Enamel microabrasion: An overview of clinical and scientific considerations

Superficial stains and irregularities of the enamelare generally what prompt patients to seek dental intervention to improve their smile. These stains or defects may be due to hypoplasia, amelogenesis imperfecta, mineralized white spots, or fluorosis, for which enamel microabrasion is primarily indicated. Enamel microabrasion involves the use of acidic and abrasive agents, such as with 37% phosphoric acid and pumice or 6% hydrochloric acid and silica, applied to the altered enamel surface with mechanical pressure from a rubber cup coupled to a rotatory mandrel of a lowrotation micromotor. If necessary, this treatment can be safely combined with bleaching for better esthetic results. Recent studies show that microabrasion is a conservative treatment when the enamel wear is minimal and clinically imperceptible. The most important factor contributing to the success of enamel microabrasion is the depth of the defect, as deeper, opaque stains, such as those resulting from hypoplasia, cannot be resolved with microabrasion, and require a restorative approach. Surface enamel alterations that result from microabrasion, such as roughness and microhardness, are easily restored by saliva. Clinical studies support the efficacy and longevity of this safe and minimally invasive treatment. The present article presents the clinical and scientific aspects concerning the microabrasion technique, and discusses the indications for and effects of the treatment, including recent works describing microscopic and clinical evaluations.

Transformation of leather waste into microwave-activated and N-doped carbon for Li-S batteries

The use of carbon from waste biomass has the potential to buffer the drawbacks of Li-S batteries and improve their overall performance. Chrome-Tanned-LeatherShavings(CTLS) are undesirable wastes, highly available for transformation into porous carbon. Microwave activation is an efficient state-of-the-art procedure to obtain unique porous structures and nitrogen incorporation can improve the performance of porous carbons. This study proposes the transformation of CTLS into a microwave activated porous carbon, and then was modified to introduce nitrogen. A microporous activated carbon with a specific surface area of 556 m2g-1 with honeycomb-like structure was obtained. Both urea thermal decomposition and ethanolamine impregnation improves the nitrogen and amine presence of surface groups. However,the results indicated that only urea treatment was effective in improving the initial capacity of the cell(1363 mAhg-1), which is probably linked with the sorption of long-chain polysulfides. This investigation confirms that it is possible to obtain carbon materials from CTLS, for use as sulfur-host cathodes in Li-S batteries, and improve their performance with urea thermal decomposition. Furthermore, a Radial basis function neural network was fitted to provide statistical support to the experimental results. The trained model corroborated the importance of nitrogen content of the carbons in the discharge capacity of the cells.

Methylation of DAPK and THBS1 genes in esophageal gastric-type columnar metaplasia

AIM: To explore methylation of DAPK, THBS1, CDH-1, and p14 genes, and Helicobacter pylori(H. pylori) status in individuals harboring esophageal columnar ***: Distal esophageal mucosal samples obtained by endoscopy and histologically diagnosed as gastric-type(non-specialized) columnar metaplasia, were studied thoroughly. DNA was extracted from paraffin blocks, and methylation status of deathassociated protein kinase(DAPK), thrombospondin-1(THBS1), cadherin-1(CDH1), and p14 genes, was examined using a methyl-sensitive polymerase chain reaction(MS-PCR) and sodium bisulfite modification protocol. H. pylori cag A status was determined by ***: In total, 68 subjects(33 females and 35 males), with a mean age of 52 years, were included. H. pylori cag A positive was present in the esophageal gastric-type metaplastic mucosa of 18 individuals. DAPK, THSB1, CDH1, and p14 gene promoters were methylated by MS-PCR in 40(58.8%), 33(48.5%), 46(67.6%), and 23(33.8%) cases of the 68 esophageal samples. H. pyloristatus was associated with methylation of DAPK(P = 0.003) and THBS1(P = 0.019).CONCLUSION: DNA methylation occurs in cases of gastric-type(non-specialized) columnar metaplasia of the esophagus, and this modification is associated with H. pylori cag A positive infection.

