Effect of propranolol on the splanchnic and peripheral renin angiotensin system in cirrhotic patients

AIm:To evaluate the effect of β-blockade on angiotensins in the splanchnic and peripheral circulation of cirrhotic patients and also to compare hemodynamic parameters during liver transplantation according to propranolol pre-treatment or not. mETHODS:Patients were allocated into two groups:outpatients with advanced liver disease(LD) and during liver transplantation(LT). Both groups were subdivided according to treatment with propranolol or not. Plasma was collected through peripheral venipuncture to determine plasma renin activity(PrA),Angiotensin(Ang) Ⅰ,Ang Ⅱ,and Ang-(1-7) levels by radioimmunoassay in LD group. During liver transplantation,hemodynamic parameters were determined and blood samples were obtained from the portal vein to measure renin angiotensin system.rAS) ***:PrA,Ang Ⅰ,Ang Ⅱ and Ang-(1-7) were signifi cantly lower in the portal vein and periphery in all subgroups treated with propranolol as compared to non-treated. The relationships between Ang-(1-7) and Ang Ⅰ levels and between Ang Ⅱ and Ang Ⅰ were significantly increased in LD group receiving propranolol. The ratio between Ang-(1-7) and Ang Ⅱ remained unchanged in splanchnic and peripheral circulation in patients under β-blockade,whereas the relationship between Ang Ⅱ and Ang Ⅰ was significantly increased in splanchnic circulation of LT patients treated with propranolol. During liver transplantation,cardiac output and index as well systemic vascular resistance and index were reduced in propranolol-treated subgroup. CONCLUSION:In LD group,propranolol treatment reduced rAS mediators,but did not change the ratio between Ang-(1-7) and Ang Ⅱ in splanchnic and peripheral circulation. Furtherm.re,the modification of hemodynamic parameters in propranolol treated patients was not associated with changes in the angiotensin ratio.

背景抗血小板治疗导致的血小板功能减退可产生围手术期出血，数项对手术前5日内摄入阿司匹林的影响研究结果不一，有的认为摄入阿司匹林会增加围手术期输血量，而有的认为并无显著影响。本项研究的目的在于对比患者自身报告服药情况与血小板功能测定两种方法评估阿司匹林摄入对围手术期输血需求的影响。方法在一项前瞻性研究中，对100例行冠状动脉旁路移植术的患者，术前5日阿司匹林的摄入进行标准化问卷调查。另外，使用multiplate血小板功能分析仪测定花生四烯酸介导的全血血小板聚集率。结果23例手术前5日内服用阿司匹林的患者中有11例血小板聚集功能异常，77例手术前未服用阿司匹林的患者中有9例血小板功能异常。服用与未服用阿司匹林的患者相比，手术后24小时内胸管引流量及红细胞输注量无显著差别。手术前血小板聚集功能异常的患者（〈51U）所需的血小板输注量（1．1U）显著高于血小板聚集功能正常的患者（0．3U，P=0．001）。结论本研究结果显示全血花生四烯酸诱导血小板聚集率对于血小板相关的凝血功能异常及血小板输注量的预测优于患者自身报告阿司匹林的摄入情况。

AIm To present clinical characteristics from.renal transplant recipients with dengue fever and its impact on graft *** We retrospectively evaluated 11 renal transplant recipients(rTr) with dengue infection confirmed by laboratory test, between January 2007 and July 2012, transplanted in the renal Transplant Center of Walter Cantídio University Hospital from Federal University of Ceará. rESULTS Positive dengue serology(Igm) was found in all patients. The mean time between transplant and dengue infection was 43 mo. Fever was presented in all patients. Nine patients presented with classical dengue and two(18%) with dengue hem.rrhagic fever. All cases had satisfactory evolution with complete recovery of the symptoms. The time for symptom.resolution varied from 2 to 20 d, with an average of 9 d. An increase of creatinine after the infection was observed in three(27.2%) patients with no clinically impact on the kidney graft function. CONCLUSION rTr with dengue infection seems to have a clinical presentation and evolution similar to those seen in the general population, with no long-term damage to patient and to the graft.

Diversity among Isolates of the Tangerine Pathotype of Alternaria alternata

The tangerine pathotype of Alternaria alternata is the aetiological agent of Alternaria brown spot on tangerines. In the state of Paraíba, Brazil, its occurrence on “Dancy” tangerine trees is associated with genetic aspects as well as the influence of environmental conditions on reproduction and dissemination within and between populations. The aim of the present study was to evaluate the diversity of isolates of this pathogen using m.rphophysiological and molecular m.rkers. For the analysis of mycelial growth and sporulation, 30 isolates from different locations were examined at 24-hour intervals until the seventh day, when the spores were quantified. The 30 isolates were characterised based on molecular m.rkers (ISSr) and genetic similarity (Jaccard index). A factor arrangement was used: 30 isolates, four media (ODA, PDA, LEA and V8), three light regimes (continuous dark, alternating light and continuous light) and three temperatures (15°C, 25°C and 35°C), with 12 repetitions. Groups 1, 2 and 3 presented low genetic variability. Group 4 showed high genetic variability of the isolates obtained from the massaranduba (state of Paraíba-Brazil) producing region and higher mycelial growth and sporulation of A. alternata. The continuous light regime and the temperature 25°C in PDA and V8 media were the ideal conditions for the mycelial growth and sporulation, respectively, of the isolates of A. alternata.

