Inorganic phosphate in the development and treatment of cancer:A Janus Bifrons?

Inorganic phosphate(Pi) is an essential nutrient to living organisms. It is required as a component of the energy metabolism,kinase/phosphatase signaling and in the formation and function of lipids,carbohydrates and nucleic acids and,at systemic level,it plays a key role for normal skeletal and dentin mineralization. Pi represents an abundant dietary element and its intestinal absorption is efficient,minimally regulated and typically extends to approximately 70%. Maintenance of proper Pi homeostasis is a critical event and serum Pi level is maintained within a narrow range through an elaborate network of humoral interactions and feedback loops involving intestine,kidney,parathyroid gland and bone,and depends on the activity of a number of hormones,including parathyroid hormone,1,25-dihydroxy vitamin D,and fibroblast growth factor 23 as major regulators of Pi homeostasis. Notably,Pi intake seemingly continues to increase as a consequence of chronic high-phosphorus(P) diets deriving from the growing consumption of highly processed foods,especially restaurant meals,fast foods,and convenience foods. Several recent reports have generated significant associations between high-P intake or high-serum Pi concentration and morbidity and mortality. Many chronic diseases,including cardiovascular diseases,obesity and even cancer have been proposed to be associated with high-P intakes and high-serum Pi concentrations. On the other hand,there is also evidence that Pi can have antiproliferative effects on some cancer cell types,depending on cell status and genetic background and achieve additive cytotoxic effects when combined with doxorubicin,illustrating its potential for clinical applications and suggesting that up-regulating Pi levels at local sites for brief times,might contribute to the development of novel and cheap modalities for therapeutic intervention in some tumours. Simulating the fate of indigenous antibiotic resistant bacteria in a mild slope wastewater polluted stream

The fate of indigenous surface-water and wastewater antibiotic resistant bacteria in a mild slope stream simulated through a hydraulic channel was investigated in outdoor *** effect of（i） natural（dark） decay,（ii） sunlight,（iii） cloudy cover,（iv） adsorption to the sediment,（v） hydraulic conditions,（vi） discharge of urban wastewater treatment plant（UWTP）effluent and（vii） bacterial species（presumptive Escherichia coli and enterococci） was ***-life time（T1/2） of E. coli under sunlight was in the range 6.48–27.7 min（initial bacterial concentration of 10^5 CFU/mL） depending on hydraulic and sunlight conditions. E. coli inactivation was quite similar in sunny and cloudy day experiments in the early 2 hr, despite of the light intensity gradient was in the range of 15–59 W/m^2; but subsequently the inactivation rate decreased in the cloudy day experiment（T1/2= 23.0 min） compared to sunny day（T1/2= 17.4 min）. The adsorption of bacterial cells to the sediment（biofilm） increased in the first hour and then was quite stable for the remaining experimental time. Finally, when the discharge of an UWTP effluent in the stream was simulated, the proportion of indigenous antibiotic resistant E. coli and enterococci was found to increase as the exposure time increased, thus showing a higher resistance to solar inactivation compared to the respective total populations.

Long-term results of endosurgical and open surgical approach for Zenker diverticulum

AIM: To assess the effectiveness of minimally invasive versus traditional open surgical approach in the treatment of Zenker diverticulum. METHODS: Between 1976 and 2006, 297 patients underwent transoral stapling (n = 181) or stapled diverticulectomy and cricopharyngeal myotomy (n = 116). Subjective and objective evaluations of the outcome of the two procedures were made at 1 and 6 mo after operation, and then every year. Long-term follow-up data were available for a subgroup of patients at a minimum of 5 and 10 years. RESULTS: The operative time and hospital stay were markedly reduced in patients undergoing the endosurgical approach. Overall, 92% of patients undergoing the endosurgical approach and 94% of those undergoing the open approach were symptom-free or were significantly improved after a median follow-up of 27 and 48 mo, respectively. At a minimum follow-up of 5 and 10 years, most patients were asymptomatic after both procedures, except for those individuals undergoing an endosurgical procedure for a small diverticulum (< 3 cm). CONCLUSION: Both operations relieve the outflow obstruction at the pharyngoesophageal junction, indicating that cricopharyngeal myotomy has an important therapeutic role in this disease independent of the resection of the pouch and of the surgical approach. Diverticula smaller than 3 cm represent a formal contraindication to the endosurgical approach because the common wall is too short to accommodate one cartridge of staples and to allow complete division of the sphincter.

