Objective:To evaluate the impact of duration of untreated symptoms in childre n with juvenile dermatomyositis(JDm)on clinical and laboratory status at diagn *** design:We examined physical and laboratory data from the first phys ician visit for 166 untreated children with *** activity scores(DASs)assessed skin and muscle *** and weight were compared with the N ational Health and Nutrition Examination Survey iii *** of untreat ed illness was designated as the time from first sign of rash or weakness to dia gnostic ***:Boys and girls with untreated JDm were shorter and light er than national norms(P >.0005 for both),and nonwhite children were weaker than wh ite children(P >.0005).Older children had more dysphagia(P =.017)and arthr itis(P >.001).Duration of untreated JDm was negatively associated with DAS we akness(P >.0005),unrelated to DAS skin,and positively associated with pathol ogical calcifications(P =.006).With untreated disease months,serum lev els of 4 muscle enzymes(aldolase,lactic dehydrogenase,creatine kinase,serum glutamic-oxaloacetic transaminase/aspartate aminotransferase)tended toward nor mal(P >.01 for each).Conclusions:Duration of untreated symptoms is an import ant variable and should be included in decisions concerning both diagnostic crit eria and intensity of therapy for children with JDm.

目的探讨CAR-T细胞治疗中枢神经系统白血病（CNSL）的有效性和安全性。方法报道两例合并CNSL白血病患者抗CD19 CAR-T细胞治疗的过程及结果，并对相关文献进行复习。结果两例患者骨髓细胞形态学特征分别为伴B淋系表达的急性髓系白血病（AmL）-m2和急性B淋巴细胞白血病（B-ALL），免疫分型符合AmL-m2和B-ALL的异常表现，临床和实验室检查符合CNSL诊断标准，诊断明确后予以抗CD19 CAR-T细胞治疗，输注总量均为5.0×106/kg。回输后两例患者中枢神经系统症状缓解。例1影像学异常消失，治疗过程中出现细胞因子释放综合征（CRS），药物干预后好转；例2脑脊液微小残留病转阴，未出现明显CRS反应。结论CAR-T细胞治疗有望缓解CNSL，改善患者预后。

*** diameter,tumour height,and tumour volume are considered important pro gnostic indicators of survival in choroidal *** this study,we inves-tigated the prognostic impact on sur vival of the easily cal-culated volume estimate based on the assumption of a half-rotation *** largest tumour diam-eter and tumour height were measured by ultrasound A-and B-scan in 93patients with choroidal *** volume was calculated by the half volume of a rotation el-lipsoid formula,rotated around the y-axis,and compared to tumour diameter and tumour height.A ll parameters were correlated to the clinical outcome o f the *** the time of diagnosis,the mean dia meter was 10.4mm(range 4.1-18.9mm),and the mean height was 5.7mm(range 1.74-14.9mm).The range of the calculated tu-mour volume was between 11and 628mm 3 (mean volume 190mm 3 ).Among all patients,distant metastases occurred in 10patients(10.8%).in a univariate approach using mantel-Haenszel log-rank test,the calculated tumour vol-ume at the time of diagnosis was the best prognostic indicator of survival followed by tu mour diameter and tu-mour height (P=0.028).When tumour volume,hori-zontal and vertical tumour diameter,age,sex,and primary tumour therapy were considered in a multivariate approach using Cox proportional Hazard model,only tumour volume turned out as a significant prognostic parameter(P=0.001).*** tumour volume is a bet-ter prognostic indicator of survival of patients with choroidal melanomas than the largest tumour di ameter and tumour height,and might be established in d aily routine.

BACKGROUND: The available drugs to treat neuropathic pain have incomp lete efficacy and dose-limiting adverse effects. We compared the efficacy of a combination of gabapentin and morphine with that of each as a single agent in patients with painful diabetic neuropathy or postherpetic neuralgia. mETHODS: in this randomized, double-blind, active p lacebo-controlled, four-period crossover trial, patients received daily active placebo (lorazepam), sustained-release morphine, gabapentin, and a combination of gabapentin and morphine -each given orally for five weeks. The primary outc ome measure was mean daily pain intensity in patients receiving a maximal tolera ted dose; secondary outcomes included pain (rated according to the Short-Form m cGill Pain Questionnaire), adverse effects, maximal tolerated doses, mood, and q uality of life. RESULTS: Of 57 patients who underwent randomization (35 with dia betic neuropathy and 22 with postherpetic neuralgia), 41 completed the trial. me an daily pain (on a scale from 0 to 10, with higher numbers indicating more seve re pain) at a maximal tolerated dose of the study drug was as follows: 5.72 at b aseline, 4.49 with placebo, 4.15 with gabapentin, 3.70 with morphine, and 3.06 w ith the gabapentin-morphine combination (P mbination vs. place bo, gabapentin, and morphine). Total scores on the Short-Form mcGill Pain Quest ionnaire (on a scale from 0 to 45, with higher numbers indicating more severe pa in)-at a maximal tolerated dose were 14.4 with placebo, 10.7 with gabapentin, 1 0.7 with morphine, and 7.5 with the gabapentin-morphine combination (P mbination vs. placebo, gabapentin, and morphine). The aim of this study was to assess the quality of colonoscopic procedures in our endoscopy unit with the goal of improving performance. methods - We prospe ctively audited 500 consecutive colonoscopic procedures and assessed sixty- two process or outcome indicators for each procedure. Results - most of the measu red indicators were within standard limits: cecal intubation rate (92% ), inade quate bowel preparations (24% ) , inappropriate procedures (9.7% ), normal pro cedures (54% ), yield for neoplasia (32% ), morbidity (0.4% ), and overall pa tient satisfaction (95.8% ). Some indicators were outside standard limits sugge sting our practices should be modified: endoscopy withdrawal time less than 6 mi nutes (78% ), forceps removal of polyps (31% ), resected polyps not recovered for pathological examination (12% ), adenomas with villous elements (22% ) , p atients unsatisfied because of time spent waiting for the procedure (19% ), pat ients unsatisfied because of inadequate explanations (10% ). There was no stand ard for a few indicators: patient discomfort (6.9% ), diagnostic success (89% ), therapeutic success (92% ). Three new indicators were proposed: proportion of patients aged m ber of normal colonoscopic procedures to perform to detect one advanced adenoma or cancer, and proportion of colonoscopic procedures causing discomfort. The dia gnostic yield of colonoscopy was dependent on age, gender, indication and approp riateness of indication but not on the prescriber. Conclusion- This audit allow ed us to evaluate our endoscopic practices and to detect certain shortcomings an d deviations from standards. it enabled us to change some or our practices with the goal of improving the quality of our colonoscopic procedures.
