Rapid sequence induction(RSI) in trauma patients: Insights from healthcare providers

BACkGROUND: We aimed to describe the current practice of emergency physicians and anaesthesiologists in the selection of drugs for rapid-sequence induction(RSI) among trauma ***: A prospective survey audit was conducted based on a self-administered questionnaire among two intubating specialties. The preferred type and dose of hypnotics, opioids, and muscle relaxants used for RSI in trauma patients were sought in the questionnaire. Data were compared for the use of induction agent, opioid use and muscle relaxant among stable and unstable trauma patients by the intubating ***: A total of 102 participants were included; 47 were anaesthetists and 55 were emergency physicians. Propofol(74.5%) and Etomidate(50.0%) were the most frequently used induction agents. Significantly higher proportion of anesthesiologist used Propofol whereas, Etomidate was commonly used by emergency physicians in stable patients(P=0.001). Emergency physicians preferred Etomidate(63.6%) and ketamine(20.0%) in unstable patients. The two groups were comparable for opioid use for stable patients. In unstable patients, use of opioid differed significantly by intubating specialties. The relation between rocuronium and suxamethonium use did change among the anaesthetists. Emergency physicians used more suxamethonium(55.6% vs. 27.7%, P=0.01) in stable as well as unstable(43.4 % vs. 27.7%, P=0.08) ***: There is variability in the use of drugs for RSI in trauma patients amongst emergency physicians and anaesthesiologists. There is a need to develop an RSI protocol using standardized types and dose of these agents to deliver an effective airway management for trauma patients.

Nature of urban interventions in changing the old center of a globalizing Doha

Doha, Qatar is continuously positioning itself at the forefront of international urbanism with different qualities of expression in terms of economy, culture, and global outlook, and is characterized by fast-tracked urban development process with large-scale urban interventions in the old center. Although the unprecedented urban growth of this city continues to be a subject of discussion, little attention has been given to investigate the new interventions and the resulting effects they have on the old center. This study aims to examine three important urban interventions, namely, the museum of Islamic Art, the reconstruction of the traditional market called Souq Waqif, and the msheireb urban regeneration project. It examines local and global issues, universal standard practices, and traditional knowledge. This study employs a descriptive analysis of these interventions to explore the impact of change in the old center, exemplified by socio-spatial and typo-morphological aspects. Reference is made to a number of empirical studies, including behavioral mapping, GIS population statistics, and analysis of historical maps. Results analytically narrate the reactions of these interventions to the possibility of simultaneously adopting universal practices with local knowledge, and whether prioritizing local influences would represent narrow-mindedness in shaping the city.

Pulmonary hypertension Induced by Thalidomide (and Derivatives) in Patients with multiple myeloma: A Systematic Review

Thalidomide is widely used in the treatment of multiple myeloma (mm). In recent years, several cases of pulmonary hypertension have been reported following treatment with thalidomide. The aim of this review was to evaluate the published literature on multiple myeloma patients with pulmonary hypertension following thalidomide treatment. A literature search was performed between 2000 and 2016. A total of 7 eligible studies were identified and deemed eligible, including 11 cases—approximately 37% (4 cases) with IgA (k), 27% (3 cases) with IgG (λ) mm, 27% (3 cases) with IgG (k) mm, and one case (9%) with primary plasma cell leukemia (PPCL). The vast majority of cases—82% (9 cases)—are associated with thalidomide, while only 18% (2 cases) are related to thalidomide derivatives (lenalidomide and pomalidomide). In conclusion, pulmonary hypertension induced by thalidomide or derivatives in multiple myeloma (mm) patients is related to a multifactorial etiology including the pathophysiology of the disease, thromboembolic events, preexisted cardiovascular conditions, comorbidities, and combination with other chemo- or bio-therapeutic agents. mm patients should be evaluated for signs and symptoms underlying cardiopulmonary disease before initiating, and during treatment with thalidomide.

