Background schistosomiasis is highly endemic in sub-saharan Africa and frequently imported to Europe. male urogenital manifestations are often neglected. We aimed to ascertain the prevalence of genitourinary clinical signs and symptoms among long-term African migrants in a non-endemic European country using a serology *** We carried out a prospective, community-based cross-sectional study of adult male migrants from sub-saharan Africa living in *** serology tests and microscopic urine examinations were carried out, and clinical data were obtained from an electronic medical record search and a structured *** We included 388 adult males, mean age 43.5 years [standard Deviation (sD) = 12.0, range: 18–76]. The median time since migration to the European Union was 17 [Interquartile range (IQR): 11–21] years. The most frequent country of origin was senegal (N = 179, 46.1%). Of the 338, 147 (37.6%) tested positive forschistosoma. Parasite eggs were present in the urine of only 1.3%. Nine genitourinary clinical items were significantly associated with positiveschistosoma serology results: pelvic pain (45.2%;OR = 1.57, 95%CI: 1.0–2.4), pain on ejaculation (14.5%;OR = 1.85, 95%CI: 1.0–3.5), dyspareunia (12.4%;OR = 2.45, 95%CI: 1.2–5.2), erectile dysfunction (9.5%;OR = 3.10, 95%CI: 1.3–7.6), self-reported episodes of infertility (32.1%;OR = 1.69, 95%CI: 1.0–2.8), haematuria (55.2%;OR = 2.37, 95%CI: 1.5–3.6), dysuria (52.1%;OR = 2.01, 95%CI: 1.3–3.1), undiagnosed syndromic sTIs (5.4%), and orchitis (20.7%;OR = 1.81, 95%CI: 1.0–3.1). shifting cultivation effects on creek water quality around Barkal Upazila in Chittagong Hill Tracts,Bangladesh

Abstract： We report the effects of shifting cultivation on water quality in 16 creeks investigated once in 2007 and twice in 2008 in 16 apparently similar small neighboring watersheds, each of 3 to 5 ha, at four locations around Barkal sub-district under Rangamati District of Chittagong Hill Tracts in Bangladesh. Concentrations of 5042- and K＋, and pH in creek water were lower, and NO3-N and Na＋ concentrations were higher in shifting-cultivation land compared to land with either plantation or natu- ral forest or a combination of these cover types. shifting cultivation effects on some water quality parameters were not significant due to change in land cover of the watershed between two sampling periods either through introduction of planted tree species or naturally regenerated vegetation. Conductivity and concentrations of HCO3- PO43-, Ca2-- and mg2＋ in creek water showed no definite trend between shifting cultivation and the other land cover types. At one area near the Forest Range Office of Barkal, creek water pH was 5.8 under land cover with a com- bination of shifting cultivation and plantation. At this area Na~ concentra- tion in shitting-cultivation land ranged from 32.33 to 33.00 mg-L＂ and in vegetated area from 25.00 to 30.50 mg.L-1 in 2007. At another area, Chaliatali Chara, sO42 concentration in a shifting-cultivation watershed ranged from 4.46 to 10.51 mg-L-1, lower than in a vegetated watershed that ranged from 11.69 to 19.98 mg.L-1 in 2007. s042-concentration in this shifting-cultivation area ranged from 1.28 to 1.37 mg.L^-1 and in the vegetated area from 1.37 to 3.50 mg-L^-1 in 2008.

