To the Editor,Sarcoidosis is a disease known for over a century and remains enigmatic in etiology and pathophysiology.1 While the pulmonary manifestation prevails in more than 90%of cases,sarcoidosis can affect any ***,it targets the eyes,skin,lymph nodes,and,in rare instances,the liver and spleen.2 The development of the disease is significantly influenced by genetic and environmental factors,as well as gene variations on chromosome ***,first-degree relatives of patients have a heightened risk factor of *** adverse prognostic factors involve being of Black race,female gender,and having a lower *** increased incidence of sarcoidosis among firefighters who responded to the 9/11 disaster highlighted environmental triggers,including agricultural employment,metalworking,firefighting,exposure to inorganic and silica dust,and handling building supplies.1 Clinical symptoms can vary widely depending on the affected *** often relies on characteristic chest radiography findings,but approximately 10%of patients show noncaseating granulomas on biopsy without pulmonary abnormalities.2 Herein,we present a unique case of multiorgan sarcoidosis,where the sole manifestations were severe thrombocytopenia,weight loss,and a history of cerebral vascular events.

Purpose: Neck node metastasis in squamous cell carcinoma of the oral tongue (SCCOT) is a recognized prognostic factor。However, the significance between number of positive nodes and survival is still not defined。This study was conducted to determine the impact of number of pathologically positive nodes on survival in patients with T1-2 SCCOT。methods: Between August 2004 and September 2007, 50 patients with Tl-2 SCCOT treated with curative surgery at Shaukat Khanum memorial Cancer Hospital and Research Centre were analyzed retrospectively。m 50%:F 50% (range 15—68 years)。All patients underwent partial glossectomy with neck dissection。Neck dissection was ipsilateral in 46 patients (92%) and bilateral in 4 patients (8%)。Forty four (88%) percent had post operative radiotherapy。Patients were divided in to 3 subgroups to determine the significance of number of positive nodes on survival: node negative (GroupⅠ), 1 - 3 positive nodes (GroupⅡ) and 4 or more nodes (Group III)。Results: AJCC stage was pTlpN0 31%, pT1pN+ 18%, pT2pN0 27% and pT2pN+ 24%。A total of 2230 cervical lymph nodes were analysed。Number of nodes in levelⅠ,Ⅱ,Ⅲ,ⅣandⅤwas 387, 721,465, 562 and 25 respectively。The mean and median number of nodes removed was 44 and 41 respectively。Twenty nine (58%) patients had NO (GroupⅠ), 15 (30%) patients had 1 to 3 positive lymph nodes (GroupⅡ) and 6 (12%) had 4 or more positive cervical lymph nodes(GroupⅢ)。Kaplan meier estimate for 3 year disease free survival (DFS) for GroupⅠ,ⅡandⅢwas 71%, 48% and 0% respectively (P=-0.004)。DFS for the whole group was 57%。Conclusion: Increasing number of pathologically positive cervical lymph node metastasis has an inverse influence on survival。In patients with 4 or more positive nodes aggressive adjuvant post operative treatment with radiation and chemotherapy should be considered as the prognosis is poor。

Purpose: Fractionated stereotactic radiosurgery (FSRS) can be given with at least three modalities: Gamma Knife, with the noninvasive frameless extend system (GKE);linear accelerator-based volumetric modulated arc therapy (VmAT);and intensity-modulated proton therapy (ImPT). We extracted treatment plans for patients who had received FSRS with GKE for recurrent skull base or intracranial tumors, created corresponding plans for VmAT and ImPT, and compared the quality of the three sets of plans. methods and materials: Plans were extracted for 9 patients with recurrent malignant skull-base tumors (n = 6) or large intracranial tumors (n = 3) who had received FSRS with GKE (median dose 24 Gy in 3 fractions) in 2013 through 2015. Plans for VmAT were generated with a TrueBeam STx LINAC machine using 6-mV photons, and plans for ImPT were generated with multi-field optimization. The optimized VmAT and ImPT plans were normalized to achieve the best possible target coverage while meeting the same dose-volume constraints on organs at risk (OARs) as the GKE plans. Plans were evaluated on the basis of target coverage, conformity index, homogeneity index, gradient index, and treatment efficiency. Results: The median target volume was 10.2 cm3 (range 1.9 - 33.8 cm3). The VmAT and ImPT plans met all OAR constraints, and target coverage and conformity were comparable among all plans. VmAT and ImPT plans showed significantly better target uniformity and treatment delivery efficiency (P P Conclusion: FSRS for skull base and large intracranial lesions delivered by VmAT and ImPT can achieve comparable target coverage, conformity, and sparing of critical structure as the GKE while providing superior target uniformity and treatment delivery efficiency. The GKE had superior high-dose gradients outside the target and thus better protected surrounding normal structures.

目的:探讨微血管减压术后疱疹病毒感染的临床特征、治疗及预后,以提高临床医生对此病的认识。方法:报道2例微血管减压术后疱疹病毒感染患者的临床资料并结合相关文献进行回顾性分析。结果:既往文献共有7例符合纳入标准,加上本文报道的2例,共9例患者。其中5例为水痘带状疱疹病毒感染,3例为单纯疱疹病毒感染,1例病毒类型不明。6例原发病为三叉神经痛,3例为面肌痉挛。其最常见症状为面部疱疹(5例),部分患者出现不典型症状如发热头痛(4例)、眼结膜炎(1例)、面瘫(1例)、面肌痉挛(1例)。经阿昔洛韦抗病毒后所有患者临床症状均好转,且预后较好。结论:微血管减压术后如出现发热、面部疼痛、面肌痉挛、面瘫、眼部症状时,需考虑有无疱疹感染可能,确诊后需及时抗病毒治疗。

Background:In this study,we aimed to investigate the molecular basis of lactation as well as to identify the genetic factors that influence milk yield and composition in *** achieve these two goals,we have analyzed how the mRNA profile of the mammary gland changes in seven murciano-Granadina goats at each of three different time points,i.e.78 d(T1,early lactation),216 d(T2,late lactation)and 285 d(T3,dry period)after ***,we have performed a genome-wide association study(GWAS)for seven dairy traits recorded in the 1st lactation of 822 murciano-Granadina ***:The expression profiles of the mammary gland in the early(T1)and late(T2)lactation were quite similar(42 differentially expressed genes),while strong transcriptomic differences(more than one thousand differentially expressed genes)were observed between the lactating(T1/T2)and non-lactating(T3)mammary glands.A large number of differentially expressed genes were involved in pathways related with the biosynthesis of amino acids,cholesterol,triglycerides and steroids as well as with glycerophospholipid metabolism,adipocytokine signaling,lipid binding,regulation of ion transmembrane transport,calcium ion binding,metalloendopeptidase activity and complement and coagulation *** regard to the second goal of the study,the performance of the GWAS allowed us to detect 24 quantitative trait loci(QTLs),including three genome-wide significant associations:QTL1(chromosome 2,130.72-131.01 mb)for lactose percentage,QTL6(chromosome 6,78.90-93.48 mb)for protein percentage and QTL17(chromosome 17,11.20 mb)for both protein and dry matter ***,QTL6 shows positional coincidence with the casein genes,which encode 80%of milk ***:The abrogation of lactation involves dramatic changes in the expression of genes participating in a broad array of physiological processes such as protein,lipid and carbohydrate metabolism,calcium homeostasis,cell death and tissue remo
