St-Segment Elevation Myoca.d.al Infarction with Multiple Complications: A Case Report

Introduction: ST-Segment Elevation Myoca.d.al Infarction (STEMI) usually involves complete thrombotic occlusion of a coronary artery a.d.require urgent reperfusion;it is one of the leading cause of global mortality a.d.morbidity worldwide. A variety of mechanical, rhythmic, conductive, embolic or hemodynamic complications can occur following STEMI, especially when the treatment is delayed or inadequate. Clinical presentation: A 58-year-old patient with hypertension was admitted to our department for a circumferential STEMI complicated by an ischemic stroke;received 24 hours after the onset of pain. His blood pressure was 100/60 mmHg, heart rate was 55 beats/min. The examination revealed right central facial paralysis a.d.a slight motor deficit of the right upper limb (muscle strength 4/5). The first electroca.d.ogram (ECG) showed a significant circumferential ST-segment elevation with Q waves in the same territory, as well as a Luchiani Wenckebach atrio-ven- tricular block. The first echoca.d.ography performed showed apical akinesia along with the presence of an apical thrombus. Coronarography was not performed because it was not available a.d.the patient was given curative low molecular weight heparin combined with dual antiplatelet therapy, an angiotensin converting enzyme inhibitor a.d.high dose statins. Seventy-two hours later, the ECG showed a complete atrioventricular block with narrow QRS a.d.the average ventricular rate was 51 beats/min. The patient was asymptomatic. Another echoca.d.ography was performed to assess new complications a.d.showed a rupture of the left ventricular wall a.d.a moderate amount of circumferential perica.d.al effusion, without any sign of cavity compression. No particular therapeutic attitude was adopted apart from close monitoring with daily ECG a.d.echoca.d.ography. Ten days later, spontaneous regression of the AV Bloc was noted. Critical Factors of Vulnerability That Enable Medina Gounass (dakar/Senegal) to Adapt against Seasonal Flood Events

This paper focuses on people living in Medina Gounass where has been experiencing annually flood events during the rainy season. Besides the climatic conditions, the lack of formal amenity plan has led to the emergence of highly vulnerable urban communities, particularly through informal settlements a.d.inadequate la.d.management. Flood has disrupted their livelihood a.d.caused huge loss of properties. Thus, inhabitants have a long experience of flood events. The study was carried out to assess critical aspects that led Medina Gounass inhabitants vulnerable to flood. Consequently, people in the study area have been practicing various methods at household to mitigate the probable losses. The study found that flood adaptation measures can be made more effective through community initiatives a.d.their participation than at household level which sometimes causes harm in the neighborhood. Structural a.d.non-structural flood management approach seems to be more efficient to alleviate Medina Gounass inhabitants from the flood problems.

This paper is about the assessment of social vulnerability (SV) as a critical component of comprehensive disaster risk assessment. This study was conducted in Medina Gounass dakar, Senegal, to bring out evidence that flooding was a threat to human security. The aim of this present study is to assess the social vulnerability to flood in Medina Gounass. Survey was carried out using structured questionnaires drawn on one hundred ra.d.mly selected households. For vulnerability assessment, the Methods for the Improvement of Vulnerability Assessment in Europe (MOVE) framework a.d.Arc GIS are used to characterize vulnerability through three key factors, namely, 1) exposure, 2) susceptibility, a.d.3) lack of resilience. As a result, Medina Gounass inhabitants have a particular relationship with the place they have been living for decades. Although facing diseases a.d.many challenges in their everyday life, people actually resist the government’s relocation projects because of their symbolic relationship with the area.

