Diagnosis of acute intermittent porphyria in a renal transplant patient:A case report

BACKGROUND Acute intermittent porphyria(AIP)is an inherited disorder of porphyrin metabolism with a worldwide distribution and a prevalence ranging from 1 to 9 per million *** is caused by an autosomal dominant-inherited mutation of low penetrance resulting in a deficiency of porphobilinogen deaminase(PBGD)*** attacks are provoked by stressors such as certain medications,alcohol,and *** herein present the first case report of AIP detected in a post-renal transplant *** SUMMARY The patient was a 65-year-old man who underwent transplantation 2 years previously for suspected nephroangiosclerosis and chronic interstitial *** subsequently developed diabetes mellitus which required insulin *** had been treated in the recent past with local mesalamine for *** presented with classic but common symptoms of AIP including intense abdominal pain,hypertension,and *** had multiple visits to the emergency room over a 6-mo period for these same symptoms before the diagnosis of AIP was *** urinary postprandial blood glucose level was 60 mg/24 h(normal,<2 mg/24 h).He was placed on a high carbohydrate diet,and his symptoms slowly *** This case report describes a common presentation of an uncommon disease,in which post-transplant complications and medications may have contributed to precipitating the previously undiagnosed *** hypothesize that the lowcarbohydrate diet and insulin with which our patient was treated may have led to the attacks of ***,our patient’s mesalamine treatment for proctitis may have led to an acute AIP crisis.A high index of suspicion is needed to consider the diagnosis of a heme synthesis disorder,which presents with the common symptoms of abdominal pain,high blood pressure,and anxiety.

目的评价骨骼肌蛋白质合成的传统方法是在静脉输注L-[环13C6]苯丙氨酸(13C6-Phe)后做骨骼肌活组织检查,通过多次活组织检查计算蛋白质合成的分数合成率（FSR）。通过对健康老年女性行11C-L-甲基蛋氨酸(11C-MET)PET/CT动态显像,估算进食后和吸收后的骨骼肌蛋白质合成水平,旨在评价上述2种方法的一致性。方法 13名老年女性[年龄（73±5）岁]禁食后口服25 g乳清蛋白,1 h后行11C-MET PET/CT动态显像,作图分析动态图像,通过计算Patlak系数Ki估算11CMET进入股中肌束的量。间隔2周,在相同条件下口服25 g乳清蛋白,1h后行稳定同位数药物13C6-Phe的输注,并进行股外侧肌活组织检查,计算FSR。结果口服乳清蛋白后的Ki值及FSR[（0.009 5±0.001）/min和（0.083±0.008）%/h]比吸收后的高[（0.007 85±0.001）/min和（0.049±0.006）%/h,P<0.05或P<0.001]。Ki和FSR升高的百分数有显著相关性（r=0.79,P=0.015）。结论 11C-MET PET/CT动态显像可以估算口服蛋白质后的骨骼肌蛋白质合成情况,其结果与传统的有创活组织检查法一致。研究结果表明,11C-MET PET/CT动态显像今后可用于无创地评价骨骼肌蛋白质合成的影响因素。

Isolative Synthesis and Characterization of Cellulose and Cellulose Nanocrystals from Typha angustifolia

The application potential of cellulosic materials in natural composites and other fields needs to be explored to develop innovative, sustainable, lightweight, functional biomass materials that are also environmentally friendly. This study investigated Typha angustifolia (Typha sp.) as a potential new raw material for extracting cellulose nanocrystals (CNCs) for application in wastewater treatment composites. Alkaline treatments and bleaching were used to remove cellulose from the stem fibres. The CNCs were then isolated from the recovered cellulose using acid hydrolysis. The study showed a few distinct functional groups (O-H, -C-H, =C-H and C-O, and C-O-C) in the Fourier Transform Infrared (FTIR) spectra. A scanning electron microscope (SEM) revealed the smooth surface of CPC and CNCs, which resulted from removing lignin and hemicellulose from powdered Typha angustifolia. Based on the crystalline index, the powdered Typha angustifolia, CPC, and CNCs were 42.86%, 66.94% and 77.41%. The loss of the amorphous section of the Typha sp. fibre resulted in a decrease in particle size. It may be inferred from the features of a Typha sp. CNC that CNCs may be employed as reinforcement in composites for wastewater treatment.

