AIm: To describe endoscopic treatment of bile leaks in these patients and to identify risk factors in these patients which can predict the development of bile leaks. mETHODS: Retrospective case-control study examining consecutive patients who underwent partial hepatectomy for polycystic liver disease (PLD) and developed a postoperative bile leak managed endoscopically over a ten year period. Each case was matched with two controls with PLD who did not develop a postoperative bile leak. RESULTS: Ten cases underwent partial hepatectomy with fenestration for symptoms including abdominal distention, pain and nausea. Endoscopic retrograde cholangiopancreatography (ERCP) showed anatomic abnormalities in 1 case. A biliary sphincterotomy was performed in 4 cases. A plastic biliary stent was placed with the proximal end at the site of the leak in 9 cases; in 1 case two stents were placed. The overall success rate of ERCP to manage the leak was 90%. There were no significant differences in age, gender, comorbidities, duration of symptoms, history of previous surgery or type of surgery performed between cases and controls. CONCLUSION: ERCP with stent placement is safe and effective for management of post-hepatectomy bile leak in patients with PLD.

AIm: To investigate the survival of individuals with colorectal cancer (CRC) with inflammatory bowel disease (IBD-associated CRC) compared to that of individuals without IBD diagnosed with CRC. mETHODS: Epidemiologic, clinical, and follow-up data were obtained from the Colon Cancer Family Registry (Colon CFR). IBD-associated cases were identified from self-report of physician diagnosis. For a subset of participants, medical records were examined to confirm self-report of IBD. Cox proportional hazards regression was applied to estimate adjusted hazard ratios (aHR) and 95%CI of mortality, comparing IBD-associated to non-IBD-associated CRC, adjusted for age at CRC diagnosis, sex, Colon CFR phase, and number of prior endoscopies. Following imputation to complete CRC stage information, adjustment for CRC stage was examined. RESULTS: A total of 7202 CRC cases, including 250 cases of IBD-associated CRC, were analyzed. Over a twelve year follow-up period following CRC diagnosis, 2013 and 74 deaths occurred among non-IBD associated CRC and IBD-associated CRC patients, respectively. The difference in survival between IBD-associated and non-IBD CRC cases was not statistically significant (aHR = 1.08; 95%CI: 0.85-1.36). However, the assumption of proportional hazards necessary for valid inference from Cox regression was not met over the entire follow-up period, and we therefore limited analyses to within five years after CRC diagnosis when the assumption of proportional hazards was met. Over this period, there was evidence of worse prognosis for IBD-associated CRC (aHR = 1.36; 95%CI: 1.05-1.76). Results were similar when adjusted for CRC stage, or restricted to IBD confirmed in medical records. CONCLUSION: These results support the hypothesis that IBD-associated CRC has a worse prognosis than non-IBD-associated CRC.

This project integrates recent innovations of recycled materials used in designing and building sustainable pavements. An increasing environmental awareness and the demand for improving economic and construction efficiencies, through measures such as con- struction warrantees and goals to reduce air pollution under the Kyoto Protocol, have increased the efforts to implement sustainable materials in roadways. The objective of this research is to develop a systematic approach toward selecting optimum combinations of sustainable materials for the construction of asphalt pavements. The selected materials, warm mix asphalt （WmA）, recycled asphalt shingles {RAS）I and reclaimed asphalt pave- ment {RAP） were incorporated in this study. The results of this research are intended to serve as guidelines in the selection of the mixed sustainable materials for asphalt pave- ments. The approach developed from this project draws upon previous research efforts integrating graphical modeling with optimizing the amount of sustainable materials based on the performance. With regard to moisture susceptibility and rutting potential test re- suits, as well as the mIm analysis based on a 95% confidence interval, the rutting perfor- mance and moisture susceptibility of asphalt mixtures are not significantly different regardless of the percentages of RAS, RAP, or WmA. The optimum mixture choices could be made by the plant emission rankings with consideration of the optimal WmA types, per- centages of RAS/RAP, and WmA production temperatures. The WmA mixtures prepared with 75% RAP and Advera WmA have produced the lowest CO2 emissions among the investigated mixture types.

