Multiple Myeloma (MM) is a disseminated plasma cell tumor caused by the proliferation of a single plasma cell clone which results from the production of monoclonal immunoglobulin, most commonly class G (IgG). MM is a cancer of the elderly, with a mean age at diagnosis of 68 years. A 79-year-old white patient sought the outpatient clinic of the Discipline of Stomatology, S?o José dos Campos Dental School, UNESP, because of a fracture associated with an osteolytic lesion in the right ascending ramus of the mandible. In view of the clinical-radiographic findings, exploratory fine needle aspiration (FNA) of the lesion was performed and the sanguinolent material obtained was submitted to the preparations of smears for cytology analysis. The diagnosis of MM was based on laboratory and radiographic findings. FNA cytology analysis of the lesion permitted a diagnostic hypothesis of MM.

The prevalence and severity of obstructive sleep apnea(OSA) is higher in specific population: children, elderly,obese and patients with pulmonary and cardiovascular diseases, compared to the general population. OSA is associated with greater morbidity and mortality in these patients. Although full-night polysomnography is still the gold standard diagnostic sleep study for OSA, it is a time consuming, expensive and technically demanding exam. Over the last few years, there is growing evidence on the use of portable monitors(PM) as an alternative for the diagnosis of OSA. These devices were developed specially for sleep evaluation at home, at a familiar environment, with easy selfapplication of monitoring, unattended. The use of PM is stablished for populations with high pre-test probability of OSA. However, there is a lack of studies on the use of PM in age extremes and patients with comorbidities. The purpose of this review is to present the studies that evaluated the use of PM in specific population, as well as to describe the advantages, limitations and applications of these devices in this particular group of patients. Although the total loss rate of recordings is variable in different studies, the agreement with fullnight polysomnography justifies the use of PM in this population.

The aim of this article is to emphasize the importance of nuclear medicine in fungal infection characterization through a case report. Case： patient female, with AML-M2, hospitalized with fever, dyspnea, nausea, myasthenia, abdominal pain and diarrhea. In the physical examination exacerbated lesion in abdominal wall and subcutaneous nodules in upper and lower limbs. The computed tomography of the chest evinced area of attenuation in ground-glass opacities in the middle lobe, tending to consolidation and bilateral nodular opacities with ground-glass halo which suggested an infiammatory/infectious process of fungal etiology. Immunoscintigraphy with ^99mTc-besilesomab （Scintimun） identified focal infectious processes in activity, scattered over cutaneous and subcutaneous tissues, predominantly in extremities. In blood culture test, there was a growth ofFusarium sp. There is a difficulty in the diagnosis of fungal infections, both in clinical and imaging methods. There is no specific method to reaching the target. In addition, fungal diseases can be divided into focally localized or disseminated infections. There is a greater difficulty in the diagnosis of immunocompromised individuals due to the deficiency of basic defense mechanisms. Scintimun （scintigraphy with besilesomab） is a useful tool of nuclear medicine in the diagnosis of infectious and inflammatory diseases to allow the image of the whole-body and in vivo detection of early pathological and physiological phenomena, even before anatomical alterations occur.

This paper presents a methodology for analytical calculation and computational simulation using the finite element method for piezoresistive graphite sensor element on flexible polymer substrate,A4 *** computer simulation aims to find the region of greatest mechanical tension and deflection of the circular diaphragm set in the circumference *** steps for simulation are geometry definition,mesh generation,inclusion of material physical properties and simulation *** mathematical modeling of maximum mechanical stress and deflection is described analytically and *** analytical calculations were compared with the computer simulation and presented a relative percentage error of 3.38%for the maximum *** results show that the piezoresistor should be positioned at the edges of the circular diaphragm to take advantage of maximum mechanical stress by defining the best location for graphite film deposition for sensor device designs and fabrications.

Background : Osteoporosis can make bone repair difficult. Low-level laser therapy( LLLT) has been shown to be a promising tool for bone neoformation. This study aimed to analyze the effect of LLLT on calvaria bone defects of ovariectomized rats using stereology. Methods : Fifty-four Wistar rats were subjected to bilateral ovariectomy, and bone defects were created in calvaria after 150 days. The animals were divided into nine groups(n = ?6 per group), and 24 hours after the bone defects were created they received three, six or 12 sessions of LLLT at 0, 20 or 30 J/cm 2, using a 780-nm low-intensity GaAlAs laser. One-way ANOVA followed by Tukey ' s post hoc test was used for data processing. A difference of P < 0.05 was considered statistically significant. The parameters evaluated were osteocyte density( Nv_(ost)), total osteocyte number( Nto ost), trabecular surface density( Sv_t), and trabecular surface area( Sa_t). Results : Data obtained showed that Nto ost, Sv t, and Sa t in group G2 rats were significantly different from G1(0 J/cm^2)( P < 0.05). Compared to group G4, G5 presented higher values for the parameters Sv t and Sa t, and G6 presented significantly higher values for almost all the analyzed parameters( Nv _(ost), Nto_(ost), Sv_t, and Sa t)( P < 0.05). Compared to group G7, G8 showed a higher value only for the parameter Sa t, and G9 showed significantly higher values for parameters Nv ost, Nto ost, Sv_t, and Sa_t. Conclusion : We conclude that LLLT stimulated bone neoformation and contributed to an increase in the total number of osteocytes, especially with a laser energy density of 30 J/cm^2 given for six and 12 sessions.

This study investigated the influence of different titanium surfaces on the differentiation of rat osteoblast-like cells (osteo-1). Osteo-1 cells were cultured on the following titanium surfaces: 1) pretreated, smooth surface (PT);2) sandblasted and acid etched surface (SLA);and 3) sandblasted and acid-etched surface rinsed under nitrogen protection to prevent exposure to air and preserved in isotonic saline solution (modSLA). Cell metabolism, total protein content, collagen content and alkaline phosphatase (AP) activity and the formation of calcified nodules were analyzed. The titanium surface did not influence cell metabolism, total protein content and collagen content. The SLA surface influenced cell differentiation, with the observation of a significant reduction of AP activity and formation of calcified nodules after 21 days compared to the PT surface. No difference was observed between the PT and modSLA surfaces. All titanium surfaces tested permitted the full expression of the osteoblast phenotype by osteo-1 cells, including matrix mineralization.

This paper presents an optimization model for solving the planning problem of collection and transportation of solid waste in medium-sized cities. As final results, are expected to promote cost savings to the public coffers, as well as environmental benefits. The developed mathematical model is formulated as a problem of linear programming with mixed-integer variables and transcribed into software GAMS （general algebraic modeling system）. The practical application was tested using data collected in the central region of a Brazilian city with approximately 90,000 inhabitants. The deterministic model used allowed an optimal solution. It was found after inclusion of restrictions that eliminated the appearance of sub-routes. It was concluded that the optimal routes allow for a 38% reduction in total distance traveled, which can generate savings of $320.00 per day regarding maintenance and fuel trucks.
