Introduction: Valvular heart diseases, often linked to rheumatic heart disease, represent a major cause of cardiovascular morbidity in sub-Saharan Africa. This study reports the preliminary outcomes of surgical treatment for valvular heart diseases initiated at the Yaoundé General Hospital (YGH) by an entirely local team. materials and methods: Between September 2022 and November 2024, 37 patients underwent surgery for valvular heart diseases at YGH. A retrospective analysis of clinical, operative, and postoperative data was conducted using R software (version 4.4.0). Results: male patients predominated, with a sex ratio of 1.17. The median age at the time of surgery was 40 years (interquartile range [IQR]: 25.0 - 51.0). Dyspnea was the main symptom at admission, reported in 36 patients (97.3%). Rheumatic etiologies were the most frequent (25 cases, 67.57%), followed by atherosclerotic lesions (6 cases, 16.2%) and degenerative lesions (4 cases, 10.83%). Among the 37 patients, 19 (51.4%) underwent mitral valve replacement, 11 (29.7%) aortic valve replacement, 6 (16.2%) double mitral-aortic valve replacement, and 1 (2.7%) tricuspid valve replacement. mechanical prostheses were implanted in 27 patients (73%), while 10 patients (27%) received biological prostheses. The median duration of cardiopulmonary bypass and aortic clamping was 101 minutes (IQR: 84.75 - 146.25) and 73 minutes (IQR: 55.75 - 116.25), respectively. The median duration of mechanical ventilation and ICU stay were 2 hours (IQR: 2 - 3) and 3 days (IQR: 2 - 4), respectively. The most frequent complication was postoperative anemia requiring blood transfusion in 16 patients (43.2%). Three patients (8.1%) required reoperation for bleeding. There was no in-hospital mortality. Conclusion: Valvular heart diseases treated at YGH are predominantly of rheumatic origin. Valve replacement was by far the most commonly used technique. Early outcomes are satisfactory and encouraging.

This paper studies the problem of locating breakdown mechanic. We consider a public transport network in which it can provide buses failure. The objective is, taking into account the statistics of breakdowns registered on the network, to locate optimally breakdown mechanics so as to minimize the response time (to ensure the network coverage of break- down mechanics). In this work, we present a binary linear programming model for this location problem which provides assignments-locations of areas served. Once the location made, we discuss dynamic assignment of breakdown mechan- ics depending on their position in the network at a given time t. Numerical simulation results are presented based on real data of urban transportation society of Dakar Dem Dikk.

Optimizing Dead mileage in Urban Bus Routes. Dakar Dem Dikk Case Study

This paper studies the buses assignment from their depots to their routes starting points in urban transportation network. It describes a computational study to solve the dead mileage minimization to optimality. The objective of this work is to assign the buses to depots while optimizing dead mileage associated with pull-out trips and pull-in trips. To do so, a new mixed-integer programming model with 0 - 1 variables is proposed which takes into account the specificity of the buses of Dakar Dem Dikk (the main public transportation company in Dakar). This company manages a fleet of buses which, depending on road conditions some buses cannot circulate on some roads of the network. Thus, buses are classified into two categories and are assigned based on these categories. The related mixed-integer 0 - 1 linear program is solved efficiently to minimize the cumulative distance covered by all buses. Numerical simulations on real datasets are presented.

Recycling Tailings Seepage Water for Diogo Heavy minerals mine Sustainability (Northern Senegal)

The sandy Quaternary and the deep maastrichtian aquifers located in the northern coastal zone of Senegal, from the locality of Kayar in the south to Saint-Louis in the north, constitute the main sources of water supply for urban and local needs as well as mining activities. The Quaternary aquifer that provides the water required for the irrigation of local farmlands, hosts a significant heavy mineral sands deposit currently being mined by the Grande Cote Operations (GCO). As a result of variable rainfall and increased water abstraction, this shallow aquifer has recorded a continuous water level decline since 1970, with potential negative effects on both the social and economic development of the region. The mining of heavy minerals (zircon, ilmenite, leucoxene and rutile) at GCO is realised through conventional dredging techniques that require large volumes of water (up to 60,000 m3/d). The water pumped by the dredge to enable the extraction of the heavy minerals, infiltrates into the shallow aquifer, runs-off into the dredge pond or evaporates. The objective of this study is to evaluate a water balance that enables the provision of a permanent water supply to the dredge pond, whilst minimising the risk of flooding of the cropping depressions adjacent to the mine site or drying out of the farming wells. The hydrodynamic model implemented for this purpose was calibrated and tested during the first year of operation. The Root mean Squared Error (RmSE) obtained for the calibration is approximately 0.52 m. The predictions indicate a requirement for the system to recover part of the tailings infiltration through dewatering boreholes. The quantity of recycled water is estimated at 16,000 m3/d on average. The model simulations show an additional water requirement, extracted from the deep maastrichtian aquifer, varying between 23,000 and 28,000 m3/d to achieve the optimum pond water level.

