Background:Systemic lupus erythematosus(SLE)is a systemic autoimmune disease characterized by the involvement of multiple organs and autoantibody *** study aimed to determine the association of human leukocyte antigen(HLA)-DQ and-DR alleles with SLE at a tertiary hospital in ***:This case-control study was conducted in the Department of Rheumatology,Bangabandhu Sheikh mujib medical University(BSmmU),Dhaka,*** patients who fulfilled the 1997 revised American College of Rheumatology classification criteria for SLE were considered cases(n=41).The controls(n=32)were selected from the hospital staff of BSmmU,including doctors and nurses who did not suffer from any known rheumatic diseases,and there was no such disease in their first-degree *** chain reaction using a sequence-specific primer(PCRSSP)in a low-resolution typing method was carried out on blood samples collected for HLA-DQ and-DR allele ***:The cases and controls had comparable mean ages(31 vs.32 years),and most cases were female(95.1%vs.34.4%).The most frequent clinical manifestations were mucocutaneous(85.4%),followed by musculoskeletal(70.7%)and constitutional symptoms(58.5%).The cases had a significantly higher frequency of HLA-DR2 allele compared with healthy controls(70.7%vs.43.8%;odds ratio:3.1;95%confidence interval:1.2-8.2).In addition,HLADQ6 allele was found to have a significant association with mucocutaneous manifestations of the cases(88.9%,P=0.03).Conclusions:Bangladeshi adults having HLA-DR2 may be more susceptible to ***-DQ6 allele might be associated with mucocutaneous manifestations of SLE.

One of the most important aspects of Bangladesh’s textile industry is denim. Bangladesh now has a new opportunity thanks to the global demand for denim among fashion industry professionals. Entrepreneurs from Bangladesh provide denim products to well-known international merchants all over the world. The worldwide denim market is predicted to expand by roughly 8% through the year 2020. We must raise the standard of denim if we are to keep up with the expanding industry. In contrast to projectile and rapier systems, air-jet weaving machines nowadays can weave practically all types of yarns without any issues and at higher rates. Due to this, air-jet looms are an excellent substitute for other weft insertion techniques. This kind of device still has one significant flaw, though, and that is the enormous power consumption brought on by the creation of compressed air. Researchers and manufacturers of air-jet looms have therefore worked very hard to find a solution to this issue and achieve a huge reduction in air consumption without compromising loom performance or fabric quality. Therefore, the purpose of this project is to look into ways to decrease air consumption and reduce auxiliary selvedge waste without any decrease in loom performance and fabric quality on existing air-jet weaving looms which reduce the manufacturing costs with process improvement. Just updating the air pressure allowed a weaving mill to reduce air usage by 11 cfm. So, with just almost no cost, a company with 100 looms could save $0.15 m each year, on compressed air. Two new methods for decreasing process costs on air jet looms have also been developed by this project work.

Background: Lupus nephritis (LN) is one of the most common presentations of Systemic lupus erythematosus (SLE). Cyclophosphamide is one of the key immunosuppressive agents for the management of LN. Leflunomide is an isoxazole immunomodulatory agent has been shown to be safe, well tolerated and effective in SLE and LN. Objective: To evaluate the outcome of leflunomide in the treatment of proliferative lupus nephritis compared to cyclophosphamide. method: This randomized clinical trial was held in Bangabandhu Sheikh mujib medical University (BSmmU), Dhaka, Bangladesh from July 2017 to August 2019. A total of 66 patients of proliferative lupus nephritis who need induction therapy were enrolled in this study. Leflunomide 100 mg/day for consecutive 3 days followed by 0.5 mg/kg/day in divided dose was given in experimental group (n = 32) and intravenous cyclophosphamide 0.5 gm/m2 of body surface area monthly pulse was given in control group (n = 34). All study patients have received prednisolone and hydroxychloroquine according to KDIGO guideline then followed up monthly for 6 months. Outcomes were measured at 6th month by renal function [S. Creatinine, 24 hours urinary total protein (24-hr UTP)], changes in SELENA-SLEDAI score, anti-ds DNA level, serum complement levels (serum C3 & C4), remission (complete/partial) and adverse drug responses. Result: In experimental group, remission occurred in 18 (56.3%) patients and no remission in 14 (43.7%) patients. In control group, remission occurred in 24 (70.6%) patients and no remission in 10 (29.4%) patients. Adverse effects in experimental group were: elevated ALT (6.3%), hypertension (12.5%), infection (6.3%) and amenorrhea (12.5%). In control group, adverse effects were mainly leucopenia (5.9%), infection (17.7%) and amenorrhea (29.4%). Intergroup analysis for treatment responses and adverse effects showed no significant difference (p > 0.05). Concl

