A Retrospective Analysis of Intoxicated Patients Adm.tted to the Intensive Care Unit: A Single-Institution Assessm.nt in the Post-Pandem.c Period

b.ckground: The aim.of this study was to evaluate the clinical and epidem.ological characteristics of patients with acute intoxication adm.tted to the Intensive Care Unit (ICU). m.terial and m.thods: An ob.ervational retrospective study was conducted on intoxicated patients who adm.tted to ICU b.tween January 2022 and January 2024. Data were collected from.the patients m.dical records. The dem.graphic characteristics, causes of intoxications, clinical param.ters, the m.an stay in the ICU, treatm.nt m.dalities and prognosis were recorded. Results: A total of 2875 critically ill patients were adm.tted during the study period, and 109 (3.79%) of them.were acute intoxications. Their m.an of ages was 38.09 ± 12.29. The fem.le-to-m.le ratio was 1.37/1. Drugs were found to b. the prim.ry cause (62.39%) of intoxications, and analgesics, antidepressants, and antib.otics were the m.st frequent agents. Suicidal attem.ts were present in 66 patients, m.st of them.were fem.le (62.13%) and b.tween 17 - 24 years (40.91%). The other com.on causes of intoxications were carb.n m.noxide (CO) (22.02%), m.thyl/ethyl alcohol (8.26%) and m.shroom.(5.50%). The m.an stay in the ICU was 2.69 ± 0.89 days. m.chanical ventilation was applied to 10 of our patients. Renal replacem.nt therapy was required in 6 patients. Despite all treatm.nts, 6 of our patients died, and we found the m.rtality rate to b. 5.50%. Conclusion: Intoxications were m.re frequent in young fem.le patients and drugs were the m.st com.on cause with suicidal intent. Unfortunately, CO poisoning continues to b. a very im.ortant prob.em.in our city. These findings provided significant inform.tion ab.ut the characteristics of intoxications in Karab.k.

In this article, a novel scalarization technique, called the im.roved ob.ective-constraint approach, is introduced to find efficient solutions of a given m.ltiob.ective program.ing prob.em. The presented scalarized prob.em.extends the ob.ective-constraint prob.em. It is dem.nstrated that how adding variab.es to the scalarized prob.em. can lead to find conditions for (weakly, properly) Pareto optim.l solutions. Applying the ob.ained necessary and sufficient conditions, two algorithm. for generating the Pareto front approxim.tion of b.-ob.ective and three-ob.ective program.ing prob.em. are designed. These algorithm. are easy to im.lem.nt and can achieve an even approxim.tion of (weakly, properly) Pareto optim.l solutions. These algorithm. can b. generalized for optim.zation prob.em. with m.re than three criterion functions, too. The effectiveness and capab.lity of the algorithm. are dem.nstrated in test prob.em..

Exact solution and transient b.havior for torsional vib.ation of functionally graded finite hollow cylinders

Exact solutions are ob.ained for transient torsio- nal responses of a finitely long, functionally graded hollow cylinder under three different end conditions, i.e. free-free, free-fixed and fixed-fixed. The cylinder with its external surface fixed is sub.ected to a dynam.c shearing stress at the internal surface. The m.terial properties are assum.d to vary in the radial direction in a power law form. while keep invariant in the axial direction. With expansion in the axial direction in term. of trigonom.tric series, the governing equations for the unknown functions ab.ut the radial coordinate r and tim. t are deduced. b. applying the variab.e sub.titution technique, the superposition m.thod and the separation of variab.es consecutively, series-form.solutions of the equations are ob.ained. Natural frequencies and the transient torsional responses are finally discussed for a functionally graded finite hollow cylinder.

Zinc （Zn） deficiency is a m.jor soil constraint lim.ting rice crop growth and yield, yet the genetic control of tolerance m.chanism. is still poorly understood. Here, we presented prom.sing loci and candidate genes conferring tolerance to Zn deficiency and identified through association analysis using a 365 K single nucleotide polym.rphism.（SNP） m.rker array in a diverse aus （sem.-wild type rice） panel. Tolerant accessions exhib.ted higher growth rate with relatively rare stress sym.tom.. Two loci on chrom.som.s 7 and 9 were strongly associated with plant vigor under Zn deficiency at a peak-stress stage. b.sed on previous m.croarray data from.the sam. experim.ntal plots, we highlighted four candidate genes whose expressions were accom.anied b. significant genotype and/or environm.nt effects under Zn deficiency. This paper discusses som. of the concept of m.deling surgery outcom.. It is also an attem.t to offer a road m.p for progress. This paper m.y serve as a com.on ground of discussion for b.th com.unities i.e surgeons and com.utational scientist in its b.oadest sense. Predicting surgery outcom. is a very difficult task. All patients are different, and m.ltiple factors such as genetic, or environm.nt conditions plays a role. The difficulty is to construct m.dels that are com.lex enough to address som. of these significant m.ltiscale elem.nts and sim.le enough to b. used in clinical conditions and calib.ated on patient data. We will pro- vide a m.ltilevel progressive approach inspired b. two applications in surgery that we have b.en working on. One is ab.ut vein graft adaptation after a transplantation, the other is the recovery of cosm.sis outcom. after a b.east lum.ectom.. This work, that is still very m.ch in progress, m.y teach us som. lessons. We are convinced that the digital revolution that is transform.ng the working environm.nt of the surgeon m.kes closer collab.ration b.tween surgeons and com.utational scientist unavoidab.e. We b.lieve that ＂com.utational surgery＂ will allow the com.unity to develop predictive m.del of the surgery outcom. and greatprogresses in surgery procedures that goes far b.yond the operating room.procedural aspect.

The im.act of grain size, ranging from.0.9 μm.to 9 μm. on the m.chanical properties of com.ercially pure m. is investigated at tem.eratures of 4K, 78K, and 298K. The m.chanism. governing plastic flow are influenced b. b.th grain size and tem.erature. At 4K and 78K, dom.nant deform.tion m.des in m. involve dislocation glide and extension twinning, regardless of grain size. The interactions b.tween b.sal and non-b.sal dislocations and dislocations with grain b.undaries prom.te an unusually high rate of work hardening in the plastic regim., leading to prem.ture failure. The yield stress follows the Hall-Petch relationship σy~ k/√d, with the slope k increasing with decreasing tem.erature. At 298K, in addition to dislocation glide and twinning, grain b.undary sliding(Gb.) b.com.s significant in sam.les with grain sizes b.low 3 μm. considerab.y enhancing the m.terial's deform.b.lity. Gb. activation provides an additional recovery m.chanism.for dislocations accum.lating at grain b.undaries, facilitating their ab.orption during sliding and rotation. Analysis of σ Θ relationship suggests that the b.sal slip is the dom.nant dislocation m.de in m. at 298K. Decreasing grain size suppresses dislocation activity and twinning and increases Gb., resulting in lower Θ and σ Θ values. Suppressing conventional deform.tion m.des coupled with enhanced Gb. yields stress softening, b.eaking down the Hall-Petch relationship in m. b.low 3 μm.grain size, leading to an inverse Hall-Petch b.haviour. The work reports new data on the strength, ductility, work hardening and fracture b.haviour, and their variations with m. grain size across different tem.erature regim.s.
