Patterns of local recurrence in rectal cancer after a multidisciplinary approach

Improvements in surgery and the application of combined approaches to fight rectal cancer have succeeded in reducing the local recurrence (LR) rate and when there is LR it tends to appear later and less often in isolation. Moreover, a subtle change in the distribution of LRs with respect to the pelvis has been observed. In general terms, prior to total mesorectal excision the most common LRs were central types (perianastomotic and anterior) while lateral and posterior forms (presa-cral) have become more common since the growth in the use of combined treatments. No differences have been reported in the current pattern of LRs as a function of the type of approach used, that is, neo-adjuvant therapies (short-term or long-course radiotherapy, orchemoradiotherapy versus extended lymphadenectomy, though there is a trend towards posterior or presacral LR in patients in the Western world and lateral LR in Asia. Nevertheless, both may arise from the same mechanism. Moreover, as well as the mode of treatment, the type of LR is related to the height of the initial tumor. Nowadays most LRs are related to the advanced nature of the disease. Involvement of the circumferential radial margin and spillage of residual tumor cells from lymphatic leakage in the pelvic side wall are two plausible mechanisms for the genesis of LR. The patterns of pelvic recurrence itself (pelvic subsites) also have important implications for prognosis and are related to the potential success of salvage curative approach. The re-operability for cure and prognosis are generally better for anastomotic and anterior types than for presacral and lateral recurrences. Overall survival after LR diagnosis is lower with radio or chemoradiotherapy plus optimal surgery approaches, compared to optimal surgery alone.

The prevalence of renal anaemia in new dialysis patients before and after routine eGFR reporting: A single centre experience

Background: In March 2006, evidence-based guidelines for the management of chronic kidney disease (CKD) in adults in the UK were published including the recommendation that kidney function should be assessed by formula-based estimation of glomerular filtration rate (eGFR), using the 4-variable modification of diet in renal disease (MDRD) equation. The purpose of this study was to evaluate whether or not improved assessment of renal function by eGFR reporting followed by an intensive local education programme and local protocols affected the prevalence of renal anaemia at the time of starting dialysis for the first time. Methods: To do this, we collected data on haemoglobin levels in people starting renal replacement therapy (RRT) for the first time, during the 12 months immediately preceding eGFR reporting. We collected data for a further 12 months after eGFR was introduced;starting 6 months after the official date of introduction. Results: The proportion of people with Haemoglobin (Hb) levels ≥ 11 g/dl increased from 25.4% in the pre-eGFR era to 41.1% in the post eGFR era. In addition, average Hb levels were better in the post eGFR era (9.89 vs. 10.2 g/dl) although this did not reach statistically significance. In general, in the post eGFR era, people known to the renal services for less than 1 month prior to starting RRT had the worst Hb levels (8.7 g/dl). Hb levels were higher according to time of referral prior to RRT with peak Hb levels in people referred 6 - 9 months (11.5 g/dl) beforehand. Conclusions: It has been suggested that estimated GFR reporting may be associated with earlier recognition of kidney disease. This may have contributed to the increase in the proportion of people with optimal haemoglobin (≥11 g/dl) levels prior to starting renal replacement therapy. Efficacy of non-pharmacological interventions in controlling type 2 diabetes in patients of African descent: A systematic review

Purpose: The purpose of this review is to systematically assess the potential effectiveness of targeted educational and other non-pharmacological interventions on diabetes control in populations of African descent in developed countries. Such information can inform intervention strategies and highlight evidence-based approaches to deal with this significant problem in this population. Methods: A systematic review and a meta-analysis of random controlled trials and cohort studies evaluating the influence of education and other non-pharmacological interventions on HbA1Cconcentrations in patients of African descent with diabetes. A comprehensive search of PubMed, EMBASE, CINAHL, ZETOC, SIGLE databases was carried out. Results: Although nine studies (8 randomised controlled trials and 1 cohort study) met the inclusion criteria, relevant HbA1Cdata were available for 6 of the studies for the subsequent meta-analysis. Heterogeneity of meta-analysis was high (I2 = 92%), the random effects pooled standard mean difference favoured the intervention -0.66 (-1.15, -0.17), p = 0.009. After sensitivity analysis, I2 remained moderate to high at 69%. The random effects pooled standard mean difference continued to favour the intervention -0.48 (-0.81, -0.16), p = 0.009. Conclusion: There is evidence supporting the efficacy of educational and other non-pharmacological interventions in diabetes control in populations of African descent in English speaking developed countries. This conclusion is tempered by the significant heterogeneity of selected interventions and paucity of high quality research in the target population.

Young People’s Conceptualization of Their Wellbeing: Culturally Situated Understandings in the Context of Kazakhstan

Little is known about the wellbeing of young people living in post-Soviet nation contexts such as Kazakhstan. The qualitative data reported represents the views of 309 Kazakhstani young people on their conceptualizations of wellbeing that fell broadly into three major thematic categories: external factors from one’s surroundings;relationships with others;and internal aspects of the self. The direct consequence of inadequate physical environment on health and the environment’s impact on psychological wellbeing through leisure, lifestyle, education, and employment opportunities were emphasized. Affirmative connections with others were associated with increased levels of emotional wellbeing. The importance of making independent choices as well as being actively involved in leisure, volunteering and extracurricular school activities for achieving optimal levels of wellbeing was highlighted.
