Health care has become an essential social-economic concern for all stakeholders(e.g.,patients,doctors,hospitals etc.),health needs,private care and the elderly class of *** massive increase in the usage of health care Internet of things(IoT)applications has great technological evolvement in human *** are various smart health care services like remote patient monitoring,diagnostic,disease-specific remote treatments and *** applications are available in a split fashion and provide solutions for variant diseases,medical resources and remote service *** main objective of this research is to provide a management platform where all these services work as a single unit to facilitate the *** ontological model of integrated healthcare services is proposed by getting requirements from various existing healthcare *** were 26 smart health care services and 26 smart health care services to classify the knowledge-based ontological *** proposed ontological model is derived from different classes,relationships,and constraints to integrate health care *** model is developed using Protégébased on each interrelated/correlated health care service having different *** querying SPARQL protocol and RDF query language(SPARQL)were used for knowledge *** Pellet Reasoner is used to check the validity and relations coherency of the proposed ontology *** to other smart health care services integration systems,the proposed ontological model provides more cohesiveness.

m>Background: m>The integration of different imaging technologies such as computed tomography (CT) and magnetic resonance imaging (mRI) assists clinicians in diagnosing several neurological diseases at an initial stage, thereby inhibiting their development into serious medical conditions. With respect to the severity of neurological diseases and the increasing popularity of imaging tools, the current study has evaluated the findings of CT and mRI in examining neurological disorders. m>methods: m>A retrospective study was conducted at the faculty of applied medical sciences, King abdul-aziz University from September 2017 to 2018 including details of 43 patients with complaints of potential neurological disturbances such as headache, oligodendroglioma, CNS infiltration, hypoxia, weakness, epilepsy and multiple sclerosis. The history of patients including their age, gender, reason for examination and findings of CT and mRI was evaluated in the study. m>Results: m>The results showed that patients in the age group of 21 to 40 years were more prone to developing neurological disorders. The major reasons for neurological examination in individuals were found to be headaches (n = 7), multiple sclerosis (n = 5) and epilepsy (n = 3). The evaluation of patients’ records revealed that only 4 patients underwent CT scans;however, all of them went through the mRI. Statistically significant results were obtained for the patients scanned and examined through mRI rather than CT. m>Conclusions: m>The results revealed mRI as more successful in diagnosing different neurological conditions in comparison with CT scan. The study further suggests that mRI can be used as an integral diagnostic approach for neurological understanding of major diseases of the central nervous system.

A Long-term Follow-up: Suture versus mesh Repair for Adult Umbilical Hernia in Saudi Patients. A Single Center Prospective Study

Objective: To report results of mesh repair vs. the modified mayo’s suture overlap in the surgical treatment of adult umbilical and paraumbilcal hernias in our medical center. Patients & methods: The study is a Saudi single center single surgeon trial composed of sixty two patients. It was performed in the Surgical Department of King abdul-aziz University Hospital at Jeddah. The patients were randomly assigned into 2 groups. Group A patients underwent onlay mesh repair while modified mayo’s repair was used in group B patients. median follow-up was 28 months, and data were collected regarding size of hernia, type of the operation, complications, length of follow-up and the recurrence rate. Chi square test was used to compare results at 0.05 levels. Results: Complication was reported in 17% in group A and 8% in group B .There was no difference in scar pain, cosmetic result, and overall patient satisfaction between both groups. The recurrence rate was 10% for mesh repair and 18.8% for suture repair. Conclusions: Despite higher complication rate, mesh repair is superior to suture repair due to lower recurrence rate. Suture repair still has a place under certain circumstances, also it is simple less costly and has insignificant infection rate.

In search for new potential inhibitors some new fluorine substituted thiobarbituric acid derivatives (2-4, 7, 8) and their fused/isolated heterobicyclic nitrogen systems (5, 6, 9, 10, 11, 12) have been synthesized from heterocyclization of fluorinated 1,3-diketoamine (1) with CS2 followed by ring closure reactions with primary nitrogen reagents. Structures of the targets have been established from elemental analysis and spectral data. Some synthesized systems have been evaluated as anti-HIV-1 and of cyclin-dependent kinase 2 (CDK2) for cell tumor division.

Utilization of Primary Health Care and Its Associated Factors among Women of Childbearing Age Living in mogadishu-Somalia

Background: The Somali people have some of the worst health indicators in the world, with women and children most affected. Despite modest improvements in maternal and child mortality rates in the last five years, inequities amongst communities meant that some children and mothers are disproportionately exposed to otherwise avoidable and treatable diseases. The broad aim of this study is to assess utilization of Primary Health Care (PHC) and Its associated factors among Women of Childbearing age living In mogadishu Somalia. methodology: Descriptive and Analytical cross-sectional community-based study was conducted among Women of Childbearing age that were living in two districts (Hodan and abdul-aziz Districts)-mogadishu-Somalia-From may 2019 to September 2019. A total of 395 of Women of Childbearing age were targeted during the study period. Descriptive and analytical statistics was used using Statistical package for social science (SPSS) version 20 statistical software. The data were showed tables, figures and graphs for describing the data. Results: The study found that most of respondents were between 15 to 25 years 41.5% (395 sample sizes), while the age of group of >45 to 55 was only 6% (395 sample size). According to the district 76.5% of respondents were from Hodan, while 23.5% (395 sample size) of respondents were from abdul-aziz district. According to awareness to the need of a pregnant mother to be vaccinated at the mother and child health (mCH) most respondents 48.2% (395 Sample size) said that pregnant women may or may not need to be vaccinated. According to health-seeking decisions only 3% of ever married respondents were made by themselves whilst 97% of them were made by their husband. Age group was not significantly associated with the level of utilization of Primary Health Care (PHC);this is different from study in Kenya showing that age is an important factor in determining the use of skilled assis

Objectives: To evaluate the effect of Juvenile Idiopathic Rheumatoid Ar-thritis (JIA) on the health-related quality of life (HRQOL) in Saudi children. methods: A cross-sectional study was conducted in a tertiary hospital in Jeddah, Saudi Arabia to evaluate the HRQOL of children aged ≤ 18 years who had JIA using the childhood health assessment questionnaire modified for Arab children (CHAQ-mAC). Such questionnaire investigates 34 activities of daily life (ADL) classified into 8 life domains. Children or their parents were invited for face-to-face interview, and a phone interview was done for patients who missed their appointments during the period between February and July 2017. A statistical model was used to calculate a total *** score (range = 0 - 33;Cronbach’s alpha = 0.966);with higher values indicating poorer HRQOL. Results: Of a total of 44 children (male ratio = 0.63;mean ± SD age = 9.95 ± 5.44), Systemic-onset JIA was the most frequent type (27.3%), followed by polyarticular (15.9%) and oligoarticular (13.6%). Pain was reported among 43.2% (frequently in the knee, in 27.3%) whereas morning stiffness was reported in 20.5%. The mean *** score = 2.89 (75th centile = 3.00). With respect of ADLs, up to 22.7% of the children complained of difficulty;and 31.8% reported a difficulty in at least one of the 34 investigated ADLs. With respect of the life domain, children reported difficulties for activities (27.3%), dressing & grooming and hygiene (13.6%), and eating (6.82%). According to the life domain, 4.5% to 13.6% of the children needed help to execute the related ADLs and up to 9.1% used aids or devices. Poor HRQOL was associated with articular pain (p = 0.003) and specific medication (p = 0.043). Con-clusion: Children with arthralgia and those on specific treatment are at higher risk of impaired QOL, which emphasizes the need for systematic screening for treatment adverse effects and joint pain and implementation of efficient management to improve HRQO
