Kidney transplantation is the best option for kidney replacement therapy,even considering that most of the times the grafts do not survive as long as their *** the Khalil et al's experience,published in this issue of the Journal,they analyze their second kidney graft survival and describe those significant predictors of early *** editorial comments on the results and put in perspective that most of the times,long-term graft survival could be inadvertently jeopardized if the immunosuppressive therapy is reduced or withdrawn for any reason,and that it could happen frequently if the transplant physician intends to innovate with the clinical care without proper evidence-based data.

The effectiveness of monitoring and early-warning systems for ground deformation phenomena,such as sinkholes,depends on their ability to accurately resolve the ongoing ground displacement and detect the subtle deformation preceding catastrophic *** sinkholes with a slow subsidence rate and diffuse edges pose a significant challenge for subsidence monitoring due to the low deformation rates and limited lateral strain *** this work,we satisfactorily illustrate the practicality of the Brillouin optical time domain analysis(BOTDA)to measure the spatial-temporal patterns of the vertical displacement in such challenging slow-moving sagging *** assess the performance of the approach,we compare the strain recorded by the distributed optical fiber sensor with the vertical displacement measured by high-precision *** results show a good spatial correlation with the ability to identify the maximum subsidence *** is also a good temporal correlation with the detection of an acceleration phase in the subsidence associated with a flood event.

Globally,type 2 diabetes mellitus(T2Dm)is one of the most common metabolic disorders.T2Dm physiopathology is influenced by complex interrelationships between genetic,metabolic and lifestyle factors(including diet),which differ between populations and geographic *** fact,excessive consumptions of high fat/high sugar foods generally increase the risk of developing T2Dm,whereas habitual intakes of plant-based healthy diets usually exert a protective ***,genomic studies have allowed the characterization of sequence DNA variants across the human genome,some of which may affect gene expression and protein functions relevant for glucose *** comprehensive literature review covers the impact of gene-diet interactions on T2Dm susceptibility and disease progression,some of which have demonstrated a value as biomarkers of personal responses to certain nutritional ***,novel genotype-based dietary strategies have been developed for improving T2Dm control in comparison to general lifestyle ***,progresses in other omics areas(epigenomics,metagenomics,proteomics,and metabolomics)are improving current understanding of genetic insights in T2Dm clinical *** more investigation is still needed,the analysis of the genetic make-up may help to decipher new paradigms in the pathophysiology of T2Dm as well as offer further opportunities to personalize the screening,prevention,diagnosis,management,and prognosis of T2Dm through precision nutrition.

metabolism is a fundamental process by which biochemicals are broken down to produce energy(catabolism) or used to build macromolecules(anabolism). metabolism has received renewed attention as a mechanism that generates molecules that modulate multiple cellular responses. This was first identified in cancer cells as the Warburg effect, but it is also present in immunocompetent cells. Studies have revealed a bidirectional influence of cellular metabolism and immune cell function, highlighting the significance of metabolic reprogramming in immune cell activation and effector functions. metabolic processes such as glycolysis, oxidative phosphorylation, and fatty acid oxidation have been shown to undergo dynamic changes during immune cell response, facilitating the energetic and biosynthetic demands. This review aims to provide a better understanding of the metabolic reprogramming that occurs in different immune cells upon activation, with a special focus on central nervous system disorders. Understanding the metabolic changes of the immune response not only provides insights into the fundamental mechanisms that regulate immune cell function but also opens new approaches for therapeutic strategies aimed at manipulating the immune system.

This paper presents an analytical framework for evaluating the impact of the two main processes identified in madrid timber-framed courtyard buildings reinforcing the courtyard's crucial role in architectural heritage sustainability:their development caused dwellings to darken(1737–1950),and the existing challenge is to open new patios according to the listed buildings'protection *** findings in archival research and existing building assessments led to completed data collection and *** exploring construction expansion,from the house towards the collective prototype around a gradually reduced patio and dwelling darkening(1737–1786),buildings resulted in open,semiopen,underlighted and closed *** area on-site assessment uncovered the transition from the older low-rise type(1–3)floors to the modern closed model of up to(4–6)*** enlargements,consequently reducing original green areas and courtyards,call for reversing that process or formulating a new *** debates the partial demolition of listed buildings in upgrades and its agreement with the needed“blue courtyard”strategy(PGOUm),resulting from joining new rear individual patios to create a communal building block *** uncovered“in-between”type(1864),which integrates two open-air collective spaces,an inner courtyard and a back garden,appears challenging to explore.

The probability of developing primary dysfunction(PD)is a function of the probability of ischemia/reperfusion(I/R)*** probability of I/R injury in turn,is a function of several donor and transplantation process variables,among which is ischemia *** et al studied the duration of a special type of warm ischemia and showed,contrary to what is known,that a longer duration is not statistically different from a shorter one in PD *** finding opens the door to the unforeseen opportunity of training fellows in performing hepatectomies,since the duration will not jeopardize liver transplant outcomes,albeit with some precautions.

The procurement process for organ donation begins with the identification of potential organ donors in emergency or critical care units(ccU),followed by their clinical evaluation,diagnostic procedures,and therapeutic interventions,mostly conducted in *** concludes with the request for organ donation and,if accepted,the retrieval of *** most interventions occurring in detection units,there has been a neglect of the strategic role played by critical care specialists(ccS)in managing and caring for brain-dead or near-brain-death *** arise:Are they willing to undertake this responsibility?Do they fully comprehend the nature of organ procurement?Are they aware of the specific interventions required to maintain possible organ donors in optimal physiological condition?Our objective is to examine the role of ccS in organ procurement and propose ways to enhance it,ultimately aiming to increase and enhance organ donation rates.
