Background:There are no clearly defined indications for pancreas-preserving *** present study aimed to analyze postoperative morbidity and the outcomes of patients undergoing pancreaspreserving ***:Patients undergoing pancreas-preserving duodenectomy from April 2008 to May 2020 were *** divided the series according to indication:scenario 1,primary duodenal tumors;scenario 2,tumors of another origin with duodenal involvement;and scenario 3,emergency ***:We included 35 *** duodenectomy was performed in 1 patient of adenomatous duodenal polyposis,limited duodenectomy in 7,and third+fourth duodenal portion resection in *** indications for scenario 1 were gastrointestinal stromal tumor(n=13),adenocarcinoma(n=4),neuroendocrine tumor(n=3),duodenal adenoma(n=1),and adenomatous duodenal polyposis(n=1);scenario 2:retroperitoneal desmoid tumor(n=2),recurrence of liposarcoma(n=2),retroperitoneal paraganglioma(n=1),neuroendocrine tumor in pancreatic uncinate process(n=1),and duodenal infiltration due to metastatic adenopathies of a germinal tumor with digestive hemorrhage(n=1);and scenario 3:aortoenteric fistula(n=3),duodenal trauma(n=1),erosive duodenitis(n=1),and biliopancreatic limb ischemia(n=1).Severe complications(Clavien-Dindo≥IIIb)developed in 14%(5/35),and postoperative mortality was 3%(1/35).Conclusions:Pancreas-preserving duodenectomy is useful in the management of primary duodenal tumors,and is a technical option for some tumors with duodenal infiltration or in emergency interventions.

BACKGROUND： Rheumatoid arthritis（ra） is a systemic inflammatory disease characterized by functional disability and pain. Although acupuncture is widely used, until now Western acupuncture studies on ra have not shown conclusive positive results. Acupuncture is regarded as a reflex therapy that has effects on the human autonomic nervous system. By establishing a traditional Chinese medicine（TCM） diagnosis first, the practitioner is able to choose acupoints according to the state of each individual ***/DESIGN： We are interested if acupuncture, using a classical diagnostic procedure to allocate acupoints to the patient according to the Shang Han Lun theory, can be effective in relieving pain, improving hand function and increasing health-related quality of life in *** authors intend to harmonize TCM diagnosis according to clinical and genetic profiles. Objective: To identify and synthesize the most recent available evidence of effectiveness of acupuncture on pain, physical function and health-related quality of life(HRQoL) in patients with rheumatoid arthritis(ra). Methods: A comprehensive search of 12 Western and Chinese databases was undertaken from their inception up to end of 2016. randomized controlled trials(RCTs), concerning patients with ra treated with needle acupuncture, written in English, Portuguese, German or Chinese were included. Primary outcomes included pain, physical function and HRQoL. Secondary outcomes included morning stiffness, functional impairment, number of tender and swollen joints and serum concentrations of inflamatory markers. Methodological quality was assessed by three independent reviewers using the standardized critical appraisal instrument from the Joanna Briggs Institute Meta-Analysis of Statistics Assessment and Review Instrument. Results: Twenty-two studies met the inclusion criteria. Of those, 9 studies were excluded after assessment of their methodological quality. The remaining 13 original RCTs included 974 patients. Ten of these studies published in China, showed favorable statistical significant effects of acupuncture in relieving symptoms of ra compared with controls. Conclusions: Evidence suggests that acupuncture interventions may have a positive effect in pain relief, physical function and HRQo L in ra patients. However, due to the heterogeneity and methodologic limitations of the studies included in this systematic review, evidence is not strong enough to produce a best practice guideline.

Breast cancer is the predominant form of carcinoma among women worldwide,with 70%of advanced patients developing bone metastases,with a high mortality *** this sense,the bone marrow(BM)mesenchymal stem/stromal cells(MSCs)are critical for BM/bone homeostasis,and failures in their functionality,transform the BM into a premetastatic niche(PMN).We previously found that BM-MSCs from advanced breast cancer patients(BCPs,infiltrative ductal carcinoma,stage III-B)have an abnormal *** work aims to study some of the metabolic and molecular mechanisms underlying MSCs shift from a normal to an abnormal profile in this group of patients.A comparative analysis was undertaken,which included self-renewal capacity,morphology,proliferation capacity,cell cycle,reactive oxygen species(Ros)levels,and senescence-associatedβ‑galactosidase(SA‑β‑gal)staining of BMderived MSCs isolated from 14 BCPs and 9 healthy volunteers(HVs).Additionally,the expression and activity of the telomerase subunit TERT,as well as telomere length,were *** levels of pluripotency,osteogenic,and osteoclastogenic genes(OCT-4,SOX-2,M-CAM,RUNX-2,BMP-2,CCL-2,M-CSF,and IL-6)were also *** results showed that MSCs from BCPs had reduced,self-renewal and proliferation *** cells also exhibited inhibited cell cycle progression and phenotypic changes,such as an enlarged and flattened ***,there was an increase in Ros and senescence levels and a decrease in the functional capacity of TERT to preserve telomere *** also found an increase in pro-inflammatory/pro-osteoclastogenic gene expression and a decrease in pluripotency gene *** conclude that these changes could be responsible for the abnormal functional profile that MSCs show in this group of patients.

