In mali, cholesteatoma surgery remains a challenge due to the limited number of otological practitioners, and the low socio-economic level of patients reflecting the difficulties in performing CT or mRI for post-operative follow-up. We therefore initiated this work with the objective of analyzing, through a broad bibliographic review, the clinical, paraclinical, topographical, therapeutic and progressive aspects of a series of ear cholesteatoma. materials and method: This was a retrospective and prospective, descriptive study that took place in the ENT and head and neck surgery department of the Gabriel Touré University Hospital over 36 months from November 2020 to October 2023. These were patients admitted to the department for chronic cholestematous otitis media. Result: In total, we collected 34 files of patients admitted for cholesteatoma. This represented 9% of cases compared to all otological surgeries carried out during the same period, i.e. 362 cases. The average age of our patients was 35.31 years, with extremes ranging from 7 years to 80 years, there were 24 men and 10 women, i.e. a m/F ratio of 2.4. The average time to diagnosis was 7 years. The main functional signs were dominated by chronic fetid purulent otorrhea associated with hypoacusis in 94.6% of cases. One case of meningeal complication and three cases of cerebral empyema were reported. Clinically, a postero-superior and marginal tympanic perforation was observed in 53%, a retraction pocket in 5.9%, the sentinel polyp in 9%, and a non-marginal tympanic perforation in 32%. Pure-tone audiometry performed preoperatively for all patients showed conductive hearing loss in 83%. In 82% of cases we created a recess with the creation of a mini box. An ossiculoplasty was performed immediately with fragments of cartilage. The postoperative course found the disappearance of otorrhea (88.23%), the persistence of otorrhea in one case, two cases of facial paralysis, and one case of tinnitus. We did not identify

Investigates the asymptotic stability of numerical solution of linear delay differential equations(DDEs) of the form y′(t)=Ly(t)+my(t-τ), t>0, y(t)=(t), -τ≤t≤0, where τ>0, L, m∈ C n×n  and ∈ (,C n), which is formulated using the semigroup instead of classical step by step methods for numerical solution of this equation, as an abstract Cauchy problems and then discretized in a systems of ordinary differential equations(ODEs), and examines the asymptotic stability properties with respect to the class of DDEs with the complex coefficients which preserve the stability properties for a fixed delay.

Lifestyle Changes for Abdominal Obesity Prevention and Encouraging Fruit Consumption may Be Beneficial in Preventing Left Ventricular Hypertrophy in Sub-Saharan African and maghreb

There is a growing body of evidence showing a close correlation between left ventricular mass with cardiovascular morbidity and overall mortality. Therefore, identifying the determinants of left ventricular hypertrophy can be of great importance for cardiovascular prevention, for prognosis and therapeutic intervention. Objective: To assess the prevalence and identify the independent determinants of echocardiographic left ventricular hypertrophy in The mA-Ghreb and Sub-Saharan Africa Left-Ventricul ArGEometry Study (mAG-SALVAGES) participants. methods: The mAG-SALVAGES is a community based study in which 100 asymptomatic Black Sub-Saharan African (BSSA) and 189 white skin maghreb within the age of 18 to 55 years underwent a resting echocardiography. multivariate logistic regression analysis was utilized to identify the independent determinants of LVH left ventricular hypertrophy. Results: men represented the majority of the enrolled participants: 173 (59.9%). Echocardiographic left ventricular hypertrophy was seen in 10 (3.5%) participants. Age ≥40 years, female gender, overall obesity, abdominal obesity, hypertension status and less fruit consumption were significantly associated with echocardiography left ventricular hypertrophy. After adjusting for confounding factors, age ≥40 years, female gender, abdominal obesity and less fruit consumption were independently and significantly associated with echocardiographic left ventricular hypertrophy, as illustrated in the following equation: Y = 0.36 + 0.162 age >40 years + 2.69 female gender + 2.52 abdominal obesity + 1.31 less fruit consumption. Conclusion: Lifestyle changes for the prevention of abdominal obesity and encouraging fruit consumption may be beneficial in preventing left ventricular hypertrophy.

