The association between stroke and meningioma is rarely reported in the data. In most etiology classification, there is no compressive cause. The association between meningioma and stroke is increasingly reported. We report a case of 52-year-old woman, previously healthy, presented with sudden right hemiplegia with some transient mood disorders. The CT-scan revealed acute left anterior cerebral artery infarction associated with fronto-basale meningioma infiltrating both anterior cerebral arteries. She was treated by corticosteroid and rehabilitation, with partial recovery. No stroke prevention was used before surgical treatment.

In case of hypertension, hypertensive retinopathy is normally an indicator of target organ damage. This sign can be frequently detected even in normotensive subjects. Among 94 normotensive volunteers chosen with regard of the hypertensive status of their parents, we could report the case of 3 black women who had hypertensive retinopathy. As changes in retinal microvasculature precede other signs of damage, we suggest fundus photography routine to be performed more frequently in assessing cardiovascular risks in hypertensive individual and their offspring, especially in black African population, where hypertension is severe.

Intravesical Prostate Protrusion (IPP) versus Middle Lobe Volume on Ultrasonography in Assessing the Impact of Benign Prostatic Hyperplasia

Background: Ultrasound is the main method of exploring the prostate. In benign prostatic hyperplasia (BPH), it provides important morphological information and assesses its impact, helping to guide the treatment. Objective: To compare intravesical prostatic protrusion (IPP) and middle lobe volume by ultrasound in BPH. Method: This was a single center prospective, descriptive and analytical study, over a period of 6 months, including 95 patients, undergoing prostatic trans-abdominal ultrasound. Patients were selected by a single urologist for clinical suspicion of benign prostatic hypertrophy. The ultrasound examination was done by a single senior radiologist. Results: The mean age of the patients was 66.63 ± 11.55 years with ranges from 38 to 98 years. The prevalence of BPH was 76.84%. The rate of patient with middle lobe protrusion was 48.42%. The mean middle lobe volume was 11.29 ± 12.90 ml. More than half of the patients (50.91%) had an IPP stage 3 of. The mean bladder wall thickness was 6.08 ± 2.58 mm, with 50.53% being pathological. The post-voiding residue (PVR) was significant in 38.75% of patients. Renal repercussions were present in 17.89%. The correlation analysis did not note a statistical link between prostate volume and quality of life score (p > 0.05). There was a statistically significant correlation between IPP values and quality of life score (p = 00461), IPSS score (p = 0.0424) and PVR (p = 0.0395). For middle lobe volume, there was a correlation with PVR (p = 0.0018). There was no correlation with clinical impact (quality of life score and IPSS score). Conclusion: The IPP appears to be an easy element to measure and better than the volume of the prostate and the middle lobe in assessing the impact of BPH.

Trauma of the Male Genitals in the Teaching Hospital Sylvanus Olympio of Lome: Lesional and Therapeutic Aspects

Introduction: Trauma of the external male genital organs (TEMGO) is multifaceted and rare. The objective of this work was to provide an overview of these lesions by describing the epidemiology, the diagnostic approach, their therapeutic and prognostic aspects. Materials and Methods: It was a retrospective study carried out over a ten-year period, from January 1, 2009 to December 31, 2018. It included male patients, admitted and treated in surgical emergencies and in the urology department of the Sylvanus Olympio Teaching Hospital in Lomé for trauma of male external genital organs. Results: Thirty-five cases had been collected. They had represented 1.5% of urological emergencies. The subjects were young with a mean age of 29.6 ± 3.1 years, with extremes of 18 and 60 years. Trauma to the penis was more frequent and found in 60% of cases, with the mechanism of coitus misstep in 54.3%, leading to a fracture of the penis. Testicular contusions were found in 22.9% of the cases. The ultrasound was performed in 6 patients. Four orchidectomies were performed for an unsustainable testicle found at scrototomy. The postoperative had been simple in all cases. Fifteen patients (42.8%) had been regularly followed on 18 months. Residual testicular pain and mode-rate erectile dysfunction was noted respectively in 3 and 2 patients. Conclusion: TEMGO, although rare, require perfect knowledge for adequate therapeutic decisions, as they can compromise the functional prognosis of concerned organs.

Factors Associated with Non-Adherence to Antiretroviral Therapy among HIV-Infected Adolescents at Pediatric Department of Yopougon University Hospital, Côte d’Ivoire

Context: The consequences of non-adherence to antiretroviral therapy (ART) are harmful in terms of morbidity and mortality among HIV-infected adolescents. There is a little data in this population in West Africa, about non-adherence to ART. Objective: To identify factors associated with non-adherence to ART in adolescents at the Pediatric Ambulatory Treatment Center (PATC) of the pediatric department of Yopougon University Hospital. Materials and Methods: A cross-sectional study was conducted from May to July 2017 among adolescents aged 10 to 19 years old at PATC of pediatrics department of Yopougon University Hospital. We have considered that adolescent was not adherent to ART when he affirm that he have not taking ART at least once in the week before the survey. A logistic regression model was used to identify factors associated with non-adherence to ART. Results: Overall 166 adolescents were included. The median age was 15 years old [IQI = 13 - 17 years]. They had a detectable viral load greater than 1000 copies/ml in 41.8% of cases. The rate of non-adherence to ART was 40.4%. The factors associated with non-adherence to ART were male [OR = 2.5 (CI = 1.1 - 10.0)], having parent widowed [OR = 7.8 (CI = 1.6 - 39.3)] or divorced [RC= 3.7 (IC=1.1 - 13.5)], CD4 inclusion rate ≥ 500 cells/ml [OR = 8.5 (CI = 1.6 - 45.5)] and duration on ART ≥ 10 years [OR = 8.9 (CI = 1.6 - 50)]. Conclusion: A rigorous therapeutic education taking into account associated factors in this study is necessary to reduce the rate of non-adherence among adolescents at PATC.