Curved footbridges supported by a shell obtained through thrust network analysis

After Maillart's concrete curved arch bridges were built before the Second World War, in the second half of the past century and this century, many curved bridges have been built with both steel and concrete. Conversely, since the construction of Musmeci's shell supported bridge in Potenza, few shell bridges have been constructed. This paper explains how to design a curved footbridge supported by an anticlastic shell by shaping the shell via a thrust network analysis(TNA). By taking advantage of the peculiar properties of anticlastic membranes, the unconventional method of shaping a shell by a TNA is illustrated. The shell top edge that supports the deck has an assigned layout, which is provided by the road curved layout. The form of the bottom edge is obtained by the form-finding procedure as a thrust line, by applying the thrust network analysis(TNA) in a non-standard manner,shaping the shell by applying the boundary conditions and allowing relaxation. The influence of the boundary conditions on the bridge shape obtained as an envelope of thrust lines is investigated. A finite element analysis was performed. The results indicate that the obtained shell form is effective in transferring deck loads to foundations via compressive stresses and taking advantage of concrete mechanical properties.

Heart-touching Chilaiditi's syndrome

Symptomatic hepato-diaphragmatic interposition of a bowel loop or Chilaiditi's syndrome is a peculiar anatomical condition most often found by chance. Its described symptoms range from intermittent, mild abdominal pain and dyspepsia to acute intestinal obstruction. We report a case of hepato-diaphragmatic migration of the hepatic flexure of the colon associated to an unusual, heretofore unreported, angina-like pain exclusively evoked by the left lateral decubitus. To maximize the chance of observing anatomical changes in different postures, computed tomography of the chest and abdomen was performed after air insufflation into the colon. While frank herniation into the chest was excluded, the scan showed that the hepatic flexure-with the interposition of the diaphragm-came in contact with the right side of the heart in the left lateral, but not in the supine, decubitus. This finding was reproduced by echocardiography which also showed virtually unaltered hemodynamics after the change of posture. ECG, left and right ventricular global and regional function as well as cardiac injury markers also remained unchanged during the maneuver, indicating that the pain evoked by the latter was unlikely due to myocardial ischemia. This case suggests that Chilaiditi's syndrome should be included among the possible, although rare,causes of unexplained angina-like symptoms.

Natural history of cytomegalovirus infection in a series of patients diagnosed with moderate-severe ulcerative colitis

AIM： To evaluate the natural history of human cytomegalovirus （HCMV） infection in a series of 28 ulcerative colitis patients in whom the search for HCMV was positive. METHODS： A series of 85 patients with moderate-se- vere ulcerative colitis flare-up were evaluated for a HCMV search by performing a haematoxylin and eosin stain, immunohistochemical assay and nested polymerase chain reaction on rectal biopsies. Among 85 screened patients （19 of whom were steroid resistant/dependant）, 28 were positive for HCMV; after remission the patients were followed up clinically and histologically. RESULTS： Among the 22 patients with complete follow- up, in 8 （36%） patients HCMV-DNA persisted in the in- testinal specimens. Among the HCMV positive patients, 4 （50%） experienced at least one moderate-severeflare-up of colitis without evidence of peripheral HCMV. Among the 14 HCHV negative patients, 3 with pouches developed pouchiUs and 5 out of 11 （45%） experienced a colitis flare-up. CONCLUSION： Our preliminary results suggest that HCHV may remain in the colon afber an acute coltis flare- up despite remission; it seems that the virus is not responsible for the disease relapse.

Laparoscopic diagnosis of pleural mesothelioma presenting with pseudoachalasia

Pseudoachalasia due to pleural mesothelioma is an extremely rare condition. A 70-year-old woman presented with progressive dysphagia for solid and liquids and a mild weight loss. A barium swallow study revealed an esophageal dilatation and a smoothly narrowed esophagogastric junction. An esophageal manometry showed absence of peristalsis. Endoscopy demonstrated an extrinsic stenosis of the distal esophagus with negative biopsies. A marked thickening of the distal esophagus and a right-sided pleural effusion were evident at computed tomography （CT） scan, but cytological examination of the thoracic fluid was negative. Endoscopic ultrasound showed the disappearance of the distal esophageal wall stratification and thickening of the esophageal wall. The patient underwent an explorative laparoscopy. Biopsies of the esophageal muscle were consistent with the diagnosis of epithelioid type pleural mesothelioma. An esophageal stent was placed for palliation of dysphagia. The patient died four months after the diagnosis. This is the first reported case of pleural mesothelioma diagnosed through laparoscopy.

Burning rate analysis of laser controlled 5-aminotetrazole propellant

As an innovative propulsion technique, laser augmented chemical propulsion(LACP) seems superior to the traditional ones. However, the corresponding combustion theories have still to be ascertained for LACP. Burning rate of 5-aminotetrazole(5-ATZ) propellant has been studied by testing pressed samples under different combustor pressures and laser powers. Based on micro computed tomography(Micro CT),an advanced thickness-over-time(TOT) method to characterize the regression of the produced nonplanar burning surface is established. Because of a shell structure covering the combustion surface,the burning rate of the implemented 5-ATZ propellant is not constant during laser ablation. Resorting to functional fitting, a new law of non-constant burning including the effect of the observed unique burning surface structures is proposed. Accordingly, applicable combustion conditions of 5-ATZ based propellants have been preliminarily speculated for future research activities.
