Connective tissue diseases (CTds) are Autoimmune diseases (AIds) characterized by the appearance of autoantibodies, which are diagnostic markers. Investigations of these autoantibodies play a major role in the management of several autoimmune diseases. The objective of this study was to describe the profile of anti-ENA antibodies according to the clinical symptoms of mixed CTds in Conakry teaching Hospital. We performed a cross-sectional study during six months. A total of 20 patients was recruited and we measured antibodies using the ELISA technique. The mean age of our patients was 36.5 years, with a predominance of females. Cutaneous and rheumatological signs were the main clinical manifestations. SLP was the most frequent CTds;the threshold of ENA antibodies positivity was higher in scleroderma with and SLP. Anti-ENA identification reveals the frequency of anti-SSA (83.33%), anti-U1RNP (66.66%) and anti-histone (50%) antibodies. Antinuclear antibodies (ANA) react with various components of the cell nucleus. Their detection is of major interest in the diagnosis of CTds. Our results highlight the importance of determining the specificity of these antibodies to guide differential diagnosis.

Comparative Study of Available Solar Potential for Six Stations of Sahel

Sahel is an African area with high solar potential. However, this potential is not uniform across the region. This paper examines the spatial distribution of the available solar potential by using six stations across the Sahel area. This comparative study was based on the analysis of in situ measurements in dakar in Senegal, Niamey in Niger, Ouagadougou, Gaoua, dori in Burkina Faso and N’djamena in Chad. The results showed the presence of a good global solar potential with an average value of about 5.43 kWh/m2/day. The maxima of global potential are noted in the northern part in Niamey with a value of 6.24 kWh/m2/day while the minima are recorded in the south-eastern part in N’djamena with an irradiation close to 4.71 kWh/m2/day. Then, the monthly evolution of this potential shows similar trends for all stations. Indeed, two maximums are observed during the year in Spring (march) and Autumn (October). However, for most of these stations, the minima of global potential are recorded in Winter (November, February) and during the rainy season (July, October). moreover, the direct normal potential also shows seasonal trends for the two stations (dakar, Niamey) where it was measured. The maxima of direct normal irradiation (dNI) are observed between February and may with a value of 5.5 kWh/m2/day in dakar and in Niamey with a value around 5.32 kWh/m2/day between February and November.

Serious Burn by Sulfuric Acid: Case Report of Particular Therapeutic Strategy in a Non-Specialized ICU at Hôpital Principal de dakar (HPd) Sénégal

managing severe burns remains problematic due to the lack of specialized units, but also because of the delay in implementing emergency care. The aim is to show that an adapted strategy, can lead to satisfying management of chemical burns. The authors report retrospectively the case of a patient admitted for chemical burns, and treated in a non-specialized intensive care unit;a 38 years old male, referred for burns by sulfuric acid at his workplace. On admission to H15, the clinic did not reveal any vital organs failure. Burns were localized on two legs and soles of the two feet (18% TBSA). Treatment combined daily dressings with silver sulfadiazine. On day 14, the wound healing associated occlusive gauze dressing, iodine cream application, and mechanical debridement. On day 47, a 5% dermal autograft performed on right foot favored with good attachment grafts. On day 58, the patient was released after complete skin recovery. Then, in a non-specialized burn unit and without early surgery access, our wound healing adapted strategy was successful. In Senegal, chemical burns represent about 2.5% of burn cases. They are often from accidents on occupation job, while generally in Africa chemical burns result from criminal attacks. Patients with severe lesions are admitted in non-specialized environments after an extended time of transfer, and don’t have efficient initial care. This may explain the high morbidity and mortality after burns in our country. The lack of surgical facilities such as skin substitutes, in non-specialized unit on low or median income countries (LmICs), explains this long period of wound healing. The treatment of severe burn in LmICs is hazardous.

Prevalence of Renal Failure in Children and the Need for dialysis in Paediatric Nephrology in a developing Country

Background: Renal failure is among the major visceral failures responsible for morbidity and mortality in children. Epidemiological data on renal failure in children are limited in sub-Saharan Africa, including Senegal. We conducted this study to assess the prevalence of renal failure (RF) and the need for dialysis in the paediatric nephrology unit. Patients and methods: This was a retrospective and descriptive study in the pediatric nephrology unit from 1st January 2020 to 30 June 2021, including children with acute or chronic RF. The need for dialysis was judged to be met by its effectiveness in face of an indication. The data collected were analysed on sphinx plus 2017 software. Results: The prevalence of RF was 60% (n/N = 132/220). RF was acute in 59% (n/N = 78/132) of cases. The mean age was 6.53 ± 4.77 years with a sex ratio of 1.8. The mean serum creatinine level was 27.22 mg/L (240.9 μmol/L) and BUN at 0.85 g/L. The aetiologies were dominated by prerenal AKI (Acute Kidney Injury) complicating nephrotic syndrome (NS) in 37.2% (n/N = 29/78) and dehydration due to stomach flu in 17.6% (n/N = 9/51). Acute post-infectious glomerulonephritis including group A streptococcus and plasmodium falciparum accounted for 16.7% (n = 13/78) of intrinsic AKI. The need for dialysis was unmet in 50% (n/N = 7/14) with a mortality of 14.1% (n/N = 11/78). The mean age of the patients with chronic RF was 8.68 ± 4.74 years with a sex ratio of 2.6. The mean serum creatinine level was 36.56 mg/L (323.5 μmol/L) and the BUN level was 0.99 g/L. Half of the children were classified as having CKd stage II (early stage). The aetiologies were dominated by primary focal and segmental glomerulosclerosis (FSGS), 53.7% (n/N = 29/54), renal hypoplasia, 22.2% (n/N = 12/54) and reflux nephropathy, 29.4% (n/N = 5/17). The need for dialysis was unmet in 46.1% (n/N = 6/13). The overall mortality of chronic kidney disease (CKd) with RF was 29.6% (n/N = 8/27). Conclusion: The prevalence of RF was high i

