Objective: Analysis of clinical documents such as bone mineral density (BmD) reports is an important component of program evaluation because it can provide insights into the accuracy of assessment of fracture risk communicated to patients and practitioners. Our objective was to compare fracture risk calculations from BmD test reports to those based on the 2010 Canadian guidelines. methods: We retrieved BmD reports from fragility fracture patients screened through a community hospital fracture clinic participating in Ontario’s Fracture Clinic Screening Program. Fracture risk was determined according to the 2010 Canadian guidelines using age, sex, and T-score at the femoral neck, in addition to three clinical factors. Three researchers classified patients’ fracture risk until consensus was achieved. Results: We retrieved reports for 17 patients from nine different BmD clinics in the Greater Toronto Area. Each patient had a different primary care physician and all BmD tests were conducted after the 2010 Canadian guidelines were published. The fracture risk of 10 patients was misclassified with 9 of the 10 reports underestimating fracture risk. Nine reports acknowledged that the prevalence of a fragility fracture raised the risk category by one level but only four of these reports acknowledged that the patient had, or may have sustained, a fragility fracture. When we raised fracture risk by one level according to these reports, eight patients were still misclassified. Fracture risk in the majority of these patients remained underestimated. Inconsistent classification was found in the majority of cases where reports came from the same clinic. Four reports described risk levels for two different types of risk. Conclusions: more than half of patients received BmD reports which underestimated fracture risk. Bone health management recommendations based on falsely low fracture risk are likely to be sub-optimal.

Due to global land surface warming, severe temperature events are expected to occur more frequently and more extremely causing changes in biodiversity and altering movement and survival of large herbivores. There are increasing observations of escalating wildlife range losses worldwide. In this study, we investigated 15 large wild herbivores (4 migratory, 1 dispersing and 10 residents) and their potential range changes in relation to projected temperatures changes based on three Representative Concentration Pathways (RCPs) 2.6, 4.5 and 8.5. Previous studies of Kenyan savannah have shown that increases in temperature can reduce the densities of wildlife significantly and after certain thresholds the species can be lost in those landscapes. The range maps of the 15 species were developed from aerial censuses that have been conducted in the arid and semi-arid lands of Kenya. We analysed temperature changes for the three RCPs for the periods 2030s, 2050s and 2070s. And based on the temperature threshold for each of the 15 species we analysed which wildlife range areas will be lost. Our results project that for the RCP 2.6, 3 out of the 15 species are projected to lose more than 50% of their range by the year 2030s, and 5 out 15 by 2050s and 4 of 15 by 2070s. The second climate scenario of RCP 4.5 projects that by 2030s, 3 species will lose more than 50% of their range, and in 2050s and 2070s 5 species. The RCP 8.5 which is the extreme scenario of temperature changes projects 5 species to lose their range by 50% in 2030s, 7 species by 2050s and 10 species by 2070s. The extent of range loss was different among species but was severe for buffalo, Thomson’s gazelle, waterbuck, and wildebeest which are also water dependent species. However, the elephant, gerenuk, hartebeest, lesser kudu, and oryx are expected to retain most of their range in all the RCPs scenarios. Background: Low level of transfusion transmissible infections (TTIs) is an indicator of a well-performing blood donor program. Aim: The study was designed to estimate the prevalence of TTIs and to evaluate the demographic characteristics of reactive and non-reactive blood donors in Zimbabwe in 2018. methods: A cross-sectional study was conducted using routinely available data from January to December 2018 in five branches of National Blood Service Zimbabwe (NBSZ). After initial screening for high-risk behavior with a questionnaire, weight, blood pressure and hemoglobin level, eligible donors were invited for blood donation. The following laboratory tests for TTIs were done: antibodies and antigen tests for human immunodeficiency virus 1 and 2 (HIV 1/2), tests for the surface antigen of hepatitis B virus (HBV), testing for hepatitis C virus (HCV) antibody and antibodies for treponema pallidum. Information on age, gender, NBSZ branch, marital status, occupation, donor type (first time/repeat) and TTIs test results were extracted from the NBSZ electronic database (e-Delphyn blood bank software). Results: Out of a sample of 1586 blood donors, thirteen (0.81%) were reactive to at least one TTI marker;five (0.32%) were reactive for human immunodeficiency virus, seven (0.44%) for hepatitis B and one (0.06%) for syphilis. There were no samples with co-infection and hepatitis C virus markers. The prevalence of TTIs was highest in the 31 - 45 years age group (2.3%) and among first-time blood donors (4.7%). The prevalence of all TTI was low with the highest prevalence of 0.44% for the hepatitis B virus. Conclusion: Continued concerted efforts will help to maintain satisfactory blood safety in Zimbabwe.