Marjolin’s ulcer is an aggressive tumor that occurs on a chronic non-healing wounds or burn scars. It’s a rare disease. Histologically, it is most often a squamous cell carcinoma. The real frequency is not known. Its incidence would be higher in developing countries where ancestral beliefs and financial difficulties cause consultation delays. Thus the diagnosis is delayed, due in part to the lack or insufficiency of diagnostic means. Our study aimed to describe the epidemiological, diagnostic and therapeutic particularities through three cases treated in our center. The delay in consultation resulted in the death of one patient, while for the other two, amputation prevented the extension of the tumor. Limb amputation is increasingly rare in the management of Marjolin’s ulcer, but in our difficult conditions it is still widely practiced when the tumor is localized at the extremity of the limb, without signs of remote invasion.

Prostate Biopsy: Which Technique for Which Results at LoméUniversity Hospital in 2017?

Background: Diagnosis of prostate cancer is certified by histology true prostate biopsies. The aim of our study was to evaluate our prostate biopsy method. Material and Methods: It was a prospective study including patients underwent prostate biopsy. Inclusion criteria were prostate specific antigen (PSA) level up to 4ng/ml and/or abnormal prostate at digital rectal examination. Patients who had risk factors of bleeding have been excluded of the study. The preparation before biopsy included antibioprophylaxy (Ciprofloxacine-Tinidazole) and rectal hypertonic cleaning (Normacol*). Twelve cores have been taken in each prostate by transrectal digital-guided way, using Biopty Gun 18 Gauge. Local anesthesia has been done previously by intrarectal application of 20 ml of gel of Lidoca?ne. Two other cores were taken into each abnormal area at rectal examination. The follow-up have been done during twelve weeks. Results: Eighty patients of 65 years of age were included. Nine patients had familial history of prostate cancer. PSA levels ranged from 5 to 6400 ng/ml with a median of 26.77 ng/ml ± 11.2. Complications occurred in 11.25% of patients, principally infectious complications which caused death of one patient by septicemia. The rate of cancer detection was 20%. Prostate abnormality at digital rectal examination and the presence of familial history of prostate cancer were not predictive factors of the presence of cancer on cores. Conclusion: Our prostate biopsy method is limited by the lack of ultrasonographic guidance and is at important risk of infectious complications.

Prognostic Role of Pulse Pressure in the Acute Phase of Myocardial Infarction

Objective: Pulse pressure is a cardiovascular risk factor. The aim of our study was to assess the short and medium term prognosis of myocardial infarction in the African black having a high pulse pressure. Methods: We performed a comparative retrospective study of 200 patients hospitalized in the Cardiology Institute of Abidjan. The first group (100 patients) had a high pulse pressure (≥60 mmHg). The second (100 patients) had normal pulse pressure (≤60 mmHg). The primary outcome was mortality. Secondary endpoints were represented by hemodynamic complications, rhythmic and conduction disturbances. Both groups of patients were matched. The matching is performed by socio-demographic criteria, the seat of necrosis and cardiovascular risk factors. The authors explained the choice of 60 mmHg as references because in all their studies they have realized that it is from 60 mmHg pulse pressure morbid events that occur in prospective studies and retrospective studies in stiffness of the great arteries is responsible for all these events. Pulse pressure is in itself, an independent cardiovascular risk factor in systolic blood pressure, diastolic blood pressure when associated with myocardial infarction, it becomes from 60 mmHg poor prognostic factor especially in the acute phase. Results: The mean age was 54.46 ± 10.10 years for patients with normal pulse pressure and 52.32 ± 10.89 years in patients with high pulse pressure. Patients with elevated pulse pressure were twice as left ventricular failure (Chi2 = 3.71;P = 0.048 = 2 OR 95% CI = 0.93 to 4.29). These patients had an ejection fraction and fractional shortening double bass (Chi2 = 31.23 P = 0.00001, OR = 2.2 95% CI 0.8 to 17.4) 10 mmHg for increased pulse pressure we observed an average growth of 18% mortality rate. Mortality was three times higher in patients with elevated pulse pressure (Khi2 = 15, 06 ddl = 1 P = 0.0001 OR = 3.34 95% CI 1.72 to 6.52). Background: Breast cancer is the leading cause of cancer-related death in women around the world. The objective of this study was to assess knowledge and attitudes about breast cancer screening among students in the Faculty of Health Sciences at the University of Lomé. Methods: This was a prospective descriptive transversal study conducted among students enrolled in the Faculty of Health Sciences (FSS) of the University of Lomé (UL) in the 2019 school year account 2020. The purpose of this study was to assess the knowledge and attitudes of medical students about breast cancer screening. Results: In total, the questionnaire was completed by 934 students, for a response rate of 57.1%. The most frequently mentioned risk factors were the presence of a family history of breast cancer (n = 799, 85.55%) and the use of oral or injectable contraceptives (n = 606, 64.88%). There is a statistically significant link between the participants’ knowledge of the self-examination and clinical examination of the breasts and certain socio-demographic characteristics, namely age (OR = 0.2, 95% CI [0.11 - 0.75], p-value 0.0001), the level of study (OR = 1.5, 95% CI [0.55 - 2.46], p-value = 0.0054), the follow-up of a course on breast cancer (OR = 2.63, 95% CI [0.32