Acute Coronary Syndromes: Epidemiological, Clinical and management Aspects in Thies (Senegal)

Purpose: To describe the epidemiological, clinical and management characteristics of acute coronary syndromes (ACS) in Thies, Senegal. methods: This was a prospective study that included consecutively from October 1st, 2018 to march 03rd, 2019, patients aged 18 years or older admitted for an ACS at the dIABCARmET department of the Saint Jean de dieu Hospital in Thies. Results: Of the 516 patients admitted, 29 had ACS, the prevalence was 5.62%. The average age of the patients was 64.2 ± 11.6 years. Nineteen patients (65.5%) were male and 10 (34.5%) were female, the sex ratio was 1.91. The number of direct admissions was 18 (62%) versus 11 patients (38%) referred. The mode of transport was a private vehicle in 19 patients (65.5%), an ambulance in 7 patients (24.1%) and public transport in 3 patients (10.3%). STEmI (ST segment elevation myocardial infarction) was the most common presentation observed in 19 patients (65.5%). Fifteen patients (51.7%) were admitted before the 12th hour. Of the 19 patients with STEmI, 11 (57.9%) had arrived in hospital before the 12th hour and 10 patients had streptokinase thrombolysis. No patients received primary PCI. Ten patients (34.5%) had heart failure and five patients (17.2%) died during hospitalization. Conclusion: delayed consultation and high mortality characterize ACS in Thies. Prevention must be the rule.

Progression of minimally Invasive Urological Surgery at Hôpital Général Idrissa Pouye in dakar in 20 Years of Practice

Introduction: Open surgery is gradually being supplanted by minimally invasive surgical techniques worldwide. Our study aimed to describe the place of minimally invasive surgery at the Hôpital Général Idrissa Pouye (HOGIP) in dakar. materials and methods: This is a descriptive cross-sectional study over a 20-years period from 1st June 2000 to 31st december 2021 in the urology department of HOGIP in dakar. The list of all surgical procedures performed was computed. We evaluated the distribution of minimally invasive procedures (endoscopy, PCNL, laparoscopy) overall and over the years. We made calculations of proportions and statistical significance was considered for alpha = 0.05. Results: In 20 years, 14,855 surgical procedures were performed, of which 5344 (36%) were minimally invasive surgeries. The average age of men was 53.19 years (standard deviation: 21.77) vs 47.32 years (standard deviation: 18.43) for women. minimally invasive procedures in the lower urinary tract accounted for 71.93% and involved 1033 cystoscopies (26.87%), 1020 Trans Urethral Resection of Prostate (TURP) (26.53%), 931 direct Vision Internal Urethrotomy (dVIU) (24.21%, and 612 Trans Urethral Resection of the Bladder (TURB) (15.92%). In the upper urinary tract, 1461 (28.07%) minimally invasive procedures were performed, including PCNL in 193 cases (3.61%), laser endopyelotomy in 104 cases (1.95%), ureteroscopy in 486 cases (1.7%) and laparoscopy in 39 cases (0.46%). The proportion of minimally invasive surgery has gradually increased. Among the 5344 minimally invasive procedures, 333 (5.23%) were performed in 2000-2004 and 2332 (43.63%) in 2015-2019. Conclusion: minimally invasive surgery represents an important part of the activity of the HOGIP urology department. Its development has improved over the years offering its advantages to the patients.

This study aims to evaluate dust impact on climate parameters over the Sahel region by RegCm3 regional model during 2006. Indeed, aerosols are one of the main uncertainties in climate models. The aerosol optical depth (AOd) derived from RegCm3 model has been validated with various observed datasets. The aerosol sources are identified over North Algeria and East of Sahel (Bodele depression). discrepancies are noted when considering dust temporal and spatial distribution. dust season extends between march and October, with two peaks of AOd recorded in march (spring) and June (summer). The dust vertical distribution showed that the mineral aerosol layer is located between 850 hPa and 300 hPa (1.5 km to 7 km). The RegCm3 model simulates fairly well the transport in the upper layers, especially in the Saharan Air Layer (SAL) during the summer. However, RegCm3 simulates poorly the transport and sedimentation of particles in the lower layers (below 2 km). The investigation of dust radiative impact shows a general cooling. The maximum of radiative forcing is located around 18°N - 20°N, with values of about -80 W/m2 in June - August (JJA) and -40 W/m2 at the surface during march - may (mAm). This study also showed the indirect effect of dust with a decrease in precipitation about -0.7 mm/day around 15 - 20°N during the rainy season.

Evaluation of the Performances of the First Grid-Connected Photovoltaic System in Sénégal

This document presents the evaluation and the monitoring of the performances of the first grid-connected photovoltaic system installed in the Center of Studies and Researches on the Renewable Energies (CERER) inaugurated on december 4th, 2012 by the governmental authorities of Senegal and Tenerife. This mini power plant of 3.15 kWc is a perfect example of the political will of the government which is to reduce the production cost of the electricity, with the diversification of the sources of production, and the greater use of the other sources such as the natural gas, the coal, the renewable energies. The evaluation of the performances of the installation is realized by using the indicators of efficiency and performance as the photovoltaic surface yield, the ratio of photovoltaic performance, the photovoltaic specific yield, and the losses of captures. The obtained results show that a big part of the energy shone during the period of observation was not able to be used further to circumstances such as the losses of conductivity, the heat losses or for example the defects on components. The analysis also shows that a large part of the produced energy is not injected because of the dilapidation of the network, the defects of landing but especially one disjunction sees frequently at the level of the point of injection.