Patient Peak Skin Dose and Dose Area Product from Interventional Cardiology Procedures

Information about the peak skin dose and Dose Area product (DAP) from percutaneous transluminal coronary angioplasty (PTCA) and coronary angiography (CA) was collected from three catheter application rooms. The range of maximum photon energy was 50 - 125 kVp and the fluoroscopy time was 0.6 - 52 seconds. Values of up to 143 Gy·cm2 for DAP and 0.752 mGy for cumulative dose (CD) were found in CA procedures. Otherwise the DAP and CD for PTCA were found to be 143 Gy·cm2 and 2.287 mGy respectively in 3rd Quartile. The relation between the fluoroscopy time and the DAP is also considered. Objectives: The objective of this study is to obtain information about patient peak skin doses (PSD), dose area product (DAP), Fluoroscopy Time (FT) and Cumulative Dose (CD) from PTCA and CA which is the most predominant with respect to high skin doses in addition to other procedures. The aim of this study is also to assess the radiation dose received by patients undergoing interventional radiology procedures, by identifying the procedures that deliver the highest doses. This study is also helpful to establish the reference dose level for adult patients undergoing interventional procedure, and to provide recommendations on how to reduce dose on selected procedures that have been identified to deliver patient dose values near the ICRP (International Commission on Radiological Protection) threshold values.

Assessment of Radiation Dose for Non-Radiation Workers in the medical Field Practices

Radiation protection programs aims to reduce the radiation dose to the lowest possible level under the Dose Limit (DL) limit by the national or international laws, while the dose monitoring programs working as scale used to evaluating the efficiency of these programs and tools. In this study, the average of the annual Eff dose for the intensive care units at hamad General hospital (hGh) is less than the 50% of DL. It was aiming also to evaluate the efficiency of the radiation safety requirements (especially the shielding Adequacy) for the non radiation workers at oncology centers, hence several monitors were installed in chosen locations outside the radiation treatment machine from 2007 to 2011.

Colonoscopy in Elderly Patients with hemorrhoidal Disease and Average-Risk for Colorectal Cancer, a Qatari Community hospital Experience

Introduction: hemorrhoids are a frequent and familiar concern of patients in the general practice and surgery settings. Colonoscopy is both diagnostic and therapeutic. In Qatar, There are little data available about coincidental pathology in elderly patients with hemorrhoids. Objective: To assess the outcome of colonoscopy in management patients with hemorrhoidal disease and average-risk for colorectal cancer and its clinical significance. method: A retrospective study of 200 patients with hemorrhoids with average risk colorectal cancer (anal bleeding and anal symptoms) and subjected to colonoscopic examination at Al khor hospital’s endoscopy unit during the period from may 2005 till August 2012. Patients below 50 years and high risk patients for colorectal cancer;with the following alarm signs: Positive personal history of colorectal neoplasms or Inflammatory Bowel Disease (IBD), positive first degree family history of colorectal neoplasms, history of altered bowel habits, recent significant weight loss, presence of iron deficiency anemia, and history of previous colonoscopy were excluded. All significant endoscopic co-findings (diverticuli, polyps, cancer, angiodysplasia and varices, or colitis) were recorded. Results: There were 200 patients;134 male, 66 female;mean patient age was 56.3 years (range, 50 to 82 years), who met the eligibility criteria. 200 colonoscopies were performed. Evaluation of these patients revealed Polyps in 16 patients (13 benign and 3 malignant), seven malignant lesions(one annular lesion and 6 fungating masses), Ulcerative colitis in 5 patients, no crohn’s disease, diverticular disease in 4 patients, no Vascular malformations, bleeding piles in 3 patients controlled by injection, and 65.5% of patients were free from any additional pathology. Colonoscopy changed the treatment plan in 58 patients (29%). No complications were encountered. 目的:报告16例颅内结核瘤患者的临床、影像学表现及抗结核治疗的结果。设计:本文共纳入苏丹khartoun市全国神经系统疾病中心连续观察的16例脑结核瘤患者。诊断则依据其临床、头颅影像学特征及对抗结核治疗有效等方面。本组中共有7例具有脑组织或脑外组织的病理组织学资料。结果与结论:本组病例最常见的临床表现是头痛(100%)、全身性抽搐(68.7%)及偏瘫(56.2%)。头颅横断面电子计算机体层摄影(CT)/核磁共振成像(mRI)显示单个或多个明显增强的病变及病灶周围水肿。经抗结核治疗后,多数存活患者(13/15)病变完全吸收。病变部分吸收与就诊延迟、颅内多个大病灶及晚期粟粒性结核病有关。我们主张对颅内结核瘤,即使是拟诊病例,应早期经验性试用抗结核治疗,尤其在结核病高发地区。
