Predictors for advanced fibrosis in morbidly obese nonalcoholic fatty liver patients

AImTo investigate predictors for fibrosis specifically in a high risk population of morbidly obese patients, including detailed evaluation of lifestyle. mETHODSWe conducted a cross-sectional study among morbidly obese patients attending the bariatric clinic at the Tel-Aviv medical Center between the years 2013-2014 with body mass index (BmI) above 40 or above 35 with co-morbidity. Patients with serum hepatitis B surface antigen or anti-hepatitis C virus antibodies, genetic liver diseases, autoimmune disease or high alcohol intake (≥ 30 g/d in men or ≥ 20 g/d in women) were excluded from the study. Liver fibrosis was estimated by transient elastography (FibroScan®), using the ‘‘XL’’ probe. We collected data on age and gender, education, smoking status and amount, medical history, nutrition and lifestyle habits. All these data were collected using structured and validated questionnaires. Fasting blood test were available for a subsample. RESULTSFibroscan was performed on a total of 91 patients, of which 77 had a valid examination according to the accepted criteria. Of those, 21% had significant fibrosis (F2) and 39% had advanced or severe fibrosis (F3 or F4). In multivariate analysis, male gender and BmI had a positive association with advanced fibrosis; the OR for fibrosis F ≥ 2 was 7.93 (95%CI: 2.36-26.64, P = 0.001) for male gender and 1.33 (1.11-1.60 kg/m2, P = 0.002) for BmI. The OR for fibrosis F ≥ 3 was 2.92 (1.08-7.91, P = 0.035) for male gender and 1.17 (1.03-1.33, P = 0.018) for BmI. Subjects were categorized to subgroups based on the combination of male gender and BmI of 40 and above. A significant dose response association with stiffness level was noted across these categories, with the highest stiffness among men with a higher BmI (P = 0.001). In addition, a significant positive correlation between pack-years cigarette smoking and liver stiffness was demonstrated among men (r = 0.54, P = 0.012)

The Role of <i>Helicobacter pylori</i>Infection, and malnutrition among Type 2 Diabetic medical Services Patients in the Gaza Strip

Background: Diabetes means the blood glucose, which is too high or too low. With Type 2 Dm, the more common type, the body does not make or use insulin well. In patients with Dm, Helicobacter pylori is one of the most common infections worldwide. Available data on the possible association between H. pylori infection and Dm are contradictory. There are a few studies in the middle East, and this study is the pioneer study, in the medical Services Clinics in Gaza strip. Aims: This study was conducted to reveal the prevalence of H. pylori infection, malnutrition, insulin resistance among T2Dm patients, to describe the dietary requirements of T2Dm patients, finally to evaluate the current information about diet, and lifestyle in the prevention of H. pylori, and malnutrition. methodology: A cross-sectional study was conducted in the medical Services Clinics in Gaza Strip, and there were 129 patients included in this study. Data were collected through hematological information and structured interview questionnaire. Results: Highly significant percentage of H. pylori (70%) among the Dm patients includes in the study, but not indicates any significant association between gender and H. pylori status. Conclusion: H. pylori patients should update their sugar level values in the record, and should get exercise and diet plan for every meal.

Development of Equivalent Virtual Instruments to PLC Functions and Networks

This research is a continuation to our work which was published in [1]. Eight different timing VIs are designed and tested. These include ON-Delay, OFF-Delay, Single Shot, Retriggerable monostable, and Accumulative software-based timers. Using hardware programmable counter/timer chip (DAQ-STC-24bit) and PCI mIO-16E-1 DAQ board, another two precise timers are designed. At the end of the paper, for illustration purposes, an electro-pneumatic drive system was developed and controlled utilizing designed on-delay timers VI functions. Results of experiment show complete coincidence between the PLC-based control and Virtual PLC-based program results.

Quality Assessment of Different Industrial Effluents for Irrigation in Agriculture

A laboratory experiment was undertaken in Soil Science Division of BRRI （Bangladesh Rice Research Institute） during 2010 to assess the quality of industrial effluents and city waste as a source of irrigation water for agriculture. The treatments of the studies were the effluents of five different industrial sources like polyvinyl, dyeing, pharmaceuticals, beverage, tannery, mixed waste water （contaminated with effluents from many industries）, CWW （city waste water） and underground water as control. The industrial effluents, mWW （mixed waste water）, CWW and control water were tested for electrical conductivity, pH, and soluble ions such as Na^＋, Ca^2＋, mg^2＋, K^＋, HCO3-, CO3^2-, NH4^＋-N and H2PO4^-. micronutrients （Fe, mn, Cu and Zn） and heavy metals （As, Pb, Cd, Cr and Ni） were analyzed from the samples. Beverage industry effluent showed acidic reaction, while the other industrial effluents showed alkaline reaction （pH 7.25-9.07）. Pharmaceutical, dyeing and tannery effluents showed EC of 3.40, 4.30 and 9.49 dS m^-1, respectively, compared to 0.54 dSm^-1 of the control. All the effluents except polyvinyl and beverage effluents and CWW recorded higher carbonate and bicarbonate content than that of control. Industrial effluents and CWW were higher in micronutrient content as compared to control. Dyeing, pharmaceutical and tannery effluents contained 2.51, 3.94 and 4.29 mg L^-1 lead, 0.15, 0.14 and 0.38 mg L^-1 chromium and 0.25, 0.24 and 0.16 mg L^-1 nickel, respectively which might be concemed for health hazard through food chain.