Introduction: The incidence of acute renal failure (ARF) varies between 20% and 40% of cases for COVID-19 patients admitted to the intensive care unit, with very high mortality, but heterogeneous according to the different epidemic waves, probably due to the genetic variant phenomenon of the virus. The aim of this study is to determine the morbidity and mortality of COVID-19 patients admitted with ARF to the intensive care unit of the Grand H?pital Est Francilien (GHEF) according to the waves and variants. methods: Cross-sectional observational study of COVID-19 patients with ARF admitted to the intensive care unit of the GHEF site in meaux covering the period from march 1sup>stsup> 2020 to December, 31sup>stsup> 2021. Per-hospitalisation and outcome data were collected and analysed with sPss version 25.0 software using the Chi-square or Fischer’s exact test or student’s t-test and logistic regression for p Results: A total of 86 patients were included. The mean age was higher (70 ± 8.5) in patients in the fourth wave than in the other waves (p = 0.015), with male predominance in all waves without significant difference. Co-morbidities: hypertension, diabetes, heart disease, dyslipidaemia and arrhythmia complete with fibrillation were present in all waves. The majority of patients were classified as KDIGO 1 for the different waves (1st: 61.9%, 2nd: 86.5%, 3rd: 80%, and 4th: 75%), with the same trend according to variant (alpha: 80%, beta: 75%, delta: 81.3%, omicron: 75%). mortality by the wave was: 1st: 28.5%, 2nd: 37.5%, 3rd: 23% and 4th: 11%) and by variant: alpha: 24.2%, beta: 44.8%, delta: 20.7%, omicron: 10.3%). Importance: This post-marketing surveillance study was conducted to evaluate real-world information about the efficacy and safety of oral Tinefconsup>?sup> tablets (sphaeranthus indicus based) in plaque psoriasis patients. materials and methods: Patients aged at least 18 years and older with clinical diagnosis of plaque psoriasis, were enrolled in this open label, non-comparative, multicenter trial. All eligible subjects received four 700 mg Tinefconsup>?sup> tablets/day for 12 weeks. The primary outcome measure was percent change in Psoriasis Area severity Index (PAsI) score from baseline to week 12. The secondary outcome measures were Physician Global Assessment (PGA), Nail Psoriasis severity Index (NAPsI), Psoriatic Arthritis Evaluation and Gene Expression Profiling and Immunohistochemistry. Results: After completion of Tinefconsup>?sup> treatment at 12 weeks, more than half of subjects (52%) achieved PAsI 50 response;PAsI 75 response was attained in 68 (23%) subjects and PAsI 90 response in 22 (7%) subjects. Five subjects with severe psoriasis achieved PAsI 90 without receiving any concomitant medication. Reduction in severity as assessed by PGA was observed in more than half of patients with moderate disease. Histopathological evaluation revealed that epidermal thickness was considerably reduced in 66% of subjects. The expression of inflammatory marker s100A9 protein was(meaningfully reduced in 60% patients with non-significant reduction of Keratin 10 protein expression. Gene expression analysis showed increase down regulation of sERPINB4;PI3 and KRT16 genes after a 12-week treatment period in subjects with higher PAsI scores. Conclusion: Oral Tinefconsup>?sup> tablets showed good efficacy and had a favorable safety profile in plaque psoriasis patients.

Background: Timely management determines the prognosis of patients admitted for acute coronary syndrome with persistent sT-segment elevation (sTEmI). Coronavirus COVID-19 appeared in China, in Wuhan in December 2019, span>which has spread at a lightning speed across the planet earth, becoming a pandemic. Senegal, as well as countries around the world, has also been affected by this pandemic, which has had a strong impact on all its sectors of activity, particularly the health system. The objective of this study was to identify and evaluate the factors that increase the time required to treat myocardial infarctions, received late at H?pital Aristide le Dantec and to assess the impact of the Covid-19 pandemic on these delays. Results: A total of 25 patients were included: 17 patients in pre-pandemic period and 8 patients in pandemic period. The predominance was male with a sex ratio of 2.5. The mean age was 59.20 years. Hypertension was the leading risk factor with a prevalence of 60% followed by sedentary lifestyle (48%). Typical chest pain was the most frequent clinical symptom found on admission (80%) followed by nausea and vomiting (40%). The first medical contact took place in a hospital in most patients (44%) and was made by a general practitioner (64%). The majority of patients (76%) were referred to the cardiology department. The personal car was the most frequently used means of transportation for our patients (60%). The average distance by car between the place of occurrence and the hospital was 22.6 km with extremes of 3.30 and 36 km. The average travel time from the place of occurrence to the cardiology department was 43 min 30 span style="font-family:Verdana;">s with extremes of 11 and 57 min. Advanced Bladder Cancer in senegal: Epidemiological and Clinical Aspects

Advanced bladder cancer remains particularly frequent in our practice. Aim: To evaluate the proportion of advanced bladder cancer at diagnosis and to describe the characterisitics at diagnosis. materials and methods: We conducted a descriptive and retrospective study assessing 97 cases of advanced bladder cancer over a period of 10 years (January 2002 to January 2012) at the department of Urology of H?pital Principal de Dakar and H?pital Général de Grand Yoff. We included the records of all patients with a pathologic confirmation of locally advanced bladder cancer (T3, T4) and/or a visceral or lymph node metastasis. Results: mean age was 47 years (Range: 25 - 80 years). The cohort comprised 69 men and 28 women with a sex ratio of 2.46. The reasons for referral were a hematuria (60.82%), pelvic mass (19.2%), irritative urinary symptoms (8.2%). Reported medical histories were: urinary schistosomiasis (13 patients), tobaccoo(10 patients), recurrent cystitis (8 patients). Indications of local extention were: inguinal lymph nodes (6 patients), tum.ral hepatomegaly (5 patients), bone pain (15 patients). A cystoscopy was performed in 64.95% of patients in a mean time of 2.5 months. A Trans Urethral Resection of Bladder Tumour (TURBT) was performed in 77 patients with a mean time from referral of 4 months. Pathologic examination showed squamous cell carcinoma (42%), urothelial carcinoma (28%) and adenocarcinoma (9%). Thoraco-abdomino pelvic CT scan showed a loco regional extension in 18 patients, extension to the peri vesical fat in 3 patients and metastasis in 25 patients. Conclusion: Delayed diagnosis of bladder cancer is still common in Africa with a high mortality rate. A better management requires an improvement of the equipment in the hospital with an emphasis on the access to endoscopy allowing for an early diagnosis.