利用 L’vov 平台和普通基体改进剂NH4H2PO4,有效地清除了用石墨炉原子吸收法分析生物试样时普遍遇到的基体干扰。将改进的 L’vov 平台置于市售 massmann 型的石墨管中,添加磷酸铵的试样溶液从平台上原子化,大大地降低了测定血、肝、尿和 NBS标准样品中 Pb、Cd、Cr 和 Ni 的干扰。在1%硝酸介质条件下,将标准加入法的曲线斜率与标准工作曲线的斜率相比,可估算出基体的影响,将六种不同的 NBS 标准样品从管壁上原子化时,测定 Pb 的斜率比有所改善,达到0.91～1.06。分析十二种不同尿样中的Pb,得到斜率此的平均值为0.97±0.03。这说明,标准加入法曲线的斜率不仪接近于标准工作曲线的斜率,而且对于不同的试样是一致的。因此,减少了对每个样品都加入标准的必要。平台和基体改进剂联用的另一个优点是改善测定精密度。处理过的全血样品,从管壁上原子化测定其中 Pb 时,得到的精密度差,相对标准偏差为13%;采用平台和改进剂时,精密度大大改善,相对标准偏差达0.9%。

Background: Depression is a common and serious medical illness around the world. It occurs more frequently in patients with chronic illness than in the general population. It is a common psychiatric problem in patients with chronic kidney disease (CKD). Objective: To assess the level of depression in pre-dialytic CKD patients attending at a tertiary care hospital in Bangladesh. methodology: This cross sectional study was conducted at Department of Nephrology, Bangabandhu Sheikh mujib medical University (BSmmU), Dhaka, Bangladesh from January 2020 to June 2020. A total of 100 pre-dialytic CKD patients were selected by convenience sampling technique and their level of depression was assessed by Zung Self-Rating Depression Scale (ZSDS). Data were collected by existing questionnaire with face to face interview and analyzed by statistical test. Results: The mean age of the participants was 49.70 ± 11.80 years. Among them 64% were male, 95% were muslim, 98% were married, 65% were completed secondary/above higher secondary level education, 65% were unemployed and 55% were lived in urban area. It was observed that 65% participants were economically dependent to their family, only 6% were engaged in regular exercise, 14% had family history of depression, 8% were smoker and 12% were alcohol/drug abuser. According to the Zung Self-Rating Depression Scale (ZSDS) we found 29% participants were mildly depressed, 39% participants were moderately depressed and 18% participants were severely depressed. Prevalence of depression was 86% in pre-dialytic CKD patients. Of them, 7% was in CKD stage-I, 17.4% was in CKD stage-II, 19.8% was in CKD stage-III, 22.1% was in CKD stage-IV and 33.7% was in CKD stage-V. Conclusion: Depression is highly prevalent in pre-dialytic CKD patients and more frequent in the advanced stages of CKD.

Impact of technology on indications and limitations for transanal surgical removal of rectal neoplasms

Transanal surgery has and continues to be well accepted for local excision of benign rectal disease not amenable to endoscopic resection. more recently, there has been increasing interest in applying transanal surgery to local resection of early malignant disease. In addition, some groups have started utilizing a transanal route in order to accomplish total mesorectal excision(TmE) for more advanced rectal malignancies. We aim to review the role of various transanal and endoscopic techniquesin the local resection of benign and malignant rectal disease based on published trial data. Preliminary data on the use of transanal platforms to accomplish TmE will also be highlighted. For endoscopically unresectable rectal adenomas, transanal surgery remains a widely accepted method with minimal morbidity that avoids the downsides of a major abdomino-pelvic operation. Transanal endoscopic microsurgery and transanal minimally invasive surgery offer improved visualization and magnification, allowing for finer and more precise dissection of more proximal and larger rectal lesions without compromising patient outcome. Some studies have demonstrated efficacy in utilizing transanal platforms in the surgical management of early rectal malignancies in selected patients. There is an overall higher recurrence rate with transanal surgery with the concern that neither chemoradiation nor salvage surgery may compensate for previous approach and correct the inferior outcome. Application of transanal platforms to accomplish transanal TmE in a natural orifice fashion are still in their infancy and currently should be considered experimental. The current data demonstrate that transanal surgery remains an excellent option in the surgical management of benign rectal disease. However, care should be used when selecting patients with malignant disease. The application of transanal platforms continues to evolve.