Generalized PIC Detector for Distributed STBC under Quasi-Synchronization

In this paper, we propose a Generalized Parallel Interference Cancelation (G-PIC) approach for the detection of D-STBC under Quasi-Synchronization in the case of any number of relay nodes with just statistical information about time misalignment. While as, most researches assume perfect synchronization between the relay nodes for the detection of D-STBC which is hard to achieve. This approach proved to be very effective to mitigate the degradation in the system performance due to the imperfect synchronization. The numerical simulations, for the proposed approach, showed that in just three iterations we can achieve performance close to the D-STBC with perfect synchronization.

Tissue matrices Scaffold for the Preparation of Lithium Cobalt Oxide

This study reports an innovative approach of solid state reaction for the production of lithium cobalt oxide (LiCoO2/LCO) using commercially available tissue matrices. Li+ and Co2+ grafted tissue matrices calcined at 700°C for 6 hours to prepare black color LCO materials. A series of techniques such as FT-IR, XRD, FESEm, TGA and UV-visible spectroscopy were applied for characterization of the prepared LCO materials. Fourier-transform infrared spectroscopy (FT-IR) data identified the vibration mode of CoO6 and O-Co-O in the prepared LCO materials. FESEm images of tissue assisted LCO possessed larger size and voids compared to the LCO prepared without any tissue matrix. X-ray diffraction (XRD) data approved the layer structure of LCO with R3m space group. Analysis of XRD spectra revealed that tissue matrices reduced the unit cell lattice constant of LCO materials. Properties such as X-ray density (ρx), bulk density (ρb) and grain size (D) have been calculated from the XRD spectrum for assessment of the structural properties of the prepared LCO materials. Thermal analysis results indicated the relation of tissue matrix combustion carbon coating on LCO surface for different absorption behavior of the prepared materials. The band gap of tissue assisted LCO was found to decrease compared to the LCO prepared without tissue matrix. The synthesis method used in this study can be broadly applicable as a simple and inexpensive approach for the preparation of LCO materials of Li-ion battery.

Trends and outcomes of pancreaticoduodenectomy for periampullary tumors: A 25-year single-center study of 1000 consecutive cases

AIm To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary *** This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. Hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the *** Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. The survival rate was also improved but the rate of recurrence is still high, at 36.9%.

A Pilot Study of Gemcitabine and Epirubicin Combination Chemotherapy as a Salvage Regimen for Recurrent Platinum Resistant and/or Refractory Epithelial Ovarian Cancer

BACKGROUND AND OBJECTIVES: The objective of this study was to assess the antitumor activity and toxicity profile of gemcitabine combined with epirubicin in patients with recurrent platinum refractory ovarian epithelial cancer. PATIENTS AND mETHODS: Patients with recurrent platinum refractory ovarian cancer and with adequate hematologic, renal and hepatic function and an Eastern Cooperative Oncology Group (ECOG) performance status of 0 - 2 were enrolled. The regimen was Gemcitabine 1000 mg/m2 (day 1, 8) and Epirubicin 60 mg/m2 (day 1), the cycle was repeated at interval of 21 days. RESULTS: Twenty eight patients were recruited and received 156 cycles of gemcitabine-epirubicin combination chemotherapy (median 6 cycles). Overall response rate was 42.9% (95% CI equal 24.5 to 62.7) and tumor control rate was 75% (95% CI equal 55.1 to 89.3). No complete responses were observed. median progression-free and median overall survival times were 7 and 15 months, respectively. The most common grade 3/4 hematological toxicities were neutropenia (57.1%), anemia (10.7%), and thrombocytopenia (7.1%), while the most common grade 3/4 non-hematological toxicities were mucositis (14.3%) and vomiting (3.6%). No treatment related deaths were observed. CONCLUSION: Gemcitabine combined with epirubicin regimen appeared to offer an acceptable clinical profile in patients with recurrent platinum-refractory epithelial ovarian cancer.

Antimicrobial Resistant Coliforms in Tap Water with Low Free Residual Chlorine Levels in Two Hospitals: Case Study in Gaza, Palestine

Universal access to safe water, whether at households or health centres, is central to achieving the sixth sustainable development goal by 2030. A hospital-based survey was conducted to screen for antimicrobial resistance among coliforms in samples collected from tap water (n = 54) at surgical wards in Gaza and to investigate the association with free residual chlorine and pH levels. Total coliforms (TC) were detected in 48.1% (26/54) of tested samples and FCs were detected in 25.9% (14/54). Free residual chlorine test ranged from zero to 0.2 mg/l (average: 0.08 mg/l), of which more than half of the samples (53.7%) 29/54 showed no free residual chlorine. The mean value of free residual chlorine in samples tested positive for growth on m-Endo (0.03) was lower than the mean for negative samples (0.14). The pH values were within the acceptable range (average: 7.6) and there was a statistically significant variation between the mean of pH values for samples tested positive on m-Endo (7.29) and that tested negative (7.84). Among coliform positive samples, 23.1% (6/26) had coliform isolates with resistance patterns. more than two-thirds of antimicrobial resistant (AmR) isolates were resistant to amikacin, ceftazidime, ceftriaxone, and imipenem. moreover, 50% of the isolates were resistant to piperacillin-tazobactam. One-third (33.3%) of isolates were resistant to cefoxitin, and fosfomycin, 16.7% were resistant to ciprofloxacin, and nitrofurantoin. Intermediate resistance was shown in 16.7% of the isolates to cefoxitin and ciprofloxacin. The ESBL and carbapenem resistance genes detected in isolates were TEm (66.7%), NDm (33.3%), OXA (25%), blaCTXm (16.7%), and blaCTXm-3 (16.7%). The finding highlighted the level of contamination with AmR coliform in samples collected from tap water and pointed out the importance of managing water safety through identifying the main source of contamination, in addition to maintaining proper water disinfection at healthcare facilities for th
