Epidemio-Clinical Profile of Psychotrauma in military Victims of Terrorists Attacks in Benin in December 2021

Introduction: In December 2021, Beninese soldiers were victims of armed terrorist attacks. This study aims to determine the psychological impact of the events experienced on the victims in order to better prevent post-traumatic stress disorder. materials and methods: This is a descriptive cross-sectional study conducted from December 13 to 14, 2021, i.e. 12 days after the oldest event and 4 days after the most recent. The experience of the event was assessed with the Peritraumatic Distress Inventory and the Peritraumatic Dissociation Experiences Questionnaire. Symptoms experienced in the hours and days after the event were assessed with the Revised Event Impact Scale. Results: 36 soldiers were included out of the 38 survivors, i.e. 94.73%. They were 51.43% victims of the night assault, 37.14% of the ambush on patrol and 11.43% of the explosion of vehicle on mine, all male. The average age was 31 years with extremes of 25 and 49 years. The 25 - 30 year olds predominated, 52.78%. 100% of soldiers had significant peritraumatic dissociation, of which 94.44% also had significant peritraumatic distress. 100% of the soldiers had manifestations of intrusion, 89% of manifestations of avoidance and 83% of manifestations of hyperexcitation. 88.89% had an index for acute stress including 100% of victims of night assault and vehicle explosion on mine. Conclusion: This study shows a high prevalence of peritraumatic reactions, recognized factors favoring post-traumatic stress disorder, and the interest of their evaluation for medical and psychological help adapted to the victims.

Obstetrical Complications of Female Genital mutilation: management maternal-Fetal medical Care and Prognosis, Obstetrical Gynecology Regional Hospital, Unit of N'zérékoré

Introduction: Female Genital mutilation (FGm) is a public health problem. There are 100 to 140 million girls and women who suffer every year in the world [1]. The aim of this study is to improve the medical care and reduce complications of FGm at the Regionalhospital of N’zérékoré, to determine their frequency, and to evaluate the maternal-fetalprognosis. methods: The study was conducted at the Regional Hospital of N’zérékoré. This was a cross-sectional, descriptive and analytic study of 6 months, from 1 September 2016 to 28 February 2017, including all pregnant women admitted for childbirth who had a complication of female genital mutilation. Results: A total of 1295 women gave birth in the service, of which 1204 women were women with female genital mutilation. Given a frequency of 92.97%, of these 1204 mutilated women, 223 presented obstetrical complications during their delivery, a proportion of 17.22%. They were mostly young patients, mostly housewives who were not in school. Type II FGm was the most common (53.06%). Obstetric complications were dominated by complicated perinatal tears (54.08%), and hemorrhages (40.81%). The catch was dominated by perineorrhaphy. Conclusion: The frequency of FGm was 92.97% and that of their obstetric complications 17.22%. most were house-wives, not in school. There was FGm type II. The abandonment of FGm would reduce maternal and perinatal morbidity and mortality.

Undifferentiated Arthritis Revealing Cogan’s Disease

Background: Cogan’s syndrome is a rare autoimmune vasculitis (less than 300 cases described in the literature) characterized in its typical form by cochleovestibular involvement, interstitial keratitis, and an inflammatory involvement of large vessels. It is frequently accompanied by general signs and cardio-vascular, rheumatologic, and digestive complications. Its management is essentially based on the use of corticosteroids and immunosuppressants. The outcome of the treatment, even if rapid, does not always avoid functional complications. This disease is probably under-diagnosed in Africa. Objective: The objective of this study was to report a case of Cogan syndrome in sub-Saharan Africa. Case report: A 78 year old black African female patient presented with arthritis of the left knee without fever. The examination also revealed a bilateral deafness of rapidly progressive onset. The patient was known to be diabetic and hypertensive, and was regularly monitored. The biology revealed an inflammatory syndrome and an inflammatory joint puncture fluid. Pure tone audiometry confirmed a 77% hearing loss. The ophthalmological examination revealed stromal oedema and hypertensive retinopathy of Kendall stage III. The rheumatological evolution was good under methylprednisolone and methotrexate, but the deafness persisted. The occurrence of a fever on day 12 of treatment required further investigations (blood culture, PCR-Covid 19 test, cytobacteriological examination of sputum, thoracic CT). The evolution was unfavourable with the death of the patient. Conclusion: Cogan’s disease is a rare inflammatory disease. Its diagnosis and management are multidisciplinary. The treatment can be disappointing.

