Off-Pump Coronary Artery Bypass Grafting in Patients with Left Ventricular Dysfunction: Short-Term Results from a Single Center in Bangladesh

Background: Off-pump coronary artery bypass grafting (OPCAB) is considered a safer alternative to on-pump surgery, especially in patients with left ventricular dysfunction (LVD). Objectives: This study assessed short-term outcomes and functional improvements in LVD patients post-OPCAB. methods: The study included 200 coronary artery disease patients who underwent isolated off-pump coronary artery bypass grafting (OPCAB) at the National Heart Foundation Hospital and Research Institute between January 2019 and June 2020. Patients were categorized into Group 1, with a left ventricular ejection fraction (LVEF) of 30% - 39%, and Group 2, with an LVEF of 40% or higher. Echocardiographic assessments of left ventricular dimensions and ejection fraction were performed preoperatively, at discharge, and one month postoperatively. Results: In Group 1, preoperative left ventricular internal dimensions during diastole (LVIDd) and systole (LVIDs) were 53.48 ± 4.40 mm and 44.23 ± 3.93 mm, respectively, with a left ventricular ejection fraction (LVEF) of 35.28% ± 2.26%. At discharge, these values improved to 51.58 ± 4.04 mm (LVIDd), 41.23 ± 5.30 mm (LVIDs), and 39.25% ± 3.75% (LVEF). One month postoperatively, further improvements were observed: 46.29 ± 3.76 mm (LVIDd), 37.45 ± 3.68 mm (LVIDs), and 43.22% ± 4.67% (LVEF). Group 2 showed similar positive outcomes, with preoperative values of 47.09 ± 5.06 mm (LVIDd), 35.11 ± 5.25 mm (LVIDs), and 50.13% ± 7.25% (LVEF), improving to 42.37 ± 4.18 mm (LVIDd), 31.05 ± 4.19 mm (LVIDs), and 55.33% ± 7.05% (LVEF) at one month postoperatively. Both groups demonstrated significant improvements in left ventricular function and NYHA class, with most patients moving from class III/IV to I/II. Complications were minimal, and no mortality was observed. Conclusion: OPCAB is safe and effective for patients with LVEF 30% - 39% and LVEF ≥ 40%, providing significant short-term functional improvements without increased risk.

The Spectrum of Nail Disorders in Patients with End-Stage Renal Disease Undergoing Conservative Treatment: A Case-Control Study

Objective: Various nail disorders have been reported in patients with chronic kidney disease. However, few studies have investigated nail disorders in patients with advanced chronic kidney disease. This study focused on nail disorders seen in patients with end-stage renal disease (ESRD) undergoing conservative treatment. methods: A case-control study was conducted at Baraha medical City in Khartoum State, Sudan. Data on patients with ESRD who were on conservative treatment and presented for follow-up between march and September 2021 were collected. Patients' demographic features, nail findings on clinical examination, and laboratory results were recorded. The obtained data were compared with those on age- and sex-matched healthy individuals from the general population. The chi-square test and Student t test were performed to analyze categorical and numerical variables, respectively. Results: Data on 78 patients with ESRD were studied. Their mean age was 54.6 ± 17.6 years, and 45 (57.7%) were men. These patients were compared with 129 controls. Nail disorders were seen in 65 (83.3%) ESRD patients and 86 (66.6%) controls (P = 0.009). Patients with ESRD were found to be at increased relative risk of developing a wide spectrum of nail disorders. An absent lunula, half-and-half nails, nail dystrophy, Terry's nails, onychomycosis, and koilonychia were the most common disorders and were seen in 44 (56.4%), 13 (16.7%), 10 (12.8%), 9 (11.5%), 6 (7.7%), and 5 (6.4%) patients, respectively. Patients with ESRD had a statistically significant risk of developing above diseases (all P monstrated a wide spectrum of nail disorders in patients with ESRD undergoing conservative treatment. In this article, a novel scalarization technique, called the improved objective-constraint approach, is introduced to find efficient solutions of a given multiobjective programming problem. The presented scalarized problem extends the objective-constraint problem. It is demonstrated that how adding variables to the scalarized problem, can lead to find conditions for (weakly, properly) Pareto optimal solutions. Applying the obtained necessary and sufficient conditions, two algorithms for generating the Pareto front approximation of bi-objective and three-objective programming problems are designed. These algorithms are easy to implement and can achieve an even approximation of (weakly, properly) Pareto optimal solutions. These algorithms can be generalized for optimization problems with more than three criterion functions, too. The effectiveness and capability of the algorithms are demonstrated in test problems.

