The brain's extracellular matrix(ECm),which is comprised of protein and glycosaminoglycan(GAG)scaffolds,constitutes 20%-40% of the human brain and is considered one of the largest influencers on brain cell functioning(Soles et al.,2023).Synthesized by neural and glial cells,the brain's ECm regulates a myriad of homeostatic cellular processes,including neuronal plasticity and firing(miyata et al.,2012),cation buffering(moraws ki et al.,2015),and glia-neuron interactions(Anderson et al.,2016).Considering the diversity of functions,dynamic remodeling of the brain's ECm indicates that this understudied medium is an active participant in both normal physiology and neurological diseases.

Achieving optimum outcomes after liver transplantation requires an understanding of the interaction between donor,graft and recipient *** the cohort of patients waiting for a transplant,better matching of the donor organ to the recipient will improve transplant outcomes and benefit the overall waiting list by minimizing graft failure and need for re-transplantation.A Pubmed search was conducted to identify published literature investigating the effects of donor factors such as age,gender,ethnicity,viral serology;graft factors such as size and quality,recipient factors such as age,size,gender and transplant factors such as major or minor blood group incompatibility and immunological *** also report technical and therapeutic modifications that can be used to manage donor-recipient mismatch identified from literature and the authors’clinical *** donor and recipient factors impact graft survival after liver *** matching based on donor-organ-recipient variables,modification of surgical technique and innovative peri-transplant strategies can increase the donor pool by utilizing grafts from marginal donors that are traditionally turned down.

Inflammatory bowel disease(IBD)is a complex disease with multiple pathogenic *** the pathogenesis of IBD is still unclear,a current hypothesis suggests that genetic susceptibility,environmental factors,a dysfunctional immune system,the microbiome,and the interactions of these factors substantially contribute to the occurrence and development of *** existing and emerging drugs have been proven to be effective in treating IBD,none can cure IBD permanently.G protein-coupled receptors(GPCRs)are critical signaling molecules implicated in the immune response,cell proliferation,inflammation regulation and intestinal barrier *** in the understanding of the structures and functions of GPCRs have provided a driving force for exploring the roles of GPCRs in the pathogenesis of diseases,thereby leading to the development of GPCR-targeted *** date,a number of GPCRs have been shown to be associated with IBD,significantly advancing the drug discovery process for *** associations between GPCRs and disease activity,disease severity,and disease phenotypes have also paved new avenues for the precise management of patients with *** this review,we mainly focus on the roles of the most studied proton-sensing GPCRs,cannabinoid receptors,and estrogen-related GPCRs in the pathogenesis of IBD and their potential clinical values in IBD and some other diseases.

Pulmonary alveolar microlithiasis： an interesting case report with systematic review of Indian literature

Pulmonary alveolar microlithiasis is a rare disease characterized by intra-alveolar presence of microliths. This study reports an interesting case of pulmonary alveolar microlithiasis and provides a systematic review of cases reported from India. A 23-year-old female presented with a history of cough, wheeze, chest pain, and episodic wheeze for five months. Pulmonary function tests demonstrated an obstructive pattern, and chest X- ray showed fine micronodular opacities predominantly involving the middle and lower zones of both lungs. Transbronchial lung biopsy revealed the diagnosis. She responded well to inhaled steroid therapy. A systematic review of literature was performed and identified 73 cases of pulmonary alveolar microlithiasis reported from India. The mean （SD） age of the patients was 28.8 （14.9） years, with an almost equal male：female ratio. many patients were asymptomatic at presentation. Breathlessness and cough were the most common symptoms, and the disease progressed into respiratory failure associated with cor pulmonale. About one-third of the cases were initially misdiagnosed and treated as pulmonary tuberculosis. Extra-pulmonary manifestations and co- morbidities were also evident in our series. This systematic review helps to determine epidemiological and clinical characteristics of pulmonary alveolar microlithiasis. Further research is needed to elucidate the etiopathogenesis, diagnosis, and therapeutic options, which are beneficial in developing and identifying cost- effective treatment for pulmonary alveolar microlithiasis.

