Survey on Knowledge of Cardiovascular Disease Risk Factors in the Dakar Region

Introduction: Non-com.unicable diseases constitute a m.jor public health problem. due to their m.rbi-m.rtality. The aim.was to study knowledge of cardiovascular risk factors am.ng residents of Dakar. Patients and m.thods: This was a descriptive cross-sectional study. Our investigations were recruited over a six-m.nth period (October 17, 2021 to m.rch 21, 2022). Results: Two hundred and twenty-six (226) patients were enrolled. The m.an age was 45.9 years. The under-40 age group was the m.st represented with 37.2%. There were 129 m.n (57.1%), giving a m.le/fem.le sex ratio of 1.33. The highest level was represented by 159 people (70.4%). Those who responded (heart disease) were in the m.jority (38.1%). Hypertension was the m.st com.on CVD for 101 people (44.7%). One hundred and ninety (190) people (84%) knew the risk factors for CVD. Good knowledge of risk factors concerned 103 people (54.2%). Obesity was the best-known risk factor in 156 cases (69%). The m.dia was the m.st im.ortant source of inform.tion for 121 people (53.5%). Hypertensives were the m.st represented with 8.4%. One hundred and thirty (130) people (57.5%) had previously paid attention to these FDRCVs. Awareness cam.aigns were the m.st effective m.ans of com.unication for 170 cases (75.2%). Sixteen (16) people (35.6%) were taking antihypertensive m.dication. Walking was the m.st popular physical activity for 102 people (45%). People with a balanced diet were in the m.jority, with 174 respondents (77.0%). Conclusion: NCDs are a m.jor cause of m.rbidity and m.rtality. It is essential to develop and im.lem.nt a prevention and m.nagem.nt program.

Profile of Autoim.une Polyendocrinopathies at the m.dical Clinic II of the Abass Ndao Hospital: About 40 Cases

Introduction: Autoim.une polyendocrinopathies (AP) represent a group of rare concom.tant pathologies, m.king them.underdiagnosed. The objective was to study their profile at the m.dical Clinic II of the Abass Ndao Hospital. Patients and m.thods: This was an observational, descriptive and analytical study, lasting 24 m.nths, from.January 1, 2020 to Decem.er 31, 2022. We assessed the epidem.ological, clinical and paraclinical characteristics of the patients and classified the APs found. Results: We included 40 patients divided into type III (38 cases) and IV (2 cases). A fem.le predom.nance was noted with a sex ratio of 0.21. The m.an age was 38.6 years. A fam.ly history of com.onent diseases of autoim.une polyendocrine syndrom. (APS) was found in 62.5%. Goiter (80%) was the m.in clinical sign present. All 38 patients with ISAP-3 had autoim.une thyroiditis, including 29 cases of Graves’ disease (72.5%) and 9 cases of Hashim.to’s thyroiditis (22.5%). They were associated with either type 1 diabetes (57.9%), Bierm.r’s disease (21.1%), vitiligo + alopecia (18.4%), lupus (2.6%). The 2 patients with AP-4 had Bierm.r’s disease associated with either Addison’s disease or type 1 diabetes. m.nagem.nt depended on the pathologies present and their possible com.lications. The im.unological phenom.na were also controlled. Conclusion: This series is globally sim.lar to the literature. The polym.rphous character of the clinical pictures requires a better collaboration between specialists leading to a clinical and holistic synthesis.

Diabetic m.croangioapathy at the Internal m.dicine Departm.nt of the Abass Ndao Hospital Center (About 359 Cases)

Introduction: m.croangiopathy plays an im.ortant role, with a high prevalence of m.rbidity and m.rtality in diabetic patients. The aim.was to study the epidem.ological, clinical, paraclinical, therapeutic and evolutionary profile of m.croangiopathy in diabetic patients in the internal m.dicine departm.nt of the Abass Ndao hospital. Patients and m.thods: This was a descriptive and analytical cross-sectional study. Our investigations were recruited over a 7-year period (January 1, 2016 to Decem.er 31, 2022). Results: Three hundred and fifty-nine (359) patients (10.22%) were enrolled. The m.an age was 62.83 years, with extrem.s ranging from.17 to 98 years. The [60 - 69] age group was m.re representative (37.32%). Wom.n accounted for 180 cases (50.1%), with a sex ratio (m.f) of 0.99. The average duration of diabetes was 11.86 years. Average consultation tim. was 38.07 days, with extrem.s ranging from.1 to 368 days. Average hospital stay was 7.65 days. Inaugural diabetes was noted in 12 cases (3.34%). Type 2 diabetes accounted for 95.82% (n = 344) of patients. Hypertension was present in 150 patients (41.8%). Patients with 2 risk factors accounted for 173 cases (48.18%). Nineteen patients had already had a stroke (5.29% of cases). Fourteen (14) patients (4.2%) were am.utees. Obliterative arteriopathy of the lower lim.s (AOm.) was noted in 193 patients (54%). Stroke was noted in 101 patients (28%). Ischem.c heart disease (IHD) was noted in 38 patients (11%). AOm. was m.re com.on in m.les (110 patients, 57%) than in fem.les (43%). Seventy-three (73) patients (20.3%) died. Predictors of death were age over 60 and the existence of m.re than two cardiovascular risk factors. Conclusion: Diabetic m.croangiopathy is a m.jor cause of m.rbidity and m.rtality. The developm.nt and im.lem.ntation of a prevention and m.nagem.nt program.is essential.