Posterior Urethral Valves without Reflux Associated with Bilateral Renal Cortical Atrophy: Diagnosis and management

The objective of this case report was to highlight the characteristics of posterior urethral valves observed in a 3-year-old patient and to describe their management. The case of a 4-year-old patient treated for a posterior urethral valve with bilateral renal cortical atrophy was reviewed. The diagnosis was confirmed by abdominal ultrasound, cystography and abdominal CT. He was referred to martinique (a tertiary health establishment) for effective support. From the acute situation to the fortuitous discovery, its understanding must be deepened because of the potential immediate symptomatic impact in the form of renal colic which can be associated with sepsis, as well as in the long term on renal function. This observation is intended to help the attending physician to initiate his diagnosis and treatment.

Nutritional Profile of Children with major Sickle Cell Syndrome at the Centre of medical and Health Advice of Kipé, Conakry, 2018

Introduction: Growth is a reflection of a child’s health and nutritional status. Children with sickle cell disease often have slower statural and weight development. The aim of this study was to evaluate the nutritional profile of children with sickle cell disease (SCD) registered in the CEmECO centre database. methodology: This was a cross-sectional study with simple random sampling of children aged 1 to 16 years registered in the clinic database. Results: We collected information on 208 children, 121 of whom had sickle cell disease and 87 of whom were normal, with a sex ratio of 1.02. The mean age of the sickle cell patients was 8.7 ± 4.4 years, while that of the non-sickle cell patients was 9.5 ± 4 years. Haemoglobin electrophoresis revealed 103 homozygous (SS), 18 double heterozygous (SC, SBetaThal, SE) and 87 normal (AA) and/or sickle cell trait (AS) sickle cell cases. We observed a significant difference in the height/age ratio (P ¥). Conclusion: The results of our study revealed stunted growth in children with sickle cell disease.

Transient Diabetes Induced by L-Asparaginase

Background: Although hyperglycaemia is one of the known side effects of L-asparaginase, its contribution to the development of diabetic ketoacidosis (DKA) is less well known in the literature. Asparaginase is an essential component of treatment protocols for acute lymphoblastic leukaemia (ALL) in combination with other chemotherapeutic drugs. On further evaluation, she was found to have high anion gap metabolic acidosis, hyperglycaemia and ketonuria. In recent decades, the use of these chemotherapeutic agents has led to a decrease in mortality and disease-free survival in ALL. L-asparaginase is one of the chemotherapy protocols used in the treatment of acute lymphoblastic leukaemia (ALL) and can induce hyperglycaemia which is aggravated by the concomitant use of corticosteroids. We report the observation of a 14-year-old girl treated with chemotherapy (GFA LAL protocol) who developed transient diabetes following the use of L-asparaginase. She was treated at the Donka paediatric haemato-oncology unit by a multidisciplinary team including a paediatric oncologist, a psychologist and a diabetologist. The aim of this study was to highlight blood glucose monitoring before and after the use of asparaginase in acute lymphoblastic leukaemia. Conclusion: We conclude that the occurrence of ketoacidosis following the use of asparaginase is a rare event. We recommend close monitoring of blood glucose levels for hyperglycaemia in patients with ALL receiving L-asparaginase.

Child Victims of Road Traffic Accidents: Epidemiological and Clinical Aspects in the medical and Surgical Emergency Department of the Donka National Hospital

Introduction: Children involved in road accidents most often suffer head or limb injuries. The severity of these injuries varies according to the age of the child, the type of road user and whether or not protective devices are used. The aim of this study was to carry out a census of admissions and to identify the epidemiological and clinical profile of children involved in road traffic accidents at Donka National Hospital. methods: This is a descriptive cross-sectional study of children involved in traffic accidents over a 3-month period from September 1 to November 30, 2021. All children admitted to the emergency department for a traffic accident were included in the study. Results: During the study period, 435/530 children (82.1%) were victims of road accidents. Boys accounted for 54.94% of cases, compared with 45.06% for girls. The age group most affected was 12-17 years old (48.97%). October had the highest accident rate (36.4%). Pedestrians are most affected (52.87%). Bicycles and mopeds were involved in 59.77% of accidents. Head injury was the most frequent pathology (33.33%). The study showed that 12.64% of victims were referred to intensive care, pediatric surgery, etc. We recorded 8 cases of death (1.84%). Conclusion: This study highlighted the characteristics, incidence and risk factors for accidents in children admitted to emergency departments. Accident prevention, particularly serious accidents in children, requires more precise knowledge of the factors and circumstances leading to their occurrence.

