Background: the quality of life (QoL) of patients with pancreatic ductal adenocarcinoma (PDAc), with its limited survival, can be affected by chemotherapy-induced toxicity. The main objective was to evaluate the effect of introducing ocoxin oral solution (OOS) in combination with standard therapy on quality of life. Methods: Thirty patients were enrolled in an exploratory, prospective, single-centre clinical trial in the oncology department of “Hermanos Ameijeiras” University Hospital in Havana, cuba. Quality of life was measured using the EORTc QLQ-c30 questionnaire and toxicity was assessed using the NcI-cTc-AE classification version 5.0. Results: There was stability in the scores over time for overall QoL and the functional scale criteria, while in terms of symptoms, fatigue, pain and loss of appetite were reduced. No grade 3 - 4 adverse events (AEs) were recorded, and only 14.9% of toxicities were classified as grade 2, and these were considered to be unrelated to OOS. Biochemical and nutritional parameters were normalised at 12 months compared to the baseline values. conclusions: This clinical study is the first report of the use of OOS in patients with advanced pancreatic cancer, and demonstrates that it is able to maintain optimal quality of life with reduced severity of toxicity during and after combination treatment with gemcitabine-based chemotherapy.

Minimally Invasive Surgery for Necrotizing Pancreatitis: A case Report

Introduction: Necrotizing pancreatitis management is complex and varies significantly among clinicians. Minimally invasive approaches like transgastric necrosectomy via laparoscopy are emerging as effective treatment options. This case report underscores the technique’s efficacy, clinical outcomes, and role in reducing complications. clinical Observation: A 59-year-old male with a history of smoking and alcoholism presented with severe abdominal pain, nausea, and vomiting. Over the following weeks, he developed symptoms including asthenia, weight loss, and melena. Diagnostic workup revealed severe anemia and Balthazar E necrotizing pancreatitis, with significant intra-abdominal fluid collections and signs of infection. After initial conservative management, the patient underwent transgastric necrosectomy via laparoscopy due to deteriorating clinical status. The procedure involved removing necrotic tissue and performing a cystogastroanastomosis and jejunostomy. Postoperative care included fasting, parenteral nutrition, broad-spectrum antibiotics, and enzymatic replacement. The patient recovered well, with reduced necrotic tissue on follow-up imaging, and was discharged twelve days post-surgery [1]. conclusion: Transgastric necrosectomy by laparoscopy is a valuable first-line surgical option for patients with symptomatic necrotizing pancreatitis, particularly in cases without prior interventions. This minimally invasive technique helps reduce major complications and mortality, offering a less invasive alternative to traditional open necrosectomy. The multidisciplinary approach and careful postoperative management were crucial to the patient’s favorable outcome. The case highlights the potential of transgastric necrosectomy as an effective treatment strategy in managing complex pancreatitis cases, including those with associated duodenal perforation [2].

Substance use and inhalation injury in adult burn patients:retrospective study of the impact on outcomes

Background:Substance use,alcohol use,and smoking use have all been associated with burn *** studies have investigated associations with substances,alcohol,smoking,inhalational only burns,and patient *** purpose of the study was to identify risk factors for pulmonary failure in patients suffering inhalation injury,focusing on the impact of substance,alcohol,and cigarette ***:This is a single-center retrospective analysis of 115 patients admitted to the Johns Hopkins Bayview Burn center with inhalational injury from January 1,2010,through September 30,*** were excluded if they were under the age of 18 years or had burn involvement of the skin>5%.Primary outcome variables measured were if patients were intubated,length of total time intubated,substance use,alcohol use,and smoking *** outcome variables measured were types of substances used(amphetamines,barbiturates,benzodiazepines,cannabinoids,cocaine,methadone,codeine/morphine),total number of substances used,intensive care unit(IcU)length of stay(LOS),hospital LOS,secondary complications,and patient *** was performed with Fisher’s exact test and the Mann-Whitney U test.A sub-group analysis for each substance,alcohol,smoking,and control subgroup was compared to its respective sub-group without substance,alcohol,smoking,and control within the no intubation and intubation group.A sub-group analysis of substance use,alcohol use,smoking use,and control was further analyzed with binomial logistic regression within the intubation ***:Following inhalation injury,50/115(43%)patients required ***-two of the 50(84%)patients intubated had substance use(pcific substances used,26/50(52%)patients intubated were using benzodiazepines(pcocaine(p=0.022).The lengths of intubation,IcU LOS,and hospital LOS with no substanc

