BACKGROUND Cytoreductive surgery(CRS)with hyperthermic intraperitoneal chemotherapy(HIPEC)for peritoneal carcinomatosis can be performed in two ways:Open or closed abdominal *** To evaluate the impact of HIPEC method on post-operative and long-term survival *** Patients undergoing CRS with HIPEC from 2000-2017 were identified in the United States HIPEC collaborative ***-operative,recurrence,and overall survival outcomes were compared between those who received open vs closed *** Of the 1812 patients undergoing curative-intent CRS and HIPEC,372(21%)patients underwent open HIPEC and 1440(79%)underwent closed *** was no difference in re-operation or severe complications between the two *** HIPEC had higher rates of 90-d readmission while open HIPEC had a higher rate of 90-d *** multi-variable analysis,closed HIPEC technique was not a significant predictor for overall survival(hazards ratio:0.75,95%confidence interval:0.51-1.10,P=0.14)or recurrence-free survival(hazards ratio:1.39,95%confidence interval:1.00-1.93,P=0.05)in the entire *** findings remained consistent in the appendiceal and the colorectal *** In this multi-institutional analysis,the HIPEC method was not independently associated with relevant post-operative or long-term *** technique may be left to the discretion of the operating surgeon.

目的:评价骨质疏松肱骨骨折中Schuhli锁定螺母替代骨水泥增强钢板内固定稳定性的效果。方法:分别用Schuhli锁定螺母配标准螺钉和骨水泥加强螺钉对骨质疏松的肱骨干骨折模型行钢板内固定,在3种加载模式下测试其内固定的强度和稳定性,并与标准固定组进行对比。再在4.5N m扭矩下将肱骨干循环扭转1000次,在同样模式下测试内固定的强度和稳定性。结果:循环扭转前后,锁定螺母组和骨水泥加强组在扭转加载模式下的内固定稳定性均明显高于标准固定组(循环前高出6～14倍,循环后高出3～3.6倍);而两组组间比较,在各加载模式下的内固定稳定性差异均不明显(P>0.05)。在轴压和四点弯曲模式下,两组循环扭转前的内固定稳定性与标准固定组比较亦无明显差异(P>0.05)。结论:使用Schuhli锁定螺母可获得与骨水泥加强同样的稳定性,还可避免水泥外渗和骨质热坏死的发生而影响骨折的愈合。

Prevalence and Risk Factors for Hypertension, Diabetes and Obesity among Lecturers and Support Staff of Bishop Stuart University in mbarara, Uganda

Aim: To establish the prevalence and risk factors for hypertension, diabetes and obesity among teaching and non-teaching staff of Bishop Stuart University, mbarara-Uganda. Background: None communicable diseases (NCDs) are projected to exceed communicable diseases as the most common causes of death by 2030 in Africa. most sub-Saharan African countries however lack detailed countrywide data on hypertension and other NCDs risk factors. Cognizant of Uganda’s recent inclusion of Hypertension and diabetes in the health policy agenda, this study was conducted among the university staff in a rural setting to provide benchmark information for design of appropriate interventions. Study Design: This was a descriptive cross-sectional, institutional based survey design. Place and duration of the study: This study was conducted among the teaching and none teaching staff of Bishop Stuart University mbarara, Uganda from 18th April-6th June 2017. methodology: A structured questionnaire was used to gather social demographic and risk factors data from the university workers at their work stations. Blood pressure of each participant was measured and Hypertension was defined as systolic BP > 140 and/or diastolic (BP) > 90 mmHg. Anthropometric measurements: Body mass index (BmI) was done by weight in (Kg) and Height in (m2) to establish weight levels. Obesity was considered at BmI > 40. A random Blood Sugar (RBS) > 200 mg/dl was considered as diabetic. Epi-info version 7 was used to enter data, analyzed using SPSS version 19. Results: A total of 156 University staff aged 25 - 75 years of both sexes, mean age 42 ± 8 were interviewed of whom 51% were males. About 15% were administrative staff, 55% teaching staff, 3% senior lecturers and professors and 25% non-teaching staff. The prevalences of Hypertension, Diabetes, and obesity were 7.7%, 16%, and 28% respectively. But also majority (60.2%) were found at risk of developing both hypertension and diabetes. 当急性穿孔性溃疡病采取简单修补术并盖大网膜加固缝合法临床证明无效时,可代之以有活力的镰状韧带加固修补缝合术。溃疡病急性穿孔导致急腹症是外科的一种常见病,外科手术方法或简单修补闭合法的选择依据污染范围大小、溃疡病征候以及患者的体质状况而定。自1937年 Graham 首先提出后,急性穿孔性胃溃疡的简单修补闭合术已成为一种普通的方法。他报告了一组51例十二指肠连续溃疡穿孔手术后有一例死亡。他提倡将游离大网膜移植于穿孔处作三针间断缝合,使其产生支撑作用。但有组织水肿和硬化时,采取简单修补缝闭法是冒险的。此外,有些临床情况,大网膜作为一种自原性移植是不适宜的,特别当一些外科手术前摘除大网膜时,或在下腹部有严重粘连,或随年龄或疾病而变薄时,均不适宜。在这种情况下,利用镰状韧带蒂移植于简单修补闭合面已取得成功。在文献中有 Fry 等应用本法治疗十二指肠溃疡穿孔的病例报告。

Testing of Lightning Protective System of a Residential Structure： Comparison of Data Obtained in Rocket-triggered Lightning and Current Surge Generator Experiments

