Retrospective analysis of hepatitis C infected patients treated through an integrated care model

AIm To determine if our health system's integrated model reflects sustained virologic response(SVR) outcomes similar to those in clinical trial data, maximizes adherence, and averts drug *** Subjects with chronic hepatitis C had their medical records reviewed from November 1st, 2014 through march 1st, 2016. Patients eligible for treatment were entered into an integrated care model therapy algorithm. The primary outcome was SVR12 based on intention to treat(ITT) analysis. Inclusion criteria consisted of both treatment na?ve and experienced patients over the age of 18 who were at least twelve weeks post-therapy completion with any genotype(GT) or mETAVIR score. Secondary outcomes included adherence, adverse events, and number of drug interaction *** At the time of analysis, 133 patients had reached twelve weeks post therapy with ITT. In the ITT analysis 70 patients were GT 1a, 26 GT 1b, 23 could not be differentiated between GT 1a or 1b, 8 GT 2, 4 GT 3, and 2 patients with multiple genotypes. The ITT treatment regimens consisted of 97 sofosbuvir(SOF)/ledipasvir(LDV), 8 SOF/LDV and ribavirin(RBV), 7 SOFand Simeprevir(SmV), 6 3D and RBV, 1 3D, 11 SOF and RBV, and 1 SOF, peg interferon alpha, and RBV. The overall SVR12 rate was 93% in the ITT analysis with a total of 6 patients relapsing. In patients with cirrhosis, 89% obtained SVR12. All 33 patients who were previous treatment failures achieved SVR12. Drug-drug interactions were identified in 56.4% of our patient population, 69 of which required interventions made by the pharmacist. The most common side effects were fatigue(41.4%), headache(28.6%), nausea(18.1%), and diarrhea(8.3%). Hepatoid adenocarcinoma of the colon in a patient with inflammatory bowel disease

A case is presented of a 36-year-old male with primary sclerosing cholangitis-associated inflammatory bowel disease(PSC-IBD) and two synchronous stage 1 adenocarcinomata of the colon,who was initially treated with a subtotal colectomy with *** year later,the patient presented with extensive intra-abdominal lymphadenopathy and peritoneal carcinomatosis,as well as a markedly elevated serum level of alpha-fetoprotein(AFP).Fine needle aspiration biopsy of a porta hepatis lymph node revealed a metastatic hepatoid *** review of the previous colectomy specimen showed that one of the previously identified adenocarcinomata had features suggestive of a hepatoid colonic *** patient was subsequently treated with a cytotoxic regimen of FOLFOX(oxaliplatin,leucovorin,5-fluorouracil) and bevacizumab,with stable results being achieved after six *** case presents the first known report of PSC-IBD associated with synchronous typical and hepatoid adenocarcinomata of the colon and highlights the importance of considering hepatoid adenocarcinoma as a differential diagnosis in patients with an increasing serum AFP level.

Circulating immunoglobulin m(Igm)exists in a pentameric form,possessing a polyreactive nature that responds not only to foreign antigens but also to autoantigens;thus,it is involved in both beneficial and detrimental immune responses,including protection from infection and the progression of *** the other hand,Igm also behaves as a carrier of the apoptosis inhibitor of macrophage(AIm)protein,storing a large amount of the inactivated form of AIm in the blood through this *** different disease conditions,AIm can dissociate from Igm locally or systemically to exert its function,inducing the removal of various biological debris such as excess fat,bacteria,cancer cells or dead cell *** typically,upon induction of acute kidney injury(AKI),Igm-free AIm is filtered by the glomerulus in the kidney,which stimulates the clearance of intraluminal dead cells debris at the obstructed proximal tubules,thereby facilitating the repair of kidney ***,cats exhibit a deficiency in AIm release from Igm,which may increase their susceptibility to renal ***,association with AIm inhibits Igm binding to the Fcα/μreceptor on follicular dendritic cells at the splenic germinal center,thereby protecting the Igm immune complex from Fcα/μreceptor-mediated internalization,which supports Igm-dependent antigen presentation to B cells and stimulates high-affinity IgG antibody *** regulation of AIm–Igm binding,resulting from the discovery of reciprocal actions between AIm and Igm,could lead to the development of novel therapies against different diseases.

Biomechanics associated with tibial stress fracture in runners:A systematic review and meta-analysis

Background:Tibial stress fracture(TSF)is an overuse running injury with a long recovery *** many running studies refer to biomechanical risk factors for TSF,only a few have compared biomechanics in runners with TSF to *** aim of this systematic review and meta-analysis was to evaluate biomechanics in runners with TSF compared to ***:Electronic databases Pubmed,Web of Science,SPORTDiscus,Scopus,Cochrane,and CINAHL were *** of bias was assessed and meta-analysis conducted for variables reported in 3 or more ***:The search retrieved 359 unique records,but only the 14 that compared runners with TSF to controls were included in the *** studies were retrospective,2 were prospective,and most had a small sample size(5-30 per group).many variables were not significantly different between ***-analysis of peak impact,active,and braking ground reaction forces found no significant differences between *** studies found larger tibial peak anterior tensile stress,peak posterior compressive stress,peak axial acceleration,peak rearfoot eversion,and hip adduction in the TSF ***:meta-analysis indicated that discrete ground reaction force variables were not statistically significantly different in runners with TSF compared to *** individual included studies,many biomechanical variables were not statistically significantly different between ***,many were reported by only a single study,and sample sizes were *** encourage additional studies with larger sample sizes of runners with TSF and controls and adequate statistical power to confirm or refute these findings.

