AIm: To evaluate the role of microRNA (miR)-146a, -155 and -122 in the duodenal mucosa of pediatric patients with Crohn’s disease (CD) and the effect of transforming growth factor-β (TGF-β) on these miRs in duodenal epithelial and fibroblast ***: Formalin-fixed, paraffin-embedded biopsies derived from the macroscopically inflamed (CD inflamed: n = 10) and intact (CD intact: n = 10) duodenal mucosa of pediatric CD patients and control children (C: n = 10) were examined. Expression of miR-146a, -155 and -122 was determined by real-time polym.rase-chain reaction (PCR). The expression of the above miRs was investigated in recombinant human TGF-β (1 nmol/L, 24 h) or vehicle treated small intestinal epithelial cells (CCL-241) and primary duodenal fibroblast cells derived from healthy children as ***: Expression of miR-146a was significantly higher in the inflamed duodenal mucosa compared to the intact duodenal mucosa of children with CD (CD inflamed: 3.21 ± 0.50 vs CD intact: 0.62 ± 0.26, P ≤ 0.01) and to the control group (CD inflamed: 3.21 ± 0.50 vs C: 1.00 ± 0.33, P ≤ 0.05). The expression of miR-155 was significantly increased in the inflamed region of the duodenum compared to the control group (CD inflamed: 4.87 ± 1.02 vs Control: 1.00 ± 0.40, P ≤ 0.001). The expression of miR-122 was unchanged in the inflamed or intact mucosa of CD patients compared to controls. TGF-β treatment significantly decreased the expression of miR-155 in small intestinal epithelial cells (TGF-β: 0.7 ± 0.083 vs Control: 1 ± 0.09, P ≤ 0.05) and also the expression of miR-146a (TGF-β: 0.67 ± 0.04 vs Control: 1 ± 0.15, P ≤ 0.01) and miR-155 (TGF-β: 0.72 ± 0.09 vs Control: 1 ± 0.06, P ≤ 0.05) in primary duodenal fibroblasts compared to corresponding vehicle treated control

Evaluation of 5 versus 10 granulocyteaphaeresis sessions in steroid-dependent ulcerative colitis: A pilot, prospective, multicenter, randomized study

AIm： To evaluate the efficacy of 5 compared to ：tO granulocyteaphaeresis sessions in patients with active steroid-dependent ulcerative colitis. mETHODS： In this pilot, prospective, multicenter randomized trial, 20 patients with moderately active steroid-dependent ulcerative colitis were randomized to 5 or 10 granulocyteaphaeresis sessions. The primary objective was clinical remission at wk 17. Secondary measures included endoscopic remission and steroid consumption.REsuLTS： Nine patients were randomized to 5 granulocyteaphaeresis sessions （group 1） and 11 patients to 10 granulocyteaphaeresis sessions （group 2）. At wk 17, 37.5% of patients in group 1 and 45.45% of patients in group 2 were in clinical remission. Clinical remission was accompanied by endoscopic remission in all cases. Eighty-six percent of patients achieving remission were steroid-free at wk 17. Daily steroid requirements were significantly lower in group 2. Eighty-nine per cent of patients remained in remission during a one year follow-up. One serious adverse event, not related to the study therapy, was reported. CONCLUSION： Granulocyteaphaeresis is safe and effective for the treatment of steroid-dependent ulcerative colitis. In this population, increasing the number of aphaeresis sessions is not associated with higher remission rates, but affords a significant steroid-sparing effect.

AIm: To determine the presence of Helicobacter species DNA in the liver of chronic hepatitis C (CHC) patients with and without cirrhosis as compared to controls, and to identify the bacterial species involved. mETHODS: Seventy-nine consecutive patients (HBV and HIV negative) with a liver sample obtained after liver biopsy or hepatic resection were studied: 41 with CHC without cirrhosis, 12 with CHC and cirrhosis, and 26 controls (HCV negative). Polym.rase chain reactions (PCRs) targeting Helicobacter 16S rDNA and species- specific were performed on DNA extracted from the liver. A gastric infection with H pylori was determined by serology and confirmed by 13C-urea breath test. REsuLTS: Overall, Helicobacter 16S rDNA was found in 16 patients (20.2%). Although positive cases tended to be higher in CHC patients with cirrhosis (41.6%) than in those without cirrhosis (17.0%) or in controls (15.4%), the difference was not statistically significant (P =0.08). H pylori-like DNA was identified in 12 cases and H. pullorum DNA in 2, while 2 cases remained unidenti- fied. Gastric infection with H pylori was found in only 2 of these patients. CONCLUSION: Our results do not confirm the associ- ation of Helicobacter species DNA in the liver of CHC patients with advanced liver disease. The lack of correlation between positive H pylori serology and the presence of H pylori-like DNA in the liver may indicate the presence of a variant of this species.

