AIM:To summarize the experience of response to COVID-19 outbreak at a tertiary eye care institute and its network of health facilities in ***:Our responses are based on the principles of social distancing,hand hygiene,respiratory etiquettes,surface disinfection protocol,and rational use of appropriate personal protective equipment(PPE).We describe our response in terms of administrative controls,clinical protocols,staff protection,environmental controls,and social distancing *** also discuss our communication strategies and monitoring systems,to ensure compliance to ***:Administrative control is mainly related to formation of task force and its *** protocols are related to patient triaging methods and clinical examination guidelines in Outpatient,Inpatient and Operating *** protection is focused on training staff on the protocols to be followed in hospital as well as at home,and use of *** protocol is focused on cleaning and disinfectant methods to be used in the *** addition,there are systems for communication as well as monitoring compliance to ***:We hope that these protocols and our experience would help the ophthalmic community globally and serve as a guide to protect ophthalmologists and ophthalmic care personnel,and their patients across the world.

How to Approach Real Mathematical Modeling in Surge Capacity: Clinical Review

Objectives: Science of surge is one of most important topics in the realm of disaster preparedness. Since 2006, after Academic Emergency Medicine (AEM) Consensus Conference, few articles with quantitative data address decision making in surge capacity. The aim of this article is looking forward to the facts about mathematical modeling and proposes real modeling in decision making to have better outcome. Methods: Literature Research was performed on database for the last ten years (2007-2017). Articles with mathematical modeling were separated and classified based on the usage of them in the field. Results: All current mathematical studies compared based on pre-hospital and hospital setting and flexibility in change of global level of care in time. Integrated model of sigmoid curve and HASC (Hospital Acute Care Surge Capacity) with name B-H integrated modeling in two-hour interval proposed. Conclusion: This study shows dynamic process of disaster planning based on outcome and reality. The proposed model makes surge capacity more predictable and adjustable.

Flavonoid-rich beverage effects on lipid profile and blood pressure in diabetic patients

AIM: To compare freeze-dried strawberry(FDS) beverage and strawberry-flavored drink effects on lipid profile and blood pressure in type 2 diabetic(T2D) ***: In a randomized, double-blind, controlled trial, 36 subjects with T2D(23 females; mean ± SE age: 51.57 ± 10 years) were randomly divided into two groups. Participants consumed two cups of either pure FDS beverage(each cup containing 25 g freeze-dried strawberry powder equivalent to one serving of fresh strawberries; intervention group) or an iso-caloric drinkwith strawberry flavoring(similar to the FDS drink in fiber content and color; placebo group) daily for 6 wk. Anthropometric measurements, 3 d, 24 h dietary recall, and fasting blood samples were collected at baseline and at weeks 6 intervention. After lying down and relaxing for approximately 10 min, each participant's blood pressure was recorded in triplicate with 5 min intervals; recordings were made at baseline and the trial end-point. Each participant's lipid profile was assessed before and after ***: Assessment at the weeks 6 intervention showed a significant reduction from baseline in total cholesterol levels and total cholesterol to high-density lipoprotein cholesterol(HDL-C) ratio in the intervention group(179.01 ± 31.86 to 165.9 ± 32.4 mg/L; P = 0.00 and 3.9 ± 0.88 to 3.6 ± 0.082 mg/L; P = 0.00 respectively), but the change was not significantly different between the two groups(P = 0.07, P = 0.29 respectively). Systolic blood pressure levels were significantly reduced from baseline in both the FDS and placebo drink groups(129.95 ± 14.9 to 114.3 ± 27.5 mm Hg; P = 0.02 and 127.6 ± 15.6 to 122.9 ± 14.47 mm Hg; P = 0.00 respectively), but the reduction was not significantly different between the two groups. Diastolic blood pressure was significantly reduced post-intervention in the FDS drink group compared to placebo group(78.7 ± 7.2 vs 84.4 ± 5.8; P = 0.01), the reduction was also significant within the FDS drink group(84

