Objective: There is accumulated evidence that mesial temporallobe seizures are preceded by a preictal transition that evolves over minutes to hours. In the pr esent study, we investigated these possible preictal changes in long-term intra cranial recordings of five patients by a measure of phase *** ord er to clearly distinguish preictal changes from all the other interictal states, we developed an automatic extraction of representative patterns of interictal s ynchronization *** reference library was used to classify the successi ve synchronization patterns of long-term recordings into groups of similar patt erns. Altered states of brain synchronization were identified as deviating from patterns in the reference library and were evaluated relative to the times of se izure onset in terms of sensitivity and specificity. methods: A phase-locking m easure was estimated using a sliding window analysis on 15 frequency bands (2 Hz steps between 0 and 30 Hz), for all pairs of EEG channels in the epileptogenic temporal lobe (14-20 channels), over the entire data sets (total analyzed durat ion 305 h). The preictal identification encompasses three basic stages: (1) a pr eprocessing stage involving the determination of a reference library of characte ristic interictal synchronization patterns using a K-means algorithm, and the identification of discriminant variables different iating interictal from preictal states, (2) a classification stage of the synchr onization pattern via a minimum mahalanobis distance to the reference patterns,a s well as detection of outliers, (3) an evaluation stage of the sensitivity and specificity of the detection by receiver-operating characteristic curves. Resul ts: In most of the cases (36 of 52 seizures, i.e. 70%), a specific state of bra in synchronization can be observed several hours before the actual seizure. AIm: To investigate whether performing immuno-histochemical CD3 staining, in order to improve the detection of intra-epithelial lymphocytosis, has an additional value in the histological diagnosis of celiac ***: Biopsies obtained from 159 children were stained by hematoxylin and eosin(HE) and evaluated using the marsh classification. CD3 staining was subsequently evaluated separately and ***: Differences in evaluation between the routine HE sections and CD3 staining were present in 20(12.6%) cases. In 10(6.3%) patients the diagnosis of celiac disease(marsh Ⅱ and Ⅲ) changed on examination of CD3 staining: in 9 cases, celiac disease had initially been missed on the HE sections, while 1 patient had been over-diagnosed on the routine sections. In all patients, the final diagnosis based on CD3 staining, was concordant with serological results, which was not found previously. In the other 10(12.3%) patients, the detection of sole intra-epithelial lymphocytosis(marsh Ⅰ) improved. Nine patients were found to have marsh Ⅰ on CD3 sections, which had been missed on routine sections. Interestingly, the only patient with negative serology had Giardiasis. Finally, in 1 patient with negative serology, in whom marsh Ⅰ was suspected on HE sections, this diagnosis was withdrawn after evaluation of the CD3 ***: Staining for CD3 has an additional value in the histological detection of celiac disease lesions, and CD3 staining should be performed when there is a discrepancy between serology and the diagnosis made on HE sections.

Cardiovascular disease including stroke is a major complication that tremendously increases the morbidity and mortality in patients with diabetes mellitus(Dm). Dm poses about four times higher risk for stroke. Cardiometabolic risk factors including obesity, hypertension, and dyslipidaemia often co-exist in patients with Dm that add on to stroke risk. Because of the strong association between Dm and other stroke risk factors, physicians and diabetologists managing patients should have thorough understanding of these risk factors and management. This review is an evidence-based approach to the epidemiological aspects, pathophysiology, diagnostic work up and management algorithms for patients with diabetes and stroke.

Diabetic foot ulcer (DFU) is a debilitating and severe manifestation ofuncontrolled and prolonged diabetes that presents as ulceration, usually locatedon the plantar aspect of the foot. Approximately 15% of individuals with diabeteswill eventually develop DFU, and 14%-24% of them will require amputation ofthe ulcerated foot due to bone infection or other ulcer-related complications. Thepathologic mechanisms underlying DFU are comprise a triad: Neuropathy,vascular insufficiency, and secondary infection due to trauma of the *** local and invasive care along with novel approaches like stem celltherapy pave the way to reduce morbidity, decrease amputations, and preventmortality from DFU. In this manuscript, we review the current literature withfocus on the pathophysiology, preventive options, and definitive management ofDFU.

PURPOSE: Clinicopathologic staging is even today the best prognostic factor in both colon and rectal cancers. There is still considerable variation in surviva l within the stages. To find other prognostic indicators we investigated six bio logic markers associated with apoptosis and cell proliferation. mETHODS: Formali n-fixed, paraffin-embedded tissue samples of 363 patients with primary colon o r rectal cancer of Dukes Stages A to D were chosen for immunohistochemical stain ing of five tumor markers: bcl-2, p53, Ki-67, cyclin D1, and carcinoembryonic antigen. Also, the number of apoptotic cells was studied by the terminal deoxynu cleotidyl transferase-mediated d-UTP nick end labeling method in 347 cases. Th e study was done on specially prepared tissue arrays. RESULTS: In rectal cancer, patients with a Ki-67 labeling index of 5 percent or higher had a better progn osis than those with a lower index. Also, positive cytoplasmic p53 expression pr edicted a favorable outcome in rectal cancer. In colon cancer, positive nuclear staining of cyclinD1 reflected better survival. Weak and moderate staining of ca rcinoembryonic antigen correlated with better prognosis than strong staining, bu t negative staining predicted poor outcome. High apoptotic index of 100 or highe r correlated with poor prognosis in colon cancer. However, in rectal cancer, the trend was the opposite. Bcl-2 staining tended to be more intense in samples of patients living 5 years or longer compared with those with worse prognosis. CON CLUSIONS: Colon cancer and rectal cancer seem to have different biologic behavio r, at least with respect to apoptosis, cytoplasmic p53 expression, and perhaps K i-67 and carcinoembryonic antigen. Further studies are needed to clarify the si gnificance of these factors.