Acute pancreatitis (AP) is a serious inflammatory disease with rising incidence both in the adult and pediatric populations. It has been shown that mitochondrial injury and energy depletion are the earliest intracellular events in the early phase of AP. moreover, it has been revealed that restoration of intracellular ATP level restores cellular functions and defends the cells from death. We have recently shown in a systematic review and meta-analysis that early enteral feeding is beneficial in adults; however, no reviews are available concerning the effect of early enteral feeding in pediatric AP. In this minireview, our aim was to systematically analyse the literature on the treatmentof acute pediatric pancreatitis. The preferred reporting items for systematic review(PRISmA-P) were followed, and the question was drafted based on participants, intervention, comparison and outcomes: P: patients under the age of twenty-one suffering from acute pancreatitis; I: early enteral nutrition (per os and nasogastric- or nasojejunal tube started within 48 h); C: nil per os therapy; O: length of hospitalization, need for treatment at an intensive care unit, development of severe AP, lung injury (including lung oedema and pleural effusion), white blood cell count and pain score on admission. Altogether, 632 articles (Pub med: 131; EmBASE: 501) were found. After detailed screening of eligible papers, five of them met inclusion criteria. Only retrospective clinical trials were available. Due to insufficient information from the authors, it was only possible to address length of hospitalization as an outcome of the study. Our mini-meta-analysis showed that early enteral nutrition significantly(SD = 0.806, P = 0.034) decreases length of hospitalization compared with nil per os diet in acute pediatric pancreatitis. Effect of Regulated Deficit Irrigation on Productivity,Quality and Water Use in Olive cv“manzanilla”

The objective of this experiment was to determine the effect of different regulated deficit irrigation (RDI) strategies on productivity, oil quality and water-use efficiency on olive grown in the Sonoran Desert. The experiments were carried out in 2009 and 2010, and in a ten years old traditional (10 × 5 m) “manzanilla” olive orchard. The control treatment was irrigated at 100% ETc during the whole season while RDI treatments were applied at 75% ETc or 50% ETc. The two RDI were applied during two phenological stages: at postharvest to evaluate the effect on table olive or from pit hardening to harvest to evaluate the effect on oil olive. Our results indicated that RDI applying 50% ETc during postharvest period reduced significantly fruit set and table olive yield, while applied during pit hardening to harvest period, it decreased oil yield but increased oil content. The RDI applying an ETc of 75% during the postharvest period gave similar table olive yield to the control, and applied form of pit hardening to harvest also gave similar oil yield to the control. The RDI using an ETc of 75% resulted in the highest water-use efficiency for oil or table olive production.

Introduction: malnutrition is a pathological state resulting from the relative deficiency or excess of one or more essential nutrients, whether manifested clinically or detected only by biochemical, anthropometric or physiological analyses. The overall objective was to assess the quality of management of acute malnutrition in children aged 0 - 24 months at the Boulbinet health center. methodology: This was a prospective descriptive study lasting six (06) months from may 5 to October 5, 2018. The study included all children aged 0 to 24 months. Results: Acute malnutrition in children aged 0 - 24 months accounted for 2.11% of cases. The sex ratio was 1.41 in favor of males. The mean age of our patients was 5 months 7 days, with extremes of 1 month and 6 months. The majority came from.ra toma (40.24%). Exclusive breastfeeding was most common (54.02%). The main clinical signs were: pallor 49.42%, diarrhea 46.67, oral lesions37.96%. SAm represented 89.66% and mAm 10.34%. most associated pathologies: anemia 49.42% and oral candidiasis 37.93%. In terms of outcome, we recorded 56.32% cures, 20.69% deaths, 18.39% dropouts and 4.60% cures. Conclusion: Improving the quality of care for malnourished children aged 0 - 24 months requires raising awareness among mothers and the general public of the consequences of malnutrition.

建筑周边道路交通的快速发展与建设,道路交通荷载已经成为导致振动问题的新的因素。道路交通荷载产生的振动通过土地传播到临近建筑的基础,然后传递到结构的其它部分。建筑的振动可能会引起室内人员的烦扰,或者干扰精密仪器的运行,甚至导致古建筑的轻微损伤。因此,对周边交通荷载导致的建筑物振动量级的预测分析非常有意义。本文中为了研究交通引起的环境振动对飞云楼进行了现场测试。测试中,道路边地面、地面层和一座三层古建筑不同位置处的振动加速度进行了测量。地面测点距建筑物外柱19.8m,距道路4m。地面层的测点布置在靠近主柱的基础上。1～3层的测点分别布置在靠近主柱的梁上。对测试数据分别在时域与频域进行了分析,研究了建筑物各层振动的振动级与频率成分。结果表明建筑物的横向振动加速度大于纵向加速度,但小于竖向加速度。研究中运用了两种预测理论对于建筑物的振动响应进行了预测。第一种基于振动衰减理论,即Barkan定律。根据Barkan定律,材料的阻尼系数可以通过地面与建筑的两个的测试数据估计得到。获得材料的阻尼系数之后,与振动源距离为d处的质点加速度峰值在时域和频域内都可以进行预测。第二种预测理论是基于传递比理论。在此理论中,首先确定建筑物外土壤中一点与建筑物内一点之间的三分之一倍频程的振动传递比。知道传递比之后,交通荷载引起的结构振动响应可以利用已知的室外地面振动预测得到。为了验证预测理论的准确性,预测得到的结构振动响应与现场实测数据进行了对比。结果表明,基于Barkan定律和传递比的预测理论能有效的预测结构的振动响应。此外,为了研究振动对建筑物的影响,对在普通交通与重载交通荷载作用下的结构振动也进行了分析。普通交通荷载作用下的建筑物振动加速度由风云楼现场测试获得;重载交通荷载作用下的结构振动通过前述理论利用别处重载的交通下地面振动预测得到。建筑物的结构振动响应与GB10071-1988、JGJ/T170-2009和GB/T5042-2008等规范中规定的古建筑振动限值进行比较,结果表明,普通交通与重载交通荷载引起的建筑物振动满足规范要求。