Purpose: The purpose of this study was to determine HIV prevalence among participants admitted to the delivery room and recent birth attendants. Materials and Methods: This was a descriptive, cross-sectional, analytical study, with a prospective collection from February 1, 2018 to January 31, 2019 at fousseyni Daou Hospital in Kayes, Mali. Results: During our study period, we recorded 4269 deliveries. HIV-positive patients known before labour accounted for 0.21%. Palpebral Reconstruction in a Secondary Ophthalmology Center in mali: About a Case

Introduction: Significant trauma to the periocular region can seriously damage ocular structures and their adnexa. The eyelids can be damaged during these traumas. The most frequent lesions are lacerations, the surgical treatment of which is generally simple. In some cases, there are complex traumas where there is a loss of tissue, which is difficult to treat. We report the case of a 26-year-old young man, farmer with no medical and surgical history, referred by the odontostomatology and maxillofacial surgery department for burns to the left hemi face by the exhaust pipe of his motorbike which occurred following a public road accident (AVP). Observation: A 26-year-old young man with no medical-surgical history, visual acuity was 5/10 with good mobility of the globe. In collaboration with the maxillofacial surgeon, a graft of the hemi face was performed using a flap from the inner side of the thigh first. In the second time we carried out a recovery by a flap which consisted in taking a supra-superciliary flap and suturing it at the level of the palpebral edges which, in spite of a fragile vitality of the tissues. Conclusion: The reconstruction of the eyelid is a real problem in our service because of the technical platform and the availability of consumables, which limits us in the choice of the operating technique.

The Influence of Ethnicity in the Relationship between Sedentary Screen Time and Left Ventricular mass: Insights from the mAG-SALVAGES

Background: The influence of race/ethnicity on the relationship between sedentary screen time and left ventricular mass has been recently suggested, but remains a subject of debate, and has never been explored in Africa. Purpose: To determine whether there is a racial/ethnic influence on the relationship between sedentary screen time and left ventricular mass in mAGhreb and Sub Saharan Africa Left-Ventricul ArGEometry Study (mAG-SALVAGES) participants. methods: 100 blacks sub-Saharan African and 187 white maghreb aged 18 - 55 years underwent an interview on their behavioral measures, physical activity and eating habits. Their left ventricular mass has also been measured by a resting transthoracic echography according to the American Society of Echography. Generalized linear models evaluated a test-for-trend across higher levels of sedentary screen time in progressive models with left ventricular measurements as dependent variables. The study population was stratified into quartiles of sedentary screen time (separately for whites and blacks) and examined the joint association of sedentary screen time and LVm within quartiles of physical activity. Results: Among White maghreb, higher screen time was associated with smaller left ventricular mass (P Conclusions: Sedentary screen time is associated with smaller left ventricular mass in White maghreb, not in black sub-Saharan African. The lack of association in blacks supports a potential qualitative difference in the cardiovascular consequences of sedentary screen based behavior.

Epidemiological and Clinical Profile of Breast Cancer at Bamako Radiotherapy Center

Breast cancer is a major public health problem because of its incidence and mortality. Purpose: To establish the epidemiological and clinical characteristics of breast cancer seen at the radiotherapy center at the mali Hospital of Bamako. Patients and methods: It was a retrospective, descriptive study of data from patients seen for breast cancer at the Center of Radiotherapy of mali Hospital between April 2014 and December 2016. The parameters studied were: age, sex, family history of breast cancer, menopausal status, parity, breast tumor location, histological type, histological grade, cancer classification stage. Results: 134 cases of breast cancer were collected, with a frequency of 15%. The sex ratio (H/F) of 0.007. The patient’s mean age was 47 ± 11 years old. The most represented age groups were 33 - 47 years old with 45.5% and 48 - 62 years old with 39%. Three percent (3%) of patients had a family history of breast cancer. Fifty (50%) of the patients were menopausal. The main clinical signs found were : mammary nodules (98%), mastodynia (65%), nodes (67%). Invasive ductal carcinomas were found in 94% of patients, followed by infiltrating lobular carcinomas with 3.7% and metaplastic carcinomas with 1.7%. SBR Grades II and III were mostly found with 37% and 23%. The average tumor size was 87 mm ± 43. Stage III was predominantly represented with 72%, followed by Stage II with 24% and Stages I and IV with 2% each. Conclusion: Breast cancer is common and reaches both before and after 50 years;the diagnosis is usually late;hence it is the importance of raising awareness and screening before the age of 50 and popularizing some complementary tests to better understand the prognosis of this disease and promote more targeted and conservative treatments that will improve survival.