Classical Indian Buddhist philosopher Nfigfirjuna is known for his philosophical interpretations of the central conception of Buddha＇s teachings, the philosophy of middle Path （mfidhyamika）. Notably he had introduced the unique concept of ＂emptiness＂（gnyata） to explain the middle Path philosophy： the philosophical meaning of ＂emptiness＂ is dependent co-arising of various elements that support the worldly experience. This study investigates how this concept is used in explaining the subjectivity of a human person and how it is used for interpreting the unique process of human existence. The discussions on subjectivity are imprecise in modern and contemporary philosophy. But Ngrjuna＇s philosophy enables us to explain subjectivity conclusively, without it having to be explained using metaphysical positions. Sunyata may introduce a new definition for the concept of non-self： not for negating the self but for caring self from the problems of life by making it centered in the middle Path （madhyama-pratipat）, where one may naturally be able to use his wisdom （prajh） as the guiding principle： not mere knowledge （na）. Sanyat is understood using fourfold （***.i） logical analysis, not twofold analysis employed normally by other philosophers. Here, the Buddhist notion of self as the co-dependent evolution process of five aggregates （pahcaskandhas） is reinterpreted using the unique method of tetralemma （***. i）. This critique explores the Western philosophy＇s conceptions on ＂human reasoning, ＂logocentrism,＂ and the objective analytical method of modern science.＂ After careful cross examination of the rival philosophical positions, it reasons out why the ＂rationale of nature＂ is always superior to ＂human reasoning＂ and ＂logocentrism,＂

Peer-to-Peer(P2P)electricity trading is a significant research area that offers maximum fulfilment for both prosumer and *** also decreases the quantity of line loss incurred in Smart Grid(SG).But,uncertainities in demand and supply of the electricity might lead to instability in P2P market for both prosumer and *** recent times,numerous machine Learning(mL)-enabled load predictive techniques have been developed,while most of the existing studies did not consider its implicit features,optimal parameter selection,and prediction *** order to overcome fulfill this research gap,the current research paper presents a new multi-Objective Grasshopper Optimisation Algorithm(mOGOA)with Deep Extreme Learning machine(DELm)-based short-term load predictive technique i.e.,mOGOA-DELm model for P2P Energy Trading(ET)in *** proposed mOGOA-DELm model involves four distinct stages of operations namely,data cleaning,Feature Selection(FS),prediction,and parameter *** addition,mOGOA-based FS technique is utilized in the selection of optimum subset of ***,DELm-based predictive model is also applied in forecasting the load *** proposed mOGOA model is also applied in FS and the selection of optimalDELm parameters to improve the predictive *** inspect the effectual outcome of the proposed mOGOA-DELm model,a series of simulations was performed using UK Smart meter *** the experimentation procedure,the proposed model achieved the highest accuracy of 85.80%and the results established the superiority of the proposed model in predicting the testing data.

The development of two simple methods for wavelength-optical intensity modulation techniques for fiber Bragg grating （FBG） sensors is presented. The performance is evaluated by measuring the strain and temperature. The first method consists of a narrow band source, an optical circulator, an FBG; and a power meter. The source and Bragg reflected signal from the FBG need to be matched to get linear results with good power levels. The source spectral power levels are very critical in this study. The power reflected from a matched reference FBG is fed into the measuring FBG in the second method. Since the FBGs are matched, the entire power is reflected back initially. During the measurement, the change in the measurand causes the reflected power from the sensing FBG to vary. A costly high resolution spectrum analyzer is required only during the characterization of the FBG and source. The performances of two interrogators are compared by measuring the strain and temperature. In the second method, the strain measurements can be made insensitive to the temperature variation by selecting a source with a flat spectrum at the measurement range. Highlights of these methods are the portability, cost effectiveness and better resolution.

Background: Acute otitis media (AOm) is an inflammatory disease of the middle ear causing significantmorbidity in early childhood. A pilot study was undertaken to identify the role of various risk factorsSouth Indian children with AOm, especially the role of nasopharyngeal ***: A prospective case control pilot study was conducted in children aged below six years,presenting to a single tertiary care from 2018 to 2019. Fifty cases with AOm and 45 age and gendermatched controls were recruited. Two nasopharyngeal swabs were collected, one was processed forbacterial culture. The other swab was processed according to the CDC recommended broth enrichmentmethod to identify carriage of S. pneumoniae. Subsequent serotyping was done by Quellung method andconventional sequential multiplex ***: Otalgia was the major presentation seen in 92% of the children with AOm. None of the clinical anddemographic characteristics were found to be statistically significant between the cases and controls. Themost common otopathogen was S. pneumoniae (55%) followed by H. influenza (29%). The commonS. pneumoniae serotypes encountered were 11A and *** colonization with S. pneumoniae[OR 6.57, p m inchildren. The risk increased with co-colonization (OR 13.89,p m as a risk factor that is enhanced by co-colonization.S. pneumoniae was the mainotopathogen in this population, serotypes 11A and 19F being the most common.