Neuropathic pain is a severe and chronic condition widely found in the general *** reason for this is the extensive variety of damage or diseases that can spark this unpleasant constant feeling in *** the processing of pain,the dorsal root ganglia constitute an important region where dorsal root ganglion neurons play a crucial role in the transmission and propagation of sensory electrical ***,the dorsal root ganglia have recently exhibited a regenerative capacity that should not be neglected in the understanding of the development and resolution of neuropathic pain and in the elucidation of innovative ***,we will review the complex interplay between cells(satellite glial cells and inflammatory cells)and factors(cytokines,neurotrophic factors and genetic factors)that takes place within the dorsal root ganglia and accounts for the generation of the aberrant excitation of primary sensory neurons occurring in neuropathic *** importantly,we will summarize an updated view of the current pharmacologic and nonpharmacologic therapies targeting the dorsal root ganglia for the treatment of neuropathic pain.

To elucidate potential antioxidant, antidiarrheal, cytotoxic, and antibacterial activities of the ethanol extract of A/ocasia indica Schott tuber in different experimental models established in vitro and in vivo. mETHODS： In vitro antioxidant activity was evaluated by 2, 2-diphenyl-l-picrylhydrazyl （DPPH） radical-scavenging assay. Phenolic content was estimated by using Folin-Ciocalteu＇s reagent while reducing ability was measured by ferric reducing power assay./n vivo antidiarrheal studies were carried out in mice, and the activity was evaluated in castor oil and magnesium sulfate- induced diarrhea. Disk diffusion assay was utilized to determine antibacterial activity against a number of pathogenic bacterial strains. Acute toxicity test was carried out to measure the safe doses for the extract. RESULTS： In DPPH radical-scavenging assay, the extract exhibited strong radical-scavenging activity with the 50% inhibitory concentration value of 42.66 IJg/mL. Total phenolic content was found to be 542.26 mg gallic acid equivalent per 100 g of dried tuber extract, whereas flavonoid content was found to be 4.30 mg quercetin equivalent/g of dried tuber extract. In reducing power assay, the extract showed strong reducing power in a concentration-dependent manner. The extract significantly （P 〈 0.01） enhanced the latent period and decreased defecation in both castor oil- and magnesium sulfate-induced diarrhea. The extract also lessened gastrointestinal motility in mice. Potential antibacterial activity was exhibited by the extract against all the tested bacterial strains in disk diffusion assay. The 50% lethal concentration against brine shrimp nauplii was 81.09 μg/mL. CONCLUSION： The results demonstrated that the ethanol extract of A. inc/ica has potential antioxidant, antidiarrheal, cytotoxic, and antibacterial activity.

The CycloneTm System (Castlewood Surgical, Inc., Concord, mA) is a novel device that facilitates the attachment of the saphenous vein onto the ascending aorta for the purpose of creating a bypass graft during a coronary artery bypass grafting (CABG) operation. It allows the surgeon to perform a hand-sewn anastomosis with no disruption of the intima of the aorta, and no need for partial clamping. During a 36-month period 109 CABG operations were performed, and the CycloneTm System (and its predecessor, the HexalonTm) was utilized to create 138 proximal anastomoses. This study demonstrates that this is a safe and effective method of creating a clampless, no-touch proximal anastomoses during off-pump CABG.

