Liver transplantation in patients with incidental hepatocellular carcinoma/cholangiocarcinoma and intrahepatic cholangiocarcinoma： a single-center experience

BACKGROUND： Reports of fiver transplantation （LT） in patients with mixed hepatocellular carcinoma/cholangiocarcinoma （HCC/CC） and intrahepatic cholangiocarcinoma （ICC） are modest and have been mostly retrospective after pathological categorization in the setting of presumed HCC. Some studies suggest that patients undergoing LT with small and unifocal ICC or mixed HCC/CC can achieve about 40%-60% 5-year post-transplant survival. The study aimed to report our experience in patients undergoing LT with explant pathology revealing HCC/CC and ICC. mETHODS： From a prospectively maintained database, we performed cohort analysis. We identified 13 patients who underwent LT with explant pathology revealing HCC/CC or ICC. RESULTS： The observed recurrence rate post-LT was 31% （4/13） and overall survival was 85%, 51%, and 51% at 1, 3 and 5 years, respectively. Disease-free survival was 68%, 51%, and 41% at 1, 3 and 5 years, respectively. In our cohort, four patients would have qualified for exception points based on updated HCC Organ Procurement and Transplantation Network imaging guidelines. CONCLUSIONS： Lesions which lack complete imaging characteristics of HCC may warrant pre-LT biopsy to fully elucidate their pathology. Identified patients with early HCC/CC or ICC may benefit from LT if unresectable. Additionally, incorporating adjunctive perioperative therapies such as in the case of patients undergoing LT with hilar cholangiocarcinoma may improve outcomes but this warrants further investigation.

I discuss several recent highly accurate theoretical predictions for masses of baryons containing the b quark,especially Ωb （ssb） very recently reported by CDF.I also point out an approximate effective supersymmetry between heavy quark baryons and mesons and provide predictions for the magnetic moments of Λc and Λb .Proper treatment of the color-magnetic hyperfine interaction in QCD is crucial for obtaining these results.

The principle of the stretch reducing process is analyzed and three models of pass design are established. The simulations are done about variables, such as, stress, strain, the stretches between the stands, the size parameters of the steel tube, and the roll force parameters. According to its product catalogs the system can automatically divide the pass series, formulate the rolling table, and simulate the basic technological parameters in the stretch reducing process. All modules are integrated based on the developing environment of VB6. The system can draw simulation curves and pass pictures. Three kinds of database including the material database, pass design database, and product database are devised using microsoft Access, which can be directly edited, corrected, and searched.

The effectiveness of phosphodiesterase type 5 inhibitors （PDE5-1s） for erectile dysfunction （ED） varies considerably among trials, but available studies investigating the factors that affect the effectiveness are few and findings are not consistent. A systematic search was performed in Pubmed, Cochrane Library, and EmBASE to identify randomized controlled trials comparing PDE5-1s with placebo for the treatment of ED. The methodological quality of included studies was assessed by the Cochrane Collaboration＇s tool for assessing risk of bias. The associations between prespecified study-level factors and effectiveness were tested by a random effects meta-regression model. This study included 93 trials with 26 139 patients. When all PDE5-1s were grouped together, Caucasian ethnicity was associated with 15.636% （95% confidence interval [CI]： 0.858% to 32.579%） increase in risk ratio （RR） for Global Assessment Questionnaire question-1 （GAQ-1）, and 1.473 （95% CI： 0.406 to 2.338） score increase in mean difference （mD） for posttreatment International Index of Erectile Function-Erectile Function domain score （IIEF-EF）, compared to Asian ethnicity. A one-score increase in baseline IIEF-EF was associated with -5.635% （95% CI： -9.120% to -2.017%） reduction in RR for GAQ-1, and -0.229 （95% CI： -0.425 to -0.042） score decrease in mD for posttreatment IIEF-EF. In conclusion, PDE5-1s are more effective in Caucasians than Asians, and in patients with more severe ED.

Advances in biomimetic three-dimensional(3D) melanoma models have brought new prospects of drug screening and disease modeling, since their physiological relevancy for recapitulating in vivo tumor architectures is more accurate than traditional two-dimensional(2D) cell culture. Gelatin methacryloyl(GelmA) is widely used as a tissue-engineered scaffold hydrogel for 3D cell culture. In the present study, an in vitro 3D malignant melanoma model based on Gel mA was fabricated to evaluate the efficiency of hypericin(Hy)-loaded microemulsion(mE) in photodynamic therapy against melanoma. The mE was produced by the spontaneous emulsification method to enhance the bioavailability of Hy at tumor sites. Hy-loaded mEs were applied to a 3D malignant melanoma model made using 6% Gel mA and the co-culture of B16F10 and Balb/c 3T3 cells,followed by crosslinking using violet light(403 nm). The observation revealed excellent cell viability and the presence of F-actin cytoskeleton network. Hy-loaded mEs exhibited higher phototoxicity and cell accumulation(about threefold) than free Hy, and the cells cultured in the 3D system displayed lower susceptibility(about 2.5-fold) than those in 2D *** findings indicate that the developed mEs are potential delivery carriers for Hy;furthermore, Gel mA hydrogel-based modeling in polydimethylsiloxane(PDmS) molds is a user-friendly and cost-effective in vitro platform to investigate drug penetration and provide a basis for evaluating nanocarrier efficiency for skin cancer and other skin-related diseases.

In the present paper, the elastic scattering of6 Li + 209 Bi system is reanalyzed by using the double folding model (DFm) at energies near the Coulomb barrier (ELab=29.9 and 32.8 meV). With this goal, a new density distribution of6 Li nucleus, the no-core full configuration (NCFC) density distribution (DD), is used to obtain the real potentials in DFm calculations. The NCFC DD results are compared with the results of both gaussian shape (GS) DD and an earlier study as well as the experimental data. This comparison provides information about the similarities and differences of the models used in calculations.
