直流电场作用下绝缘聚合物老化演变的仿真研究,对促进计算高电压工程学的发展有重要的理论及工程价值。为此使用mATLAB自定义编程,在二维坐标中建立有限差分老化数值模型;用绝缘薄膜的老化寿命数据所拟合的小尺寸结构单元的通用参数,表征大尺寸绝缘样品的材料不均匀性;并基于化学反应速率理论,量化计算绝缘老化过程。通过仿真发现,绝缘聚合物内部的薄弱点率先发生微小劣化,导致局部电场畸变;增强的局部电场进一步加速局部区域的老化进程,引起劣化区域的扩大与延伸;最终当局部场强达到临界值时,绝缘内部发生快速的击穿过程,并形成贯穿上下电极的细丝状导电路径,在靠近极板时由于局部电场分散性而出现分形的失效结构。该研究提出的数值仿真模型可高效、直观地反映绝缘聚合物老化过程中,从随机性局部劣化演变到确定性宏观失效的物理规律,为深入理解绝缘老化失效提供了理论参考和依据。

BACKGROUND Pancreatic acinar cell carcinoma(PACC)is a rare *** to 45%of PACCs have alterations in the DNA damage repair pathway and 23%harbor rearrangements in the BRAF or RAF1 *** present a PACC case with a germline BRCA2 likely pathogenic variant(LPV)to highlight the impact of genomic testing on treatment decisions and patient *** our larger case series,we provide clinic-based information on additional 10 PACC patients treated in our *** SUmmARY A 70-year-old male was diagnosed with advanced *** presentation,he was cachectic with severe arthralgia despite prednisolone and a skin rash that was later confirmed to be *** was treated with modified FOLFIRINOX(mFFX)with the knowledge of the germline BRCA2 *** 11 cycles of mFFX,a computed tomography(CT)scan demonstrated significant tumor response in the pancreatic primary and hepatic metastases,totaling 70%from baseline as per Response Evaluation Criteria in Solid *** of the skin panniculitis was also *** identified two additional PACCs with druggable targets in our case *** data contribute to practical evidence for the value of germline and somatic profiling in the management of rare diseases like *** This patient and others in our larger case series highlight the importance of genomic testing in PACC with potential utility in personalized treatment.

提出并设计了一种新型的低温两相中微子探测器,它利用电子泡在液氦池中特殊的传输特性,实时、高效地测量了来自太阳质子-质子反应产生的低能量中微子.该电子泡探测器的工作原理类似于时间投影室,当入射中微子进入到探测介质液氦池中后,与氦原子发生作用,会激发弹性散射电子,通过测量这些散射电子的能量及轨迹并与放射性背景信号分开,就可以反推出入射中微子的能量和其他性质.由于散射电子的信号很弱,因此使用位于液面上方饱和蒸汽区的气体电子倍增器放大电子信号.这种技术的突出优点是具有极高的空间分辨率和很好地抑制电离信号反馈的功能.基于气体电子倍增器读取电信号和高精度CCD相机以进行探测光信号的新型时间投影室的研究,目的是建造一个三维的空间分辨率为几个毫米量级的大型液氦低温探测器,以探测能量低至100～200 keV的太阳中微子.

＜正＞ GPRC6A is an "orphan". G protein-coupled receptor （related to the calcium sensing receptor） which is activated by basic amino acids. Al（3 + ） and the cal cimimetic NPS-568 （see Pi et al., 2005, J Biol Chem 280:40201-9）. The aim of this study was to investigate the possible presence of GPRC6A in rat mesenteric artery （RmA）. In sharp micro-electrode recordings. L-omithine （0.3 mm） produced an endothelium-dependent hyperpolarisation and potentiated the hyperpolarisation to the calcimimetic calindol （100. 300 nm）. The effects of both L-or-nithine and calindol were abolished by the IKCa channel inhibitor TRAm-34 （0.01 mm）. Similar effects （TRAm-34 sensitive hyperpolarisation and potentia-tion of calindol effect） were produced by Al（3+ ）（0.1 mm）. RT-PCR using mRNA extracted from RmA produced an amplicon of the predicted size, which was sequenced and confirmed as GPRC6A. Furthermore, the protein was also i dentified by Western blot using a selective polyclonal antibody. We conclude that GPRC6A is present in RmA endothelial cells and may play a role in the regulation of vascular tone. Funded by the British Heart Foundation