The study evaluated the role of different substrates on water quality, growth and survival of Labeo victorianus fry. Random sampling was used to select 225 fry, stocked at 100/m3 and fed on shell free artemia® (40% - 60% crude protein). Each treatment was carried out in triplicates using gravel and sand as substrates and water media without substrate (control). Growth and survival of the fry was monitored over 4 weeks period at a hatchery set-up. Physicochemical parameters were measured weekly while mortality was documented daily. Substrate type had an effect on water quality, growth and survival of fry. Dissolved Oxygen, growth and survival was higher in tanks under gravel substrate than sand and control while mortality was higher (22%) for sand (p ≤ 0.05) than gravel and sand treatments. Fry in gravel substrate had the best growth response (1.77 ± 0.92 g) while control and sand recorded 1.57 ± 0.90 g and 1.45 ± 0.97 g respectively. In contrast, growth was homogenous during the first 2 weeks of the trial but gravel substrate fry significantly out-grew sand and control in the remaining 3 weeks. However, fry in sand and control experienced compensatory growth towards the end of the study. Therefore, gravel substrate enhances L. victorianus performance and is recommended in the introduction of the species into aquaculture and re-stocking back to the wild.

Development of an effective vaccine against the leading human bacterial pathogen group A Streptococcus(GAS)is a public health *** species defining group A cell wall carbohydrate(GAC,Lancefield antigen)can be engineered to remove its immunodominant N-acetylglucosamine(GlcNAc)side chain,implicated in provoking autoimmune cross-reactivity in rheumatic heart disease,leaving its polyrhamnose core(GACPR).Here we generate a novel protein conjugate of the GACPR and test the utility of this conjugate antigen in active *** of conjugation to a standard carrier protein,we selected SpyAD,a highly conserved GAS surface protein containing both B-cell and T-cell epitopes relevant to the bacterium that itself shows promise as a vaccine *** was synthesized using the XpressTm cell-free protein expression system,incorporating a non-natural amino acid to which GACpr was conjugated by site-specific click chemistry to yield high molecular mass SpyAD-GACPR conjugates and avoid disruption of important T-cell and B-cell immunological *** conjugated SpyAD-GACPR elicited antibodies that bound the surface of multiple GAS strains of diverse m types and promoted opsonophagocytic killing by human *** immunization of mice with a multivalent vaccine consisting of SpyAD-GACPR,together with candidate vaccine antigens streptolysin O and C5a peptidase,protected against GAS challenge in a systemic infection model and localized skin infection model,without evidence of cross reactivity to human heart or brain tissue *** general approach may allow GAC to be safely and effectively included in future GAS subunit vaccine formulations with the goal of broad protection without autoreactivity.

The treatment and management of diabetes mellitus has been a challenge to achieve a total cure using conventional drugs. Hypoglycemic and hypolipidemic effects of bitter kola (m>Garcinia kolam>) seed extract on alloxan-induced diabetic rats were studied as a local product. Albino rats weighing about 200 g were investigated in groups using 200 - 800 mg/kg weight of bitter kola seed extract and the sample examined for glucose and lipid profiles using enzymatic methods and statistically analyzed using statistical package for social science (SPSS) windows, version 20.0. The results were expressed as the mean ± SD. The results for the serum glucose and low density lipoprotein cholesterols (LDL-C) concentrations of untreated diabetic rats, and treated rats showed a significant (P m 200 - 800 mg/kg when compared to the control rats. Total cholesterol (TC) and triglycerides (TG) concentrations for untreated diabetic rats, and treated rats showed increase in concentration which was not significant (P > 0.05) from 200 - 800 mg/kg weight when compared with the controls. High density lipoproteins (HDL-C) concentrations for untreated diabetic rats, and treated rats showed increase in concentration which was significant (P m 200 - 800 mg/kg weight when compared with the controls. This study confirms the hypoglycemic and hypolipidemic effects of bitter kola (m>Garcinia kolam>) seed extract.

Background In 2015,herpes simplex virus 1(HSV-1)-derived talimogene laherparepvec(T-VEC)was the first oncolytic virus approved by the US Food and Drug Administration as a therapeutic agent for cancer ***,its antitumor application is limited to local treatment of melanoma,and there is a lack of understanding of the mechanisms underlying the regulation of HSV-1 replication in cancer cells and the associated antitumor *** hypothesized that increasing the replication capacity of HSV-1 in tumor cells would enhance the antitumor effect of this *** We systematically identified IFN-stimulated genes induced by HSV-1 by performing functional screens and clarified the mechanism by which BACH1 acts against ***,we tested the effect of BACH1 deficiency on immunogenic cell death induced by ***,we investigated the antitumor effect of BACH1 deficiency on HSV-1 in mCA205 and B16 murine tumor *** We identified eight IFN-stimulated genes(ISGs)controlling HSV-1 replication,among which BTB and CNC homology 1(BACH1)suppressed HSV-1 replication by inhibiting the transcription of ICP4,ICP27,and *** of Bach1 function not only increased HSV-1 proliferation but also promoted HSV-1-induced cell apoptosis,HmGB1 secretion,and calreticulin exposure in tumor *** importantly,hemin,an FDA-approved drug known to downregulate BACH1,significantly enhanced HSV-1-mediated antitumor activity with increased T lymphocyte infiltration at the tumor *** Our studies uncovered a novel antiviral activity of BACH1 and provided a new strategy for improving the clinical efficiency of the oncolytic virus HSV-1.