We have fabricated m doped (m = Al, Co, Fe, Ga, Ni and Zn) PrBa2Cu3O7 (PBCO), i.e. PrBa2(Cu1-xmx)3O7. The doping levels x are 0.05, 0.10, 0.15, and 0.20. X-ray data indicated no significant second phase for substituting Cu by Al, Co, Fe and Ga up to 20%. However impurity phases were detected for Ni and Zn substituted samples with doping levels equal to and higher than 15%. At 77 K the electrical resistivity of these compounds is orders in magnitude higher than that of PBCO. We also found that although the lattice parameters in the doped samples differ from PBCO, all samples remain orthorhombic. The lattice parameters of the doped sample are very close to those of YBa2Cu3O7-δ (YBCO) and PBCO. For this reason these compounds are better materials to be used as the I-layer for YBCO SIS junctions. Results of structural and transport studies on 2000 thick PrBa2[Cu0.80G0.2]3O7 (PBCGO) and YBCO/PBCGO multilayers are presented in this paper.

Prevalence and Associated Factors of Diabetic Kidney Disease in Senegalese Patients: A Cross-Sectional Study in Saint-Louis

Introduction: Diabetes is a leading cause of chronic kidney disease in the world. During the next decade, its burden is expected to increase in Africa with potential complications such as chronic kidney disease. However, epidemiology and risk factors of diabetic kidney disease are poorly described at population level. This study aimed to determine prevalence of diabetic kidney disease (DKD) in adult diabetics living in Saint-Louis, northern Senegal. methods: A cross-sectional study including diabetic patients followed-up aged ≥18 years during a five-year period (2013-2018) in Saint-Louis. Clinical and biological parameters were collected during annual community-based mass screening. Diabetes was defined as fasting blood glucose ≥ 1.26 g/L confirmed by a second lab dosage. DKD was defined as persistence of albuminuria ≥ 30 mg/24h and/or estimated glomerular filtration rate (eGFR) 2. Data were analyzed with Stata 12.0. Results: We included a total of 1310 diabetic patients among whom 3.7% (95% CI = 1.4% - 9.8%) presented DKD. Their mean age was 46.2 ± 11.8 years and sex-ratio was 0.7. micro-albuminuria and macro-albuminuria were present respectively in 59.2% and 18.4% of patients with DKD and half of them had a normal eGFR. Before the survey 89.8% of patients with DKD were not aware of their renal disease and only four of them had seen a nephrologist. After multivariate analysis, age (OR = 1.5;95% CI = 1.1 - 3.4), duration of diabetes (OR = 1.2;95% CI = 1.6 - 4.4) and hypertension (OR = 2.5;95% CI = 1.4 - 4.6) were associated with the presence of DKD in diabetic patients while no significant association was not found with gender, blood glucose level, smoking and familial history. Conclusion: DKD is a frequent complication in diabetic adult population living in Saint-Louis. Early detection and management should be promoted in order to prevent progression to end-stage renal disease.

Humoral Response and Tolerance of Vaccination against SARS-CoV-2 in Adults Senegalese Patients Undergoing Hemodialysis: A multicenter Prospective Study

Introduction: Following the COVID-19 pandemic, vaccination has been proposed in several countries as the main preventive measure despite very limited data, particularly in dialysis patients. We conducted this study to assess the immunological response to vaccination in Senegalese hemodialysis patients. Patients and methods: We conducted a prospective study, in two dialysis centers in Dakar from march 30th to August 30th, 2021 including patients on hemodialysis for >6 months, vaccinated against SARS-CoV-2 according to the vaccination schedule recommended by WHO. A vaccine response was considered positive when seroconversion was observed after one dose of vaccine. The clinical efficacy of immunization was defined as the absence of new COVID-19 infection in patients who received a complete vaccination. Results: Among the 81 patients included in the study, 7.4% had anti-Spike Igm antibodies before their first vaccination. Seroprevalence of Igm antibodies was 38.3% one month after the first vaccine dose (at m1) and 8.6% one month after the second dose (at m4). Anti-Spike IgG antibodies were present in 40.3% of patients before vaccination, in 90.1% at m1, and in 59.7% at m4. Among patients previously infected with SARS-CoV-2, 10.2% had Igm antibodies at m0, 31.6% at m1, and 10.5% at m4 post-vaccination. Similarly, seroprevalences of IgG antibodies in this subgroup were 31.5%, 61.3%, and 50.0% respectively at m0, m1, and m4 post-vaccination. A comparison of seroconversion rates between m0 and m4 showed significant differences only for IgG in COVID-19 naive patients. mean duration in dialysis and the existence of previous COVID-19 infection were associated with patients’ vaccinal response after the two doses. Age, gender and the use of immunosuppressive treatment did not influence post-vaccinal antibody production. Conclusion: Vaccination against COVID-19 in Senegalese hemodialysis patients induced a low seroconversion rate but it was well tolerated. m

