Neurological Involvem.nt in System.c Lupus Erythem.tosus (SLE): Our Recent Experience

Introduction: The central, psychiatric and peripheral neurological m.nifestations of lupus are am.ng the m.st severe visceral disorders and are grouped under the general term.of “neuro-psychiatric system.c lupus erythem.tosus” (NPSLE). We conducted a cross-sectional observational study within our Departm.nt of Internal m.dicine aim.d at describing the clinical and evolutionary aspects of central neurological disorders of SLE, excluding lupus m.elopathy. Patients and m.thods: This was a retrospective and observational cross-sectional study carried out from.1 January 2015 to 31 October 2017, in the Departm.nt of Internal m.dicine of Aristide le Dantec University Hospital in Dakar (Senegal). All patients hospitalized during this period who m.t the 1997 ACR classification criteria of SLE and who presented with a central neuropsychiatric syndrom. attributable to SLE (as defined by ACR 1999) were included. Patients with isolated headache, acute m.elitis or secondary neurological involvem.nt attributable to a toxic, m.tabolic, infectious or tum.ur-related cause were excluded from.our study. Results: During the study period, 10 patients with neuropsychiatric lupus involvem.nt were treated at our institution, including 9 wom.n and 1 m.n;the m.dian age was 29 years (20 - 55 years). Neurological involvem.nt occurred during the course of lupus evolution in 9/10 cases. The m.dian tim. to SLE evolution was 18 m.nths (0 - 60 m.nths). Neuropsychiatric syndrom.s as defined by the 1999 ACR were com.only associated and m.re than half of our patients had m.ltiple neuropsychiatric syndrom.s. There were 5 cases of confusion syndrom. and com., 4 cases of seizure, 3 cases of psychosis, 2 cases of acute cerebrovascular disease and 1 case of aseptic m.ningitis. Am.ng the extra-neurological m.nifestations of SLE, haem.tological and derm.tological involvem.nts were com.on. Renal involvem.nt affected half of the patients. Long-Term.Oral Corticosteroid-Therapy: Description of the Practice of Adjuvant m.asures by Dakar m.dical Specialists

Introduction: The extended adm.nistration of corticoids by oral way for a length of m.re than 3 m.nths defines the long-term.corticosteroid-therapy. This one, used in num.rous indications, displays m.st often at the risk of undesirable effects linked som.tim.s to the habits of prescription of the doctors. Patients and m.thods: In order to study the prescription m.dalities of this treatm.nt, we conducted a cross-sectional, m.lticentric and descriptive study from.June 1st, 2017 to August 1st, 2017, over a period of 2 m.nths. It involved a questionnaire given to m.dical specialists of all m.dical specialties and practicing in the University Hospital of Dakar. Results: 170 doctors were interviewed. Derm.tologists and internists were m.stly found 19.4% and 18.8% or 33% and 34% doctors. System.c autoim.une diseases alone accounted for 48% of prescription reasons. Prednisone was prescribed in 88% of cases. The im.unosuppressive dose of 1 m./kg was m.st often prescribed. Practitioners very heterogeneously prescribe m.st adjuvant m.asures to prolonged system.c corticosteroid therapy. Thus, the recom.endation of a low-sodium.diet (38% of physicians), and the system.tic prescriptions of proton pum. inhibitors (44.7% of physicians) and vitam.n-calcium.supplem.ntation were frequently perform.d by physicians (34% of physicians). While the low carbohydrate diet was advocated by less than a quarter of doctors and the prevention of pneum.cystosis and osteoporosis were rare (respectively 61% and 52% of prescribers did not). Conclusion: The global analysis of the habits of our m.dical specialists concerning the use of long-term.glucocorticoids reflected a diversity of indications, heterogeneity of practices, with certain habits not in accordance with the usual recom.endations.

Introduction: System.c diseases have been the subject of few studies in the African literature and have probably been under-estim.ted. The objective of our study was to specify their spectrum. their epidem.ological aspects and diagnostic delay in Internal m.dicine Departm.nts of Dakar (Senegal). m.terial and m.thod: It was a m.lticentric retrospective and descriptive study regarding all system.c diseases during 119 m.nths from.1st January 2005 to 30 Novem.er 2014 in 5 hospital centers down Dakar. System.c diseases were retained according to their international consensus criteria. Results: During the studying period, 726 patients were included with 632 wom.n and 94 m.n (sex ratio of 0.14). The average age was 43.76 years. Inflam.atory rheum.toid fam.ly history was noted in 10.06% of cases. Rheum.toid arthritis (RA) was the predom.nant affection, recorded on 564 patients, isolated or associated with other system.c diseases. It was followed in a decreasing order, in the system.c auto-im.une diseases sub-groupe, by system.c lupus (56 cases), Sj?gren’s syndrom. (32 cases), System.c Sclerosis (26 cases), Idiopathic inflam.atory m.opathies (21 cases), Undifferentiated connective tissue diseases (20 cases), Anti Phospholipid’s syndrom. (6 cases) and m.xed connective tissue disease (6 cases). A diagnosis of system.c vasculitis was recorded in 19 patients. The other system.c affections were represented by system.c sarcoidosis (8 cases), Adult-onset Still’s disease (03 cases), am.loidosis (02 cases) and 02 cases of system.c syndrom. associated to im.unodeficiency. The m.an diagnostic delay duration before the diagnostic was 3.46 years. Conclusion: System.c diseases in internal m.dicine are characterized by their diversity, the clear predom.nance of RA, and significant diagnostic delay.

Osgood Schlatter Disease: Study of 2 Observations in Conakry, Guinea

Introduction: OSD is a growth osteochondrosis affecting the apophyseal cartilage of the anterior tibial tuberosity of the knee, 20 to 30% of young growing athletes suffer from.this pathology, described by Osgood and Schlatter in 1903. Objective: To report 2 observations and review the literature. Observation 1: This was a 14-year-old Guinean teenager, Halpoular, living in Conakry, a right-handed footballer who presented with a painful swelling of the right knee that looked m.chanical, whose father had HLA B27 positive ankylosing spondylitis. X-ray of the right knee showed fragm.ntation of the anterior tibial tuberosities, and ossicle with edem. of the patellar tendons at stage 4 according to Ehrenbord and Lagergren (1961). Thus, the disease of OSD is retained, He received 120 m. of diclofenac, 20 m. om.prazole for 1 m.nth and the cessation of sports activity, the evolution was favorably m.rked by the regression of pain. Observation 2: A 16-year-old girl, initials m.L.T, Guinean, residing in Coyah (Republic of Guinea), a handball player, consulted for intense pain (VAS: 8/10) in her knees after a violent shock received when she hit the post. The physical exam.nation found a painful, soft swelling at the anterior tibial tuberosity of the right knee, the rest of the exam.nation was norm.l. The X-ray of the right knee shows fragm.ntation of the anterior tibial tuberosity, and ossicle with edem. of the patellar tendon at stage 4 according to Ehrenbord and Lagergren 1961, thus the OSD disease is retained, the treatm.nt instituted was 120 m. of diclofenac, the cessation of sports activity which gives a good evolution (EVA at 2/10). Conclusion: OSD is a growth osteochondrosis affecting the apophyseal cartilage of the anterior tibial tuberosity of the knee in young growing athletes, resolving at rest.