We present a comparison of data obtained during testing of lightning protective system of a residential structure in rocket-triggered lightning experiment at the International Center for Lightning Research and Testing(ICLRT)at Camp Blanding,Florida,and current surge generator experiment at Rzeszow University of Technology in *** different configurations of LPS were tested in Poland with the dc grounding resistances of the entire system 4.09 Ω(LPS 1a),1.65 Ω(LPS 1b),and 2.88 Ω(LPS 2).For LPS 1a with three ground rods the value of the peak current entering the electrical circuit neutral was about 56% of the injected current peak,and for LPS 1b with two additional ground rods and two 5 m long buried horizontal conductors it was about 16%.For LPS 2 with five ground rods interconnected by a buried loop conductor this ratio was 21%.The current waveshapes in the ground rods differed from the injected current waveshapes and the current waveshapes in other parts of the test system,especially,for poorer-grounding LPS *** surge-generator results are consistent with those of triggered-lightning experiments at Camp Blanding,Florida(DeCarlo et al.,2008 [2]).

Laparoscopic vs. Robotic Approach to Partial Colon Resection

We retrospectively collected data on 130 minimally invasive colon resection cases performed by the same surgeon from April 2005 to February 2017, of which 35 were completed laparoscopically and 95 were completed robotically. The first 30 robotic cases were allocated to a Training group to account for the learning needed to stabilize operative ability. The 35 laparoscopic cases and 65 of the 95 robotic cases, starting with the 31st case, were compared. Robotic partial colon resection was significantly more capable than the laparoscopic approach in limiting blood loss (p mph node harvest (p ms of operation time (p milar rates of several aspects of morbidity and mortality. The robotic approach to partial colon resection is a technically capable modality of operation intraoperatively, as shown by increased lymph node harvest and less blood loss, and may provide some benefits to patients postoperatively, evidenced by shorter hospital stays, and potentially an oncologic benefit by obtaining higher lymph node harvest in cancer cases. Another benefit of the implementation of a robotic approach is the increased number of patients that are eligible for and ultimately undergo a safe and successful minimally invasive surgery as compared to a more traditional surgical approach.

13-Lipoxygenases(LOXs)initiate the synthesis of jasmonic acid(JA),the best-understood oxylipin hormone in herbivory ***,the roles of 9-LOX-derived oxylipins in insect resistance remain ***,we report a novel anti-herbivory mechanism mediated by a tonoplast-localized 9-LOX,ZmLOX5,and its linolenic acid-derived product,9-hydroxy-10-oxo-12(Z),15(Z)-octadecadienoic acid(9,10-KODA).Transposon-insertional disruption of ZmLOX5 resulted in the loss of resistance to insect ***5 knockout mutants displayed greatly reduced wound-induced accumulation of multiple oxylipins and defense metabolites,including benzoxazinoids,abscisic acid(ABA),and JA-isoleucine(JA-Ile).However,exogenous JA-Ile failed to rescue insect defense in lox5 mutants,while applications of 1 mm 9,10-KODA or the JA precursor,12-oxo-phytodienoic acid(12-OPDA),restored wild-type resistance *** profiling revealed that exogenous 9,10-KODA primed the plants for increased production of ABA and 12-OPDA,but not *** none of the 9-oxylipins were able to rescue JA-Ile induction,the lox5 mutant accumulated lower wound-induced levels of Ca^(2+),suggesting this as a potential explanation for lower wound-induced *** pretreated with 9,10-KODA exhibited rapid or more robust woundinduced defense gene *** addition,an artificial diet supplemented with 9,10-KODA arrested fall armyworm larvae ***,analysis of single and double lox5 and lox10 mutants showed that ZmLOX5 also contributed to insect defense by modulating ZmLOX10-mediated green leaf volatile ***,our study uncovered a previously unknown anti-herbivore defense and hormonelike signaling activity for a major 9-oxylipin α-ketol.

Assessment of UAV Based Vegetation Indices for Nitrogen Concentration Estimation in Spring Wheat

Unmanned Aerial Vehicles (UAVs) have become increasingly popular in recent years for agricultural research. High spatial and temporal resolution images obtained with UAVs are ideal for many applications in agriculture. The objective of this study was to evaluate the performance of vegetation indices (VIs) derived from UAV images for quantification of plant nitrogen (N) concentration of spring wheat, a major cereal crop worldwide. This study was conducted at three locations in Idaho, United States. A quadcopter UAV equipped with a red edge multispectral sensor was used to collect images during the 2016 growing season. Flight missions were successfully carried out at Feekes 5 and Feekes 10 growth stages of spring wheat. Plant samples were collected on the same days as UAV image data acquisition and were transferred to lab for N concentration analysis. Different VIs including Normalized Difference Vegetative Index (NDVI), Red Edge Normalized Difference Vegetation Index (NDVIred edge), Enhanced Vegetation Index 2 (EVI2), Red Edge Simple Ratio (SRred edge), Green Chlorophyll Index (CIgreen), Red Edge Chlorophyll Index (CIred edge), medium Resolution Imaging Spectrometer (mERIS) Terrestrial Chlorophyll Index (mTCI) and Red Edge Triangular Vegetation Index (core only) (RTVIcore) were calculated for each flight event. At Feekes 5 growth stage, red edge and green based VIs showed higher correlation with plant N concentration compare to the red based VIs. At Feekes 10 growth stage, all calculated VIs showed high correlation with plant N concentration. Empirical relationships between VIs and plant N concentration were cross validated using test data sets for each growth stage. At Feekes 5, the plant N concentration estimated based on NDVIred edge showed one to one correlation with measured N concentration. At Feekes 10, the estimated and measured N concentration were highly correlated for all empirical models, but the model based on CIgreen was the only model that had a one to