Morpho-Physiological Changes in Roots of Rice Seedling upon Submergence

Submergence is a serious environmental condition that causes large loss in rice production in rain fed lowland and flood affected area. This study evaluated morphological and physiological responses of rice roots to submergence using two tolerant rice genotypes FR13A and Swarna-Sub 1 and two sensitive ones Swarna and IR42. The tolerant genotypes had higher survival rate and less shoot elongation but greater root elongation during submergence than the sensitive ones. After submergence,the tolerant genotypes also had higher root dry weight and more active roots than the sensitive *** genotypes exhibited less root injury, with less malondialdehyde production and slower electrolyte leakage after submergence. Tolerant genotypes also maintained higher concentrations of soluble sugar and starch in roots and shoots and higher chlorophyll retention after submergence than the sensitive *** data showed that root traits such as root activity and root growth are associated with survival rate after submergence. This is probably accomplished through higher energy supply, and membrane integrity is necessary to preserve root function and reduce injury during submergence. These root traits are important for submergence tolerance in rice.

The oat panicle components were changed by the forms of nitrogen use influencing productivity. The aim of the study was to determine the direct and indirect effects of oat panicle components on grain yield by N-fertilizer rates in succession systems of high and reduced release of N-residual. The study was conducted in the years 2013 and 2014 in a randomized complete block with four replications in a factorial 3 × 2, for nitrogen rates (30, 60 and 120 kg·ha-1) and oat cultivars (Barbarasul and Brisasul) the soybean/oat and corn/oat systems. The panicle weight and grain per panicle show greater sensitivity to the change of nitrogen rates. Grain yield was favored by the direct effect of harvest index panicle and indirect by grain weight per panicle. In the system soybean/ oat, the grain weight of panicle shows greater efficiency in promoting positive direct effects on grain yield.

AIM: To investigate activity, toxicity, and prognostic factors for survival of erlotinib and fixed dose-rate gemcitabine (FDR-Gem) in advanced pancreatic cancer.METHODS: We designed a single-arm prospective, multicentre, open-label phase II study to evaluate the combination of erlotinib (100 mg/d, orally) and weekly FDR-Gem (1000 mg/m2, infused at 10 mg/m2 per minute) in a population of previously untreated patients with locally advanced, inoperable, or metastatic pancreatic cancer. Primary endpoint was the rate of progression-free survival at 6 mo (PFS-6); secondary endpoints were overall response rate (ORR), response duration, tolerability, overall survival (os), and clinical benefit. Treatment was not considered to be of further interest if the PFS-6 was 40% would be of considerable interest (p1 = 40%); with a 5% rejection error (α = 5%) and a power of 80%, 35 fully evaluable patients with metastatic disease were required to be enrolled in order to complete the study. Analysis of prognostic factors for survival was also carried out.RESULTS: From May 2007 to September 2009, 46 patients were enrolled (male/female: 25/21; median age: 64 years; median baseline carbohydrate antigen 19-9 (CA 19-9): 897 U/mL; locally advanced/metastatic disease: 5/41). PFS-6 and median PFS were 30.4% and 14 wk (95%CI: 10-19), respectively; 1-year and median os were 20.2% and 26 wk (95%CI: 8-43). Five patients achieved an objective response (ORR: 10.9%, 95%CI: 1.9-19.9); disease control rate was 56.5% (95%CI: 42.2-70.8); clinical benefit rate was 43.5% (95%CI: 29.1-57.8). CA 19-9 serum levels were decreased by > 25% as compared to baseline in 14/23 evaluable patients (63.6%). Treatment was well-tolerated, with skin rash being the most powerful predictor of both longer PFS (P os (P = 0.01) at multivariate analysis (median os for patients with or without rash: 42 wk vs 15 wk, respectively, Log-rank P = 0.03). Adjuvant Taxanes in Breast Cancer: A Critical Re-appraisal

Purpose: Several studies have reported a positive impact for taxanes in adjuvant breast cancer (BC) treatment in terms of reduced recurrence and mortality. However the impact of the magnitude on overall survival (os) remains partially controversial. Methods: We examined the impact of taxane-containing adjuvant therapy for patients with early BC on os, based on the number of deaths and on the calculated number of patients who need to be treated with taxanes to avoid one death (NNT). We classified patients in three different groups according to whether taxanes were administered concurrently or sequentially, and whether all treatment arms had the same or different duration. Results: 1) Taxanes in combination therapy: 8258 patients (4373 with taxanes and 3885 without taxanes), with 723 os events. Overall survival for taxane-treated patients was 92.7% versus 89.6% for patients not receiving taxanes. NNT was 33. 2) Sequential treatment of unequal duration in the treatment arms: 14,228 patients (7970 with taxanes and 6256 without). Overall survival in taxane-treated patients was 86% compared with 83.2% for patients not receiving taxanes. NNT was 44. 3) Sequential treatment and similar duration in treatment arms: 9511 women (5093 with taxanes and 4418 without). Overall survival in patients treated with taxanes was 87% versus 85% in patients not receiving taxanes. NNT was 50. When the results of all these trials were considered together, the NNT stands at 43 patients. Conclusion: Taxanes afford a modest increase in overall survival in BC patients regardless of how they are given. Translational trials may well help to improve patient selection in the future.