Pre-and postoperative systemic hemodynamic evaluation in patients subjected to esophagogastric devascularization plus splenectomy and distal splenorenal shunt:A comparative study in schistomomal portal hypertension

AIM: To investigate the systemic hemodynamic effects of two surgical procedures largely employed for treatment of schistosomal portal hypertension. METHODS: Thirty-six patients undergoing elective surgical treatment of portal hypertension due to hepatosplenic mansonic schistosomiasis were prospectively evaluated. All patients were subjected to preoperative pulmonary artery catheterization; 17 were submitted to esophagogastric devascularization and splenectomy (EGDS) and 19 to distal splenorenal shunt (DSRS). The systemic hemodynamic assessment was repeated 4 d after the surgical procedure. RESULTS: Preoperative evaluation revealed (mean ± SD) an increased cardiac index (4.78 ± 1.13 L/min per m2),associated with a reduction in systemic vascular resistance index (1457 ± 380.7 dynes.s/cm5.m2). The mean pulmonary artery pressure (18 ± 5.1 mmHg) as well as the right atrial pressure (7.9 ± 2.5 mmHg) were increased,while the pulmonary vascular resistance index (133 ± 62 dynes.s/cm5.m2) was decreased. Four days after EGDS,a significant reduction in cardiac index (3.80 ± 0.4 L/min per m2,P < 0.001) and increase in systemic vascular resistance index (1901.4 ± 330.2 dynes.s/cm5. m2,P < 0.001) toward normal levels were observed. There was also a significant reduction in pulmonary artery pressure (12.65 ± 4.7 mmHg,P < 0.001) and no significant changes in the pulmonary vascular resistance index (141.6 ± 102.9 dynes.s/cm5.m2). Four days after DSRS,a non-significant increase in cardiac index (5.2 ± 0.76 L/min per m2) and systemic vascular resistance index (1389 ± 311 dynes.s/cm5.m2) was observed. There was also a non-significant increase in pulmonary artery pressure (19.84 ± 5.2 mmHg),right cardiac work index (1.38 ± 0.4 kg.m/m2) and right ventricular systolic work index (16.3 ± 6.3 g.m/m2),without significant changes in the pulmonary vascular resistance index (139.7 ± 67.8 dynes.s/cm5.m2). Colorectal cancer(CRC)is the second leading cause of cancer related mortality,though improvements in CRC screening,systemic and local treatment,and surgical technique have steadily won victories in the battle against CRC,and the rate of mortality from CRC is steadily declining(1).However,perhaps no clinical-oncologic scenario is as unstandardized as the management of colorectal-liver metastases(CRLM)and their evaluation for consideration of resectability.

ALPPS and the endless pursuit of hepatic resectability

Associated liver partition and portal vein ligation for staged hepatectomy(ALPPS)is an established resection strategy well into its teenage *** initial controversy,it has proven to be an effective and safe approach for primary unresectable liver remnant anatomy in high volume centers(1-5).ALPPS involves portal vein ligation and parenchymal split to obtain rapid,enhanced hypertrophy of the future liver remnant and resection during second stage of the operation(6).This allows for resection with liver remnants as small as 20%of original volume(7,8).It is the most rapid tool in achieving hypertrophy compared to other modalities(9-11).Nevertheless,ALPPS remains a contentious topic in the HPB literature(12-15).We believe that ALPPS is an excellent option for highly selected patients with expanded indications,especially when conducted by experienced surgical teams(5,16).

Phase-sensitive optical time-domain reflectometry(CDOTDR)is a well-established technique that provides spatiotemporal measurements of an environmental variable in real *** unique capability is being leveraged in an everincreasing number of applications,from energy transportation or civil security to *** date,a wide number of different approaches have been implemented,providing a plethora of options in terms of performance(resolution,acquisition bandwidth,sensitivity or range).However,to achieve high spatial resolutions,detection bandwidths in the GHz range are typically required,substantially increasing the system cost and ***,we present a novel OOTDR approach that allows a customized time expansion of the received optical ***,the presented technique reaches cm-scale spatial resolutions over t km while requiring a remarkably low detection bandwidth in the MHz *** approach relies on the use of dual-comb spectrometry to interrogate the fibre and sample the backscattered *** phase-spectral coding is applied to the employed combs to maximize the signal-to-noise ratio of the sensing scheme.A comparison of the proposed method with alternative approaches aimed at similar operation features is provided,along with a thorough analysis of the new *** results demonstrate a radically novel high-resolution OOTDR scheme,which could promote new applications in metrology,borehole monitoring or aerospace.