Background: Pathological gambling is a rare potential complication related to treatment of Parkinson disease (PD). However, the etiology of this behavior is poorly understood. Objective: To examine the relationship between medical therapy for PD and pathological gambling. methods: In our routine movement disorders practice (2002-2004), we encountered 11 patients with idiopathic PD who had recently developed pathological gambling. We assessed the relationship to their medical therapy and compared them with cases identified by systematic review of the existing literature on pathological gambling and PD. Results: All 11 patients with PD and pathological gambling were taking therapeutic doses of a dopamine agonist; 3 of these patients were not treated with levodopa. In 7 patients, pathological gambling developed within 3 months of starting to take or escalating the dose of the agonist; in the other 4 with a longer latency, gambling resolved after the agonist use was discontinued. Pramipexole dihydrochloride was the agonist in 9 of 11 cases in our series and 10 of 17 in the literature (68%in total). Conclusions: Dopamine agonist therapy was associated with potentially reversible pathological gambling, and pramipexole was the medication predominantly implicated. This may relate to disproportionate stimulation of dopamine D3 receptors, which are primarily localized to the limbic system.

背景在美国,安全有效的人乳头瘤病毒(HPV)疫苗未得到充分利用,人们不断忽视预防癌症的机会。美国国家级指南建议在13岁前(暴露于性传播病毒之前)完成2~3剂HPV疫苗系列的注射。全面且精确地给出初级卫生保健中HPV疫苗实施情况的阻碍和促进因素,有利于有效实施策略的制定。方法采用混合方法和落实研究融合框架(CFIR)系统地调查了10个初级卫生保健诊所(16名护理人员)中影响HPV疫苗使用的因素。与护理人员进行面对面半结构式访谈,使用CFIR指导收集和分析定性数据。通过从病历中提取的数据对HPV疫苗的使用情况进行分析。通过整合定性和定量数据,识别影响HPV疫苗使用的因素构建体。结果评估72个CFIR构建体,HPV疫苗覆盖率较高和较低的护理人员共给出7个强识别模式和7个弱识别模式。大多数强识别模式的构建体是促进因素,并且与护理人员的特征(知识和信仰、自我效能、变革动能)、其对干预的认知(青少年接种疫苗时,年轻或年长的相对优势)、护理人员注射疫苗的过程(执行能力)相关。弱识别模式的构建体来自外部环境(同伴压力、财务激励)、内部环境(落实过程中的组织网络、沟通、准备工作)、实施过程(规划、参与、反思、评估)。两个强识别模式的构建体是阻碍因素,一个来自干预(注射疫苗起始年龄的适应性),另一个来自外部环境(患者需求和财力)。结论在私立初级保健诊所中,通过CFIR系统地调查HPV疫苗的使用情况,能够分别给出护理人员、人际关系、实践操作层面的阻碍和促进因素,以便确定未来的努力方向,增加HPV疫苗的使用。研究结果表明,针对护理人员并帮助其解决多层次的HPV疫苗使用障碍的实施策略值得进一步调查。

metal-on-metal(mom) hip replacements have proven to be a modern day orthopaedic failure. The early enthusiasm and promise of a hard, durable bearing was quickly quashed following the unanticipated wear rates. The release of metal ions into the blood stream has been shown to lead to surrounding soft tissue complications and early failure. The devastating destruction caused has led to a large number of revision procedures and implant extractions. The resulting research into this field has led to a new area of interest; that of the wear at the trunnion of the prosthesis. It had been previously thought that the metal debris was generated solely from the weight bearing articulation, however with the evolution of modularity to aid surgical options, wear at the trunnion is becoming more apparent. The phenomenon of "trunnionosis" is a rapidly developing area of interest that may contribute to the overall effect of metallosis in mo m replacements but may also lead to the release of metal ions in non mo m hip designs. The aim of this paper is to introduce, explain and summarise the evidence so far in the field of trunnionosis. The evidence for this phenomenon, the type of debris particles generated and a contrast between mo m, non mo m and resurfacing procedures are also presented.