An Unusual and Fatal Case of Perthes’ Syndrome

Perthes syndrome includes cervical and facial cyanosis, subconjunctival haemorrhage, petechial haemorrhages on the face, neck, and upper chest, secondary to chest compression. We report a clinical case of Perthes’ syndrome in 3-year-old boy victims of chest crush by a tractor. Clinical examination found: a Glasgow score at 13/15, polypnea, tachycardia, hypotension;wound of the occipital scalp, a low abundance of hematuria, deformation of the left lower limb. Body scan: bilateral pneumothorax and pulmonary contusions, left hemothorax;lacerations at the medial and upper renal pole grade IV, a low abundance of retroperitoneal hematoma, fracture of the left femur. Biological assessment showed major metabolic disturbances with severe acute rhabdomyolysis and multi-visceral failure. Without clinical improvement, the patient died after 36 hours of intensive care.

Left Ventricular measurements in Black Sub-Saharan Africans and White maghreb

Background: Evidence that blacks have greater left ventricular mass (LVm) than whites has been demonstrated by large population-based American studies. However, to our knowledge, there is no study to date comparing LVm in Black Sub-Saharan Africans (BSSA) and the maghreb white population. We compared LVm measured echocardiographically in asymptomatic BSSA and maghreb. methods and Results: A total of 100 asymptomatic BSSA and 189 maghreb, (18 to 55 years old), underwent resting two-dimensional transthoracic echocardiography. LVm and geometry were assessed according to the 2015 American Society of Echocardiography and the European Association of Cardiovascular Imaging updated guidelines for cardiac chamber quantification. Crude or indexed LVm to body surface area or height2.7 was similar in BSSA and in maghreb (132.7 ± 37.0 vs. 134.2 ± 35.7 g;73.1 ± 17.8 vs. 72.9 ± 16.2 g/m2;32.1 ± 9.8 vs. 33.6 ± 9.5 g/m2.7). However, the left ventricular posterior wall was thicker in BSSA. Patterns of left ventricular geometry (normal, concentric remodeling, or concentric or eccentric hypertrophy) were equally distributed among the two ethnic groups. Conclusions: Left ventricular posterior wall thickness but not LVm is greater in BSSA than in maghreb.

Effectiveness and Tolerance of Tenofovir Disoproxil Fumarate in Sub-Saharan Viral B Cirrhotic Patients

Tenofovir reduces viral load during viral cirrhosis B and improves prognosis. The aim of this study was to evaluate the effectiveness and safety of Tenofovir during the treatment of viral B cirrhosis. The study was retrospective and prospective on patients with viral B cirrhosis treated with Tenofovir in the Hepato-Gastroenterology Department of University Hospital Gabriel Touré in Bamako (mali) and evaluated between three and six months after the start of treatment. We included 89 patients. The mean age was 44.5 ± 16 years with extremes of 18 and 90 years. The sex ratio was 1.2. At inclusion all patients had a detectable viral load with an average of 2651.96 ± 1495.85 IU/ml. Follow-up viral load was undetectable in 84.3% with patients between 3 and 6 months of treatment. The average detectable viral load was only 27.04 ± 21.05 IU/ml. There was also an improvement in hepatocellular function and few side effects. Conclusion: This study shows the interest of Tenofovir in the treatment of viral cirrhosis B with very few adverse effects.