The flowerpiercers interactions with a community of high Andean plants

Fault Location Approach to Distribution Networks Based on custom State Estimator

This paper presents a properly designed branchcurrent based state estimator(BcBSE)used as the main core ofan accurate fault location approach(FLA)devoted to distribution *** to the approaches available in the literature,it uses only a limited set of conventional measurementsobtained from smart meters to accurately locate faults at busesor branches without requiring measurements provided by phasor measurement units(PMUs).This is possible due to themethods used to model the angular reference and the faultedbus,in addition to the proper choice of the weights in the stateestimator(SE).The proposed approach is based on a searchingprocedure composed of up to three stages:①the identificationof the faulted zones;②the identification of the bus closest tothe fault;and③the location of the fault itself,searching onbranches connected to the bus closest to the ***,this paper presents a comprehensive assessment of the proposedapproach,even considering the presence of distributed generation,and a sensitivity study on the proper weights required bythe SE for fault location purposes,which can not be found inthe *** show that the proposed BcBSE-basedFLA is robust,accurate,and aligned with the requirements ofthe traditional and active distribution networks.

changes in cinnamic Acid Derivatives Associated with the Habituation of Maize cells to Dichlobenil

The habituation of cell cultures to cellulose biosynthesis inhibitors such as dichlobenil （DcB） represents a valu- able tool to improve our knowledge of the mechanisms involved in plant cell wall structural plasticity. Maize cell lines habituated to lethal concentrations of DcB were able to grow through the acquisition of a modified cell wall in which cellulose was partially replaced by a more extensive network of arabinoxylans. The aim of this work was to investigate the phenolic metabolism of non-habituated and DcB-habituated maize cell cultures. Maize cell cultures were fed [14c]cinnamate and the fate of the radioactivity in different intra-protoplasmic and wall-localized fractions throughout the culture cycle was analyzed by autoradiography and scintillation counting. Non-habituated and habituated cultures did not markedly differ in their ability to uptake exogenous [14c]cinnamic acid. However, interesting differences were found in the radiolabeling of low- and high-Mr metabolites. Habituated cultures displayed a higher number and amount of radiola-beled low-Mr compounds, which could act as reserves later used for polysaccharide feruloylation. DcB-habituated cultures were highly enriched in esterified [14c]dehydrodiferulates and larger coupling products. In conclusion, an extensive and early cross-linking of hydroxycinnamates was observed in DcB-habituated cultures, probably strengthening their cellulose-deficient walls.

A renewal theory based performance and configuration framework of the IEEE 802.11ah RAW mechanism

IEEE 802.11ah is a new Wi-Fi standard for sub-1Ghz communications,aiming to address the challenges of the Internet of Things(IoT).Significant changes in the legacy 802.11 standards have been proposed to improve the network performance in high contention scenarios,the most important of which is the Restricted Access Window(RAW)*** mechanism promises to increase the throughput and energy efficiency by dividing stations into different *** this scheme,only the stations belonging to the same group may access the channel,which reduces the collision probability in dense ***,the standard does not define the RAW grouping *** this paper,we develop a new mathematical model based on the renewal theory,which allows for tracking the number of transmissions within the limited RAW slot contention period defined by the *** then analyze and evaluate the performance of RAW *** also introduce a grouping scheme to organize the stations and channel access time into different groups within the ***,we propose an algorithm to derive the RAW configuration parameters of a throughput maximizing grouping *** additionally explore the impact of channel errors on the contention within the time-limited RAW slot and the overall RAW optimal *** presented analytical framework can be applied to many other Wi-Fi standards that integrate periodic channel *** simulations using the MATLAB software validate the analytical model and prove the effectiveness of the proposed RAW configuration scheme.