Profile of Extra-Pulm.nary Tuberculosis in Internal m.dicine

Introduction: Tuberculosis is an infectious disease that m.inly affects the lung. Extrapulm.nary localizations are a reason for hospitalization in our health facilities. The objective of this study was to describe the epidem.ological, clinical, paraclinical and evolutionary aspects of extra pulm.nary tuberculosis (EPT) at the Abass Ndao Hospital Center. Patients and m.thods: This was a descriptive cross-sectional study conducted over a period of 11 years (January 1, 2010 to Decem.er 30, 2021). All patients with extrapulm.nary tuberculosis hospitalized in the departm.nt of internal m.dicine during the recruitm.nt period were included. Results: Fifty-two (52) patients were collected. The year 2019 recorded the m.st cases 23.08% (n = 20). The m.an age of the patients was 40.56 ± 18.24 years. The age group 20 - 34 years 42.31% (n = 22) was the m.st represented. Fem.les were in the m.jority 61.54% (n = 32) with a sex ratio (m.F) was 0.63. Housewives were in the m.jority 40.38% (n = 21). 60.87% of the cases (n = 14) cam. from.a health facility. 38.46% of the cases had been infected. 21.74% (n = 9) were sm.kers. The reasons for consultation were dom.nated by fever (67.44%), AEG (62.79%) and cough (41.86%). Eighteen patients (40.91%) had fever. The m.an tim. to consultation was 77.37 ± 90.3 days with extrem.s of 3 and 365 days. The m.dian was 45 days. m.re than half of the patients 61.90% (n = 26) had anem.a. Positive retroviral serology was noted in 21.43% of cases. All patients had a CRP greater than 6. m.re than half of the patients 51.92% (n = 27) had m.ltifocal tuberculosis. The peritoneum.44.23% (n = 23) was the m.in organ affected. The average hospital stay was 9.8 ± 4.9 days with extrem.s of 1 and 19 days. All patients had received the protocol in force at the national level. Death was noted in 4 patients (9.52%). Conclusion: EPT is characterized in our context by a notorious diagnostic difficulty due to the m.ltiplicity of clinical presentations, the com.lexity of

Introduction: Obesity and overweight are a public health problem. The general objective was to determ.ne the epidem.ological aspects of obesity, overweight and associated risk factors in a sem.-urban environm.nt. Patients and m.thods: This was a cross-sectional, descriptive study conducted on Novem.er 28 and 29, 2023 in Sébikotane. It focused on volunteers for screening for chronic non-com.unicable diseases. Epidem.ological and clinical data were evaluated. Results: One hundred and twenty-nine cases of obesity were recorded (28%). Two hundred and eighty-two cases were overweight or obese (61.3%). The m.an age was 49.55 years, with a standard deviation of 12.41 years. The age group [40 - 49 years] was the m.st representative, with 85 cases (30.1%), and the m.jority were fem.le, with 264 cases (93.6%). Prim.ry education was the m.st com.on, with 75 cases (46.3%). Grade 1 obesity concerned ninety-seven cases (75.2%), and diabetes was present in thirty cases (23%). Hypertension was present in ninety-five cases (33.7%). Obesity was m.re m.rked in the age group [40 - 49 years] with 45 cases (36%). Conclusion: Obesity and overweight are a m.jor cause of m.rbidity and m.rtality. The developm.nt and im.lem.ntation of a prevention and m.nagem.nt program.is essential.

