Iatrogenic Bile Duct Inj.ries after Cholecystectomy, Is the Laparoscopic Approach a Good Idea?

Obj.ctive: The treatment of iatrogenic bile duct inj.ries is still a challenge for hepatobiliary and general surgeons. Roux-en-Y hepaticoj.j.nostomy, one of the most appropriate techniques for the treatment of circumferential lesions, either occurring less than 2 cm from the bifurcation or in the bifurcation of the common hepatic duct, requires experience in advanced laparoscopy and hepatobiliary surgery. This study aims to present the results of laparoscopic hepaticoj.j.nostomy (LHj. for the treatment of iatrogenic bile duct inj.ries (IBDI). Methods: A retrospective study analyzing the medical records of patients diagnosed with IBDI and treated using LHj.of patients at the Hospital S?o j.sé do Avaí (HSj.). Sex, age, previous cholecystectomy technique, signs and symptoms, postoperative complications, length of stay, inj.ry classification, and time elapsed from inj.ry to diagnosis were analyzed. Magnetic resonance cholangiography (MRC), endoscopic retrograde cholangiopancreatography (ERCP) or intraoperative cholangiography. Results: From March 2006 to December 2018, six patients underwent LHj. In five cases (83.33%), the primary operation was a laparoscopic cholecystectomy (LC) and in one patient (13.66%) open cholecystectomy. The most frequent clinical sign was j.undice. The mean surgical time was 153.2 minutes (range: 115 to 206 minutes), and the hospital stay was 3 to 7 days (mean: 4.16 days). One patient had infection of the umbilical trocar incision and one patient presented with stenosis of the hepaticoj.j.nal anastomosis and was treated with radioscopic pneumatic dilatation. Conclusion: LHj.for circumferential and total IBDI either diagnosed early (during surgery) or late, may be a safe and effective option, with similar results to the conventional technique, a low complication rate and all the known advantages of minimally invasive surgery.

Outcomes of Total and Subtotal Laparoscopic Gastrectomy with D2 Lymphadenectomy in Advanced Gastric Cancer in a Brazilian Hospital

Background: Although laparoscopic gastrectomy is becoming more popular as a curative therapy for gastric cancer, there are concerns about its oncological adequacy. We have compared the outcomes of laparoscopic total gastrectomy (LTG) and laparoscopic subtotal gastrectomy (LSG), both with modified D2 lymphadenectomies for the treatment of advanced gastric cancers. Aim: To compare the outcomes of laparoscopic (total and subtotal) gastrectomy with modified D2 lymphadenectomy for the treatment of gastric cancer, contributing to the literature regarding the overall survival of these patients and postoperative complications. Methods: From 1993 to 2014, 239 patients were operated on laparoscopic gastrectomy at our department. The routinely laparoscopic gastrectomy was performed in all patients with gastric cancer including those presenting with obstruction and bleeding. Data could be collected, on a retrospective way, from 2006 to 2014, from the medical records of 103 patients who underwent LSG (n = 72) or LTG (n = 31). We excluded patients with metastatic disease and those who could not have a complete tumor resection. Results: Most patients were in advanced stages of cancer. Adenocarcinoma was the most common find, with 43% of cases in stage IIA and 31% in stage IIIB. Intracorporeal Roux-en-Y or Billroth II anastomoses were employed. Postoperative complications, for LSG and LTG, were 18% and 35.4%;mortality rate, during hospital stay, was 4.9% and 7.7%;three-year survival rate, 53.1% and 59.3%;and five-year survival rate, 46.9% and 40.7%. Mean hospital stay was 7.08 days, being significantly lower in LSG group (p j.j.nal or esophago-j.j.nal anastomosis leaks. Conclusions: Both LSG and LTG with modified

This study was conducted to investigate the properties of Pr3+-Dy3+doped Zn2Y-type hexaferrite for potential technological applications.X-ray diffraction（XRD）,Fourier transform infrared spectroscopy（FTIR） and field-emission scanning electron microscopy（FESEM） were used to analyze structural *** crystallite size in the synthesized samples varies between 57.54 and 68.57 *** vibrational bands at 400 and 600 cm-1,common to all hexaferrites,were confirmed through *** FESEM analysis reveals an agglomeration of magnetic grains and a decrease in the average grain size from 1.24 to1.06 μ*** M-H loops show that,with x values of 0.0,0.1,and 0.2,the saturation magnetization is determined to be 34.76,34.23,and 32.52 emu/g,*** corresponding coercivity values are21.24,30.39,and 33.99 ***-visible spectroscopy using Tauc theory reveals an increase in the optical band gap from 2.32 to 2.50 eV,indicating a tunable energy band structure by incorporating Pr3+-Dy3+*** dielectric constant increases,whereas AC conductivity decreases with increased Pr3+-Dy3+*** obtained results suggest the potential suitability of these materials for various technological applications.

