A I m : To present our extensive experience of hepatectomy for hepatocellular carcinoma using a microwave tissue coagulator to demonstrate the effectiveness of this device. mETHODS: A total of 1118 cases(1990-2013) were reviewed,with an emphasis on intraoperative blood loss,postoperative bile leakage and fluid/abscess formation,and adaptability to anatomical resection and hepatectomy with hilar ***: The median intraoperative blood loss was 250 m L; postoperative bile leakage and fluid/abscess formation were seen in 3.0% and 3.3% of cases,respectively. Anatomical resection was performed in 275 cases,including 103 cases of hilar dissection that required application of microwave coagulation near the hepatic hilum. There was no clinically relevant biliary tract stricture or any vascular problems due to heat injury. Regarding the influence of cirrhosis on intraoperative blood loss,no significant difference was seen between cirrhotic and non-cirrhotic patients(P = 0.38),although cirrhotic patients tended to have smaller tumors and underwent less invasive operations. CONCLUSION: This study demonstrated outcomes of an extensive experience of hepatectomy using heat coagulative necrosis by microwave tissue coagulator.

The dust monitor for rapid detection of α-emitting artificial radionuclides such as plutonium with Rn-progenies background compensation system was developed by applying a time interval analysis（TIA）.The TIA method,which is one kind of delayed coincidence method,was applied to β-α decay events with microsecond scale（half-life of 214Po:164μs）.Time intervals of neighboring pulses were analyzed by a single time interval analysis（STA）of the TIA *** situ experiments,dust samples on the filter（HE-40T;pore size 8μm）were collected at workplaces in reprocessing plant for measuring the 214Bi-214Po correlated *** that,the measurement of the 214Bi-214Po correlated events was carried out using the developed ***,through an experiment using the filter collected dust placed on a small 241Am source,increment of α-ray counting rate due to the 241Am source was detected clearly by background *** present method will apply for on-line monitoring of α-ray measurement,because the method is able to detect any additional artificial nuclides by compensating any changes of background due to Rn-progeny.

A low body mass index (BmI) is often associated with low nutritional status and adverse health outcomes. While the BmI is used in describing “thinness” meaning cachexia, serum albumin levels were also used in assessing patient’s nutritional conditions. Object: A retrospective study was conducted to determine whether the extremes of BmI and/or cachexia increased the morbidity and mortality in our critical emergency department. methods: A hundred and five cases with low BmI (≤18.5 kg/m2) were chosen from the patients who were admitted to our critical emergency department from Sept 2013 to Aug 2016. The relationship between serum albumin levels on admission and prognosis were analyzed. Results: No significant correlations were observed between body mass index and albumin levels in thin patients. Lower levels of serum albumin (p Conclusions: Even with low BmI, if the albumin value was 3.3 g/dL or more, the prognosis was good in our emergency department. Estimation of albumin level on admission is not only a simple and objective way to assess the nutritional status but also a useful outcome predictor of critically ill patients coming to emergency centers.

Split liver transplantation(SLT),while widely accepted in pediatrics,remains underutilized in adults. Advancements in surgical techniques and donor-recipient matching,however,have allowed expansion of SLT from utilization of the right trisegment graft to now include use of the hemiliver graft as well. Despite less favorable outcomes in the early experience,better outcomes have been reported by experienced centers and have further validated the feasibility of SLT. Importantly,more than two decades of experience have identified key requirements for successful SLT in adults. When these requirements are met,SLT can achieve outcomes equivalent to those achieved with other types of liver transplantation for adults. However,substantial challenges,such as surgical techniques,logistics,and ethics,persist as ongoing barriers to further expansion of this highly complex procedure. This review outlines the current state of SLT in adults,focusing on donor and recipient selection based on physiology,surgical techniques,surgical outcomes,and ethical issues.

To the Editor:An 82-year-old man with symptomatic severe calcific aortic valve stenosis(peak velocity:5.19 m/s;mean gradient:78.0 mmHg;aortic valve area:0.48 cm2)underwent transfemoral transcatheter aortic valve *** the procedure,a 0.035-inch straight-tip glide wire(Terumo,Somerset,NJ,USA)supported by a 6 Fr Judkins Right 4(JR4)coronary catheter(medtronic,minneapolis,mN,USA)were used to cross the severely stenotic aortic ***,the JR4 catheter could not pass the aortic valve because the wire crossed the aortic valve in the left/right coronary commissure creating an unfavorable angle and limited pushability for the *** of switching to a differently shaped catheter(eg,an Amplatz left 1 catheter),we advanced the glide wire and therefore the more supportive portion of the wire further through the aortic ***,both wire and JR4 catheter dropped into the non/left coronary commissure,allowing the JR4 catheter to cross the valve ***,a 34 mm self-expanding Evolut R valve(medtronic)was implanted,and the patient was discharged uneventfully(Figure 1A-E and Supplementary Video 1,http://***/Cm9/A353["Commissural drop"wiring technique.]).
