BACKGROUND An isthmocele is a scar defect in the uterine wall due to a cesarean section. Its prevalence is unknown,but it has been associated with secondary ***,we present a case where the patient suffered from an isthmocele that was associated with persistent hydrometra,which developed during in vitro *** SUMMARY The patient underwent hysteroscopic surgery,which successfully resolved the isthmocele as well as the hydrometra. Afterward,two high-quality,euploid embryos,determined by morphological assessment and pre-implantation genetic diagnostic testing,were transferred. This resulted in uterine pregnancy,as determined byserum β-human chorionic gonadotropin levels on day 14(180 m U/mL) and ultrasound-confirmed presence of a gestational sac with a positive embryocardia at week 6. The pregnancy reached 36 wk without any complications,and the product was born in good health. We report a successful isthmocele treatment in a patient with secondary infertility,in which the isthmocele was the cause of persistent *** Hydrometra caused by secondary cesarean is an infertility factor,which can be corrected by hysteroscopy plus ablation of the isthmocele.

AIM To demonstrate the non-inferiority(15% non-inferiority limit) of monotherapy with tenofovir disoproxil fumarate(TDF) vs the combination of lamivudine(LAM) plus adefovir dipivoxil(ADv) in the maintenance of virologic response in patients with chronic hepatitis B(CHB) and prior failure with *** This study was a Phase Iv prospective, randomized, open, controlled study with 2 parallel groups(TDF and LAM+ADv) of adult patients with hepatitis B e antigen(HBe Ag)-negative CHB, prior failure with LAM, on treatment with LAM+ADv for at least 6 mo, without prior resistance to ADv and with an undetectable viral load at the start of the study, in 14 Spanish hospitals. The follow-up time for each patient was 48 wk after randomization, with quarterly visits in which the viral load, biochemical and serological parameters, adverse effects, adherence to treatment and consumption of hospital resources were *** Forty-six patients were evaluated [median age: 55.4 years(30.2-75.2); 84.8% male], including 22 patients with TDF and 24 with LAM+ADv. During study development, hepatitis B virus DNA(HBv-DNA) remained undetectable, all patients remained HBe Ag negative, and hepatitis B surface antigen(HBs Ag) positive. Prosthetic valve thrombosis (PvT) is a rare but serious entity. Its diagnosis and treatment can be challenging and mortality remains high regardless of its management. A 50-year-old man admitted with cardiac arrest, whose physical examination and echocardioscopy suggested PvT confirmed with fluoroscopy. Intra-aortic fibrinolysis was the treatment chosen with successful results.

This contribution discusses about the rheological, kinematic and dynamic frameworks necessary to produce recumbent and upright folds from syn-orogenic granitic massifs that were formed during an early stage of magma genesis related to the onset of a migmatitic dome. Syn-kinematic granitoids occurring within the high-grade infrastructure of the Padron migmatitic dome(NW Iberia) are deformed into largescale recumbent folds(D_2) that are later affected by upright folds(D_3). Petrostructural analysis of a selected area of this dome reveals that after a period of crustal thickening(D_1), NNW-directed extensional flow gave way to recumbent folds and penetrative axial plane foliation(S_2). Superimposed subhorizontal compression resulted in upright folds(D_3). A closer view into the dynamics of the dome allows exploring the factors that may condition the nucleation of folds with contrasting geometries during progressive deformation of molten continental crust. The formation of folds affecting syn-kinematic granitoids suggests a cooling metamorphic path in migmatitic domes. Active and passive folding mechanisms require a crystallizing(cooling) magma to nucleate folds. A more competent metamorphic host inhibits fold nucleation from much less competent magmas. As it crystallizes, magma becomes more rigid(competent),and approaches viscosity values of its host. Passive folding is favored when no significant competence contrast exists between magma and host, so this folding mechanism is more likely shortly after magma genesis and emplacement. In such conditions, and under dominant subhorizontal flow accompanied by flattening(D_2),passive folding would produce isoclinal recumbent geometries. Further magma cooling introduces a shift into the rheological behavior of partially molten crust. Lymphomatoid granulomatosis, currently called as extranodal angiocentric and angiodestructive immunoproliferative disorder, is a rare entity of unclear etiology. It involves most frequently lungs, central nervous system and skin. The clinical course is variable, but mortality is high. Today, it represents a diagnostic challenge because it can emulate autoimmunity, infection, and malignancy processes. Optimal therapy scheme is still unknown. We report the case of a 20 year-old man presenting with fever, weight loss, sweating, multiple bilateral lung nodules on the chest X-ray and cutaneous involvement.

Glycogen constitutes the major carbon storage source in cyanobacteria, as starch in algae and higher plants. Glycogen and starch synthesis is linked to active photosynthesis and both of them are degraded to glucose in the dark to maintain cell metabolism. Control of glycogen biosynthesis in cyanobacteria could be mediated by the regulation of the enzymes involved in this process, ADP-glucose pyrophosphorylase （AGP） and glycogen synthase, which were identified as putative thioredoxin targets. We have analyzed whether both enzymes were subjected to redox modification using purified recombinant enzymes or cell extracts in the model cyanobacterium Synechocystis sp. PCC 6803. Our results indicate that both AGP and glycogen synthases are sensitive to copper oxidation. However, only AGP exhibits a decrease in its enzymatic activity, which is recovered after reduction by DTT or reduced thioredoxin （TrxA）, suggesting a redox control of AGP. In order to elucidate the role in redox control of the cysteine residues present on the AGP sequence （C45, C185, C320, and C337）, they were replaced with serine. All AGP mutant proteins remained active when expressed in Synechocystis, although they showed different electrophoretic mobility profiles after copper oxidation, reflecting a complex pattern of cysteines interaction.

Evaluation of a voluntary Control Program for the Detection of Bovine Leukemia virus Antibodies Based on Agar Gel Immunodiffusion Test in Dairy Farms in Costa Rica

Cattle from 20 dairy farms were serologically tested over a five-year period using agar gel immunodiffusion test (AGIDT) as part of a voluntary Bovine Leukemia virus (BLv) control program. After five years of removing infected animals from the herds based on BLv-AGIDT serological status, blood samples from 332 cattle in these farms were collected and analyzed side by side by AGIDT and enzyme-linked immunosorbent assay (ELISA) to detect antibodies against BLv. AGIDT detected 29.2% (97) and 16.0% (53) of the animals as positive and weak positive respectively, whereas ELISA detected 58.2% (193) cattle as positive. The prevalence of BLv-antibodies determined with AGIDT in the dairy farms oscillated between 0% and 86%, whereas prevalence determined by ELISA ranged between 28% and 100% in the same farms. Although both techniques showed similarly results in farms with high BLv-prevalence, ELISA detected a larger proportion of BLv-positive, especially in farms with low or no BLv-prevalence based on AGIDT, leading to wrong assumptions in terms of farm level control efforts. Our results strongly suggest that AGIDT alone is inadequate to implement BLv control programs and ELISA is a more adequate test for BLv surveillance and control programs.
