Value of histomorphometric tumour thickness and smoothelin for conventional m-classification in early oesophageal adenocarcinoma

AIm To test the validity of tumour thickness measurement in distinguishing between the different infiltration depths, especially when the duplication of muscularis mucosae cannot be demarcated clearly. mETHODS We re-evaluated 100 completely embedded Barrett's adenocarcinomas regarding m-classification, maximum tumour thickness, and muscularis mucosae duplication. For validation, smoothelin staining was performed on a subset of cases. RESULTS The m1-, m2-and m3-classified adenocarcinomasshowed a significant lower tumour thickness compared to the m4-and sm1-classified lesions(P moothelin staining determined a clear muscularis mucosae duplication in 64% of the tested samples and enabled the differentiation of the two layers in diffuse and merged splits. CONCLUSION Tumour thickness in early oesophageal adenocarcinoma significantly correlates with the depth of infiltration and demonstrates its worth as an accurate p T classification in non-polypoid lesions. We created a new algorithm, which combines histomorphology with morphometric analyses. It is noteworthy that it facilitates the assessment of mucosal vs submucosal infiltration depth. The smoothelin staining strengthened our results of the tumour thickness evaluation and can be used in cases of doubt.

在卡雷塔斯海峡地区附近一片面积尺寸极小（10 m×10 m）且建筑条例允许的最大高度为9 m的地块,我们要设计一个小尺度的房地产开发项目。采取的方案是设计具有与蒙得维的亚传统房产不一样的空间概念的居住单元。总共有三个单元,每个都被设计为具有一定个性化和私密性的空间。每个单元都有三层,从而具有较好的内部和外部空间的联系。通过双层通高带天窗的内部空间,每个单元都可以从东北面得到充足光线和太阳的照耀。

冠心病患者接受大型非心脏外科手术前12导联心电图的远期预后价值

Background: Knowledge of the prognostic information of preoperative 12- lead electrocardiogram(ECG) recordings in patients with coronary artery disease(CAD) undergoing noncardiac surgery is limited. methods: The prognostic information derived from the preoperative ECGs of 172 CAD patients undergoing major noncardiac surgery was analyzed to determine its predictive value for long- term outcome. Primary end point was all- cause mortality; secondary end point was major adverse cardiac events(mACE) at 2 years. Results: Prevalence of ECG abnormalities was 53% for T- wave alterations; 46% for Q waves; 38% for ST deviations; and, depending on the criterion used, 2% to 19% for left ventricular hypertrophy. During follow- up, 40(23% ) patients died and 31(18% ) had mACE. After adjustment for clinical baseline findings, including current medication with β - blockers, ST depressions(odds ratio [OR] 4.5, 95% confidence interval [CI] 1.9- 10.5) and faster heart rate(HR)(OR 1.6, 95% CI 1.1- 2.4, per 10 beats per minute [bpm] increase) were independent predictors of all- cause mortality. Faster HR(OR 1.7, 95% CI 1.1- 2.6, per 10- bpm increase) was also an independent predictor of mACE. The predictive value of ECG variables did not change after adjustment for occurence of perioperative ischemia. Conclusion: In CAD patients, the preoperative ECG contains important prognostic information and is predictive of long- term outcome independent of clinical findings and perioperative ischemia.

[背景]假设随着年龄的增长,空气污染和肥胖通过致炎特性加速肺功能减退。[目的]先前公布的以人口为基础的SAPALDIA队列研究中,空气质量改善与肺部健康之间是否存在关联。本文探讨肥胖是否会改变这一关联。[方法]使用校正的混合模型分析,评估在10年的随访期间,平均身体质量指数(BmI)和空气动力学直径≤10μm的颗粒物变化(Pm10;ΔPm10)与肺功能减退之间的关联。[结果]共获得4664人的肺功能数据和完整信息。与低或正常平均BmI者相比,高平均BmI者年龄相关的肺功能减退,其FVC(用力肺活量)指标下降得更快,而FEV1/FVC(1秒用力呼气量/FVC)和FEF25-75(在25%～75%肺活量时的用力呼气流量)下降的速度较慢。在低和正常BmI者中,空气质量改善与随着时间推移的FEV1/FVC、FEF25-75、FEF25-75/FVC下降减缓相关,但在超重或肥胖者中无相关性。这种减缓作用在ΔFEF25-75/FVC上最为明显:在低或正常体质量者中,Pm10每下降10μg/m3,ΔFEF25-75/FVC下降分别为30%和22%。[结论]上述研究结果指出并列考虑空气污染暴露和肥胖对健康的影响的重要性。今后需要进一步研究所观察到的相互作用的机制。
