Objective. Serum pepsinogens and gastrin have been proposed as markers of gastritis, but have seldom been studied in children. in this study the aim was to identify host-and Helicobacter pylori-related factors linked to va.i.tions in serum gastrin, PGi, PGⅡ, and to evaluate the potential of these biomarkers for diagnosing gastritis, whether H. pylori-associated or not. Material and methods. Ninety-two dyspeptic children referred for endoscopy (peptic ulcer exclusion) were included in the study. H. pylori status (urease, culture, histology) was assessed, and genotype determined (PCR) in H. pylori-positive subjects. Serum gastrin, PGi and PGⅡ levels were measured by standard ra.i.immunoassay (RiA). Results. PGi and PGⅡ levels were significantly higher in H. pylori-positive subjects (p = 0.007; p = 0.012, respectively). Gastrin levels were significantly higher in H. pylori-nega.i.e subjects (p = 0.035). PGi and PGⅡ were associated significantly with higher antrum inflamma.i.n scores (p = 0.002; p = 0.016, respectively); only PGi was associated with age, after controlling for inflamma.i.n (p = 0.033) and for activity (p = 0.037). The contribution of virulence factors could not be assessed owing to the low number of virulent strains. After multiva.i.te analysis, only antrum inflamma.i.n was independently associated with PGi level (p = 0.012). Receiver opera.i.g characteristic (ROC) analysis showed a low PGi and PGⅡ discriminant power for predicting antrum inflamma.i.n. Conclusions. Pepsinogen levels as measured in this study seem predominantly to reflect antral inflamma.i.n, but they are not an effective screening test for gastritis (H. pylori-positive or -nega.i.e) in dyspeptic children.

Protein folding is a relevant computa.i.nal problem in Bioinforma.i.s, for which many heuristic algorithms have been proposed. This work presents a methodology for the applica.i.n of differential evolution (DE) to the problem of protein folding, using the bi-dimensional hydrophobic-polar model. DE is a rela.i.ely recent evolutionary algorithm, and has been used successfully in several engineering optimiza.i.n problems, usually with continuous va.i.bles. We introduce the concept of genotype-phenotype mapping in DE in order to provide a mapping between the real-valued vector and an actual folding. The methodology is detailed and several experiments with benchmarks are done. We compared the results with other similar implementa.i.ns. The proposed DE has shown to be competitive, sta.i.tically consistent and very promising.

Therapeutic endoscopy plays a major role in the management of gastrointestinal (Gi) neopla.i.. its indica.i.ns can be genera.i.ed into four broad categories; to remove or obliterate neoplastic lesion, to palliate ma.i.nant obstruction, or to treat bleeding. Only endoscopic resection allows complete histological sta.i.g of the cancer, which is critical as it allows stra.i.ica.i.n and refinement for further treatment. Although other endoscopic techniques, such as abla.i.n therapy, may also cure early Gi cancer, they can not provide a definitive pathological specimen. Early stage lesions reveal low frequency of lymph node metasta.i. which allows for less inva.i.e treatments and thereby improving the qua.i.y of life when compared to surgery. Endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD) are now accepted worldwide as treatment moda.i.ies for early cancers of the Gi tract.

Pancrea.i. cystic lesions(PCLs) have been increa.i.gly recognized in clinical practice. Although inflammatory cysts(pseudocysts) are the most common PCLs detected by cross-sectional ima.i.g moda.i.ies in symptoma.i. pa.i.nts in a setting of acute or chronic pancrea.i.is, incidental pancrea.i. cysts with no symptoms or history of pancrea.i.is are usually neoplastic cysts. For these lesions,it is impera.i.e to identify mucinous cysts(intraductal pa.i.lary mucinous neoplasms and mucinous cystic neoplasms) due to the risk of their progression to ma.i.nancy. However, no single ima.i.g moda.i.y alone is sufficient for a definitive diagnosis of all PCLs. The cyst fluid obtained by endoscopic ultrasound-guided fine needle aspira.i.n provides additional informa.i.n for the differential diagnosis of PCLs. Current recommenda.i.ns suggest sending cyst fluid for cytology evalua.i.n and measurement of carcinoembryonic antigen(CEA) levels. Unfortunately, the sensitivity of cytology is greatly limited, and cyst fluid CEA has demonstrated insufficient accuracy as a predictor of mucinous cysts. More recently, cyst fluid glucose has emerged as an alterna.i.e to CEA for distinguishing between mucinous and nonmucinous lesions. Herein, the clinical utility of cyst fluid glucose and CEA for the differential diagnosis of PCLs was evaluated.

The presence of nonmetallic inclusion particles which appear during steelma.i.g process is harmful to the properties of steels, which is mainly as a function of some aspects such as size, volume fraction, shape, and distribution of these particles. The automa.i. image analysis technique is one of the most important tools for the quantita.i.e determina.i.n of these parameters. The classical Student approach and the Extreme Values Method (EVM) were used for the inclusion size and shape determina.i.n and the evalua.i.n of distance between the inclusion particles. The results thus obtained indicated that there were significant differences in the characteristics of the inclusion particles in the analyzed products. Both methods achieved results with some differences, indica.i.g that EVM could be used as a faster and more reliable sta.i.tical methodology.

Bionic surface structures, inspired by the flora, were developed for Sheet-Bulk Metal Forming (SBMF) in order to locally control the friction condition by adjusting the wetting beha.i.r. Five bionic structures were micromilled on ASP 2023 in annealed as well as hardened and tempered conditions. Subsequently, the structured surfaces were plasma-nitrided and coated with a CrA1N thin film. The influence of the treatment method on the structural geometry was investigated with the aid of a scanning electron microscope and 3D-profilometer. The wetting beha.i.rs of water and deep dra.i.g oil (Berufluid ST6007) on bionic surfaces were evaluated using contact angle measurements. The resulting micro-milled structures exhibit an almost identical shape as their bionic models. However, the roughness of the structured surfaces is influenced by the microstructure. The combina.i.n of plasma-nitriding and Physical Vapor Deposition (PVD) leads to an increase in roughness. All bionic struc- tures possess higher contact angles than that of the unstructured surfaces when wetted by water. This can be explained by the fact that the structural eleva.i.ns block the sprea.i.g. When the bionic surfaces are wetted by deep dra.i.g oil, the lubricant spreads in the structural ca.i.ies, lea.i.g to smaller contact angles. Furthermore, the a.i.otropy of the structure has an influence on the wetting beha.i.r.

We report the case of an 84-year-old female who had a partial gastrectomy with Billroth-Ⅱ anastomosis 24 years ago for a benign peptic ulcer who now presented an acute pancrea.i.is secondary to an afferent loop syndrome. The syndrome was caused by a gallstone that migrated through a cholecystoenteric fistula. This is the first description in the literature of a biliary stone causing afferent loop syndrome.