AIm To evaluate the rate of adverse events(AEs) during consecutive gastric and duodenal polypectomies in several Spanish centers. mETHODS Polypectomies of protruded gastric or duodenal polyps ≥ 5 mm using hot snare were prospectively included. Prophylactic measures of hem.rrhage were allowed in predefined cases. AEs were defined and graded according to the lexicon recommended by the Am.rican Society for Gastrointestinal Endoscopy. Patients were followed for 48 h, one week and 1 mo after theprocedure. rESULTS308 patients were included and a single polypectomy was performed in 205. Only 36(11.7%) were on prior anticoagulant therapy. mean polyp size was 15 ± 8.9 mm(5-60) and in 294 cases(95.4%) were located in the stomach. Hem.rrhage prophylaxis was performed in 219(71.1%) patients. Nine patients presented AEs(2.9%), and 6 of them were bleeding(n = 6, 1.9%)(in 5 out of 6 AE, different types of endoscopic treatment were performed). Other 24 hem.rrhagic episodes could be managed without any change in the outcome of the endoscopy and, consequently, were considered incidents. We did not find any independent risk factor of *** Gastroduodenal polypectomy using prophylactic measures has a rate of AEs small enough to consider this procedure a safe and effective method for polyp resection independently of the polyp size and location.

On the north coast of present-day Peru, between the Andes and the Pacific Ocean, prospered approximately between 1000 BC and 1375 AD, several relevant cultures: Chavín (1000-200 BC), Vicús and Frías (200 BC - 300 AD), moche (400 BC-700 AD), Sicán (700-1375 AD). These cultures are interconnected and characterized by a high metallurgical ability, demonstrated by the presence of beautiful artifacts on gold, silver and copper alloys. m.re than hundred metal artefacts from these cultures were analyzed with a portable equipment which uses the non-destructive and non-invasive technique of energy-dispersive X-ray fluorescence (EDXrF). Following objects were analyzed: (1) gold, silver and copper objects from the Chavín culture (museo municipal of Piura and and museo Enrico Poli in Lima); (2) gold and silver objects from the Vicús and Frías cultures (museo municipal of Piura); (3) gold, silver and copper objects from the moche culture ("museo Tumbas reales de Sipán", "museo de Sitio de Huaca rajada" in Lambayeque and museo Enrico Poli in Lima); (4) gold, silver and copper objects from the Sicán culture (museum of Sicán, Ferra?afe). Portable equipments were employed, mainly composed of a small size X-ray tube and a thermoelectrically cooled, small size, Si-PIN or Si-drift X-ray detectors. The main characteristics of the EDXrF-technique is of being non destructive and multi-elemental. Standard samples of gold and silver alloys were employed for calibration and quantitative analysis. The aims of this campaign of analysis were: (1) to analyze a large number of objects; (2) to differentiate gold, gilded copper and tumbaga (this last being a poor gold alloy enriched at the surface by depletion gilding); (3) to correlate, when possible, composition of analyzed alloys to the specific culture; (4) to determine a possible evolution of metallurgy; (5) to better determine characteristics and beginning time of tumbaga production. In the last decade, Brazil was the scene of two of the world’s biggest disasters related to mining tailings dams. The collapse of the m.riana and Brumadinho dams was catastrophic event that brought to light major environmental and humanitarian crises. Facts like this incite debate on the safety of dam construction methods and highlight the need for a broad discussion of dam safety legislation in the country. From the above, this paper promotes a review of various forms of construction of tailings dams and their associated problems, as well as discusses the techniques for soil reinforcement and their construction methods. The work also brings an overview of the Brazilian dams from April to July 2019, tracing the context of the Brazilian tailings dams months after the tragedy of Brumadinho. The methodology includes a robust literature review and discourse analysis. In Brazil, the number of ruptured dams is alarming, especially in the state of minas Gerais, in which m.re than 7 have broken in the last two decades. Among the damages, the deaths impact society abruptly and were cataloged 455 deaths, of which 216 correspond to the rupture of the dam of the Córrego do Feijão mine in Brumadinho. To evaluate and diagnose a dam, one should consider ordinance N ° 70,389, 2017 revoked by resolution ANm n ° 95, of February 7, 2022. In this context, in m.rch 2019, we experienced several disturbing situations regarding dam instability in the state of minas Gerais, being categorized as risky structures. To exemplify, one can cite dams classified as level 2 risk that became level 3. Due to the above facts, several studies are being conducted to im.rove soil aspects. For example, we highlight the increase in soil resistance that can be done using comm.rcial and alternative materials available in the environment.

Panulirus argus virus 1 (PaV1) is considered a major threat to spiny lobsters Panulirus argus. In this study Geospatial analysis was used to analyze PaV1 distribution in an artisanal fishery of spiny lobster Panulirus argus population from the north coast of the Yucatan Peninsula. Adult and sub-adult P. argus and seabed coverage data were collected from thirty artisanal fishing sites. Five seabed coverage types were identified: seagrass;sand/seagrass mixture;sand only;coral/sand mixture;and seaweed. No juveniles were examined. Of the 358 collected lobsters, PaV1 was identified in four organisms (three sub-adults and one adult) from two fishing sites (termed A & B), both found in a seagrass coverage area. Overall prevalence was of 1.12%. Prevalence was of 20% (2/10) at one site and of 12.6% (2/16) at the other.