Delivery of a peptide(APP96-110),derived from amyloid precursor protein(APP),has been shown to elicit neuroprotective effects following cerebral stroke and traumatic brain *** this study,the effect of APP96-110 or a mutant version of this peptide(mAPP96-110)was assessed following moderate(200 kdyn,(2 N))thoracic contusive spinal cord injury(SCI)in adult Nude *** received a single tail vein injection of APP96-110 or mAPP96-110 at 30 minutes post-SCI and were then assessed for functional improvements over the next 8 weeks.A cohort of animals also received transplants of either viable or non-viable human mesenchymal stromal cells(hmSCs)into the SC lesion site at one week post-injury to assess the effect of combining intravenous APP96-110 delivery with hmSC *** were perfused 8 weeks post-SCI and longitudinal sections of spinal cord analyzed for a number of factors including hmSC viability,cyst size,axonal regrowth,glial reactivity and macrophage *** of sensorimotor function revealed occasional significant differences between groups using Ladderwalk or Ratwalk tests,however there were no consistent improvements in functional outcome after any of the ***96-110 alone,and APP96-110 in combination with both viable and non-viable hmSCs significantly reduced cyst size compared to SCI *** treatments with donor hmSCs also significantly increased βIII tubulin^(+),glial fibrillary acidic protein(GFAP^(+))and laminin+expression,and decreased ED1^(+)expression in *** preliminary study demonstrates that intravenous delivery of APP96-110 peptide has selective,modest neuroprotective effects following SCI,which may be enhanced when combined with hmSC ***,the effects are less pronounced and less consistent compared to the protective morphological and cognitive impact that this same peptide has on neuronal survival and behaviour after stroke and traumatic brain *** while the efficacy of a

Background: many studies have provided evidence for an association between obesity, physical inactivity, and western diet as risk factors for colorectal cancer (CRC). Few studies directly address the association between type 2 Diabetes mellitus (Dm) and the risk of colorectal lesions at specific anatomic locations. methods: 2663 subjects with a previous history of adenoma (s) and removal of all current adenomas at study entry were followed for a mean time of three years across three different chemoprevention clinical trials. The primary endpoint was colorectal adenoma recurrence and number of lesions during the treatment phase;the secondary endpoints were presence of advanced colorectal neoplasia (CRN) and location of CRN. Using log linear regression, the effect of Dm status on the relative risk (RR) of CRN recurrence, advanced CRN, and location of CRN was assessed. Results: Dm status was not significantly associated with incidence of colorectal adenomas, incidence of advanced colorectal lesions, or left-sided colorectal neoplastic lesions. Subjects with Dm had a marginally increased risk of right-sided (p = 0.06) colorectal adenomas and a significant increased risk of multiple right-sided adenomas (p = 0.03) in the unadjusted model;this association was not significant after adjusting for age and other potential confounders (RR = 1.22, 95% CI: 0.85 - 1.76). Conclusion: We did not observe a statistically significant increased risk in CRN recurrence for overall neoplasia, advanced neoplasia or location of neoplasia in individuals with Dm compared to non-Dm individuals. However, given the patterns observed in this investigation, future studies with longer follow-up time and longer Dm exposure, incorporating objective measurements of type 2 Dm might help elucidate the risk of CRN among individuals with Dm.

Hearing Disability in Colombia Self-Perception and Associated Factors 2002-2008

Introduction: Hearing disability is a condition that affects normal ear function, as much in adulthood as in the first years of life. According to the 2005 Census, 6.3% of the Colombian population has some type of disability, of which 17.4% have hearing limitations, including those with hearing devices. Elucidating the conditions of this population and identifying the factors related to hearing disability will permit the management of strategies from different sectors to mitigate the consequences associated with this limitation. Objective: To estimate the self-perception of hearing disability in Colombia and to determine the factors associated with this limitation. materials and methods: The present study is cross-sectional, based on the analysis of secondary information obtained from the Registry for Localization and Characterization of Persons with Disability (RLCPD) during the 2002-2008 period. Socio-demographic and healthcare variables were analyzed. Results: 13.6% (102,648/750,377) of the population reported hearing limitations even with the use of special hearing devices. 43.52% (44,041) of people over 3 years of age could neither read nor write. 29.39% (30,145) of people who reported this limitation are not affiliated to any sort of health insurance system. Factors found to be associated with hearing limitations were: socioeconomic stratus (OR: 1.33;CI 95% 1.25;1.42), illiteracy (OR: 1.44;CI 95% 1.42;1.46) and lack of affiliation to a health insurance system (OR: 1.03;CI 95% 1.01;1.04). Conclusion: People registered with hearing disability live under vulnerable conditions;among them, most/the majority pertained to a segment of the population with low economic resources and had difficulty obtaining/accessing work, education and healthcare services.