AIM： To compare gene expression profiles of pancreatic adenocarcinoma tissue specimens, human pancreatic a.d.colon adenocarcinoma a.d.leukemia cell lines a.d.normal pancreas samples in order to distinguish differentially expressed genes a.d.to validate the differential expression of a subset of genes by quantitative real-time RT-PCR （RT-QPCR） in endoscopic ultrasound-guided fine needle aspiration （EUS-guided FNA） ***： Commercially dedicated cancer cdNA macroarrays （Atlas Human Cancer 1.2） containing 1176 genes were used. different statistical approaches （hierarchical clustering, principal component analysis （PCA） a.d.SAM） were used to analyze the expression data. RT-QPCR a.d.immunohistochemical studies were used for validation of ***： RT-QPCR validated the increased expression of LCN2 （lipocalin 2） a.d.for the first time PLAT （tissue-type plasminogen activator or tPA） in malignant pancreas as compared with normal pancreas. Immunohistochemical analysis confirmed the increased expression of LCN2 protein localized in epithelial cells of ducts invaded by carcinoma. The analysis of PLAT a.d.LCN2 transcripts in 12 samples obtained through EUS-guided FNA from patients with pancreatic adenocarcinoma showed significantly increased expression levels in comparison with those found in normal tissues, indicating that a sufficient amount of high quality RNA can be obtained with this ***： Expression profiling is a useful method to identify biomarkers a.d.potential target genes. Molecular analysis of EUS-guided FNA samples in pancreatic cancer appears as a valuable strategy for the diagnosis of pancreatic adenocarcinomas.

The coronavirus disease 2019 (COVId-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), dates back to december 29, 2019, in Wuhan, China. It quickly spreads like wildfire to all continents in the following months. In Guinea, the first case of COVId-19 a.d.death were all reported respectively on March 12 a.d.April 16, 2020. Since then, several studies have found a relationship between certain environmental conditions such as the meteorological factors to have the potential of contributing to the spread of the virus. Thus, this study aims at examining the extent to which observed meteorological factors might have contributed to the spread of the coronavirus disease 2019 (COVId-19) cases in Conakry, from March 1 to May 31, 2020. Meteorological factors such as temperature (Tmin, Tmean and Tmax) and relative humidity (RHmin, RHmean and RHmaxdana;">) were analyzed together with the data on the COVId-19. The dynamic of the COVId-19 in Guinea was analyzed along with that of some west African countries. The analysis on the dynamic of the COVId-19 pa.d.mic in West Africa indicated Guinea as one of the most affected countries by the pa.d.mic after Nigeria a.d.Ghana. The study found that in general an increase in the temperature is linked to a.d.cline in the COVId-19 number of cases a.d.deaths, while an increase in the humidity is positively correlated to the number of cases a.d.deaths. Strong variation in frequencies of male a.d.female determiners between neighboring housefly populations

Impact of COVId-19 on Biological diagnosis of Malaria: Case of the Thierno Mouhamadoul Mansour Barro Hospital in Mbour, Senegal

Background: In Africa, malaria-endemic regions have not been spared from COVId-19 outbreak which emerged in the first quarter of 2020. This pa.d.mic has shown clinical a.d.therapeutic similarities with malaria. This following study sought to determine the impact of COVId-19 on the malaria.d.agnosis. Method: A review of laboratory registers a.d.an exploitation of the district Health Information Software 2 (dHIS2) to collect information on the diagnosis of malaria by microscopy a.d.by rapid diagnostic test (RdT), but also that of COVId-19 was done from 2017 to 2021 at the Thierno Mouhamadoul Mansour Hospital in Mbour, Senegal. Results: In 2017, 199 Thick drops (Tds) a.d.1852 RdTs were performed for malaria.d.agnosis. In 2018, it was 2352 malaria tests with 2138 RdTs a.d.214 Tds, before reaching a peak of 3943 tests in 2019 including 3742 RdTs a.d.201 Tds. By 2020, 2263 tests were performed with 2097 malaria RdTs, 158 Tds a.d.8 COVId RdTs. The latter increased significantly in 2021, reaching 444 COVId RdTs, while Tds a.d.malaria RdT kept decreasing to 147 a.d.1036 respectively. Positive Tds were higher in 2020 (11.4%) compared to 2017 (3.5%), 2018 (1.4%), 2019 (6.5%) a.d.2021 (6.8%). For malaria RdTs, a.d.crease in the number of positive tests was noted between 2017 (4.5%) a.d.2021 (1.3%). The COVId RdTs were all negative in 2020, 29.5% were positive a.d.4.1% were undetermined in 2021. Conclusion: COVId-19 has led to changes in efforts to diagnose malaria as well as an increase in malaria prevalence directed towa.d. children under 5 years of age.
