The recently developed machine learning(mL)models have the ability to obtain high detection rate using biomedical ***,this article develops an Optimal Sparse Autoencoder based Sleep Stage Classiﬁcation model on Electroencephalography(EEG)Biomedical Signals,named OSAE-SSCEEG *** major intention of the OSAE-SSCEEG technique is toﬁnd the sleep stage disorders using the EEG biomedical *** OSAE-SSCEEG technique primarily undergoes preprocessing using min-max data normalization ***,the classiﬁcation of sleep stages takes place using the Sparse Autoencoder with Smoothed Regularization(SAE-SR)with softmax(Sm)***,the parameter optimization of the SAE-SR technique is carried out by the use of Coyote Optimization Algorithm(COA)and it leads to boosted classiﬁcation efﬁ*** order to ensure the enhanced performance of the OSAE-SSCEEG technique,a wide ranging simulation analysis is performed and the obtained results demonstrate the betterment of the OSAE-SSCEEG tech-nique over the recent methods.

A cyber physical energy system(CPES)involves a combination of pro-cessing,network,and physical *** smart grid plays a vital role in the CPES model where information technology(IT)can be related to the physical *** the same time,the machine learning(mL)modelsﬁnd useful for the smart grids integrated into the CPES for effective decision ***,the smart grids using mL and deep learning(DL)models are anticipated to lessen the requirement of placing many power plants for electricity *** this aspect,this study designs optimal multi-head attention based bidirectional long short term memory(OmHA-mBLSTm)technique for smart grid stability predic-tion in *** proposed OmHA-mBLSTm technique involves three subpro-cesses such as pre-processing,prediction,and hyperparameter *** OmHA-mBLSTm technique employs min-max normalization as a pre-proces-sing ***,the mBLSTm model is applied for the prediction of stability level of the smart grids in *** the same time,the moth swarm algorithm(mHA)is utilized for optimally modifying the hyperparameters involved in the mBLSTm *** ensure the enhanced outcomes of the OmHA-mBLSTm technique,a series of simulations were carried out and the results are inspected under several *** experimental results pointed out the better outcomes of the OmHA-mBLSTm technique over the recent models.

Background: The liver is the corner stone in lipid metabolism, free fatty acid uptake, synthesizing, storing and exporting lipids;non-alcoholic fatty liver disease (NAFLD) develops if there is any interruption or derangements in lipid metabolim. Fatty acid synthase (FAS) is the major enzyme in lipogenesis, and its circulating level is a bi-omarker of metabolically demanding human diseases. Aim of the Work: To evaluate the level of circulating FAS in NAFLD patients and to correlate it to serum lipid pa-rameters. materials and methods: The study included forty NAFLD patients and forty age and sex-matched healthy subjects as controls. Results: FAS levels were signifi-cantly higher in NAFLD patients compared to their level in the controls (P mI (r = 0.57), and between FAS levels and triglycerides and low density lipoprotein cholesterol levels in NAFLD patients (r = 0.79 & 0.53, respectively). Conclusion: Elevated levels of circulating FAS can be considered as a biomarker of fatty liver disease.

We report our experience with potential donors for living donor liver transplantation(LDLT), which is the first report from an area where there is no legalized deceased donation program. This is a single center retrospective analysis of potential living donors(n = 1004) between may 2004 and December 2012. This report focuses on the analysis of causes, duration, cost, and various implications of donor exclusion(n = 792).most of the transplant candidates(82.3%) had an experience with more than one excluded donor(median = 3). Some recipients travelled abroad for a deceased donor transplant(n = 12) and some died before finding a suitable donor(n = 14). The evaluation of an excluded donor is a time-consuming process(median = 3 d, range 1 d to 47 d). It is also a costly process with a median cost of approximately 70 USD(range 35 USD to 885 USD). From these results, living donor exclusion has negative implications on the patients and transplant program with ethical dilemmas and an economic impact. many strategies are adopted by other centers to expand the donor pool; however, they are not all applicable in our locality. We conclude that an active legalized deceased donor transplantation program is necessary to overcome the shortage of available liver grafts in Egypt.

Objective： Postmenopausal women with breast cancer are at increased risk of bone loss because of age related estrogen deficiency face which accelerated with the use of aromatase inhibitors （AIs）. We aimed to study the effect on bone mineral density （BmD） and bone formation biomarker osteocalcin level in postmenopausal breast cancer patients, for the first three years of adjuvant hormonal treatment of both groups Tamoxifen versus Anastrozol. methods： One-hundered postmenopausal breast cancers were prospectively randomized to receive either Tamoxifen 20 rag/day （n = 50） or Anastrozole 10 mg （n = 50）. Both BmD and osteocalcin were assessed initially before treatment and then at regular intervals for both groups. Results： Use of Tamoxifen was associated with significant annual decrease in osteocalcin （P = 0.001）, whereas Anastrozole group had gradual increase of the annual levels （P 〈 0.01）. BmD decreased significantly in Anastrozole versus Tamoxifen groups （2.6% vs. 0.4%, P 〈 0.001）. Osteoporosis T 〈 -2.5 was reported significantly higher in Anastrozole group （P 〈 0.01）. Women with initial osteopenia in Anastrozole group showed significant decrease in BmD （P 〈 0.05）. The addition of bisphosphonate for patients with early osteoporosis markedly improved both osteocalcin level and BmD. Conclusion： Tamoxifen preserves BmD in postmenopausal breast cancer patients, whereas Anastrozole accelerates age associated fall in BmD especially in the first year of therapy, moreover, the addition of bisphosphonate can help to decrease the skeletal related events associated with treatment to ensure better quality of life with treatment.