Recording electric field evolution in single-shot with THz bandwidth is needed in science including spectroscopy,plasmas,biology,chemistry,Free-Electron Lasers,accelerators,and material ***,the potential application range depends on the possibility to achieve sub-picosecond resolution over a long time window,which is a largely open problem for single-shot *** solve this problem,we present a new conceptual approach for the so-called spectral decoding technique,where a chirped laser pulse interacts with a THz signal in a Pockels crystal,and is analyzed using a grating optical spectrum *** borrowing mathematical concepts from photonic time stretch theory and radio-frequency communication,we deduce a novel dual-output electro-optic sampling system,for which the input THz signal can be numerically retrieved—with unprecedented resolution—using the so-called phase diversity *** show numerically and experimentally that this approach enables the recording of THz waveforms in single-shot over much longer durations and/or higher bandwidth than previous spectral decoding *** present and test the proposed DEOS(Diversity Electro-Optic Sampling)design for recording 1.5 THz bandwidth THz pulses,over 20 ps duration,in *** we demonstrate the potential of DEOS in accelerator physics by recording,in two successive shots,the shape of 200 fs Rms relativistic electron bunches at European X-FEL,over 10 ps recording *** designs presented here can be used directly for accelerator diagnostics,characterization of THz sources,and single-shot Time-Domain Spectroscopy.

Osteocytes are the most abundant cells in bone,which is a frequent site of breast cancer ***,we focused on Wnt signaling and evaluated tumor-osteocyte *** animal experiments,mammary tumor cells were inoculated into the mammary fat pad and *** role of Lrp5-mediated Wnt signaling was examined by overexpressing and silencing Lrp5 in osteocytes and establishing a conditional knockout mouse *** results revealed that administration of osteocytes or their conditioned medium(CM)inhibited tumor progression and *** overexpressing Lrp5 or β-catenin displayed strikingly elevated tumor-suppressive activity,accompanied by downregulation of tumor-promoting chemokines and upregulation of apoptosis-inducing and tumor-suppressing proteins such as *** antitumor effect was also observed with osteocyte-derived CM that was pretreated with a Wnt-activating ***,silencing Lrp5 in tumors inhibited tumor progression,while silencing Lrp5 in osteocytes in conditional knockout mice promoted tumor *** exhibited elevated Lrp5 expression in response to tumor cells,implying that osteocytes protect bone through canonical Wnt ***,our results suggest that the Lrp5/β-catenin axis activates tumor-promoting signaling in tumor cells but tumor-suppressive signaling in *** envision that osteocytes with Wnt activation potentially offer a novel cell-based therapy for breast cancer and osteolytic bone metastasis.

Outcomes and long-term survival of coronary artery surgery:The controversial role of opium as risk marker

AIM To study survival in isolated coronary artery bypass graft(CABG) patients and to evaluate the impact of preoperative chronic opium consumption on long-term *** Cohort of 566 isolated CABG patients as Tehran Heart Center cardiac output measurement was conducted. Daily evaluation until discharge as well as 4- and 12-mo and 6.5-year follow-up information for survival status were fulfilled for all patients. Long-term 6.5-year overall and opium-stratified survival, adjusted survival curves based on opium consumption as well as possible predictors of all-cause mortality using multiple cox regression were determined by statistical *** Six point five-year overall survival was 91.8%; 86.6% in opium consumers and 92.7% in non-opium consumers(P=0.035). Patients with positive history of opium consumption significantly tended to have lower ejection fraction(EF), higher creatinine level and higher prevalence of myocardial infarction. Multiple predictors of all-cause mortality included age, body mass index, EF, diabetes mellitus and cerebrovascular accident. The hazard ratio(HR) of 2.09 for the risk of mortality in opium addicted patients with a borderline P value(P=0.052) was calculated in this model. Further adjustment with stratification based on smoking and opium addiction reduced the HR to 1.20(P=0.355).CONCLUSION Simultaneous impact of smoking as a confounding variable in most of the patients prevents from definitive judgment on the role of opium as an independent contributing factor in worse long-term survival of CABG patients in addition to advanced age, low EF, diabetes mellitus and cerebrovascular accident. Meanwhile, our findings do not confirm any cardio protective role for opium to improve outcome in coronary patients with the history of smoking. Further studies are needed to clarify pure effect of opium and warrant the aforementioned findings.