Purpose:The collaboration relationships between innovation actors at a geographic level may be considered as grouping two separate layers,the domestic and the *** the level of each layer,the relationships and the actors involved constitute a Triple Helix *** paper distinguished three levels of analysis:the global grouping together all actors,the domestic grouping together domestic actors,and the foreign related to only actors from partner ***/methodology/approach:Bibliographic records data from the Web of Science for South Korea and West Africa breakdown per innovation actors and distinguishing domestic and international collaboration are analyzed with game *** core,the Shapley value,and the nucleolus are computed at the three levels to measure the synergy between ***:The synergy operates more in South Korea than in West Africa;the government is more present in West Africa than in South Korea;domestic actors create more synergy in South Korea,but foreign more in West Africa;South Korea can consume all the foreign synergy,which is not the case of West *** limitations:Research data are limited to publication records;techniques and methods used may be extended to other research *** implications:West African governments should increase their investment in science,technology,and innovation to benefit more from the synergy their innovation actors contributed at the foreign ***,the results of the current study may not be sufficient to prove that greater investment will yield benefits from foreign ***/value:This paper uses game theory to assess innovation systems by computing the contribution of foreign actors to knowledge production at an area *** proposes an indicator to this end.

This editorial delves into the research article by Zeng et al published in the latest issue of World Journal of *** manuscript contributes significantly to addressing the global health issue of nonalcoholic fatty liver disease(NAFLD)by introducing and validating the Exercise and Diet Adherence Scale(EDAS).The article effectively conveys the importance of the study,highlighting the prevalence of NAFLD,the lack of approved drugs for its treatment,and the crucial role of lifestyle *** use of the Delphi method for scale development and the subsequent evaluation of its reliability add scientific rigor to the *** results demonstrate that the scale is correlated with key lifestyle indicators,which makes it a promising tool for assessing patient adherence to *** identification of specific score thresholds for predicting adherence to daily calorie intake and exercise adds practical value to the *** differentiation among scores indicative of good,average,and poor adherence enhances its clinical *** conclusion,the manuscript introduces EDAS,a valuable instrument that can contribute substantially to the field of NAFLD research and clinical practice.

This study focuses on the preparation of nanostructured holmium oxide via the decomposition of holmium acetate precursor utilizing the non-isothermal strategy. Thermogravimetric analysis(TGA) was used to follow up the various thermal events involved in the decomposition process. Dehydration completes approximately at 150℃, which is followed by the decomposition of the anhydrous acetate leading to the formation of holmium oxide. Based on the TGA results the acetate precursor was heated non-isothermally at the temperature range of 150 e700℃. The obtained solids were characterized using powder X-ray diffraction(XRD), X-ray photoelectron spectroscopy(XPS), Fourier transform infrared spectroscopy(FT-IR), field-emission scanning electron microscopy(FE-SEm) and transmission electron microscopy(TEm). It is found that nanocrystalline Ho_2 O_3 starts to form at 500℃ and presents the only phase detected at the 500 e700℃ range. The electrical conductivity of the solids that form at the temperature range of 300 e700℃ was investigated. The obtained values were correlated with the observed structural modifications accompanying the heat treatment. The electrical conductivity of the Ho_2 O_3 samples prepared at 500, 600 and 700℃ reaches the values of 1.92 × 10^(-7), 1.61 × 10^(-7) and 8.33 × 10^(-8) Ω^(-1)cm^(-1) at a measuring temperature of 500℃, respectively. These values are potentially advantageous for high-resistivity devices.
