Background:Fluid resuscitation is a key treatment for sepsis,but limited data exists in patients with existing heartfailure(HF)and septic *** objective of this study was to determine the impact of initial fluid resuscitationvolume on outcomes in HF patients with reduced or mildly reduced left ventricular ejection fraction(LVEF)withseptic ***:This multicenter,retrospective,cohort study included patients with known HF(LVEF≤50%)presentingwith septic *** were divided into two groups based on the volume of fluid resuscitation in the first 6 h;mL/kg or≥30 mL/*** primary outcome was a composite of in-hospital mortality or renal replacementtherapy(RRT)within 7 *** outcomes included acute kidney injury(AKI),initiation of mechanicalventilation,and length of stay(LOS).All related data were collected and compared between the two groups.A generalized logistic mixed model was used to assess the association between fluid groups and the primaryoutcome while adjusting for baseline LVEF,Acute Physiology and Chronic Health Evaluation(APACHE)II score,inappropriate empiric antibiotics,and receipt of ***:One hundred and fifty-four patients were included(93 patients inmL/kg group and 61 patientsin≥30 mL/kg group).The median weight-based volume in the first 6 h was 17.7(12.2–23.0)mL/kg in themL/kg group vs.40.5(34.2–53.1)mL/kg in the≥30 mL/kg group(Pmposite of in-hospital mortality or RRT between themL/kg group compared to the≥30 mL/kggroup(55.9%vs.45.9%,P=0.25),***mL/kg group had a higher incidence of AKI,mechanicalventilation,and longer hospital ***:In patients with known reduced or mildly reduced LVEF presenting with septic shock,no differencewas detected for in-hospital mortality or RRT in patients who received≥30 mL/kg of resuscitation fluid comparedto less fluid,although this study was underpowered to detect a ***

Cochlear implantation in patients with meniere’s disease:A systematic review

Distribution of Respiratory Tract Infectious Diseases in Relation to Particulate matter(Pm_(2.5))Concentration in Selected Urban Centres in Niger Delta Region of Nigeria

Due to the visibility of soot in the environment of the Niger Delta especially Rivers State that has led to the increase of Respiratory Tract Infections(RTIs)in the region,this study was undertaken to determine the relationship between Particulate matter(Pm_(2.5))concentration and the incident of Respiratory Tract Infections(RTIs)in selected urban centres of the Niger *** on RTIs were collected from the Hospital management Boards of the ministries of Health of Rivers,Bayelsa and Delta States and the data for Pm_(2.5)were remotely sensed from 2016 to 2019,and subsequently analyzed with ANOVA and Spearman’s rank correlation *** findings of this study revealed that there was significant variation in the occurrence of Pm_(2.5)across the selected urban centres in the Niger Delta *** Pm_(2.5)for the reviewed years was far above the World Health Organization(WHO)annual permissible limit of 10μg/m^(3)thereby exacerbating Respiratory Tract Infections(RTIs).The epidemiology of the RTIs showed that there are basically four(4)prominent RTI diseases:Asthma,Tuberculosis,Pneumonia and Chronic Obstructive Pulmonary Disease(COPD).The result of this study showed that the concentration of Pm_(2.5)varies in all the selected cities,and the mean monthly variation(2016-2019)showed that Port Harcourt had 47.27μg/m^(3)for January while Yenagoa and Asaba had 46μg/m^(3)and 47.51μg/m^(3)respectively for January;while the lowest mean value in the cities were seen within the month of September and October,which also had a strong seasonal influence on the concentration of Pm_(2.5).The concentration of Pm_(2.5)and the numbers of RTIs also gradually increases in the study areas from 2016 to *** study recommends that the necessary regulatory bodies should closely monitor the activities of the companies likely to cause such pollution;guild them through their operations and give prompt sanctions and heavy fines to defaulters of the accepted standards.

Improvement of the Viscous Penalty method for Particle-Resolved Simulations

A numerical study of the parameters controlling the viscous penalty method is investigated to better set up Particle-Resolved Direct Numerical Simulations (PR-DNS) of particulate flows. Based on this analysis, improvements of the methods are proposed in order to reach an almost second order convergence in space. The viscous penalty method is validated in Stokes regime by simulating a uniform flow past a fixed isolated cylinder. moreover, it is also utilized in moderate Reynolds number regime for a uniform flow past a square configuration of cylinder and compared in terms of friction factor to the well-known Ergun correlation.