Thermal effects on wear behavior of additively manufactured NiTi shape memory alloys

NiTi alloys fabricated by laser powder bed fusion(LPBF)additive manufacturing technology not only address the compositional instability resulting from complex processes but also solve the challenges of difficult machining of intricate aerospace ***,there are very few reports on the wear behavior of LPBF-NiTi *** the present work,the effects of microstructure and thermal treatment,including heat treatment and frictional heat,on the wear behavior of LPBF-NiTi alloy and 100Cr6 ball were analyzed through a series of tribological experiments with different sliding *** the average sliding speed increases(0.079–0.216 m/s),the wear rate of the as-built and heat-treated samples tends to decrease in the range of 2.69×10^(-3)–0.97×10^(-3)mm^(3)/*** the heat-treated LPBF-NiTi alloy is 46%harder than the as-built alloy is,the latter has a higher toughness(505 Mj.m^(3))and greater transformation strain of SIM(0.097).This leads to a coupling effect of heat treatment and sliding speed on the wear *** addition,the wear track morphologies under different sliding speeds are asymmetric due to the 24% greater acceleration at the far end from the motor and the 2.15 mm deviation between the maximum speed position and the geometric center of the *** wear modes of the as-built and heat-treated samples included adhesive,abrasive and delamination ***,the wear morphologies and dominant wear modes change with the frictionally caused heat release induced by the sliding speed.

Identifying suitable areas for plenter forest management

Plenter forests, also known as uneven-aged or continuous cover forests enhance forest resilience and resistance against disturbances compared to even-aged forests. They are considered as an adaptation option to mitigate climate change effects. In this study, we present a conceptual approach to determine the potentially suitable area for plenter forest management within central European mixed species forests and apply our approach to the case study area in Styria, the south-eastern Province of Austria. The concept is based on ecological and technical-economic constraints and considers expected future climate conditions and its impact on plenter forest management. For each 1 ha forest pixel, we assess the ecological conditions for plenter forest management according to the autecological growth conditions of silver fir, and at least one additional shade tolerant tree species. The technical-economic constraints are defined by slope (elevations silver fir is likely to expand. Our results may serve forest managers to identify areas suitable for plenter forests and assist in the transformation of even-aged pure forests to uneven-aged forests to increase resistance, resilience, and biodiversity under climate change.

The Influence of Visible Light on the Consumption Rate of Expanded Polystyrene by Zophobas morio Larvae

The process of disposing of expanded polystyrene (EPS) is by burning it in municipal incinerators. This process gives off EPS microplastics, which can find their way into water, food, blood, and maj.r organ systems. Zophobas morio larvae are capable of consuming and breaking down EPS within their digestive tracts by minimizing the spread of microplastics. Studies of the consumption of EPS by Z. morio larvae are typically conducted under white or no visible light treatments. This study tested whether the color of visible light influenced the consumption rate of EPS by Z. morio larvae. If Z. morio larvae consume EPS under visible light, then visible light will influence the amount of EPS consumed. If results suggest that the consumption rate is influenced by visible light colors, then Z. morio larvae could be a solution for recycling EPS. This study’s procedure placed Z. morio larvae into 25 j.rs under one of six visible light treatments of red, yellow, green, blue, white, and no visible light. Each j.r contained a pre-weighed block of EPS and six Z. morio larvae. After two weeks, the Z. morio larvae were removed from the j.rs, and the difference between each pre-weighed EPS block and the weight of the same partially consumed block was recorded in three trials. The data indicates that green and blue visible light treatments resulted in the greatest amount of EPS consumed by Z. morio larvae while the red and yellow had the least amount of EPS consumed by the Z. morio larvae. In conclusion, results indicate that green and blue visible light, compared to the no visible light treatment, could be used to influence the Z. morio larvae to consume more EPS. Green and blue visible light and Z. morio larvae could make the recycling process of EPS more environmentally friendly when used in households or by local environmental organizations.

Thoughts Concerning the Origin of Our Fractal Universe

During the past few decades, it has become clear that the distribution, sizes, and masses of cosmic structures are best described as fractal rather than homogeneous. This means that an entirely different formalism is needed to replace the standard perturbation model of structure formation. Recently, we have been developing a model of cosmology that accounts for a large number of the observed properties of the universe. A key component of this model is that fractal structures that later regulated the creation of both matter and radiation came into existence during the initial Planck-era inflation. Initially, the vacuum was the only existence and since time, distance, and energy were uncertain, its only property, the curvature (or energy), was most likely distributed randomly. Everything that happened after the Planck era can be described by the known laws of physics so the remaining fundamental problem is to discover how such a random beginning could organize itself into the hierarchy of highly non-random self-similar structures on all length scales that are necessary to explain the existence of all cosmic structures. In this paper, we present a variation of the standard sandpile model that points to a solution. Incidental to our review of the distributions of cosmic structures, we discovered that the apparent transition from a fractal to a homogeneous distribution of structures at a distance of about 150 Mpc is a consequence of the finite size of the universe rather than a change in the underlying statistics of the distributions.

Telomeres: The Promise of New Cancer Therapies

Telomeres have been a subj.ct of genetic research since the 1930s. They play a crucial role in cancer biology, as they influence both cellular senescence and genomic stability. In cancer cells, dysfunctional telomeres can lead to chromosomal fusions and, through deregulation of telomerase, allow replication of mutated chromosomes that might otherwise lead to apoptosis. Research is now focused on improving telomere-based cancer cell detection and developing potential therapies that inhibit telomerase activity in cancerous cells. Telomere research is crucial in understanding the molecular mechanisms influencing tumor growth and invasiveness because of the central role played by telomeres in various cancer types. Several telomerase inhibitors and immunotherapy treatments are in pre-clinical or clinical development. Research on the role of telomeres in oncogenesis has made significant strides, but obstacles remain, including a lack of high-resolution structural understanding, inadequate preclinical models, and concern over potential side effects. Even so, the current path of telomere research holds promise.