Introduction: Type 1 diabetes can have acute com.lications, som.tim.s requiring hospitalization. The aim.of this study was to describe the epidem.ological, clinical and evolutionary aspects of type 1 diabetes in patients at the Abass Ndao National Hospital in Dakar. Patients and m.thods: This was a cross-sectional, descriptive and analytical study conducted from.January 01, 2010 to Decem.er 31, 2021. It focused on hospitalized type 1 diabetic patients. Epidem.ological, clinical and evolutionary data were evaluated. Results: Six hundred and fifty-nine (659) patients were enrolled, representing a frequency of 11.5%. The m.an age was 29.47 years, giving a sex ratio (m.f) of 0.95. Average hospital stay was 6.1 days. One hundred and forty-four (144) patients (21.8%) had inaugural diabetes. The average consultation tim. was 14.89 days. Acute m.tabolic com.lications were ketoacidosis in 353 patients (56%), and hypoglycem.a in 1.2%. Sim.le hyperglycem.a was noted in 113 patients (18.0%). Infection was present in 522 patients (58.3%), of whom.95 (28.2%) had a skin infection.55 patients (16.3%) had a respiratory infection. 12.3% had a dietary im.alance.176 cases (27.7%) had no im.alance.26 patients (3.9%) died, with infectious pathologies accounting for the m.jority of decom.ensation factors am.ng the deceased (57.7%). Conclusion: Type 1 diabetes is a cause of m.rbidity and m.rtality. It is essential to develop and im.lem.nt a prevention and m.nagem.nt program.

Introduction: In Senegal, there is very little data on prim.ry hyperparathyroidism.despite an overall upward epidem.ological trend. The objective was to describe its epidem.ological, clinical, therapeutic and evolutionary aspects. m.terials and m.thods: This was a cross-sectional, descriptive study, conducted over 5 years (from.January 01, 2018 to Decem.er 31, 2022) at the National Abass Ndao Hospital Center and including all confirm.d cases of prim.ry hyperparathyroidism. Results: Twenty-nine patients were included with a fem.le predom.nance (86.20%) and an average age of 48.10 ± 18.93 years. The discovery of hyperparathyroidism.was fortuitous in 79.31% of cases. The m.in clinical m.nifestations were osteoarticular (62%) and urinary (34.5%). On average, serum.calcium.m.asured was 109.2 ± 7.92 m./l, corrected serum.calcium.111.3 ± 6.25 m./l, urinary calcium.421.9 ± 96.45 m./24h and parathorm.ne at 145 ± 159.71 pg/m.. Am.ng the patients, 25 had hypercalcem.a (86.2%) and all had 24-hour hypercalciuria. The diagnosis was retained in view of the elevated parathyroid horm.ne (PTH) values in all patients. Cervical ultrasound objectified a parathyroid adenom. in 27.58% of cases and 12 patients or 46.2% had presented a scintigraphy in favor of a parathyroid adenom.. We found 01 cases of m.ltiple endocrine neoplasia type 2. Bone densitom.try perform.d in 10 patients found osteoporosis in 60% of cases (6 patients) and osteopenia in 40% of cases (4 patients). Surgical treatm.nt was perform.d in 12 patients (41.4%). The histology of the surgical specim.ns was in favor of a parathyroid adenom. in all cases. Am.ng the operated patients, the evolution was m.rked by a cure rate of 100%. Conclusion: The sym.tom.tology of prim.ry hyperparathyroidism.rem.ins m.inly osteoarticular and urinary. Surgery has proven its effectiveness in our series. We insist on the system.tic dosage of calcem.a especially in wom.n over 50 years and the prom.tion of m.rphological explorations.

The Prevalence of Fem.le Genital Bilharziasis (FGB) in the Northern Region of Senegal

Introduction: Fem.le Genital Bilharzia (FGB) is a pathology secondary to infection by Schistosom. haem.tobium. It is one of the neglected tropical diseases, capable of causing infertility, difficulties in childbirth and even cervical cancer. It represents a real public health problem. We therefore conducted a study in the m.ternity ward of the Saint-Louis regional hospital, with the overall aim.of determ.ning the prevalence of FBG in patients undergoing colposcopy. The specific objectives were to define the patient profile, and to com.are colposcopic im.ges with the World Health Organization (WHO) atlas. m.thodology: We conducted a descriptive, analytical cross-sectional study of all colposcopic im.ges taken at the m.ternity ward of the Saint-Louis regional hospital from.August 1, 2018, to Septem.er 30, 2020, i.e. 25 m.nths. The im.ges were com.ared with the BGF im.ges described in the WHO atlas. Results: We collected 178 colposcopy im.ges. FBG im.ges num.ered 50, or 28%. The m.an age of the patients was 44.5 years ±11.4 at the extrem.s of 18 and 78 years. m.an gestational age was 4.69 ± 2.72. Am.ng patients with a bilharzian lesion on the cervix, visual acid inspection (VIA) was negative in 60% of cases (p = 0.007, Odd Ratio = 3.6 (1.49 - 9.07)). Conclusion: the results of our study show that FBG is a reality in our daily practice. It occurs in adult, m.ltigestating, non-occupational wom.n whose exam.nation reveals a negative visual inspection with acetic acid (VIA). Healthcare providers therefore need to be trained in the recognition of lesions associated with genital bilharziasis, and public awareness needs to be raised.