Aims: To review one year of ultrasound impulse elastometry in a hospital in Ouagadougou. Patients and method: This is a retrospective descriptive study of data from one year’s use of impulse ultrasound elastometry. Ultrasound impulse elastometry was performed using the FibroScan® COmPACT 530. Fibrosis was considered significant when E (hepatic elasticity) ≥ 7.2 kPa (F2 fibrosis). The test was considered valid when the IQR/median ratio ≤ 30% and there were at least 10 valid measurements. Results: A total of 1911 patients underwent FibroScan®. There were 1079 men, giving a sex ratio of 1.3. The mean age of the patients was 37.9 ± 12.2 years. The indication for FibroScan® was hepatitis B virus infection in 89% of cases. The validation criteria for FibroScan® were met in all patients. The mean value for elasticity was 7.9 kPa and for steatosis 212 dB/m. Fibrosis was non-significant in 75.5% of cases. more than half of our patients (56.7%) didn’t have steatosis, 24.8% had mild steatosis, 12.4% had moderate steatosis and 6.1% had severe steatosis. Conclusion: Ultrasound pulse elastometry plays an important role in monitoring chronic liver disease. It allows non-invasive diagnosis of hepatic fibrosis and steatosis. In our context, however, access to the test is limited by its availability only in large urban centers, and by its cost.

JUNO is a multi-purpose neutrino observatory under construction in the south of *** publication presents new sensitivity estimates for the measurement of the △m_(31)^(2),△m_(21)^(2),sin^(2)θ_(12),and sin^(2)θ_(13) oscillation parameters using reactor antineutrinos,which is one of the primary physics goals of the *** sensitivities are obtained using the best knowledge available to date on the location and overburden of the experimental site,the nuclear reactors in the surrounding area and beyond,the detector response uncertainties,and the reactor antineutrino spectral shape constraints expected from the TAO satellite *** is found that the △m_(21)^(2) and sin^(2)θ_(12) oscillation parameters will be determined to 0.5%precision or better in six years of data *** the same period,the △m_(31)^(2) parameter will be determined to about 0.2%precision for each mass ordering *** new precision represents approximately an order of magnitude improvement over existing constraints for these three parameters.

The Jiangmen Underground Neutrino Observatory(JUNO)features a 20 kt multi-purpose underground liquid scintillator sphere as its main *** of JUNO's features make it an excellent location for^8B solar neutrino measurements,such as its low-energy threshold,high energy resolution compared with water Cherenkov detectors,and much larger target mass compared with previous liquid scintillator *** this paper,we present a comprehensive assessment of JUNO's potential for detecting^8B solar neutrinos via the neutrino-electron elastic scattering process.A reduced 2 meV threshold for the recoil electron energy is found to be achievable,assuming that the intrinsic radioactive background^(238)U and^(232)Th in the liquid scintillator can be controlled to 10^(-17)g/*** ten years of data acquisition,approximately 60,000 signal and 30,000 background events are *** large sample will enable an examination of the distortion of the recoil electron spectrum that is dominated by the neutrino flavor transformation in the dense solar matter,which will shed new light on the inconsistency between the measured electron spectra and the predictions of the standard three-flavor neutrino oscillation *** m^(2)_(21)=4.8times10^(-5);(7.5times10^(-5))eV^(2),JUNO can provide evidence of neutrino oscillation in the Earth at approximately the 3sigma(2sigma)level by measuring the non-zero signal rate variation with respect to the solar zenith ***,JUNO can simultaneously measureDelta m^2_(21)using^8B solar neutrinos to a precision of 20% or better,depending on the central value,and to sub-percent precision using reactor antineutrinos.A comparison of these two measurements from the same detector will help understand the current mild inconsistency between the value of Delta m^2_(21)reported by solar neutrino experiments and the KamLAND experiment.
