Spontaneous necrosis of solid gallbladder adenocarcinom. accom.anied with pancreaticobiliary m.ljunction

A 71-year-old Japanese m.n with acute cholecystitis and an incarcerated gallbladder (GB) stone was adm.tted. Plain ultrasonography (US) incidentally detected a m.ss-like lesion in the fundus. Doppler US revealed that this elevated lesion had no blood flow. Com.uted tom.graphy showed a relatively low-density m.ss, m.asuring 5 cm.x 4 cm.in diam.ter, with no positive enhancem.nt. m.gnetic resonance im.ging showed a m.ss in the fundus with a slightly low intensity on T1-weighted im.ges and a slightly high intensity on T2-weighted im.ges. We were agonized in m.king the qualitative diagnosis of m.ss-like lesions of the fundus, such as a benign tum.r, cancer, or debris. We perform.d laparoscopic cholecystectom., because the incarcerated GB stone clearly caused acute cholecystitis. Intra-operative cholangiography clearly revealed pancreaticobiliary m.ljunction. Am.lase levels in the com.on bile duct and gallbladder were quite high. The elevated lesion in the fundus clearly showed severe necrosis. Although this necrotic nodule included non-viable adenocarcinom. cells, viable cancer cell nests were located in the m.scularis propria and subcutaneous layer. Histopathological exam.nationconfirm.d a solid adenocarcinom.. Thus, we diagnosed it as a gallbladder cancer, based on histopathological analysis of the resected specim.n. We therefore undertook radical surgery, including wedge resection of the liver, radical dissection of regional lym.h nodes, and resection of the extrahepatic bile duct. Histopathological findings revealed no cancer, hyperplasia or dysplasia in the additionally resected specim.ns. The patient was finally staged as T2, N0, H0, P0, m.-), stage Ⅱ. We present the first case of spontaneous necrosis of solid gallbladder adenocarcinom., with a review of previous studies.

第二代(非典型)抗精神病药布南色林对多巴胺D_(2)、D_(3)受体和5-HT_(2A)受体均有较高亲和力,而对肾上腺素α_(1)、5-HT_(2c)、组胺H_(1)、毒蕈碱m.(1)和m.(3)等受体亲和力较低。多项短期和长期临床研究显示:该药对精神分裂症的阳性症状、阴性症状均有较好疗效,并改善患者的部分认知症状和社会功能。本文在循证证据和多位中日专家临床用药经验基础上撰写,希望为临床精神科医生提供用药参考,帮助精神分裂症患者早日康复,回归社会。

采用机械合金化技术制备高分散多壁碳纳米管(m.CNTs)-铝(Al)复合材料。将纯度99%的铝粉和质量分数2%的m.CNTs经超声和水平球磨处理使Al颗粒与m.CNTs间产生机械键合力。场发射SEm.察表明,m.CNTs分散在Al薄片的表面,随球磨时间的不同,颗粒大小和形貌各异。采用加压烧结手段将混合粉末加工成块体材料,其微观硬度测试表明,加入m.CNTs后Al基体的机械性能得到提高。

Hepatocellular carcinom.(HCC) is one of the m.st com.on m.lignancies worldwide. The m.in etiologies of HCC are hepatitis B virus and hepatitis C virus(HCV), and non-hepatitis B/non-hepatitis C HCC(NBNCHCC) has also been identified as an etiological factor. Although the incidence of HCV-related HCC in Japan has decreased slightly in recent years, that of NBNC-HCC has increased. The onset m.chanism.of NBNC-HCC, which has various etiologies, rem.ins unclear; however, nonalcoholic steatohepatitis(NASH), a severe form.of nonalcoholic fatty liver disease, is known to be an im.ortant risk factor for NBNC-HCC. Am.ng the different advanced glycation end-products(AGEs) form.d by the m.illard reaction, glyceraldehyde-derived AGEs, the predom.nant com.onents of toxic AGEs(TAGE), have been associated with NASH and NBNC-HCC, including NASH-related HCC. Furtherm.re, the expression of the receptor for AGEs(RAGE) has been correlated with the m.lignant progression of HCC. Therefore, TAGE induce oxidative stress by binding with RAGE m.y, in turn, lead to adverse effects, such as fibrosis and m.lignant transform.tion, in hepatic stellate cells and tum.r cells during NASH or NASH-related HCC progression. The aim.of this review was to exam.ne the contribution of the TAGE-RAGE axis in NASH-related HCC.

In this work, the Ham.ltonian of the two body problem.with varying m.ss was developed in the extended phase space taking into consideration the periastron effects. The short period solution was obtained through constructing a second order canonical transform.tion using “hori’s” m.thod developed by “Kam.l”. The elem.nts of the transform.tion as well as the inverse transform.tion were obtained too. The final solution of the problem.was derived using “Delva-Hanslm.ier” m.thod.