原发性胆汁性肝硬化(PBC)是一种器官特异性的进行性自身免疫疾病,常见于女性,以肝脏门脉周围的淋巴细胞浸润,胆管上皮细胞特异性损伤以及血清中高滴度抗线粒体抗体(AmA)为主要特征。最新的PBC研究采用了CD4启动子控制下的TGFβ受体2显性失活(dnTGFβRⅡ)小鼠模型,这种小鼠模型很好地模拟了PBC患者的典型特征。在这种小鼠的基因背景下,通过分别敲除Rag1,μ以及CD1d基因建立了多种双基因缺陷鼠,通过研究发现T细胞,B细胞以及CD1d限制的NKT细胞等对肝脏淋巴细胞浸润,胆管上皮细胞损伤以及AmA的产生发挥着重要的作用。一系列的细胞免疫学实验结果显示,是CD8+T细胞而非CD4+T细胞在肝脏损伤过程中起着决定性的作用,而B细胞除分泌抗自身抗体之外,还具有抑制PBC发生的免疫调节作用。这些机制的研究为揭示人类PBC疾病的细胞免疫学致病机理提供了有力的证据和依托。

Background: P40)IL-2Rα mice spontaneously develop severe autoimmune cholangitis, similar to human patients with primary biliary cirrhosis(PBC). Although there has been considerable work that has focused on immune regulation, including breach of tolerance reflected by autoreactive T and B cells, there is a relative paucity of efforts directed at the key peritoneal cavity(PC) B cell subset, coined B1 a. Aims: Herein, we took advantage of p40IL-2Rα mice, to assess the role of PC B cells, especially B1 a cells, in the progress of autoimmune cholangitis. methods: To address this issue, we have studied qualitative and quantitative characteristics of B1 cells, particularly B1 a cells, in the PC of p40IL-2Rα mice compared to controls. Results: Interestingly, the number of B1 a cells in the PC of p40IL-2Rα mice were significantly decreased compared to controls. In contrast, there was a dramatic increase of CD4+ and CD8+ T cells accompanied by elevated expression of IFN-g. Further, there was a negative correlation between the frequency of B1 a cells and pathogenic CD8+ T cells in both liver and PC of p40IL-2Rα mice. Based on these results, including functional analysis, B cells from p40IL-2Rαmice downregulated IL-10 production and CTLA-4 expression, consistent with loss of B cell regulatory function. Conclusion: We submit that the relative absence of B1 a cells in the PC in this murine model of autoimmune cholangitis results in defective regulatory function and highlights a new potential therapeutic target in PBC.

Bismuth telluride is known to wield unique properties for a wide range of device applications. However, as devices migrate to the nanometer scale, significant amount of studies are being conducted to keep up with the rapidly growing nanotechnological field. Bi2Te3 possesses distinctive properties at the nanometer level from its bulk material. Therefore, varying synthesis and characterization techniques are being employed for the realization of various Bi2Te3 nanostructures in the past years. A considerable number of these works have aimed at improving the thermoelectric （TE） figure-of-merit （ZT） of the Bi2Te3 nanostructures and drawing from their topological insulating properties. This paper reviews the various Bi2Te3 and Bi2Te3-based nanos- tructures realized via theoretical and experimental procedures. The study probes the preparation techniques, TE properties and the topological insulating effects of 0D, 1D, 2D and Bi2Te3 nanocomposites. With several applications as a topological insulator （TI）, the topological insulating effect of the Bi2Te3 is reviewed in detail with the time reversal symmetry （TRS） and surface state spins which characterize TIs. Schematics and preparation methods for the various nanostructural dimensions are accordingly categorized.

This paper reviews the original achievements and advances regarding the field effect transistor(FET) fabricated from one of the most studied transition metal dichalcogenides: two-dimensional mo S2. Not like graphene, which is highlighted by a gapless Dirac cone band structure, monolayer mo S2 is featured with a 1.9 e V gapped direct energy band thus facilitates convenient electronic and/or optoelectronic modulation of its physical properties in FET structure. Indeed,many mo S2 devices based on FET architecture such as phototransistors, memory devices, and sensors have been studied and extraordinary properties such as excellent mobility, ON/OFF ratio, and sensitivity of these devices have been exhibited. However, further developments in FET device applications depend a lot on if novel physics would be involved in them. In this review, an overview on advances and developments in the mo S2-based FETs are presented. Engineering of mo S2-based FETs will be discussed in details for understanding contact physics, formation of gate dielectric, and doping strategies. Also reported are demonstrations of device behaviors such as low-frequency noise and photoresponse in mo S2-based FETs, which is crucial for developing electronic and optoelectronic devices.