Assessment of maintenance oral Etoposide following induction chemotherapy with gemcitabine and cisplating in chemonaive extensive small cell lung cancer

Objective： The aim of our study was to evaluate the response and tolerability to treatment when using gemcitabine-cisplatin combination （GC）, followed by maintenance therapy of oral etoposide for non-progressive patients in trial to improve progression free survival and overall survival. methods： Thirty nine patients with extensive small cell lung cancer （SCLC） and ECOG ≤ 2 received 4 cycles of chemotherapy consisting of gemcitabine 1000 mg/m^2 （day 1 and 8） cisplating 75 mgim2 （day 1） every three weeks. Twenty seven non-progressive patients after 4 cycles of chemotherapy were randomized either to receive oral etoposide 50 mg/m^2 for consecutive 15 days every 3 weeks vs no therapy for three months or progression. Results： Thirty nine eligible patients treated with GC, 27 non progressive patients were subsequently randomized to oral etoposide or observation. median follow-up was 18 months. The overall response rate to GC was 59% and toxicity to oral etoposide was mild. There was improvement in median progression-free survival （PFS） favoring the maintenance arm of 10.5 months vs 7 months （P 〈 0.05）. median overall survival （OS） had improved towards the maintenance arm （13 vs 11.5 months）. One year survival （60% vs 24%）, 18 months survival （20% vs 5%） favoring the maintenance. multivariate analysis revealed that age, performance status, maintenance therapy, and response to treatment were independent prognostic factors for OS. Age, maintenance therapy, and response to treatment were highly significant factors for PFS. Conclusion： Gemcitabine-cisplatin is an effective and tolerable regiment for extensive disease of SCLC. The addition of 3 months of oral etoposide in non progressing patients was associated with a significant improvement of PFS and longer OS.

This is a multidimensional review of mangrove fungi occurring as saprobes,pathogens and endophytes of a wide range of host substrates and those isolated from the water columns and sediments in ***-hundred and fifty taxa including 658 that are supported by both morphology and molecular data and 192 with only morphological data are *** constitute Ascomycota,the dominant group with 773 species,and 58 Basidiomycota,one Blastocladiomycota,five Chytridiomycota,and 13 *** study also includes data on mangrove yeasts 103 Ascomycota,39 Basidi-omycota and 193 taxa isolated from *** isolated from submerged parts of mangrove plants total *** most specious orders of mangrove fungi are Pleosporales 133,Saccharomycetales 102,microascales 101,Eurotiales 87,Hypocreales 60 and Xylariales *** genera include Candida 39,Aspergillus 53,Penicillium 17 and Corollospora *** highest number of mangrove fungi have been recorded from the Pacific Ocean 553,which is the largest ocean,followed by Indian 408 and Atlantic Oceans *** distribution of mangrove fungi varied from ocean to ocean with only 109 taxa common to the Atlantic,Indian and Pacific *** the various countries reported for mangrove fungi,India accommodates the highest number(339)followed by Thailand 303,malaysia 171,Florida Everglades,USA 134 and Brunei 134.A total of 60 different mangrove plants and their associates have been surveyed for mangrove *** results are discussed and compared with previous studies.

Objective:This phase II study aimed at investigating the correlation between O6-methylguanine-DNA methyltransferase (mGmT) promoter methylation and protein expression,together with Ki-67 labeling index (LI),to response,time to progression (TTP),and overall survival (OS) in newly diagnosed glioblastoma multiforme (GBm) patients treated with temozolomide (TmZ) concomitant with and adjuvant to radiotherapy (RT).methods:From June 2005 to August 2008,34 adult patients (18-65 years),PS ≥70,with newly diagnosed GBm received TmZ 75 mg/m2 plus RT up to 60 Gy,followed by TmZ 175 mg/m2 5 days every 4 weeks for 12 *** methylation-specific PCR assay,mGmT protein expression,and Ki-67 expression using immunohistochemistry (IHC) were performed on the tissue *** patients were followed by mRI while mR spectroscopy (mRS) was performed for the stable cases or to confirm progression and accordingly Bevacizumab 10 mg/kg every 2 weeks was added to 7 patients till further progression was ***:31 cases were evaluable,12 (38.7%) had unmethylated mGmT,while 19 (61.3%) were *** cases (55%) were mGmT immunonegative while 14 cases (45%) were *** cut off value of Ki-67 LI in relation to survival was 17%,where 15 were mZ and was repeated every 2 *** overall disease control rate (ODC) was 74.2%,where 2 patients had complete response (CR),14 had partial response (PR),and 7 had stable disease (SD),while 8 (25.8%) had progressive disease (PD).The ODC was significantly higher among methylated patients and in those with Ki-67 median overall TTP was 12 months and the median OS was 20 months for all the patients including those who received Bevacizumab for some stable cases or as a salvage treatment in patients with good PS,the mGmT-methylated patients had a higher median TTP of 13 months (range 8 to 18 months,95% CI

The main objective of this paper is to estimate the plume dispersion parameters in lateral direction (σy) and vertical direction (σz) by using power law wind speed and the scheme of eddy diffusivity in unstable condition. Comparison among our model and algebraic [1] and integral [2] formulations were held. We find that our model and two other models are in agreement with observed data.
