian continental tholeiites, （4） Tremadocian rhyolites, and （5） upper lower Ordovician initial rift tholeiites. A rifting event linked to asthenosphere upwelling took place in the late early Cambrian but did not evolve. It renewed in the Tremadocian with abundant crustal melting due to underplating of mixed asthenospheric and lithospheric magmas. This main tectono-magmatic conti- nental rift is termed the ＂Tremadocian Tectonic Belt＂ underlined by a chain of rhyolitic volcanoes from Occitan and South Armorican domains to Central Iberia. It evolved with the setting of syn-rift coarse siliciclastic deposits overlain by post-rift deep water shales in a suite of sedimentary basins that fore- casted the South Armorican-Medio-European Ocean as a part of the Palaeotethys Ocean.