Associated Factors with Early Sexual Intercourse among In-School Adolescents in Brazzaville

Background: Early sexual intercourse (ESI) is a global public health problem. In Congo, associated factors with ESI have been under-studied. Objective: To analyse factors associated with early sexual intercourse among in-school adolescents in Brazzaville. Method: A cross-sectional analytic study with three-stage random sampling was carried out from March 2 to June 2, 2022 in Brazzaville secondary and high schools. Adolescents in the selected schools were interviewed about their sexual and social behaviour, and their family and school environment. Multivariate analysis using logistic regression was used to identify factors associated with ESI. The significance level of the p-value was set at 5% and the confidence interval at 95%. Results: Of a total of 1100 adolescents surveyed, 800 (72.7%) had ESI, 98% of them were female. The main reason for early initiation of sexual intercourse was curiosity (81.8%). Non-use of condoms and multiple sexual partnerships were noted respectively in 98.7% and 68.7% of adolescents who had experienced ESI. Female gender (p Conclusion: The prevalence of ESI is high, and the factors identified in this study may help to reduce it.

Osteocyte Remodeling of the Perilacunar and Pericanalicular Matrix

With additional functions of osteocytes being identified, the concept that osteocytes are just ＂static lacunar-dwelling cells＂ is no longer accepted. We reviewed most of the relevant literature on osteocyte＇s function in the direct remodeling of the perilucunar matrix, discussing the advantages and disadvantages. Special attention was paid to how the negative researchers argue about the ＂osteocytic osteolysis＂ principle, and how the positive side addressed the arguments. We also discussed the newly found data of osteocytic remodeling function from our group. With more biotechnology in hand, there is increased excitement in the prospect of now being able to answer the two important questions： do osteocytes have the capability to remove mineral from the perilacunar matrix and if so what are the molecular and cellular mechanisms？ do osteocytes have the capability to deposit new mineral on the perilacunar matrix and if so what are the cellular and molecular mechanisms？

How authentic leadership influences employee proactalty： the sequential mediating effects of psychological empowerment and core self-evaluations and the moderating role of employee political skill

This study aims to examine the relationship between authentic leadership and employee proactive behavior. Based on self-determination theory, we argue that such a relationship is sequentially mediated by psychological empowerment and core self-evaluations. In addition, political skill plays a moderating role in the third stage. These hypotheses are validated by a sample of 65 leaders and 275 subordinates from two private enterprises in China's Mainland. Results show that authentic leadership Clime I） influences employees＇ proactive behavior Clime 3） through the psychological empowerment （Time I） and core self-evaluations of employees （Time 2）, and the relationship between core self-evaluations and proactive behavior is positively moderated by employees＇ political skill. In addition, bootstrapping results also verify the moderating role played by employees＇ political skill in the indirect relationship between authentic leadership and proactive behavior through core self-evaluations. Theoretical and managerial implications are further discussed in the light of these findings.

This article examines the relationship among subjective well-being(SWB),income and education in China through self-perceived social *** and women appear to use different socioeconomic markers to determine their self-perceived social status,which ultimately affects their subjective reports of *** using the data from the 2010 China General Social Survey(CGSS),this study finds a positive association between income and SWB for *** result also shows a positive association between education and SWB for ***,for women self-perceived social status is a strong mediator of income and happiness;while for men,selfperceived social status mediates the relationship between education and *** study highlights the importance of the cultural influence in understanding the differences in self-perceived social status outcomes by gender and how these differences ultimately influence the subjective measures of well-being in China.