The purpose of this paper is to describe the business process modeling and simulation of a textile supply chain using Colored Petri nets. Our model takes into account both the source process and delivery logistics that exist between any two members of supply chain;moreover, we model other activities performed by the company such as manufacturing clothing. The model has been built to acquire a better understanding about the behavior of a textile company in the fulfillment of requests from costumers. The model has been built using CPN Tools. This model was built with modules for activities of the supply chain textile, e.g. receiving orders of customers, determination of production plan, procurement raw material, transportation of raw material, production and delivering products to customers. This modularized model offers some advantages to represent complex supply chains according to their structure and requirements. Thus, we can add modules easily depending on the necessary activities.

Body Composition in Sickle Cell Disease Patients in the Steady State

Introduction: Previous studies in sickle cell disease (SCD) patients have demonstrated low muscle mass (mm) and high fat mass (Fm) percentages compared to individuals with normal hemoglobin. The relationship between the evolutions of body composition (BC) parameters in the active SCD population is not well elucidated. The aim of the present study was to assess the body composition in active adult SCD patients in sub-Saharan Africa during the inter-critical period. methodology: We conducted a descriptive and analytical cross-sectional study over an 8-month period. Eighteen SCD patients (sex ratio = 1) reported regular physical activity (RPA) corresponding to level 3 (RPA of moderate intensity) of the 4-level Saltin-Grimby physical activity scale were recruited. Anthropometric and BC parameters such as BmI, percentage Fm and percentage mm were evaluated using an impedance meter. Results: The study population had a mean age of 26 ± 7 years and a mean BmI of 20.5 ± 1.9 kg/m2. We found a significantly higher percentage of Fm in women compared to the men (25.6% ± 10.2% vs 11.1% ± 2.1%;p = 0.001). For the proportion of mm, all the women had a normal percentage. The men mostly had high levels of mm compared with reference norms. men had significantly greater mm percentages than women (45.4% ± 1.8% vs. 26.5% ± 1.4%;p Conclusion: RPA appears to have positive effects on BC in active population with SCD, and a greater-than-expected effect was observed on mm in male patients. Promoting regular, adapted physical activity would be a promising way of improving BC parameters and quality of life in the SCD population. Further studies with larger cohorts will be needed to better substantiate these results.

Baseline of the Use of Solar Irrigation Pump in the Niayes Area in Senegal

Solar irrigation pumps are considered as an alternative to the use of diesel and electric pumps due to the high cost of energy. These pumps are now increasingly used in the Niayes area. Thus, a more in-depth knowledge about the components of a solar pumping system and their characteristics available on the market as well as prices can be beneficial. This study was conducted to have an idea on the baseline of the use of solar irrigation pumps in this area. To perform this study, surveys were conducted among 12 suppliers and 10 service providers located in Dakar, Thies, Louga and Saint-Louis region and among 53 users located in Potou area which is in Louga region. Results show a wide range of pump brands and characteristics. Brands found were Lorentz, LIKOU, SHImGE, Grundfos, Solar pump, Feili, Difful, JUQIANG, Solar tech, and Asaman. The pumps’ flow rate varies between 2.5 m3/h and 45 m3/h, the pressure ranges from 15 m to 160 m and the power is between 0.072 kW and 10 kW. The price of these pumps varies depending on the brand used and their characteristics. In addition, other materials are available, such as panels with power ranging from 80 W to 330 W and electric cables. The price of these components varies according on the type used. Thus, the investment cost for implementation varies between 640 euros (420,000 CFA) and 25,087 euros (16,455,919 CFA). The cost of installation varies between 76.3 euros (50,000 CFA) and 1219.6 euros (800,000 CFA). Concerning maintenance, it is generally free during the warranty period when the equipment is supplied and installed by the same company. However, the cost of maintenance varies between 152.5 euros and 457.4 euros/year or is estimated at 45.7 euro/maintenance after the warranty years. Investigations conducted among users show that farmers in general do not perform pump maintenance due to a lack of knowledge and financial means.