The Impact of Rainbow Trout Farm Effluents on Water Quality of Koohrang River, Iran

The aim ,of this study was to assess the potential impact of trout farm effluents on water quality of Koohrang river （Chaharmahal va Bakhtiary province-Iran） where seven trout farms are localized and to determine the some physico-chemichal parameters of the farms. Monthly samples were taken from inflow, outflow and 1,500 meter lower than these farms have been measured and avaluated during 6 months. Measured physico-chemical water quality parameters were DO, BOD5, PO4, NO3, NO2, NH3, TH, PH, TDS, TSS. Monthly data were converted to mean values （-4- standard error） for comparison. Data on water quality parameters of inflow and outflow water and 1,500 meters lower than trout farms are given. Amount of outflow BOD5 in farms has significant difference with inflow water and again has been decreased in 1,500 meters, DO amount in outflow rather than inflow water decreased and in distance of 1,500 meters significant statistically increased （P 〈 0.01）. TSS amount between inflow and outflow of farms has not difference but in 1,500 meters distance has been increased （P 〈 0.01）. PH in inflow and outflow and 1,500 meters lower has not so statistical difference. TH in outflow rather than inflow has been increases and in 1,500 meters distance, its amount has been higher than outflow, too. But this difference has not been significant （P 〉 0.01）. Ammonia in farms outflow rather than inflow has been significant difference （P 〈 0.01）. In flow-through aquaculture systems like raceways and tanks, effluents are discharged to the environment with enhanced concentrations of nutrients and solids. Such effluents may have a serious negative impact on the quality of the receiving water when discharged untreated. The results of this study indicated that trout farm effluents had a impact on the water quality of Koohrang river, but in 1,500 meters distance these parameters modified and suitable for fish farming, as river self purified the environment.

An advanced numerical model, developed by Swerea MEFOS AB, has been used to diagnose and optimize steel casting practices for peritectic steels for a Scandinavian steel producer. The model is based on the commercial code ANSYS-FLUENT 15.0 which solves the Navier-Stokes equations by using an interface tracking technique known as Volume of Fluid（VOF） for the multiple phase system steel/slag. As a result, the model predicts metal flow and slag infiltration as well as their influence on heat flux and solidification under transient conditions. Recent improvements to the model include separation between mould powder and slag film and considering the effect of crystallization on interfacial resistance for a Peritectic mould powder through a User Defined Function（UDF）. The casting parameters analysed consist ofcasting speed interlocked with oscillation settings, Submerged Entry Nozzle（SEN） immersion depth and Argon injection flow rate. These practises were optimised by performing parametric studies to evaluate shell growth, lubrication depth, cooling channel heat flux, etc. Application of the model allows the prediction of trends and opportunities for further improvement in the form of guidelines for the process and enhanced operational windows. These have been tested at industrial level showing improvements on surface quality and process stability.

Osteocytes are mechanosensitive bone cells, but little is known about their effects on tumor cells in response to mechanical stimulation. We treated breast cancer cells with osteocyte-derived conditioned medium(CM) and fluid flow-treated conditioned medium(FFCM) with 0.25 Pa and 1 Pa shear stress. Notably, CM and FFCM at 0.25 Pa induced the mesenchymal-to-epithelial transition(MET), but FFCM at 1 Pa induced the epithelial-to-mesenchymal transition(EMT). This suggested that the effects of fluid flow on conditioned media depend on flow intensity. Fluorescence resonance energy transfer(FRET)-based evaluation of Src activity and vinculin molecular force showed that osteopontin was involved in EMT and MET switching. A mouse model of tumorinduced osteolysis was tested using dynamic tibia loadings of 1, 2, and 5 N. The low 1 N loading suppressed tumor-induced osteolysis, but this beneficial effect was lost and reversed with loads at 2 and 5 N, respectively. Changing the loading intensities in vivo also led to changes in serum TGFβ levels and the composition of tumor-associated volatile organic compounds in the ***, this study demonstrated the critical role of intensity-dependent mechanotransduction and osteopontin in tumorosteocyte communication, indicating that a biophysical factor can tangibly alter the behaviors of tumor cells in the bone microenvironment.
